The objective: to compare effectiveness of pressure support and mandatory ventilation modes at the final stage of general anesthesia.Subjects and Methods. 58 patients were included in the study. All patients underwent laparoscopic or open surgery under combined general anesthesia with muscle relaxants and tracheal intubation. At the end of the operation, after suturing the muscle layer, patients were randomly divided into two groups, depending on the further mode of ventilation: the mandatory mode group with dual control until extubation (n = 29) and the spontaneous breathing mode group with pressure support (n = 29). The time of awakening, the severity of post-extubation cough, hemodynamic parameters and oxygenation immediately before and 5 minutes after extubation were assessed.Results. In the groups of patients, statistically significant differences were observed in the time of awakening (252 Β± 67 sec and 426 Β± 71 sec in PSV and PCV-VG Groups, respectively), extubation (287 Β± 55 sec and 464 Β± 67 sec in the PSV and PCV-VG groups, respectively), and transfer from the operating room (473 Β± 60 sec and 687 Β± 77 sec in the PSV and PCV-VG groups, respectively) (p < 0.0001). Also, patients receiving PSV respiratory support had higher saturation levels 5 minutes after extubation (p < 0.0001), and heart rate and mean arterial pressure immediately before extubation were lower than in the mandatory ventilation group (p = 0.013 and p < 0.0001, respectively). In addition, in the mode of spontaneous breathing with pressure support, a lower severity of post-extubation cough was observed (p = 0.003).Conclusion. The use of a spontaneous breathing mode with pressure support at the end of general combined anesthesia has several advantages versus mandatory ventilation mode. These advantages include faster awakening, extubation and transfer of the patient to the ward, lower severity of post-extubation cough, as well as better gas exchange after extubation, lower intensity of hypertension and tachycardia before it. Earlier, in experiments in vitro and in vivo, it was shown that the TrkA receptor agonist compound GK-2, which was a low molecular weight NGF mimetic, had pronounced angiogenic and antiischemic activity. However, it remained unclear whether this activity was associated with the activation of VEGF-A, since, on the one hand, there are reports that NGF-mediated angiogenesis can be initiated by activating NGF of the vascular endothelial growth factor VEGF-A, and on the other hand, it is shown that a selective antagonist of Flk1 receptors specific for VEGF-A, compound SU-5416 does not affect the angiogenic effect of NGF. Purpose of the investigation. Study of the selective VEGF blockade effect on the antiischemic activity of the TrkA receptor agonist GK-2. Methods. Evaluation of the compound GK-2 antiischemic activity was assessed in model experiments simulating hind limb ischemia in rats. Results. It has been shown that against the background of blockade of VEGF-A binding with specific for it (VEGFR1 / Flt-1 / and VEGFR2 / KDR /) receptors, bevacizumab (2.5 mg / kg, i.p., every 3 days for 14 days) compound GK-2 (1 mg / kg, i.p., daily, for 14 days) realizes its antiischemic activity. Conclusion. The purpose of the study. Investigation of the effect of the fabomotizol dihydrochloride systematic therapy on the morphological picture of the heart left ventricle (LV) in rats in the subacute period of myocardial infarction. Materials and methods. Myocardial infarction (MI) modeling was carried out using the A.Selye method. Fabomotizol dihydrochloride was administered to rats intraperitoneally 1 time per day from the 15th to the 28th day after MI at a dose of 15 mg/kg. At the end of the experiment, euthanasia and a pathoanatomic autopsy were performed. Samples of hearts after fixation in formalin and standard wiring were poured into paraffin blocks. Histological sections of the hearts were microscoped in transmitted light. Results. Dilation of the LV cavity and thinning of its walls in animals treated with fabomotizol dihydrochloride are less pronounced than in control rats with MI. In the periinfarction zone of the myocardium in rats treated with fabomotizol dihydrochloride, wave-like deformation and fragmentation of cardiomyocytes is less intense, and the transverse striation of myofibrils, on the contrary, is more pronounced than in the control. In animals treated with fabomotizol dihydrochloride, the periinfarction zone is well vascularized. Conclusion. According to the results of morphological studies performed on a model of subacute MI in rats, it was demonstrated that systematic therapy with fabomotizole dihydrochloride contributes not only to a significant reduction in the necrosis zone, but also to a certain extent prevents the development of early postinfarction remodeling. In rats treated with fabomotizol dihydrochloride, in contrast to control animals, reparative processes prevail in the cardiac muscle. It is known that the alcoholic cardiomyopathy (ACMP) is the main reason for lethality from chronic alcoholism. For ACMP the risk of development of malignant violations of a heart rhythm which result approximately at 40% of such patients is sudden heart death is extremely high. Materials and methods. Experiments were made on the ACMP translational model developed by us which is formed at rats by the end of the 24th week of compulsory reception of 10 % of ethanol solution. For studying the mechanisms which are the responsible of antiarrhythmic action of a fabomotizole dihydrochloride used a complex of morphohistological, electrophysiological and molecular researches. Results. It is shown that against the background of systematic therapy fabomotizole dihydrochloride (15 mg/kg, i.p.) daily within 28 days after 24 weeks of alcoholization, in comparison with alcoholized control the fat dystrophy of a myocardium significantly decreases and the threshold of electric fibrillation of heart ventricles is restored. According to results of molecular researches, a fabomotizole dihydrochloride significantly suppresses revealed in control alkoholized animals the abnormal mRNA expression of key receptor genes and proteins responsible for maintenance in cardiomyocytes of a homeostasis of ions of Ca++ and regulation of their rhythmic activity: regulatory proteins Epac1 (p = 0.021), Epac2 (p = 0.018), CaM (p = 0.00001) and also RyR2 (p = 0.031), IP3R2 (p = 0.006) receptors. Conclusion.
