    Venous Thromboembolism After Removal of Retrievable Inferior Vena Cava Filters

    The purpose of this study was to examine the incidence of new or recurrent venous thromboembolism (VTE) after retrieval of inferior vena cava (IVC) filters and risk factors associated with such recurrence. Between March 2001 and September 2008, at our institution, implanted retrievable vena cava filters were retrieved in 76 patients. The incidence of new or recurrent VTE after retrieval was reviewed and numerous variables were analyzed to assess risk factors for redevelopment of VTE after filter retrieval. In 5 (6.6%) of the 76 patients, redevelopment or worsening of VTE was seen after retrieval of the filter. Three patients (4.0%) had recurrent deep venous thrombosis (DVT) in the lower extremities and 2 (2.6%) had development of pulmonary embolism, resulting in death. Although there was no significant difference in the incidence of new or recurrent VTE related to any risk factor investigated, a tendency for development of VTE after filter retrieval was higher in patients in whom DVT in the lower extremities had been so severe during filter implantation that interventional radiological therapies in addition to traditional anticoagulation therapies were required (40% in patients with recurrent VTE vs. 23% in those without VTE; p = 0.5866 according to Fisher’s exact probability test) and in patients in whom DVT remained at the time of filter retrieval (60% in patients with recurrent VTE vs. 37% in those without VTE; p = 0.3637). In conclusion, new or recurrent VTE was rare after retrieval of IVC filters but was most likely to occur in patients who had severe DVT during filter implantation and/or in patients with a DVT that remained at the time of filter retrieval. We must point out that the fatality rate from PE after filter removal was high (2.6%)

    Cobordism of Morse functions on surfaces, the universal complex of singular fibers and their application to map germs

    We give a new and simple proof for the computation of the oriented and the unoriented fold cobordism groups of Morse functions on surfaces. We also compute similar cobordism groups of Morse functions based on simple stable maps of 3-manifolds into the plane. Furthermore, we show that certain cohomology classes associated with the universal complexes of singular fibers give complete invariants for all these cobordism groups. We also discuss invariants derived from hypercohomologies of the universal homology complexes of singular fibers. Finally, as an application of the theory of universal complexes of singular fibers, we show that for generic smooth map germs g: (R^3, 0) --> (R^2, 0) with R^2 being oriented, the algebraic number of cusps appearing in a stable perturbation of g is a local topological invariant of g.Comment: This is the version published by Algebraic & Geometric Topology on 7 April 200

    A hearing aid using cartilage conduction (CC) has been proposed as an alternative to bone conduction (BC) hearing aids. The transducer developed for this application is lightweight, requires a much smaller fixation force than a BC hearing aid, and is more convenient to use. CC can be of great benefit to patients with fibrotic aural atresia. Fibrotic tissue connected to the ossicles provides an additional pathway (termed fibrotic tissue pathway) for sound to reach the cochlea by means of CC. To address the function of fibrotic tissue pathway, BC and CC thresholds were measured in six ears with fibrotic aural atresia. The relationship between the CC thresholds and the results of computed tomography was investigated. In the ears with the presence of a fibrotic tissue pathway, the CC thresholds were lower than the BC thresholds at 0.5 and 1.0 kHz. At 2.0 kHz, no significant difference was observed between the BC and CC thresholds. The current findings suggest that sound in the low to middle frequency range is transmitted more efficiently by CC via a fibrotic tissue pathway than BC. The development of hearing devices using CC can contribute to rehabilitation, particularly in patients with fibrotic aural atresia.博士(医学)・甲621号・平成26年3月17

    A 5 m sediment core taken from Lake Kestel in southwestern Anatolia, Turkey, was analyzed for high molecular saturated fatty acids (≧C20 FAs) as a paleoclimatic indicator.In the present paper, we discussed the correlation between the fluctuations in ≧C20 FA concentrations and precipitation in Anatolia over the last 2,000 years based on previous pollen records. The discussion led to the following conclusion:1) The amounts of precipitation about 1,800 to 1,300 years ago were extremely low leading to very dry climatic conditions.2) From 1,300 to 600 years ago, the precipitation increased gradually to a level similar to that of the present with some fluctuations into very low precipitation. From about 600 years ago to the recent past, a wet climate had prevailed.3) From 200 to 40 years ago, the precipitation temporarily increased to about 1.5 times its previous level. This sequence is very similar, but not identical, to those of heat-stable enterotoxins produced by enterotoxigenic Escherichia coli and Yersinia enterocolitica

    Single-cell transcriptome atlas of Drosophila gastrula 2.0

However, the mechanisms underlying these processes in a genome-wide view remain ambiguous, partly because the single-cell transcriptomic data of early developing embryos containing accurate spatial and lineage information are still lacking. Here, we report a single-cell transcriptome atlas of Drosophila gastrulae, divided into 77 transcriptomically distinct clusters. We find that the expression profiles of plasma-membrane-related genes, but not those of transcription-factor genes, represent each germ layer, supporting the nonequivalent contribution of each transcription-factor mRNA level to effector gene expression profiles at the transcriptome level. We also reconstruct the spatial expression patterns of all genes at the single-cell stripe level as the smallest unit. This atlas is an important resource for the genome-wide understanding of the mechanisms by which genes cooperatively orchestrate Drosophila gastrulation
