    The Far-Infrared Radiation Mobile Observation System (FIRMOS) for spectral characterization of the atmospheric emission

    The Far-Infrared Radiation Mobile Observation System (FIRMOS) is a Fourier transform spectroradiometer developed to support the Far-infrared Outgoing Radiation Understanding and Monitoring (FORUM) satellite mission by validating measurement methods and instrument design concepts, both in the laboratory and in field campaigns. FIRMOS is capable of measuring the downwelling spectral radiance emitted by the atmosphere in the spectral band from 100 to 1000 cm1^{−1} (10–100 µm in wavelength), with a maximum spectral resolution of 0.25 cm1^{−1}. We describe the instrument design and its characterization and discuss the geophysical products obtained by inverting the atmospheric spectral radiance measured during a campaign from the high-altitude location of Mount Zugspitze in Germany, beside the Extended-range Atmospheric Emitted Radiance Interferometer (E-AERI), which is permanently installed at the site. Following the selection of clear-sky scenes, using a specific algorithm, the water vapour and temperature profiles were retrieved from the FIRMOS spectra by applying the Kyoto protocol and Informed Management of the Adaptation (KLIMA) code. The profiles were found in very good agreement with those provided by radiosondes and by the Raman lidar operating from the Zugspitze Schneefernerhaus station. In addition, the retrieval products were validated by comparing the retrieved integrated water vapour values with those obtained from the E-AERI spectra

    Correction to: Two years later: Is the SARS-CoV-2 pandemic still having an impact on emergency surgery? An international cross-sectional survey among WSES members

    Background: The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods: A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results: 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approaches reported by 67.3% of the participants, especially in uncomplicated appendicitis, cholecystitis and multiple-recurrent diverticulitis. Conclusions: The SARS-CoV-2 pandemic still significantly impacts on care and outcome of patients in ES. Well-known problems with in-hospital logistics are not sufficiently resolved by now; however, medical staff shortages and reduced capacities have been dramatically aggravated over last two pandemic years

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice

    A Portable Quantum Cascade Laser Spectrometer for Atmospheric Measurements of Carbon Monoxide

    Trace gas concentration measurements in the stratosphere and troposphere are critically required as inputs to constrain climate models. For this purpose, measurement campaigns on stratospheric aircraft and balloons are being carried out all over the world, each one involving sensors which are tailored for the specific gas and environmental conditions. This paper describes an automated, portable, mid-infrared quantum cascade laser spectrometer, for in situ carbon monoxide mixing ratio measurements in the stratosphere and troposphere. The instrument was designed to be versatile, suitable for easy installation on different platforms and capable of operating completely unattended, without the presence of an operator, not only during one flight but for the whole period of a campaign. The spectrometer features a small size (80 × 25 × 41 cm3), light weight (23 kg) and low power consumption (85 W typical), without being pressurized and without the need of calibration on the ground or during in-flight operation. The device was tested in the laboratory and in-field during a research campaign carried out in Nepal in summer 2017, onboard the stratospheric aircraft M55 Geophysica. The instrument worked extremely well, without external maintenance during all flights, proving an in-flight sensitivity of 1–2 ppbV with a time resolution of 1 s

    Predicting Post-Hepatectomy Liver Failure in HCC Patients: A Review of Liver Function Assessment Based on Laboratory Tests Scores

    The assessment of liver function is crucial in predicting the risk of post-hepatectomy liver failure (PHLF) in patients undergoing liver resection, especially in cases of hepatocellular carcinoma (HCC) which is often associated with cirrhosis. There are currently no standardized criteria for predicting the risk of PHLF. Blood tests are often the first- and least invasive expensive method for assessing hepatic function. The Child–Pugh score (CP score) and the Model for End Stage Liver Disease (MELD) score are widely used tools for predicting PHLF, but they have some limitations. The CP score does not consider renal function, and the evaluation of ascites and encephalopathy is subjective. The MELD score can accurately predict outcomes in cirrhotic patients, but its predictive capabilities diminish in non-cirrhotic patients. The albumin–bilirubin score (ALBI) is based on serum bilirubin and albumin levels and allows the most accurate prediction of PHLF for HCC patients. However, this score does not consider liver cirrhosis or portal hypertension. To overcome this limitation, researchers suggest combining the ALBI score with platelet count, a surrogate marker of portal hypertension, into the platelet–albumin–bilirubin (PALBI) grade. Non-invasive markers of fibrosis, such as FIB-4 and APRI, are also available for predicting PHLF but they focus only on cirrhosis related aspects and are potentially incomplete in assessing the global liver function. To improve the predictive power of the PHLF of these models, it has been proposed to combine them into a new score, such as the ALBI-APRI score. In conclusion, blood test scores may be combined to achieve a better predictive value of PHLF. However, even if combined, they may not be sufficient to evaluate liver function and to predict PHLF; thus, the inclusion of dynamic and imaging tests such as liver volumetry and ICG r15 may be helpful to potentially improve the predictive capacity of these models

    Minimally invasive versus open adrenalectomy for adrenocortical carcinoma: the keys surgical factors influencing the outcomes-a collective overview

    PurposeAdrenocortical carcinoma (A.C.C.) is a rare tumour, often discovered at an advanced stage and associated with a poor prognosis. Surgery is the treatment of choice. We aimed to review the different surgical approaches trying to compare their outcome.MethodsThis comprehensive review has been carried out according to the PRISMA statement. The literature search was performed in PubMed, Scopus, the Cochrane Library and Google Scholar.ResultsAmong all studies identified, 18 were selected for the review. A total of 14,600 patients were included in the studies, of whom 4421 were treated by mini-invasive surgery (M.I.S.). Ten studies reported 531 conversions from M.I.S. to an open approach (OA) (12%). Differences were reported for operative times as well as for postoperative complications more often in favour of OA, whereas differences for hospitalization time in favour of M.I.S.Some studies showed an R0 resection rate from 77 to 89% for A.C.C. treated by OA and 67 to 85% for tumours treated by M.I.S. The overall recurrence rate ranged from 24 to 29% for A.C.C. treated by OA and from 26 to 36% for tumours treated by M.I.S.ConclusionsOA should still be considered the standard surgical management of A.C.C. Laparoscopic adrenalectomy has shown shorter hospital stays and faster recovery compared to open surgery. However, the laparoscopic approach resulted in the worst recurrence rate, time to recurrence and cancer-specific mortality in stages I-III ACC. The robotic approach had similar complications rate and hospital stays, but there are still scarce results about oncologic follow-up

    Human dental pulp pluripotent-like stem cells promote wound healing and muscle regeneration

    BACKGROUND: Dental pulp represents an easily accessible autologous source of adult stem cells. A subset of these cells, named dental pulp pluripotent-like stem cells (DPPSC), shows high plasticity and can undergo multiple population doublings, making DPPSC an appealing tool for tissue repair or maintenance. METHODS: DPPSC were harvested from the dental pulp of third molars extracted from young patients. Growth factors released by DPPSC were analysed using antibody arrays. Cells were cultured in specific differentiation media and their endothelial, smooth and skeletal muscle differentiation potential was evaluated. The therapeutic potential of DPPSC was tested in a wound healing mouse model and in two genetic mouse models of muscular dystrophy (Scid/mdx and Sgcb-null Rag2-null γc-null). RESULTS: DPPSC secreted several growth factors involved in angiogenesis and extracellular matrix deposition and improved vascularisation in all three murine models. Moreover, DPPSC stimulated re-epithelialisation and ameliorated collagen deposition and organisation in healing wounds. In dystrophic mice, DPPSC engrafted in the skeletal muscle of both dystrophic murine models and showed integration in muscular fibres and vessels. In addition, DPPSC treatment resulted in reduced fibrosis and collagen content, larger cross-sectional area of type II fast-glycolytic fibres and infiltration of higher numbers of proangiogenic CD206+ macrophages. CONCLUSIONS: Overall, DPPSC represent a potential source of stem cells to enhance the wound healing process and slow down dystrophic muscle degeneration.status: publishe

    Deep-convective influence on the upper troposphere–lower stratosphere composition in the Asian monsoon anticyclone region: 2017 StratoClim campaign results

    International audienceThe StratoClim stratospheric aircraft campaign took place in summer 2017 in Nepal (27 July-10 August) and provided for the first time a wide dataset of observations of air composition inside the Asian monsoon anticyclone (AMA). In the framework of this project, with the purpose of modelling the injection of pollutants and natural compounds into the stratosphere, we performed a series of diffusive back trajectory runs along the flights' tracks. The availability of in situ measurements of trace gases has been exploited to evaluate the capability of the trajectory system to reproduce the transport in the upper troposphere-lower stratosphere (UTLS) region. The diagnostics of the convective sources and mixing in the air parcel samples have been derived by integrating the trajectory output with high-resolution observations of cloud tops from the Meteosat Second Generation (MSG1) and Himawari geostationary satellites. Back trajectories have been calculated using meteorological fields from European Centre for Medium-Range Weather Forecasts (ECMWF) reanalysis (ERA-Interim and ERA5) at 3 and 1 h resolution, using both kinematic and diabatic vertical motion. The comparison among the different trajectory runs shows, in general, a higher consistency with observed data as well as a better agreement between the diabatic and kinematic version when using ERA5-based runs with respect to ERA-Interim. Overall, a better capacity in reproducing the pollution features is finally found in the diabatic version of the ERA5 runs. We therefore adopt this setting to analyse the convective influence in the UTLS starting from the StratoClim observations. A large variety of transport conditions have been individuated during the eight flights of the campaign. The larger influence by convec-tive injections is found from the continental sources of China and India. Only a small contribution appears to be originated from maritime regions, in particular the South Pacific and the Bay of Bengal, which, unexpectedly, was not particularly active during the period of the campaign. In addition, a mass of clean air injected from a typhoon has also been detected at around 18 km. Thin filamentary structures of polluted air, characterized by peaks in CO, are observed, mostly associated with young convective air (age less than a few days) Published by Copernicus Publications on behalf of the European Geosciences Union. 12194 S. Bucci et al.: Deep-convective transport during the StratoClim campaign and with a predominant South China origin. The analysis revealed a case of direct injection of highly polluted air close to the level of the tropopause (anomalies of around 80 ppbv injected at 16 km) that then kept rising inside the anticyclonic circulation. Due to the location of the campaign, air from continental India, in contrast, has been only observed to be linked to air masses that recirculated within the anticyclone for 10 to 20 d, resulting in a lower concentration of the trace gas. The analysis of a flight overpassing an intense convec-tive system close to the southern Nepalese border revealed the injection of very young air (few hours of age) directly in the tropopause region (∼ 18 km), visible in the trace gases as an enhancement in CO and a depletion in the O 3 one. From the whole campaign, a vertical stratification in the age of air is observed: up to 15 km, the age is less than 3 d, and these fresh air masses constitute almost the totality of the air composition. A transition layer is then individuated between 15 and 17 km, where the convective contribution is still dominant , and the ages vary between 1 and 2 weeks. Above this level, the mean age of the air sampled by the aircraft is estimated to be 20 d. There, the convective contribution rapidly decreases with height and finally becomes negligible around 20 km
