    Jocs motors Una alternativa per enfortir els músculs de l’abdomen

    L’objectiu de l’estudi va ser comparar l’amplitud de l’electromiografia (EMG) i la forma en què van ser coactivats els músculs abdominals durant l’execució de l’exercici d’encorbament del tronc i dos jocs motors tradicionals: el carretó i l’hula-hop. Per fer-ho, es va enregistrar l’EMG dels músculs recte, oblic extern i oblic intern de l’abdomen, durant l’execució de cada una de les tasques. A l’estudi hi van participar nou voluntaris sans, sense antecedents de cirurgia abdominal, lesions raquídies o síndrome de dolor lumbar. L’amplitud de l’EMG va ser amitjanada i normalitzada respecte a la contracció voluntària màxima. Es va realitzar un ANOVA de dos factors (múscul/tasca) i un post hoc Tukey per examinar les diferències en l’activitat elèctrica de cada múscul entre les tasques i en cada tasca entre els músculs. Els jocs analitzats van produir nivells d’activitat elèctrica superiors als generats per l’encorbament del tronc, i en conseqüència, han de ser considerats com un complement o una alternativa als exercicis d’enfortiment abdominal. El carretó va generar un important nivell de coactivació abdominal, encara que va activar principalment l’oblic extern de l’abdomen.     Early Seroma Treatment Protocol Based on US-Guided Aspiration in DTI Prepectoral Reconstruction: A Prospective Study

    Introduction: Seroma is a common complication after prepectoral prosthetic breast reconstruction with ADM, leading to wound dehiscencse, infection, and even loss of reconstruction at last. A new ultrasound (US) guided follow-up protocol has been applied to compare primary and secondary complications incidence and their treatment, and evaluate the effect of precocious seroma detection and its evacuation in reducing secondary complications. Methods: We enrolled 406 patients from January 1st, 2021 to July 1st, 2023 who underwent mastectomy and 1-stage prepectoral reconstruction with ADM. Experimental group counted 96 patients, whom have been treated as protocol fashion, therefore with multiple US-guided evaluations and eventual evacuations along with postoperative period; control group (310 patients) has exclusively been clinically evaluated. Results: Seroma incidence detected rate among experimental group, after 1-year follow-up, was 32.2%, compared to 16.8% in control cohort, additionally no other secondary complications were detected in the first group. Referring to the wound dehiscence incidence, a statistically significant higher frequency was observed in control group compared with treatment 1 (21.2% vs. 0%; P = .0027). Conclusions: Seroma and correlated secondary complications may lead to additional surgeries, higher sanitary costs and even reconstructive failure. With a seriated US follow-up protocol application, the surgeon could promptly manage and treat seroma, decreasing additional complications rate, particularly wound dehiscence. Level of evidence: III

    West Nile virus transmission. results from the integrated surveillance system in Italy, 2008 to 2015

    IIn Italy a national Plan for the surveillance of imported and autochthonous human vector-borne diseases (chikungunya, dengue, Zika virus disease and West Nile virus (WNV) disease) that integrates human and veterinary (animals and vectors) surveillance, is issued and revised annually according with the observed epidemiological changes. Here we describe results of the WNV integrated veterinary and human surveillance systems in Italy from 2008 to 2015. A real time data exchange protocol is in place between the surveillance systems to rapidly identify occurrence of human and animal cases and to define and update the map of affected areas i.e. provinces during the vector activity period from June to October. WNV continues to cause severe illnesses in Italy during every transmission season, albeit cases are sporadic and the epidemiology varies by virus lineage and geographic area. The integration of surveillance activities and a multidisciplinary approach made it possible and have been fundamental in supporting implementation of and/or strengthening preventive measures aimed at reducing the risk of transmission of WNV trough blood, tissues and organ donation and to implementing further measures for vector control

    The Effect of Adjuvant Radiotherapy on One- and Two-Stage Prosthetic Breast Reconstruction and on Autologous Reconstruction: A Multicenter Italian Study among 18 Senonetwork Breast Centres

    Purpose: In modern breast cancer treatment, a growing role has been observed for breast reconstruction together with an increase in clinical indications for postmastectomy radiotherapy (PMRT). Choosing the optimum type of reconstructive technique is a clinical challenge. We therefore conducted a national multicenter study to analyze the impact of PMRT on breast reconstruction. Methods: We conducted a retrospective case-control multicenter study on women undergoing breast reconstruction. Data were collected from 18 Italian Breast Centres and stored in a cumulative database which included the following: autologous reconstruction, direct-to-implant (DTI), and tissue expander/immediate (TE/I). For all patients, we described complications and surgical endpoints to complications such as reconstruction failure, explant, change in type of reconstruction, and reintervention. Results: From 2001 to April 2020, 3116 patients were evaluated. The risk for any complication was significantly increased in patients receiving PMRT (aOR, 1.73; 95% CI, 1.33-2.24; p < 0.001). PMRT was associated with a significant increase in the risk of capsular contracture in the DTI and TE/I groups (aOR, 2.24; 95% CI, 1.57-3.20; p < 0.001). Comparing type of procedures, the risk of failure (aOR, 1.82; 95% CI, 1.06-3.12, p=0.030), explant (aOR, 3.34; 95% CI, 3.85-7.83, p < 0.001), and severe complications (aOR, 2.54; 95% CI, 1.88-3.43, p < 0.001) were significantly higher in the group undergoing DTI reconstruction as compared to TE/I reconstruction. Conclusion: Our study confirms that autologous reconstruction is the procedure least impacted by PMRT, while DTI appears to be the most impacted by PMRT, when compared with TE/I which shows a lower rate of explant and reconstruction failure.     Mobile phones and head tumours. The discrepancies in cause-effect relationships in the epidemiological studies - how do they arise?

    The uncertainty about the relationship between the use of mobile phones (MPs: analogue and digital cellulars, and cordless) and the increase of head tumour risk can be solved by a critical analysis of the methodological elements of both the positive and the negative studies. Results by Hardell indicate a cause/effect relationship: exposures for or latencies from 65 10 years to MPs increase by up to 100% the risk of tumour on the same side of the head preferred for phone use (ipsilateral tumours) - which is the only one significantly irradiated - with statistical significance for brain gliomas, meningiomas and acoustic neuromas. On the contrary, studies published under the Interphone project and others produced negative results and are characterised by the substantial underestimation of the risk of tumour. However, also in the Interphone studies a clear and statistically significant increase of ipsilateral head tumours (gliomas, neuromas and parotid gland tumours) is quite common in people having used MPs since or for 65 10 years. And also the metaanalyses by Hardell and other Authors, including only the literature data on ipsilateral tumours in people having used MPs since or for 65 10 years - and so also part of the Interphone data - still show statistically significant increases of head tumours

    Proyecto Final Integrador de Ingeniería AmbientalLa cuenca del río Reconquista es considerada la segunda más contaminada del país. El canal Aliviador es uno de los tres causes del Reconquista a la altura de su desembocadura en la cuenca baja. En función un análisis bibliográfico y campañas de monitoreo, se concluye que efectivamente el canal se halla altamente impactado producto de la contaminación hídrica. El proyecto comprende el dragado y diseño de sistema de tratamiento de los sedimentos contaminados. Se propone la aplicación de tubos geotextiles con adición de polímeros coagulantes/floculantes para deshidratar el sedimento y contener los contaminantes dentro de las unidades. El cubicaje total de sedimentos a tratar corresponde a 22.427,9 m3, requiriendo de la instalación de 21 unidades de 15,7 m de circunferencia y 30 m de largo, cada equipo de capacidad volumétrica de tratamiento de 300 m3, según las especificaciones dadas por el fabricante. La unidad modular de tratamiento posee la capacidad de ser reproducida y adaptada a diversos volúmenes de sedimento a tratar. Se concluye que el proyecto resulta ambiental, técnica, social y económicamente viable. Siendo indispensable el cumplimiento del Plan de Gestión Ambiental y requiriendo imprescindiblemente del diseño de una escala piloto para su óptima implementación a escala real."Fil: Parodi, Nadia Agustina.     Experimental Study of the Phase Equilibria in the R-Al-Si Ternary Systems (R: Rare Earth Element) the Ho-Al-Si Isothermal Section at 500\u2daC

    The effect of holmium addition to the Al-Si system has been experimentally studied, as the isothermal section at 500˚C. The constitution of the alloys has been determined by means of scanning electron microscopy (SEM), electron microprobe analysis (EDXS) and X-ray powder diffraction. The knowledge of the phase relationships in the R-Al-Si ternary systems (R: rare earths element) is essential to deeply understand the technological properties of the Al-Si based alloys, that are useful in different industrial fields. In the system investigated have been identify nineteen ternary fields and twelve two phase fields. Three ternary compounds have been found in this ternary section: τ2-Ho2Al3Si2 (mS14-Y2Al3Si2), τ4-Ho2AlSi2 (oI10-W2CoB2) and τ5-Ho6Al3Si (tI80-Tb6Al3Si). The results obtained can be useful compared with the other known R-Al-Si systems also for predictive purposes
