    Factors associated with relapse amongst substance abusers

    Background: Relapse amongst substance abusers is common throughout the world, and Bangladesh is no exception to this. In Bangladesh, drug related problems are gradually becoming a burning issue in context of social, economical and medical perspective. The present study aimed to find out factors indicating relapse amongst substance abuser. Methods: This descriptive type of observational study was conducted in the combined military hospital and other government/private hospital/institute, especially the central drug addict treatment center, Dhaka. Informed consent was obtained prior to data collection. Collected data was classified, edited, coded, and entered into the computer for statistical analysis by using SPSS-23. The chi-Square test was used to analyze the categorical variables, and a p<0.05 was considered as statistically significant. Results: The study involved 100 patients who had a history of substance abuse. The most common substance abused was Yaba (27%), followed by cannabis (21%). The average duration of abuse for Yaba was 5.8 years, while the longest mean duration was for Alcohol (14.2 years). In the 2nd admission, the largest percentage of patients was aged 21-30 years and were male. The majority of patients were Muslim and were either unemployed or had a lower socioeconomic status. Patients age, occupation, socioeconomic status, peer pressure, and family problems all had a significant association (p<0.05) with relapse at different admissions. Peer pressure and family problems were also identified as factors affecting relapse, with 67.57% and 56.76% of patients experiencing them during their 2nd admission, respectively. Conclusions: The study found Yaba to be the most commonly used drug, followed by cannabis, phensedyl, heroin, etc. Alcohol was found to have the longest duration of abuse. Most patients were aged 21-30 and unemployed in multiple admissions. Peer pressure and unemployment were major factors in substance abuse, and psychiatric illness was a common factor in relapse. The results align with global findings and highlight the need for a comprehensive approach to addressing substance abuse, considering all relevant factors.

    Interferon-γ and Proliferation Responses to Salmonella enterica Serotype Typhi Proteins in Patients with S. Typhi Bacteremia in Dhaka, Bangladesh

    Salmonella enterica serotype Typhi infection is a significant global public health problem and the cause of typhoid fever. Salmonella are intracellular pathogens, and cellular immune responses are required to control and clear Salmonella infections. Despite this, there are limited data on cellular immune responses during wild type S. Typhi infection in humans. Here we report the assessment of cellular immune responses in humans with S. Typhi bacteremia through a screening approach that permitted us to evaluate interferon-γ and proliferation responses to a number of S. Typhi antigens. We detected significant interferon-γ CD4 and CD8 responses, as well as proliferative responses, to a number of recombinantly purified S. Typhi proteins as well as membrane preparation in infected patients. Antigen-specific interferon-γ responses were present at the time of clinical presentation in patients and absent in healthy controls. These observations could assist in the development of interferon-γ-based diagnostic assays for typhoid fever

    Convalescent plasma in patients admitted to hospital with COVID-19 (RECOVERY): a randomised controlled, open-label, platform trial

    SummaryBackground Azithromycin has been proposed as a treatment for COVID-19 on the basis of its immunomodulatoryactions. We aimed to evaluate the safety and efficacy of azithromycin in patients admitted to hospital with COVID-19.Methods In this randomised, controlled, open-label, adaptive platform trial (Randomised Evaluation of COVID-19Therapy [RECOVERY]), several possible treatments were compared with usual care in patients admitted to hospitalwith COVID-19 in the UK. The trial is underway at 176 hospitals in the UK. Eligible and consenting patients wererandomly allocated to either usual standard of care alone or usual standard of care plus azithromycin 500 mg once perday by mouth or intravenously for 10 days or until discharge (or allocation to one of the other RECOVERY treatmentgroups). Patients were assigned via web-based simple (unstratified) randomisation with allocation concealment andwere twice as likely to be randomly assigned to usual care than to any of the active treatment groups. Participants andlocal study staff were not masked to the allocated treatment, but all others involved in the trial were masked to theoutcome data during the trial. The primary outcome was 28-day all-cause mortality, assessed in the intention-to-treatpopulation. The trial is registered with ISRCTN, 50189673, and ClinicalTrials.gov, NCT04381936.Findings Between April 7 and Nov 27, 2020, of 16 442 patients enrolled in the RECOVERY trial, 9433 (57%) wereeligible and 7763 were included in the assessment of azithromycin. The mean age of these study participants was65·3 years (SD 15·7) and approximately a third were women (2944 [38%] of 7763). 2582 patients were randomlyallocated to receive azithromycin and 5181 patients were randomly allocated to usual care alone. Overall,561 (22%) patients allocated to azithromycin and 1162 (22%) patients allocated to usual care died within 28 days(rate ratio 0·97, 95% CI 0·87–1·07; p=0·50). No significant difference was seen in duration of hospital stay (median10 days [IQR 5 to >28] vs 11 days [5 to >28]) or the proportion of patients discharged from hospital alive within 28 days(rate ratio 1·04, 95% CI 0·98–1·10; p=0·19). Among those not on invasive mechanical ventilation at baseline, nosignificant difference was seen in the proportion meeting the composite endpoint of invasive mechanical ventilationor death (risk ratio 0·95, 95% CI 0·87–1·03; p=0·24).Interpretation In patients admitted to hospital with COVID-19, azithromycin did not improve survival or otherprespecified clinical outcomes. Azithromycin use in patients admitted to hospital with COVID-19 should be restrictedto patients in whom there is a clear antimicrobial indication

    Abstract. This study was performed based on local perception on arsenic toxicity with respect to socioeconomic status at the Achintanagar village in the southwestern part of Bangladesh. The questionnaire was distributed randomly at the local scale in the study area. It was found that the respondents are highly contaminated with arsenic and most of them have suffered from economic crisis and ignorance. Arsenic contamination and food poisoning concept has direct bearing on educational level which is emphasized on socioeconomic status. Correlation coefficient matrix had revealed that the whole aspects of socioeconomic component negatively correlated with arsenic toxicity for group one and two. Female participant were observed to be more unconscious than male respondents on food contamination through arsenic accumulation. Results showed that, most of the respondents continued to eat and practiced the sale of contaminated food in the community. It was observed that this practice is highly contagious for human and environmental health

    Land Use and the Climatic Determinants of Population Exposure to PM<sub>2.5</sub> in Central Bangladesh

    The major industrial cities of Bangladesh are experiencing significant air-pollution-related problems due to the increased trend of particulate matter (PM2.5) and other pollutants. This paper aimed to investigate and understand the relationship between PM2.5 and land use and climatic variables to identify the riskiest areas and population groups using a geographic information system and regression analysis. The results show that about 41% of PM2.5 concentration (μg/m3) increased within 19 years (2002–2021) in the study area, while the highest concentration of PM2.5 was found from 2012 to 2021. The concentrations of PM2.5 were higher over barren lands, forests, croplands, and urban areas. From 2002–2021, the concentration increased by about 64%, 62.7%, 57%, and 55% (μg/m3) annually over barren lands, forests, cropland, and urban regions. The highest concentration level of PM2.5 (84 μg/m3) among other land use classes was found in urban areas in 2021. The regression analysis shows that air pressure (hPa) (r2 = −0.26), evaporation (kg m−2) (r2 = −0.01), humidity (kg m−2) (r2 = −0.22), rainfall (mm/h) (r2 = −0.20), and water vapor (kg m−2) (r2 = −0.03) were negatively correlated with PM2.5. On the other hand, air temperature (k) (r2 = 0.24), ground heat (W m−2) (r2 = 0.60), and wind speed (m s−1) (r2 = 0.34) were positively correlated with PM2.5. More than 60 Upazilas were included in the most polluted areas, with a total population of 11,260,162 in the high-risk/hotspot zone (1,948,029 aged 0–5, 485,407 aged 50–69). Governmental departments along with policymakers, stainable development practitioners, academicians, and others may use the main results of the paper for integrated air pollution mitigation and management in Bangladesh as well as in other geographical settings worldwide

    Increased yield performance of mutation induced soybean genotypes at varied agro-ecological conditions

    In soybean breeding program, continuous selection pressure on traits response to yield created a genetic bottleneck for improvements of soybean through hybridization breeding technique. Therefore an initiative was taken to developed high yielding soybean variety applying mutation breeding techniques at Plant Breeding Division, Bangladesh Institute of Nuclear Agriculture (BINA), Bangladesh. Locally available popular cultivar BARI Soybean-5 was used as a parent material and subjected to five different doses of Gamma ray using Co60. In respect to seed yield and yield attributing characters, twelve true breed mutants were selected from M4 generation. High values of heritability and genetic advance with high genotypic coefficient of variance (GCV) for plant height, branch number and pod number were considered as favorable attributes for soybean improvement that ensure expected yield. The mutant SBM-18 obtained from 250Gy provided stable yield performance at diversified environments. It provided maximum seed yield of 3056 kg ha-1 with highest number of pods plant-1 (56). The National Seed Board of Bangladesh (NSB) eventually approved SBM-18 and registered it as a new soybean variety named ‘Binasoybean-5’ for largescale planting because of its superior stability in various agro-ecological zones and consistent yield performance
