    Population Pharmacokinetics and Pharmacodynamics of Extended-Infusion Piperacillin and Tazobactam in Critically Ill Children

    The study objective was to evaluate the population pharmacokinetics and pharmacodynamics of extended-infusion piperacillintazobactam in children hospitalized in an intensive care unit. Seventy-two serum samples were collected at steady state from 12 patients who received piperacillin-tazobactam at 100/12.5 mg/kg of body weight every 8 h infused over 4 h. Population pharmacokinetic analyses were performed using NONMEM, and Monte Carlo simulations were performed to estimate the piperacillin pharmacokinetic profiles for dosing regimens of 80 to 100 mg/kg of the piperacillin component given every 6 to 8 h and infused over 0.5, 3, or 4 h. The probability of target attainment (PTA) for a cumulative percentage of the dosing interval that the drug concentration exceeds the MIC under steady-state pharmacokinetic conditions (TMIC) of\u3e50% was calculated at MICs ranging from 0.25 to 64 mg/liter. The mean ± standard deviation (SD) age, weight, and estimated glomerular filtration rate were 5 ± 3 years, 17 ± 6.2 kg, and 118 ± 41 ml/min/1.73m2, respectively. A one-compartment model with zero-order input and first-order elimination best fit the pharmacokinetic data for both drugs. Weight was significantly associated with piperacillin clearance, and weight and sex were significantly associated with tazobactam clearance. Pharmacokinetic parameters (mean ± SD) for piperacillin and tazobactam were as follows: clearance, 0.22 ± 0.07 and 0.19 ± 0.07 liter/h/kg, respectively; volume of distribution, 0.43 ± 0.16 and 0.37 ± 0.14 liter/kg, respectively. All extended-infusion regimens achieved PTAs of\u3e90% at MICs of/liter. Only the 3-h infusion regimens given every 6 h achieved PTAs of\u3e90% at an MIC of 32 mg/liter. For susceptible bacterial pathogens, piperacillin-tazobactam doses of\u3e80/10 mg/kg given every 8 h and infused over 4 h achieve adequate pharmacodynamic exposures in critically ill children

    Oxidative stress in relation to diet and physical activity among premenopausal women

    Abstract Higher levels of oxidative stress, as measured by F 2 -isoprostanes, have been associated with chronic diseases such as CVD and some cancers. Improvements in diet and physical activity may help reduce oxidative stress; however, previous studies regarding associations between lifestyle factors and F 2 -isoprostane concentrations have been inconsistent. The aim of this cross-sectional study was to investigate whether physical activity and intakes of fruits/vegetables, antioxidant nutrients, dietary fat subgroups and alcohol are associated with concentrations of F 2 -isoprostane and the major F 2 -isoprostane metabolite. Urinary F 2 -isoprostane and its metabolite were measured in urine samples collected at enrolment from 912 premenopausal women (aged 35–54 years) participating in the Sister Study. Physical activity, alcohol consumption and dietary intakes were self-reported via questionnaires. With adjustment for potential confounders, the geometric means of F 2 -isoprostane and its metabolite were calculated according to quartiles of dietary intakes, alcohol consumption and physical activity, and linear regression models were used to evaluate trends. Significant inverse associations were found between F 2 -isoprostane and/or its metabolite and physical activity, vegetables, fruits, vitamin C, α -carotene, vitamin E, β -carotene, vitamin A, Se, lutein+zeaxanthin and long-chain n -3 fatty acids. Although trans fats were positively associated with both F 2 -isoprostane and its metabolite, other dietary fat subgroups including SFA, n -6 fatty acids, n -3 fatty acids, MUFA, PUFA, short-chain n -3 fatty acids, long-chain n -3 fatty acids and total fat were not associated with either F 2 -isoprostane or its metabolite. Our findings suggest that lower intake of antioxidant nutrients and higher intake of trans fats may be associated with greater oxidative stress among premenopausal women

    Quantum spin chains of Temperley-Lieb type: periodic boundary conditions, spectral multiplicities and finite temperature

    We determine the spectra of a class of quantum spin chains of Temperley-Lieb type by utilizing the concept of Temperley-Lieb equivalence with the S=1/2 XXZ chain as a reference system. We consider open boundary conditions and in particular periodic boundary conditions. For both types of boundaries the identification with XXZ spectra is performed within isomorphic representations of the underlying Temperley-Lieb algebra. For open boundaries the spectra of these models differ from the spectrum of the associated XXZ chain only in the multiplicities of the eigenvalues. The periodic case is rather different. Here we show how the spectrum is obtained sector-wise from the spectra of globally twisted XXZ chains. As a spin-off, we obtain a compact formula for the degeneracy of the momentum operator eigenvalues. Our representation theoretical results allow for the study of the thermodynamics by establishing a TL-equivalence at finite temperature and finite field.Comment: 29 pages, LaTeX, two references added, redundant figures remove

    Great Expectations: Voluntary Sports Clubs and Their Role in Delivering National Policy for English Sport

The volunteers work in a mutual aid, self-production, self-consumption system whose main purpose is identifying and nurturing high-level performers. But the new HMG/Sport England strategies leading to London 2012 expects volunteers to make a major contribution to sustaining and extending participation. The study utilized six focus group sessions with a total of 36 officials and members of 36 clubs across the six counties of Eastern England to assess whether and to what extent government policy objectives can be delivered through the voluntary sector. The study focused on the perceptions and attitudes of club members about being expected to serve public policy and the current pressures they and their clubs face. The results lead the authors to question the appropriateness, sensitivity, and feasibility of current sport policy, particularly the emphasis on VSCs as policy implementers.Peer reviewe

    A deformed analogue of Onsager's symmetry in the XXZ open spin chain

    The XXZ open spin chain with general integrable boundary conditions is shown to possess a q-deformed analogue of the Onsager's algebra as fundamental non-abelian symmetry which ensures the integrability of the model. This symmetry implies the existence of a finite set of independent mutually commuting nonlocal operators which form an abelian subalgebra. The transfer matrix and local conserved quantities, for instance the Hamiltonian, are expressed in terms of these nonlocal operators.     Testing general relativity using golden black-hole binaries

    The coalescences of stellar-mass black-hole binaries through their inspiral, merger, and ringdown are among the most promising sources for ground-based gravitational-wave (GW) detectors. If a GW signal is observed with sufficient signal-to-noise ratio, the masses and spins of the black holes can be estimated from just the inspiral part of the signal. Using these estimates of the initial parameters of the binary, the mass and spin of the final black hole can be uniquely predicted making use of general-relativistic numerical simulations. In addition, the mass and spin of the final black hole can be independently estimated from the merger--ringdown part of the signal. If the binary black hole dynamics is correctly described by general relativity (GR), these independent estimates have to be consistent with each other. We present a Bayesian implementation of such a test of general relativity, which allows us to combine the constraints from multiple observations. Using kludge modified GR waveforms, we demonstrate that this test can detect sufficiently large deviations from GR, and outline the expected constraints from upcoming GW observations using the second-generation of ground-based GW detectors.Comment: 5 pages, 2 fig

    Studying the Pulsation of Mira Variables in the Ultraviolet

    We present results from an empirical study of the Mg II h & k emission lines of selected Mira variable stars, using spectra from the International Ultraviolet Explorer (IUE). The stars all exhibit similar Mg II behavior during the course of their pulsation cycles. The Mg II flux always peaks after optical maximum near pulsation phase 0.2-0.5, although the Mg II flux can vary greatly from one cycle to the next. The lines are highly blueshifted, with the magnitude of the blueshift decreasing with phase. The widths of the Mg II lines are also phase-dependent, decreasing from about 70 km/s to 40 km/s between phase 0.2 and 0.6. We also study other UV emission lines apparent in the IUE spectra, most of them Fe II lines. These lines are much narrower and not nearly as blueshifted as the Mg II lines.     We consider the open spin-s XXZ quantum spin chain with nondiagonal boundary terms. By exploiting certain functional relations at roots of unity, we derive a generalized form of T-Q relation involving more than one independent Q(u), which we use to propose the Bethe-ansatz-type expressions for the eigenvalues of the transfer matrix. At most two of the boundary parameters are set to be arbitrary and the bulk anisotropy parameter has values \eta = i\pi/2, i\pi/4,...
