    Is there an association between date of birth and date of death? A statistical investigation on exit and transcendence into the eternal world

    The idea of this study came out long time ago, in relation to some personal unfortunate events. I have extended my research on 10000 funerary inscriptions and documents from civil and military tombs to find out if the relation between the date of birth and the date of death Ā is significantly correlatedĀ . Our findings point outĀ that inĀ about 75% cases the date of death is up to 2 months before or after the date of birth.Ā This suggests a genetic determinism of death moment

    Viral infections can be a cause of CNS infection, causing mental and neurological symptoms. Even under normal conditions, good mental health is of paramount importance to the functioning of society. Healthcare workers are critical to the COVID-19 pandemic and beyond, but may have to leave work if their mental health is affected.WeĀ  present the favorable evolution of a patient, a 55 year-old woman, medical staff with a history of viral infection with SARS CoV-2 (August 2020), which is brought by a crew of the Ambulance service in the emergency department (ED) service (December 2020) for multi-drug ingestion for suicide. After treatment and investigations she is transferred to the ā€œElena Doamnaā€ Psychiatric Hospital. After 4 days of hospitalization, he returns to the emergency department by transfer from the Psychiatric Hospital for underlying seizures, confusion syndrome. During her admission to the neurology service of the ā€œSf Ap. Andreiā€ Emergency Clinical Hospital, the patient benefited from numerous clinical and paraclinical investigations, which provided information about the patient's neuropsychiatric evolution.</p

    Relationship between Feelings and Risk of Burnout Syndrome to the Medical Staff in the Emergency Department of Galați Hospital During the Covid 19 Pandemic Period

    Introduction: Burnout syndrome is a manifestation of chronic stress caused by occupational stressors. The large number of patients infected with SARS Covid 2 has had a particular psychological impact on medical staff in Emergency Departments and only.Material and Methods: This was a descriptive study, of medical staff in the Emergency Department of the Galati County Emergency Hospital, which is in the front line in the fight against coronavirus. All 120 participants signed an informed consent form and then completed a socio-demographic questionnaire, self-designed questionnaire to identify feelings experienced at work during Covid- 19 Pandemic and the MBI - HSS (MP) (Maslach Burnout Inventory) to assess their burnout level.Results: The study found that 45% of the subjects were at high risk of Burnout. Medical staff in the Emergency Department show a degree of Burnout syndrome directly proportional to their experience in the field.Conclusions: The prevalence of Burnout syndrome in the Emergency Department is high, more than half of the physicians suffer from a high level of Burnout. Older age and less experience in the field and female gender are predictive factors for Burnout syndrome. Self-reported risk factors are increased overtime, workload, bureaucracy, lack of sleep, lack of respect from patients and colleagues, fear of exposure to the virus and patient care. Self-reported feelings of exhaustion, stress, and depression are dependent on symptoms of Burnout syndromeĀ </p

    Emotional Exhaustion and Professional Satisfaction During Covid-19 Pandemic at the Level of Emergency Department Staff

    Introduction: Burnout is a syndrome conceptualized as result of the chronic stress at the work place which was not successfully managed. The possible negative effects on the medical staff, the patients and the health institutions have generated interest and motivated the research to understand the possible causes, these effects also influence the burnout predictors. The medical services suppliers within the emergency department have had one of the greatest exhaution occurences even before the pandemic. Being in the first line in contact with the suspected patients or confirmed with COVID-19 infection intensifies this fact. Both the working factors (working hours, years of practice, number of treated patients, professional development activities) and the socio- demographic factors (age, sex, civil status, income, education) are associated with the exhaustion at the work place.Material and methods: I have studied a total of 120 participants, receiving 115 valid questionnaires, the answering ratio being 95,83%. This was a descriptive, multicentre, transversal study of the medical and the auxiliary staff within the department UPU-SMURD of the County Clinic Emergency Hospital ā€œSf. Ap. Andreiā€ Galați. The staff gave their consent and attended a combined interview which consisted of a pilot questionnaire and a demographic questionnaire.Ā  Results and discussions: The average score of exhaustion at the staff level was 3.25, a great percentage of the doctors within the ED (45.8%), (n=24) indicates an increased level of exhaustion, only 26.8% (n=80) of nurses show an increased level of exhaustion. The average score of the stress level shown by the staff is 3.1, 66.6% (n=24) among which the doctors with an increased stress level 9.1% (n=9) auxiliary staff, 23.8% (n=80) of nurses. Conclusions: The results show us a high exhaustion, stress and depression ratio at the emergency doctors in comparison with the other professional categories within the ED. A high level of professional satisfaction is accompanied by an increased level of stress and of self-esteem.</p

    Use of the WBSI Questionnaire in a Study Group of Patients with Polytrauma During the Period 2015-2021

    The present study started from empathy towards the patient and the fact that medicine is always looking for optimal solutions for the patient's well-being. The state of health, however, is not limited to the physical body, but the psychological mental status, the environment, the recovery time must be taken into account. The medical act is not limited to the life-saving gesture applied at a given moment.Ā  Polytrauma requires prompt and safe medical gestures, but the patient does not recover from this tragic episode in the near future. Time, patience, attention, coping mechanisms, psychological wellbeing, and a conducive environment are needed to compete with the previous medical act in order to recover fully and as quickly as possible. The length of hospitalization means financial costs, but the full recovery of the patient, the mental well-being, translates into hidden costs, borne by both the patient and the health system.Based on these ideas, the present study tried to add value to health care. Considering polytrauma as a possible trigger for PTSD, considering recovery as closely related to coping mechanisms, the study aimed to find the linking element in this "polytrauma-recovery" pathway. Why is recovery easier in some cases? Why does the number of hospital days vary in the context of similar physical injuries? Why do cases of higher severity recover more easily than others with lower severity scores?This study set out to explain how trauma and its psychological implications (rumination, nightmares, anxiety, obsessive thoughts about the injury) may be blamed for a more difficult recovery.</p

    The Patient Hides the Truth, but the Computer Tomography Examination Restores It!

    In this article, we present the case of a patient who hid the real reason for which he came to the emergency room. This patient in this case is a 61-year-old man, who presented to the emergency department for symptoms relevant to respiratory disease. However, the initial diagnosis turned out to be only the &ldquo;tip of the iceberg&rdquo; in the evolution and treatment of this patient. Fournier gangrene is a fulminant form of infectious necrotizing fasciitis of the perineal, genital, or perianal regions, which frequently affects men, but can also occur in women and children. It is a rare but life-threatening disease. Due to potential complications, it is important to diagnose Fournier gangrene as early as possible. Although antibiotics and aggressive debridement have been widely accepted as standard treatment, the mortality rate remains high. In many cases, the doctor&rsquo;s mission is much more difficult than it first seems. The challenges to which the doctor is subjected, especially in the emergency room, are immense. In establishing a diagnosis and emergency treatment, all parties involved&mdash;medical staff, on the one hand, and patients and relatives, on the other hand&mdash;must collaborate so that the outcome is a positive one. The overall conclusion of the medical team was that Fournier gangrene in the patient was the consequence of both the presence of an intrarectal foreign body, as well as excessive alcohol consumption. The case of our patient with Fournier gangrene and an intrarectal foreign body had a positive outcome due to quick, aggressive, and adequate medical and surgical management, as well as the fact that the patient had no other associated pathologies beyond alcohol &ldquo;passion.&rdquo; Our patient&rsquo;s case is an example of the exceptional situations we sometimes encounter in medical practice

    The role of determining the nucleolar organizers in assessing the evolution of neoplasms

    Background Nucleolar organizer regions are parts of chromosomes that code ribosomal RNA and correlate to cellular proliferation. They have been identified by means of an argyrophilic technique that reveals NORs as black dots in the nuclei. The method has been recently applied in some malignancies, as lymphoma or melanoma. Only few investigations onĀ  hormonal-dependent tumours have been published. Our aim was to investigate on the total number of AgNORs and cell type of endometrial tumors. Material and Methods We studied 21 cases of formalin fixed paraffin embedded endometrial tumors including hyperplasia. The number of AgNORs was counted in both epithelial and stromal components. A total of 13 cases were benign and 8 malignant lesions.Ā  Results The total number of AgNORs was found to correlate with mitotic counts both in benign lesions and malignancies. The mean number of AgNORs increased from 2.79 in simple endometrial hyperplasia to 10.0 in adenocarcinoma. Conclusions Our data indicate that the AgNORs visualization can assist to differentiate between different malignancy grades of endometrial proliferations, especially between complex endometrial hyperplasia and atypical hyperplasia as well as endometrial adenocarcinomas

    Speaking about pregnancy we can say that include physiologic changes mediated by female sex hormones with important effects even on healthy women. Physiologic changes during pregnancy can exacerbate already existing oral pathologic conditions, such as gingivitis, periodontitis, salivary and caries lesions. Aim This paper aims to illustrate recent informations about physiologic changes during normal gestation and also the management of dental treatment in women with normal pregnancy and in women at risk for adverse pregnancy outcomes. Materials and methods We addreddes some queries to the electronic databases including clinical trials in PubMed and Thomson ISI ā€“ Web Of Science, published in English in the last five years using key words: ā€žhematological, respiratory, gastrointestinal, cardiovascular changes, pregnancy, dental caries, gingivitis, dental managementā€. From the collected materials, we have conceived a summary of the data about the physiologic changes during pregnancy wich can exacerbate already existing oral pathologic conditions, such as gingivitis, periodontitis, salivary and caries lesions. Results Oral infections during pregnancy may be associated with adverse pregnancy outcomes. Prenatal care is an essential part of a successful pregnancy and oral health assessment must be part of prenatal care. Changes produced by pregnancy present a number of unique management problems in dental treatment. Conclusion The best approach to avoid pregnancy complications and adverse pregnancy outcomes is to apply preventive strategies. One of the important direction in managing dental care for the pregnant woman is to promote a healthy oral environment free of inflammation and infection. All the changes in gingival tissues during pregnancy are associated with an exacerbated clinical inflammatory response. Immune adaptations that occur during pregnancy may further facilitate infections of oral tissues

    The Impact of Burnout Syndrome on Job Satisfaction among Emergency Department Nurses of Emergency Clinical County Hospital &ldquo;Sf&acirc;ntul Apostol Andrei&rdquo; of Galati, Romania

    Background and Objectives: Burnout syndrome is caused by a number of factors, including personal, organizational, and professional problems. Data from the literature reported a strong relationship between burnout syndrome and job satisfaction among emergency medical personnel. Materials and Methods: We studied a sample of 80 nurses working in the Emergency Department of Emergency Clinical County Hospital &ldquo;Sf&acirc;ntul Apostol Andrei&rdquo; of Galati Romania. Participants signed an informed consent and then completed a socio-demographic questionnaire and the MBI-HSS (Maslach Burnout Inventory&ndash;Human Services Survey) questionnaire to assess the level of burnout and JSS (Job Satisfaction Survey) to assess the level of professional satisfaction. The aim of this study was to measure the levels of burnout and satisfaction among nurses in the Emergency Department as well as the relationship between these two variables and a group of selected socio-demographic characteristics. Results: This study found that 36.25% of nurses reported a high level of burnout. Additionally, emotional exhaustion is directly proportional to professional experience and age. Participants also reported a sense of ambivalence and satisfaction with the workplace, but were satisfied with the nature of their work. Conclusions: The results of this study suggest the relationship between variables and this could be used to implement psychoactive intervention strategies at both individual and organizational levels, which could lead to a decrease in burnout levels. Burnout is a predictor of job satisfaction for Emergency Department nurses. Personal achievement was relatively commensurate with the nature of the work. Additionally, the increase in burnout among nurses is directly proportional to the nature of the work
