The article provides information about the XIX World Congress of Psychiatry (August 21-24, 2019, Lisbon, Portugal). The issues of implementing the new ICD-11 diagnostic principles are considered; the main results of changes and modifications in schizophrenic disorders, mood disorders and new diagnosticΒ categories in the new concept of βobsessive-compulsive disorders (OCD) and obsessive-compulsive related disordersβ are presented.The clinical utility of ICD-11 consists in the ability to use the diagnostic criteria more accurately than before. Perinatal mental health of the mother occupies a central position in healthcare, the same as caring for a newborn baby. Maternal depression is the lead pathology of postpartum period. Despite the high prevalence and importance to the overall development of children, most cases of depression are unappreciated, unrecognized and untreated. Inattention to issues which are related to physical and mental health of women, leads to severe negative social and economic consequences for society. Statistical comparison of the total number of pregnant women, the number of births in the United States in 2007, excluding cases of fetal loss, including still-births and miscarriages with similar Russian data allowed to calculate the approximate prevalence of postpartum depression in the same period. Presumable probability of postpartum depression is 15% in different regions of the world, and the diagnostic criteria of DSM-IV for postpartum depression are representative for comparison. Considering these assumptions, according to 2007 each year about 260,000 out of 1 mln. 600,000 of Russian women in childbirth may suffer from post-natal depression. The results have a probabilistic character. For the mothers the consequences of unrecognized and untreated depression after childbirth are the most frequent. It should be seen as a depressive episode or recurrent depressive disorder which has a singular coloring due to the appearance in the peak period of reproductive age of women. Postpartum depression may increase the possibility of occurrence of relapse of a depressive disorder as well as the transition to a state of chronic depression. Alcoholism or drug use is a common complication of depression; the risk of suicide increases, especially during the time of reducing the severity of the most severe symptoms of depression, though it is below the prevalence of suicide among the population. Particular attention should be paid to such symptoms as instability in mood and irritability as during the postpartum depression and in its residual period. They can cause child abuse. This paper also presents current data on the epidemiology, etiology, risk factors for postpartum depression, its clinical manifestations, the influence of untreated maternal depression on child development, therapy and educational modules to spread multidisciplinary and inter-agency approach in perinatal mental health problems.Π ΠΏΠ΅ΡΠΈΠ½Π°ΡΠ°Π»ΠΎΠ³ΠΈΠΈ ΠΏΡΠΈΡ

Historical and modern data of the need of the creation of social work faculty in Siberian State Medical University is discussed in the present paper. The need of university education renovation in the sphere of mental health protection is a ground of the social specialist training. It is suggested a development of biopsychosocial education model that requires the training of social workers in the sphere of clinical psychology, psychotherapeutic supports and psychiatry. Study objectives are dedicated to brief synthesized establishment of diagnostics general standard, management and therapy of major depressive disorders (MDD) in clinical medicine to provide education in medical practice.Study methods are based on short-term, medium-term and long-term educational programs in 24 Russian and Ukraine cities, which were based on original educational programs under the direction of World Psychiatric Association (WPA) and International Committee For Prevention and Treatment of Depression (PTD). There, about 1450 doctors of different occupations were acquainted with the detection, management and treatment of MDD. The Russian version of WPA/PTD programs was created. The program of education included 4 modules. The Core module included an overview of the epidemiology, impact, concepts and classification, and etiology of depressive disorders as well as their recognition, diagnosis, and management in the primary care setting. The second module focused on depressive disorders in physical illness and covered those major illnesses for which is reasonable evidence for an association with depressive disorders. The third module included the development of the depressive disorders in older persons. The forth module included training physicians in mental health skills. 39 theme improvements for 858 primary care setting physicians within the framework of faculty training program of physiciansβ development were the part of other programs of physiciansβ occupations.Results. A number of highlights were included in the general algorithm of the educational programs. The creation of optimum Β«physician-depressive patientΒ» contact demands a number of new skills to establish effective communication. These skills involve training of meeting of depressive patient and physician, the ability to follow a certain communication style; identify emotional, cognitive, psychomotor and nonverbal patterns of behavior. Besides that, the education included adherence to therapy and therapeutic alliance during the period of informed agreement. There was an education of fluent grasp of medicaments, which are the first choice in therapy. Integration of this knowledge is done during the periods of repeated interactive discussions of antidepressantsβ effectiveness in depression therapy which is based on three-phased MDD therapy model. The process of working out the depressive patientsβ management was based on systematization and diagnostic of depression disorders (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, or International Classification of Diseases, Revision 10); detailed discussions of manifestation and thorough depression symptomatology; management approaches in depression disorders, different in manifestation.Conclusion. The basis of physicians' education was the idea that depressive disorder in holistic approach can't be considered as an independent psychiatric disease in its mild and moderate forms of manifestation. COGNITIVE IMPAIRMENTS AFTER STROKE
Thirty-two patients (mean age 53.912.4 years) with prior stroke were examined to clarify the pattern and degree of cognitive impairments depending on the site of a lesion. A control group comprised 32 subjects (mean age 52.112.9 years) without nervous system pathology. The authors made a complete clinical study to evaluate the neurological and mental status, a standardized interview, and A.R. Luria's neuropsychological tests. The patients with prior stroke were found to have lower values of auditory-verbal short-term and long-term memory, impaired concentration and voluntary attention scope. Mild cognitive disorders were established in middle-aged patients and moderate cognitive disorders of amnestic multifunctional type were in elderly patients. Auditory-verbal memory, visual gnosis, and some speech-related thought functions were impaired when in the involvement of the temporal and posterior frontal lobes in patients with prior stroke in the middle cerebral artery bed. Impaired visual image recognition and lower visual voluntary attention were detected in patients with prior stroke in the anterior and posterior cerebral arteries

Unipolar depression is prevalent in inpatients with GIT tumors. Following types of depressions have been distinguished: with cognitive disturbances, anxious-hypochondriac and agitated ones. Open clinical comparative assessment of therapy effectiveness of depressive spectrum disorders was carried out. We compared patients who began treatment in special psychiatric department and those who underwent different consultations, investigations and took medication in primary care before hospitalization.
