    The CLARIN Knowledge Centre for Atypical Communication Expertise

    In this chapter we introduce the CLARIN Knowledge Centre for Atypical Communication Expertise. The mission of ACE is to support researchers engaged in languages which pose particular challenges for analysis; for this, we use the umbrella term “atypical communication”. This includes language use by second-language learners, people with language disorders or those suffering from lan-guage disabilities, and languages that pose unique challenges for analysis, such as sign languages and languages spoken in a multilingual context. The chapter presents details about the collaborations and outreach of the centre, the services offered, and a number of showcases for its activities

    Shadowing reduced speech and alignment

    This study examined whether listeners align to reduced speech. Participants were asked to shadow sentences from a casual speech corpus containing canonical and reduced targets. Participants’ productions showed alignment: durations of canonical targets were longer than durations of reduced targets; and participants often imitated the segment types (canonical versus reduced) in both targets. The effect sizes were similar to previous work on alignment. In addition, shadowed productions were overall longer in duration than the original stimuli and this effect was larger for reduced than canonical targets. A possible explanation for this finding is that listeners reconstruct canonical forms from reduced forms.peer-reviewe

    Selective digestive and oropharyngeal decontamination in medical and surgical ICU patients:individual patient data meta-analysis

    Objectives: Selective digestive decontamination (SDD) and selective oropharyngeal decontamination (SOD) improved intensive care unit (ICU), hospital and 28-day survival in ICUs with low levels of antibiotic resistance. Yet it is unclear whether the effect differs between medical and surgical ICU patients.& para;& para;Methods: In an individual patient data meta-analysis, we systematically searched PubMed and included all randomized controlled studies published since 2000. We performed a two-stage meta-analysis with separate logistic regression models per study and per outcome (hospital survival and ICU survival) and subsequent pooling of main and interaction effects.& para;& para;Results: Six studies, all performed in countries with low levels of antibiotic resistance, yielded 16 528 hospital admissions and 17 884 ICU admissions for complete case analysis. Compared to standard care or placebo, the pooled adjusted odds ratios for hospital mortality was 0.82 (95% confidence interval (CI) 0.72-0.93) for SDD and 0.84 (95% CI 0.73-0.97) for SOD. Compared to SOD, the adjusted odds ratio for hospital mortality was 0.90 (95% CI 0.82-0.97) for SDD. The effects on hospital mortality were not modified by type of ICU admission (p values for interaction terms were 0.66 for SDD and control, 0.87 for SOD and control and 0.47 for SDD and SOD). Similar results were found for ICU mortality.& para;& para;Conclusions: In ICUs with low levels of antibiotic resistance, the effectiveness of SDD and SOD was not modified by type of ICU admission. SDD and SOD improved hospital and ICU survival compared to standard care in both patient populations, with SDD being more effective than SOD. (C) 2017 The Author(s).     This article explores New York City\u27s non-traditional, judicially based response to prostitution. This article first recounts the history of New York City’s Women’s Court. It then examines the work of the Midtown Community Court, the “problem-solving court” established in 1993 to address criminal issues, like prostitution, in Midtown Manhattan. It also discusses the renewed concerns about sex work in New York and describe the movement, propelled by modern reformers, to address prostitution through specialty courts. It then contrasts the shared features and attributes of the Women’s Court and Midtown Court models. Finally, the article urges modern reformers to step back from the problem-solving court movement and their call for the creation of more such specialized criminal courts

    Selective decontamination of the digestive tract (SDD) in critically ill patients: a narrative review

    Selective decontamination of the digestive tract (SDD) is an infection prevention measure for intensive care unit (ICU) patients that was proposed more than 30 years ago, and that is currently considered standard of care in the Netherlands, but only used sporadically in ICUs in other countries. In this narrative review, we first describe the rationale of the individual components of SDD and then review the evidence base for patient-centered outcomes, where we distinguish ICUs with low prevalence of antibiotic resistance from ICUs with moderate-high prevalence of resistance. In settings with low prevalence of antibiotic resistance, SDD has been associated with improved patient outcome in three cluster-randomized studies. These benefits were not confirmed in a large international cluster-randomized study in settings with moderate-to-high prevalence of antibiotic resistance. There is no evidence that SDD increases antibiotic resistance. We end with future directions for research
