    Myxedema coma accompanied by sick sinus syndrome and hypoventilation: A case report

    Myxedema coma is an emergency presentation of hypothyroidism, and cardiopulmonary manifestations of the disease are related to a high mortality rate. We herein report a case of myxedema coma accompanied by sick sinus syndrome requiring temporary cardiac pacing and hypercapnic respiratory failure in an 87-year-old woman. This case is unique because both of the cardiac and pulmonary manifestations were reversible and successfully treated with thyroid hormone replacement. Our case indicates that early detection of the condition and administration of levothyroxine are essential to improve the life-threatening condition and avoid invasive treatment such as permanent pacemaker implantation or intubation

    Microsomal prostaglandin E synthase-1 in both cancer cells and hosts contributes to tumour growth, invasion and metastasis

    mPGES-1 (microsomal prostaglandin E synthase-1) is a stimulus-inducible enzyme that functions downstream of COX (cyclo-oxygenase)-2 in the PGE2 (prostaglandin E2)-biosynthesis pathway. Although COX-2-derived PGE2 is known to play a role in the development of various tumours, the involvement of mPGES-1 in carcinogenesis has not yet been fully understood. In the present study, we used LLC (Lewis lung carcinoma) cells with mPGES-1 knockdown or overexpression, as well as mPGES-1-deficient mice to examine the roles of cancer cell- and host-associated mPGES-1 in the processes of tumorigenesis in vitro and in vivo. We found that siRNA (small interfering RNA) silencing of mPGES-1 in LLC cells decreased PGE2 synthesis markedly, accompanied by reduced cell proliferation, attenuated Matrigelā„¢ invasiveness and increased extracellular matrix adhesion. Conversely, mPGES-1-overexpressing LLC cells showed increased proliferating and invasive capacities. When implanted subcutaneously into wild-type mice, mPGES-1-silenced cells formed smaller xenograft tumours than did control cells. Furthermore, LLC tumours grafted subcutaneously into mPGES-1-knockout mice grew more slowly than did those grafted into littermate wild-type mice, with concomitant decreases in the density of microvascular networks, the expression of pro-angiogenic vascular endothelial growth factor, and the activity of matrix metalloproteinase-2. Lung metastasis of intravenously injected LLC cells was also significantly less obvious in mPGES-1-null mice than in wild-type mice. Thus our present approaches provide unequivocal evidence for critical roles of the mPGES-1-dependent PGE2 biosynthetic pathway in both cancer cells and host microenvironments in tumour growth and metastasis

    Visualization of aquaionic splitting via iron corrosion

    We report a water decomposition mode called ā€˜Aquaionic Splitting (AiS)ā€™ by means of iron corrosion in aqueous solution. In this paper, we investigated the phenomenon by controlling the reaction between iron and water. A pseudo-sacrificial protection method with oil paint was employed to select the anode and cathode formation locations that govern iron corrosion. Then, the AiS reaction was visualized by using BTB solution, whose colour corresponds to pH, to produce colour patterning that corresponds to the aquaion distribution. It has become clear that water can be selectively separated into protons and hydroxide ions by corrosion control treatment. In this vein, the diffusion coefficient of protons was estimated by using the colour patterning of BTB solution that accompanies iron corrosion, and aquaion distribution was then computer simulated by solving the diffusion equation

    Abstract Abnormal peripheral perfusion (PP) worsens the prognosis of patients with septic shock. Polymyxin B-direct hemoperfusion (PMX-DHP) increases blood pressure and reduces vasopressor doses. However, the modification of PP following administration of PMX-DHP in patients with vasopressor-dependent septic shock have not yet been elucidated. A retrospective exploratory observational study was conducted in patients with septic shock treated with PMX-DHP. Pulse-amplitude index (PAI), vasoactive inotropic score (VIS), and cumulative fluid balance data were extracted at PMX-DHP initiation (T0) and after 24 (T24) and 48 (T48) h. Changes in these data were analyzed in all patients and two subgroups (abnormal PP [PAIā€‰<ā€‰1] and normal PP [PAIā€‰ā‰„ā€‰1]) based on the PAI at PMX-DHP initiation. Overall, 122 patients (abnormal PP group, nā€‰=ā€‰67; normal PP group, nā€‰=ā€‰55) were evaluated. Overall and in the abnormal PP group, PAI increased significantly at T24 and T48 compared with that at T0, with a significant decrease in VIS. Cumulative 24-h fluid balance after PMX-DHP initiation was significantly higher in the abnormal PP group. PMX-DHP may be an effective intervention to improve PP in patients with abnormal PP; however, caution should be exercised as fluid requirements may differ from that of patients with normal PP

    Currently, an agricultural method called SynecocultureTM has been receiving attention as a means for multiple crop production and recovering from environmental degradation; it helps in regreening the environment and establishing an augmented ecosystem with high biodiversity. In this method, several types of plants are grown densely, and their management relies mainly on manual labor, since conventional agricultural machines and robots cannot be applied in complex vegetation. To improve work efficiency and boost regreening by scaling-up Synecoculture, we developed a robot that can sow, prune, and harvest in dense and diverse vegetation that grows under solar panels, towards the achievement of compatibility between food and energy production on a large scale. We adopted a four-wheel mechanism with sufficient ability to move on uneven terrain, and a two orthogonal axes mechanism with adjusted tool positioning while performing management tasks. In the field experiment, the robot could move straight on shelving slopes and overcome obstacles, such as small steps and weeds, and succeeded in harvesting and weeding with human operation, using the tool maneuver mechanism based on the recognition of the field situation through camera image
