Anticoagulant selection in relation to the SAMe-TT2R2 score in patients with atrial fibrillation. the GLORIA-AF registry

Aim: The SAMe-TT2R2 score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2 scores >2 than to patients with lower scores. Methods and results: We analyzed the Phase III dataset of the Global Registry on Long-Term Oral Antithrombotic Treatment in Patients with Atrial Fibrillation (GLORIA-AF), a large, global, prospective global registry of patients with newly diagnosed AF and ≥1 stroke risk factor. We compared baseline clinical characteristics and antithrombotic prescriptions to determine the probability of the VKA prescription among anticoagulated patients with the baseline SAMe-TT2R2 score >2 and ≤ 2. Among 17,465 anticoagulated patients with AF, 4,828 (27.6%) patients were prescribed VKA and 12,637 (72.4%) patients an NOAC: 11,884 (68.0%) patients had SAMe-TT2R2 scores 0-2 and 5,581 (32.0%) patients had scores >2. The proportion of patients prescribed VKA was 28.0% among patients with SAMe-TT2R2 scores >2 and 27.5% in those with scores ≤2. Conclusions: The lack of a clear association between the SAMe-TT2R2 score and anticoagulant selection may be attributed to the relative efficacy and safety profiles between NOACs and VKAs as well as to the absence of trial evidence that an SAMe-TT2R2-guided strategy for the selection of the type of anticoagulation in NVAF patients has an impact on clinical outcomes of efficacy and safety. The latter hypothesis is currently being tested in a randomized controlled trial. Seasonal variation of biomass and productivity of the periphytic community on artificial substrata in the Jurumirim Reservoir (São Paulo, Brazil)

The seasonal variation of the periphytic community attached to an artificial substratum (glass tubes) was studied during two different periods in a lagoon connected to the Paranapanema River, the main tributary of the Jurumirim Reservoir (São Paulo-Brazil). An analysis of dry weight, ash free dry weight, chlorophyll a, phaeophytin and primary productivity of periphyton was carried out. The first experiment lasted from August to December 1993, the second from February to June 1994. Tubes were removed after 7, 14, 21, 28, 60, 90 and 119 days of incubation. In the 1st experiment, the periphytic community reached a higher biomass after the 4th week of colonization (28th day), in the 2nd experiment after the 2nd week (14th day). This discrepancy is related to seasonal differences in environmental factors (water temperature, nutrients concentrations and water discharge) that determine initial colonization. After the first stages of colonization, the biomass and primary productivity of periphyton reached their maximum values after the 60th day of incubation. In both experiments, three developmental phases could be discerned. In the initial phase, an exponential growth was observed. In the second phase, the bioderm reached its maximum biomass and productivity. In the third phase, a decrease of biomass and productivity occurred

Chemical composition of tropical macrophyte Echinochloa polystachya (HBK) Hitchcock in Jurumirim Reservoir (Sao paulo, Brazil)

Seasonal variations of C, N and P concentrations and stocks in the emergent macrophyte Echinochloa polystachya (H.B.K.) Hitchcock, of the Paranapanema River mouth zone at Jurumirim Reservoir (SP, Brazil), were examined. Marked variation in nutrient concentrations (from 287.6 to 463.2 mg C g DW-1; 3.4 to 37.8 mg N g DW-1; 0.13 to 2.31 mg P g DW-1) and stocks (from 3.8 to 11.9 t C ha(-1); 96.3 to 400.6 kg N ha(-1), 3.6 to 44.3 kg P ha(-1)) were observed along the year (August 1993 - July 1994). Peaks in concentrations and stocks were observed in January, February and March 1994. As nutrient concentrations in water and sediments are low and stocks in the plant high, the local nutrient dynamics seem to be controlled by this macrophyte. Leaf blades and sheaths participate most in recycling, while stems contain the principal nutrient stock

The planktonic community in tropical interdunal ponds (Lencois Maranhenses National Park, Maranhao State, Brazil).

The aim of this study was to characterize the LenYois Maranhenses ponds (Maranhao State, Brazil) with respect to morphometry and physical and chemical aspects of the water body, in particular, the planktonic community. LenYois Maranhenses is a tropical coastal region with sand dunes and numerous interdunal ponds, most of them temporary. Ten ponds were studied, and a nictemeral variation was carried out. They have similar physical and chemical characteristics (high temperature - 27.5 to 32.0 [degree]C, acid pH - 4.9 to 6.2, low values of dissolved nutrients, chloride - 0.75 to 1.00 mg.L[long dash]1, and electric conductivity - 18.8 to 33.2 [mu]S.cm[long dash]1, and light penetration up to the sediment). With respect to the plankton, it is likely that the different histories of colonization, the formation of the ponds, and the initial organisms, affect the species richness, density and dominance observed. The lakes were classified as polymitic and oligotrophic
