    Two- and Three-particle States in a Nonrelativistic Four-fermion Model in the Fine-tuning Renormalization Scheme. Goldstone mode "against" extension theory

    In a nonrelativistic contact four-fermion model we show that simple regularisation prescriptions together with a definite fine-tuning of the cut-off-parameter dependence of ``bare'' quantities give the exact solutions for the two-particle sector and Goldstone modes. Their correspondence with the self-adjoint extension into Pontryagin space is established leading to self-adjoint semi-bounded Hamiltonians in three-particle sectors as well. Hidden symmetry of the model is found. It could be broken or exact depending on the coupling constant value.     Taking into account the significance of the HLA complex in the inflammatory and immune responses, we can assume that the potential for limiting or developing pathology in the next generation will be determined at the stage of selection of certain alleles in the spousal genotype.The aim. To study the role of HLA assortativity in couples with healthy children and couples with children with congenital heart diseases (CHD) through the prism of immunogenetic mechanisms of mutual olfactory choice.Materials and methods. We studied the distribution of HLA-DRB1 alleles in married couples with healthy children and with children having CHD. To identify the associations of HLA-DRB1 alleles with odor preferences, we also studied the group included young males and females. HLA-DRB1 gene typing was carried out in all participants.Results. The combination of HLA-DRB1 alleles in couples with healthy children was similar to the combination of these alleles in the mutual olfactory sympathy between unfamiliar young males and females. Allele combinations in the spouses from the experimental group differ from the group of random selection. The frequency of matches for HLA-DRB1 alleles in married couples with children having CHD without was significantly higher than in the control group.Conclusion. The first stage of selection, associated with olfactory selection, is aimed to the whole population, and it is significantly manifested in the control group (married couples with healthy children). At the same time, in the experimental group (couples with children having CHD), some deviations from the main selection were discovered. Generally, specific HLA-DRB1 allele combinations obtained on the basis of olfactory assessments, indicate the involvement of HLA molecules in pheromone reception

    Polymorphism of inflammatory system genes in the pathogenesis of rheumatic heart disease

    Aim. To assess the contribution of polymorphic variants of inflammatory response genes to the predisposition to rheumatic heart disease.Material and methods. Using real-time polymerase chain reaction, we analyzed the prevalence of 18 polymorphic variants of 8 genes involved in the inflammatory process in 251 patients with rheumatic heart disease and 300 healthy donors.Results. We found that homozygous TT genotypes of rs1800871 (IL10) (p=0,02) and TT rs1800872 (IL10) polymorphisms (p=0,027), as well as TT genotypes of CRP gene (rs1205) (p=0,015) and GG genotypes of rs375947 (IL12RB) (p=0,037) are "risky" and associated with the development of rheumatic heart disease.Conclusion. Associations of polymorphic variants rs1800871 and rs1800872 of the IL10 gene, rs1205 of the CRP gene, and rs375947 of the IL12RB gene can be an important link in the pathogenesis of rheumatic heart disease and can later be used as biological markers for a personalized assessment of the disease risk

    Combination of Endobronchial Ultrasound and Modern Cytological Diagnosis in Patients With Intrathoracic Lesions

    Objective: To evaluate the effectiveness of endobronchial (EBUS) and endoscopic ultrasound (EUS) in cytomorphological verification of a malignant process using the transbronchial needle aspiration (TBNA) of the lymph nodes and mediastinal organs.Materials and methods: In 2017–2019 we analyzed the cytological samples (standard and cell block) of 172 patients (115 males and 57 females) with an average age of 61 years (25–82 years) in the clinical diagnostic laboratory of Scientific Research Institute – Ochapovsky Regional Clinical Hospital No. 1 (Krasnodar, Russian Federation). Of them 111 were used for immunocytochemical analysis. The majority of the studies (77) were performed in patients with lung cancer in order to identify the stage of the disease. We used combined bronchoscopy under general anesthesia in the operating room with high-frequency ventilation and a rapid on-site evaluation (ROSE).Results: We found the lymph node and mediastinal invasion in 140 (81%) patients who underwent EBUS-EUS TBNA. The non-diagnostic results were obtained in 32 (19%) patients. The combination of EUS and EBUS with TBNA showed the highest effectiveness in the mentioned clinical cases. The sensitivity, specificity, and accuracy of the cytologic analysis during EBUS and EUS with TBNA and no ROSE were 89%, 100%, and 90%, respectively. ROSE reduced the rate of non-diagnostic results and increased the sensitivity (96%), specificity (100%), and accuracy (97%) of the cytologic evaluation.Conclusions: According to the obtained results, we recommend using EBUS-EUS TBNA modalities for morphological verification of the mediastinal lymph nodes as a routine invasive diagnosis component of the preoperative evaluation in patients with lung cancer
