    An exploratory study using the OMI scale to assess the opinion toward mental illness among young adults

    Background: Public perception of mental illness is critical for successful treatment seeking behaviour as well as for early diagnosis and surveillance of disease. Two hundred eleven young patients were interviewed at Department of psychiatry, Sir Sunderlal Hospital, Varanasi with the help of consecutive sampling method. Objective: To assess the opinion towards mental illness among young adults by using the opinion on mental illness scale (OMI). Methods: Data was collected using the opinion about mental illness scale (OMI) which consists of 51 questions out of which, 15 questions directly related to the cause, care, and management of mental illness were randomly selected and analyzed. Analysis was done showing Percentage and chi-square tests to see the significance of scores between the respondents. Results: The finding shows that more than 70% of respondents believe that mental illness is a consequence of bad behavior, lack of willpower, or negative beliefs and gap in the treatment seeking behavior and understanding the cause of disease was found. Conclusions: Opinions on Mental health have received increasing attention as a useful strategy to promote early identification of mental illness, reduce stigma and enhance help-seeking behaviors. As part of the National Mental Health Programme, there is a need to raise awareness in order to enhance understanding of mental illness in facilitating early diagnosis and treatment of disease

    Choice of sedative for deep brain stimulation in Parkinson’s disease: Our experience and comparison of two cases

    AbstractIntroductionParkinson’s disease (PD) is a severe, debilitating disease of the extra pyramidal central nervous system, which has a significant effect on lifestyle and day to day living of the affected population. Statistically, more of the elderly are now going to present with this disease. Moving ahead from older procedures such as cingulotomy, pallidotomy and thalamotomy which had irreversible side effects, deep brain stimulation (DBS) has emerged as a new, safer and more attractive option for such patients. Anaesthetic concerns for such procedures mainly incorporate principles of awake craniotomy, for which the basic requirement is a cooperative patient. Although Propofol was somewhat of a gold standard for this purpose until a few years back, Dexmedetomidine has emerged as the new drug of choice.CaseWhile conducting two surgeries for DBS over two days, we had an obverse experience with these drugs. We describe the pre-operative assessment and intra-operative management of the two cases and a discussion of the factors which might have contributed to this contradiction.ConclusionThe choice of sedation for DBS in PD should take into consideration factors such as patient cooperation, ‘drug off’ state due to pre-op medication stoppage, GABA versus non-GABA mediated mechanism of drugs, amount of dependence on PD drugs, severity of disease and finally requirement of the testing team. No drug can be singled out to be better and must be chosen based on individual merits of the patient and disease

    XHX - A Framework for Optimally Secure Tweakable Block Ciphers from Classical Block Ciphers and Universal Hashing

    Tweakable block ciphers are important primitives for designing cryptographic schemes with high security. In the absence of a standardized tweakable block cipher, constructions built from classical block ciphers remain an interesting research topic in both theory and practice. Motivated by Mennink\u27s F[2] publication from 2015, Wang et al. proposed 32 optimally secure constructions at ASIACRYPT\u2716, all of which employ two calls to a classical block cipher each. Yet, those constructions were still limited to n-bit keys and n-bit tweaks. Thus, applications with more general key or tweak lengths still lack support. This work proposes the XHX family of tweakable block ciphers from a classical block cipher and a family of universal hash functions, which generalizes the constructions by Wang et al. First, we detail the generic XHX construction with three independently keyed calls to the hash function. Second, we show that we can derive the hash keys in efficient manner from the block cipher, where we generalize the constructions by Wang et al.; finally, we propose efficient instantiations for the used hash functions

    Career sustainability of digital micro-entrepreneurs: strategic insights from YouTubers in India

    Drawing on a framework of sustainable career theory, this paper aims to understand the interplay of agentic and contextual factors for digital micro-entrepreneurs to lead sustainable careers. Eighteen YouTube content creators in India participated in semi-structured interviews, offering coverage of digital content creators across acting, cosmetics, finance, fitness, food, law, modelling, music, teaching, travel, and video games. The findings showed three agentic and three contextual themes associated with the career sustainability of a digital micro-entrepreneur. Additionally, four paradoxes were identified, capturing the interplay between the agentic and contextual themes. The career of a digital micro-entrepreneur is a dichotomy of promising hope, stardom, and flexibility while concealing challenges like precarity, hate comments, and financial instability. Knowing this can help individuals make better-informed career decisions. The study advances sustainable career theory by capturing insights from digital micro-entrepreneurs in India to understand the interplay of agentic and contextual factors that create a series of paradoxes for such individuals to navigate over time

    Abstract: Syntheses of a small library of hydroxamates by reacting esters of different carboxylic acids with hydroxyl amine hydrochloride has been achieved. All the synthesized compounds were screened in vitro against Trypanosoma brucei brucei S427 and Plasmodium falciparum 3D7 strain. The IC 50 values of potent anti-trypanosomal agents ranged between 0.025-5.69 Îźg/mL, while these values varied in the range of 0.78 -40.89 Îźg/mL against P. falciparum. Five compounds with very good SI values were active against both the protozoans. Some compounds have also shown strong antifungal activities with MIC as low as 0.19 Îźg/mL. These simple low molecular weight hydroxamates with triple activities and high degree of SI values revealed a new strategy in malaria/HAT and fungal chemotherapy

    Abstract Background The current study reports dissemination of highly stable bla OXA-10 family of beta lactamases among diverse group of nosocomial isolates of Gram-negative bacilli within a tertiary referral hospital of the northern part of India. Methods In the current study, a total number of 590 Gram negative isolates were selected for a period of 1 year (i.e. 1st November 2011–31st October 2012). Members of Enterobacteriaceae and non fermenting Gram negative rods were obtained from Silchar Medical College and Hospital, Silchar, India. Screening and molecular characterization of β-lactamase genes was done. Integrase gene PCR was performed for detection and characterization of integrons and cassette PCR was performed for study of the variable regions of integron gene cassettes carrying bla OXA-10. Gene transferability, stability and replicon typing was also carried out. Isolates were typed by ERIC as well as REP PCR. Results Twenty-four isolates of Gram-negative bacilli that were harboring bla OXA-10 family (OXA-14, and OXA16) with fact that resistance was to the extended cephalosporins. The resistance determinant was located within class I integron in five diverse genetic contexts and horizontally transferable in Enterobacteriaceae, was carried through IncY type plasmid. MIC values were above break point for all the tested cephalosporins. Furthermore, co-carriage of bla CMY-2 was also observed. Conclusion Multiple genetic environment of bla OXA-10 in this geographical region must be investigated to prevent dissemination of these gene cassettes within bacterial population within hospital settings
