    Histological changes in the liver of diabetic rats: A review of pathogenesis of nonalcoholic fatty liver disease in type 1 diabetes mellitus

    Abstract: Abundant knowledge is present over nonalcoholic fatty liver disease (NAFLD) and type 2 diabetes mellitus but no attention has been given to the same aspect with type 1 diabetes mellitus (T1DM) in humans. However, various animal modeled studies are documented having induced T1DM using chemical induction by alloxan (ALX) or streptozotocin (SZ) to investigate and comprehend the nature and pathophysiology of NAFLD in T1DM. The objectives of this review were to assess whether T1DM induced by ALX or SZ could cause histological (morphological and ultrastructural) changes in rat liver to illustrate the effects of these drugs and diabetes, and pathophysiology of NAFLD in T1DM. To achieve these objectives, we systematically searched the studies from EconLit, Embase, Google Scholar, Medline, PubMed, ProQuest, Scopus, Springer Link, and Science Direct databases. Initially, 150 studies were retrieved. Out of these, 107 studies were removed as these failed to provide relevant details. Finally, 43 articles were selected for this review. This review concluded that diabetes induced by ALX or SZ, cause biochemical alterations in blood and pathophysiological variations in the liver of rats. These changes can vary from steatosis to steatohepatitis and liver fibrosis, and are similar to the modifications observed in human liver

    Is Trade Openness the Reason of High Energy Demand in China?

    The present study aims to examine the short-run and long-run impact of China's trade liberalization policies on its energy demand over the period from 1980 to 2018. The results of Autoregressive Distributed Lag approach of co-integration show that energy consumption significantly increases as a result of trade openness and increase in real Gross Domestic Product (GDP). The results of the granger causality test also confirm the unidirectional causality running from trade openness and real GDP to energy demand. The results of the study have an important implication because if China wants to continue its trade liberalization policies then it must increase its energy production.     Application of Geospatial Techniques in Agricultural Resource Management

    Although technological advancements have sparked the beginning of the fourth agricultural revolution, human beings are still facing severe problems such as shrinking croplands, dwindling water supplies, negative consequences of climate change, and so on in achieving agricultural resilience to meet the demands of the growing population over the globe. Geospatial techniques involving the integrated use of geographic information system (GIS), remote sensing (RS), and artificial intelligence (AI) provide a strong basis for sustainable management of agricultural resources aimed at increased agricultural production. In recent times, these advanced tools have been increasingly used in agricultural production at local, regional, and global levels. This chapter focuses on the widespread application of geospatial techniques for agricultural resource management by monitoring crop growth and yield forecasting, crop disease and pest infestation, land use and land cover mapping, flood monitoring, and water resource management. Moreover, we also discuss various methodologies involved in monitoring and mapping abovementioned agricultural resources. This chapter will provide deep insight into the available literature on the use of geospatial techniques in the monitoring and management of agricultural resources. Moreover, it will be helpful for scientists to develop integrated methodologies focused on exploring satellite data for sustainable management of agricultural resources

    Sustainable Irrigation Management for Higher Yield

    Sustainable irrigation is sensible application of watering to plants in agriculture, landscapes that aids in meeting current survival and welfare needs. Sustainable irrigation management can help with climate change adaptation, labor, energy savings, and the production of higher-value and yield of crops to achieve zero hunger in water-scarce world. To ensure equal access to water and environmental sustainability, investments in expanded and enhanced irrigation must be matched by improvements in water governance. Sustainable irrigation must be able to cope with water scarcity, and be resilient to other resource scarcities throughout time in context of energy and finance. The themes and SDGs related to clean water, water resources sustainability, sustainable water usage, agricultural and rural development are all intertwined in the concept of “sustainable irrigation for higher yield.” Sustainable irrigation management refers to the capability of using water in optimum quantity and quality on a local, regional, national, and global scale to meet the needs of humans and agro-ecosystems at present and in the future to sustain life, protect humans and biodiversity from natural and human-caused disasters which threaten life to exist.     Cancer prevalence, incidence and mortality rates in Pakistan in 2012

    According to estimates from the International Agency for Research on Cancer (IARC), there were 14.1 million new cancer cases and 8.2 million cancer deaths in 2012 worldwide. In this article, we provide an overview of the cancer burden in Pakistan, including the 1, 3, 5-years prevalence, estimated number of new cancer cases and deaths in 2012 by age. Data from GLOBOCAN 2012, produced by the IARC, were used. GLOBOCAN provides estimates of cancer prevalence, incidence and mortality worldwide, and for countries and regions. This article concluded that the most prevalent cancers in Pakistan include breast, lip and oral cavity, cervix uteri, colorectum and bladder, respectively. The cancers responsible for the highest incidence in both the genders (total = 148,041) in Pakistani population includes breast (n = 34038, 23%), lip and oral cavity (n = 12761, 8.6%), lung (n = 6800, 4.6%), non-hodgkin lymphoma (n = 5964, 4%) and colorectum (n = 5335, 3.6%), respectively. Whereas, the cancers responsible for the highest deaths (total = 101,113) in Pakistani population includes breast (n = 16232, 16.1%), lip and oral cavity (n = 7266, 7.2%), lung (n = 6013, 5.9%), oesophagus (n = 4748, 4.7%) and non-hodgkin lymphoma (n = 4374, 4.3%), respectively

    Influence of Education Level of Older Patients on Polypharmacy, Potentially Inappropriate Medications Listed in Beer’s Criteria, and Unplanned Hospitalization: A Cross-Sectional Study in Lahore, Pakistan

    Objective: To evaluate influence of education level of older patients on polypharmacy, potentially inappropriate medications (PIMs) listed in Beer’s Criteria, and unplanned hospitalization. Methods: A cross-sectional study was conducted among older people aged ≥65 years between 1 December 2017 and 28 February 2018. For data analysis, descriptive statistics and logistic regression analysis were employed. Results: Among 385 older patients, 88.8% were prescribed PIMs and 56.4% underwent PIMs associated unplanned hospitalization. Older people were less exposed to polypharmacy or excessive polypharmacy as their education levels increased (no formal education vs. primary vs. secondary vs. tertiary, 74% vs. 69.8% vs. 60.5% vs. 58.1%). Patients having higher education were also accompanied by significantly lower prescription of PIMs (no formal education vs. primary vs. secondary vs. tertiary, 96% vs. 87.3% vs. 84.5% vs. 79.1%) as well as unplanned hospitalization (no formal education vs. primary vs. secondary vs. tertiary, 64.7% vs. 76.2% vs. 40.3% vs. 46.5%). Results of regression analysis revealed that no formal education (OR = 1.202, 95% CI = 1.032–2.146, p-value = 0.003) and primary education level (OR = 1.175, 95% CI = 1.014–1.538, p-value = 0.039) were significantly associated with the use of polypharmacy among older people. On the other hand, no formal education was significantly associated with the prescription of PIMs (OR = 1.898, 95% CI = 1.151–2.786, p-value = 0.007). Furthermore, older people with no formal education (OR = 1.402, 95% CI = 1.123–1.994, p-value = 0.010) and primary education level (OR = 1.775, 95% CI = 1.281–3.018, p-value = <0.001) were significantly more likely to undergo unplanned hospitalization. Conclusions: Patients having low literacy level are more likely to receive PIMs, polypharmacy, and undergo unplanned hospitalization in comparison to highly educated patients. Hence, promotion of health literacy for patients is crucial to overcome these problems

    Causality and preventability assessment of adverse drug events of antibiotics among inpatients having different lengths of hospital stay: a multicenter, cross-sectional study in Lahore, Pakistan

    Abstract Background A large number of hospital admissions are attributed to adverse drug reactions (ADRs) and they are the fifth leading cause of death worldwide. The present study aimed to assess the causality and preventability of adverse drug events (ADEs) of antibiotics among inpatients having different lengths of hospital stay. Methods A prospective, observational study was conducted in four tertiary-care public sector hospitals of Lahore, Pakistan. Study population consisted of hospitalized patients who were prescribed one or more antibiotics. Data were collected between 1st January, 2017 and 30th June, 2017 from 1249 patients. Naranjo score, modified Schumock and Thornton scale were used for causality and preventability assessments, respectively. Medication errors (MEs) were assessed by MEs tracking form. SPSS and Microsoft Excel were used for data analysis. Results A total of 2686 antibiotics were prescribed to 1249 patients and 486 ADEs were found. The preventability assessment revealed that most of the ADEs (78.8%) were found among patients having long length of stay (LOS) in hospital and were preventable (59.3% of the ADEs were definitely preventable while 44.7% were probably preventable) and caused by MEs including wrong drug (40.1%) and monitoring errors (25%). The errors were caused due to non-adherence of policies (38.4%) and lack of information about antibiotics (32%). Most of the non-preventable ADEs or ADRs among patients having long and short LOS in hospital were “probable” (35.5%) and “possible” (35.8%), respectively. Logistic regression analysis revealed that ADEs were significantly less among females (OR = 0.047, 95% CI = 0.018─0.121, p-value = < 0.001), patients aged 18─52 years (OR = 0.041, 95% CI = 0.013─0.130, p-value = < 0.001), patients with ARTIs (OR = 0.004, 95% CI = 0.01–0.019, p-value = < 0.001), patients prescribed with 2 antibiotics per prescription (OR = 0.455, 95% CI = 0.319─0.650, p-value = < 0.001) and patients with long LOS (OR = 14.825, 95% CI = 11.198─19.627, p-value = < 0.001). Conclusion Antibiotics associated definitely preventable ADEs were more commonly found in patients having long LOS in the inpatient departments because of MEs and lack of proper pharmacovigilance system. The ADRs showed a probable and possible causal association with both β-lactams and non β-lactams antibiotics
