    Lebanon: Cholera Outbreak

    Lebanon is a country that has always been tangled with the ever looming threat of a growing health crisis. With a failing government and massive influx of refugees, mostly the victim of their war torn economy, it seemed inevitable that sanitation procedures would run short. Now the Lebanese people and government face the challenge of the spread of Cholera, a water-borne disease, within the countries vicinity as a result of mass pollution from the formation of informal settlements as well as the country being a hotbed for political instability. These have resulted in limited success in controlling the outbreak engulfing the millions of refugees and general populace. While external powers such as the WHO have extended a hand in providing medical as well as financial assistance in counter reacting to the growing disease, an absolute solution to the problem has been largely stunted due to several socio-economic factors that have unfolded over the years as well as the rate of garbage disposal. However, as mentioned in my report I give one valid method of controlling the problem from spiralling out of control. This report will help in giving an easy insight into the current Public Health situation and ways in which it is/can be countered

    Gender difference in age at onset of schizophrenia: a cross sectional study from Pakistan

    OBJECTIVE: To validate the effect of gender on age at onset of schizophrenia in a developing country METHODS: Medical records of 252 patients diagnosed with schizophrenia according to DSM-IV criteria at a tertiary care hospital in Karachi, Pakistan between 2002 and 2008 were reviewed using a structured questionnaire. Age at onset was defined as age at onset of psychotic symptoms, age at first contact with a health care provider and age at first hospitalization. Socio-demographic variables were reported using descriptive statistics and all measures of age at onset were compared across gender using t-test. RESULTS: There were 119 women and 133 men with mean age of 37.6 +/- 12.8 years. All three measures of age at onset of illness showed no difference between women and men. The mean age when first psychotic symptoms appeared in men was 24.86 +/- 8.83 years (n = 128) while that in women was 26.57 +/- 9.96 years (n = 111), p = 0.160. The mean age at index hospitalization for treatment of psychosis in men was 29.50 +/- 10.64 years (n = 123) and in women was 31.61 +/- 12.07 years (n = 103), p = 0.164. The mean age at first contact with any caregiver in men was 29.73 +/- 37.58 years (n = 119) and in women was 29.38 +/- 11.99 years (n = 108), p = 0.926. CONCLUSION: There is no significant difference in age at onset of schizophrenia across gender in our population. This validates a difference in epidemiology of schizophrenia in the South Asian population

    The present study examined the effects of psychological contract breach on attitudinal and behavioral job outcomes including job satisfaction, intention to remain with one’s organization, perceived organizational support, and organizational citizenship behaviors. This study is based on banks and organizations of d g khan business hub Additionally, the present study also examined whether the effect of the psychological contract breach on these job outcomes varied based on the type of psychological contract an employee had (i.e., transactional or relational). A total of 30 full-time employees participated in this study. Results showed that the breach of one’s psychological contract had a significant effect on one’s job satisfaction, intention to remain, and perceived organizational support. Additionally, there was a significantinteraction between psychological contract breach and psychological contract type such that when employees experienced a breach of their psychological contract, the effect of the psychological contract breach on perceived organizational support and organizational citizenship behaviors varied depending on psychological contract type

Data generated through the quantile Functions and then different limits for the full and missing data set have been developed with scale parameter. A number of such mean control limits could be constructed through purposed method but for analysis purpose few of them have discussed. The missing data limits broadened than the full data in each case, which was expected to be. The average run length (ARL) was also calculated for different sample sizes (50,100,150). The general decreasing behavior of ARL according to increasing shifts was observed that shows a worthy sign, for two distributions, as the probability of detecting an out of control signal increased due to decrease in ARL

    Physician migration at its roots: a study on the factors contributing towards a career choice abroad among students at a medical school in Pakistan

    BACKGROUND: Physician migration, also known as “brain drain,” results from a combination of a gap in the supply and demand in developed countries and a lack of job satisfaction in developing countries. Many push and pull factors are responsible for this effect, with media and internet playing their parts. Large-scale physician migration can pose problems for both the donor and the recipient countries, with a resulting reinforcement in the economic divide between developed and developing countries. The main objectives of our study were to determine the prevalence of migration intentions in medical undergraduates, to elucidate the factors responsible and to analyze the attitudes and practices related to these intentions. METHODS: This was a cross-sectional, observational, questionnaire-based study, conducted at Dow Medical College of Dow University of Health Sciences, Karachi, between January, 2012 and May, 2012. A total of 323 students responded completely. The questionnaire consisted of 3 sections, and was aimed at collecting demographic details, determining students’ migratory intentions, evaluating reasons for and against migration and assessing attitudes and practices of students related to these intentions. RESULTS: Out of 323 respondents, 195 wanted to pursue their careers abroad, giving a prevalence rate of 60.4% in our sample. United States was the most frequently reported recipient country. The most common reasons given by students who wished to migrate, in descending order, were: lucrative salary abroad followed by quality of training, job satisfaction, better way of life, relatives, more opportunities, better working environment, terrorism in Pakistan, harassment of doctors in Pakistan, desire to settle abroad, more competition in Pakistan, better management, peer pressure, longer working hours in Pakistan, religious reasons, parent pressure, political reasons and favoritism in Pakistan. A considerable number of respondents had already started studying for licensing examinations, and were also planning of gaining clinical experience in their desired country of interest. CONCLUSION: Physician migration is a serious condition that requires timely intervention from the concerned authorities. If considerable measures are not taken, serious consequences may follow, which may pose a threat to the healthcare system of the country
