Modelling the business structure of a digital health ecosystem
[EN]The current trend in digital solutions for the health sector is to move from fragmented services to progressively more integrated services provided by multiple stakeholders through technological ecosystem platforms. However, the business model is scarcely taken into account at the early stages of development of this type of ecosystems specially in the health sector. In the present paper a general approach towards the exploitation of a technological ecosystem focused on caregivers is presented. It follows the Business Process Model and Notation (BPMN) in order to develop different ecosystem’s exploitation alternatives, taking into account the ecosystem stakeholders and their main value propositions. This serves as a starting data model in the software development process from which different business exploitation alternatives can be elaborated
Do national resources have to be centrally managed? Vested interests and institutional reform in Norwegian fisheries governance
Corporatism -with its privileged access, restricted participation and centralized
structures - has a long history in Norwegian fisheries governance. Co-management –
understood as a decentralized, bottom-up and more inclusive form of fisheries
governance - has not been considered a relevant alternative.. Why does corporatism
still prevail in a context where stakeholder status in fisheries governance globally – both
in principle and practice - has been awarded environmental organizations, municipal
authorities and even consumer advocacy groups? Why then have alternatives to the
corporatist system of centralized consultation and state governance never been seriously
considered in Norway, in spite of the growing emphasis on fish as a public resource and
fisheries management as human intervention in geographically confined and complex
ecosystems? We suggest that thismay have to do with the fundamental assumptions
behind Norwegian fisheries governance that since fish is a national resource, it must be
centrally managed. We argue that this is an assumption that may be contested
A Kernel to Exploit Informative Missingness in Multivariate Time Series from EHRs
A large fraction of the electronic health records (EHRs) consists of clinical
measurements collected over time, such as lab tests and vital signs, which
provide important information about a patient's health status. These sequences
of clinical measurements are naturally represented as time series,
characterized by multiple variables and large amounts of missing data, which
complicate the analysis. In this work, we propose a novel kernel which is
capable of exploiting both the information from the observed values as well the
information hidden in the missing patterns in multivariate time series (MTS)
originating e.g. from EHRs. The kernel, called TCK, is designed using an
ensemble learning strategy in which the base models are novel mixed mode
Bayesian mixture models which can effectively exploit informative missingness
without having to resort to imputation methods. Moreover, the ensemble approach
ensures robustness to hyperparameters and therefore TCK is particularly
well suited if there is a lack of labels - a known challenge in medical
applications. Experiments on three real-world clinical datasets demonstrate the

Listening In on the Past: What Can Otolith δ18O Values Really Tell Us about the Environmental History of Fishes?
Oxygen isotope ratios from fish otoliths are used to discriminate marine stocks and reconstruct past climate, assuming that variations in otolith δ18O values closely reflect differences in temperature history of fish when accounting for salinity induced variability in water δ18O. To investigate this, we exploited the environmental and migratory data gathered from a decade using archival tags to study the behaviour of adult plaice (Pleuronectes platessa L.) in the North Sea. Based on the tag-derived monthly distributions of the fish and corresponding temperature and salinity estimates modelled across three consecutive years, we first predicted annual otolith δ18O values for three geographically discrete offshore sub-stocks, using three alternative plausible scenarios for otolith growth. Comparison of predicted vs. measured annual δ18O values demonstrated >96% correct prediction of sub-stock membership, irrespective of the otolith growth scenario. Pronounced inter-stock differences in δ18O values, notably in summer, provide a robust marker for reconstructing broad-scale plaice distribution in the North Sea. However, although largely congruent, measured and predicted annual δ18O values of did not fully match. Small, but consistent, offsets were also observed between individual high-resolution otolith δ18O values measured during tag recording time and corresponding δ18O predictions using concomitant tag-recorded temperatures and location-specific salinity estimates. The nature of the shifts differed among sub-stocks, suggesting specific vital effects linked to variation in physiological response to temperature. Therefore, although otolith δ18O in free-ranging fish largely reflects environmental temperature and salinity, we counsel prudence when interpreting otolith δ18O data for stock discrimination or temperature reconstruction until the mechanisms underpinning otolith δ18O signature acquisition, and associated variation, are clarified
Interaction between expectancies and drug effects: an experimental investigation of placebo analgesia with caffeine as an active placebo
In a randomised placebo-controlled clinical trial it is assumed that psychosocial effects of the treatment, regression to the mean and spontaneous remission are identical in the drug and placebo group. Consequently, any difference between the groups can be ascribed to the pharmacological effects. Previous studies suggest that side effects of drugs can enhance expectancies of treatment effects in the drug group compared to the placebo group, and thereby increase placebo responses in the drug group compared to the
placebo group.
The hypothesis that side effects of drugs can
enhance expectancies and placebo responses was tested.
Painful laser stimuli were delivered to 20 healthy
subjects before and after administration of a drink with 0 or 4 mg/kg caffeine. The drink was administered either with information that it contained a painkiller or that it was a placebo. Laser-evoked potentials and reports of pain, expectancy, arousal and stress were measured.
Results Four milligrammes per kilogramme of caffeine reduced pain. Information that a painkiller was administered increased the analgesic effect of caffeine compared to caffeine administered with no drug information. This effect
was mediated by expectancies. Information and expectancies had no effect on pain intensity when 0 mg/kg was administered.
The analgesic effect of caffeine was increased by
information that a painkiller was administered. This was due to an interaction of the pharmacological action of the drug and expectancies. Hence, psychosocial effects accompanying a treatment can differ when an active drug is administered compared to a placebo
Halogenase Genes in Nonribosomal Peptide Synthetase Gene Clusters of Microcystis (Cyanobacteria): Sporadic Distribution and Evolution
Cyanobacteria of the genus Microcystis are known to produce secondary metabolites of large structural diversity by nonribosomal peptide synthetase (NRPS) pathways. For a number of such compounds, halogenated congeners have been reported along with nonhalogenated ones. In the present study, chlorinated cyanopeptolin- and/or aeruginosin-type peptides were detected by mass spectrometry in 17 out of 28 axenic strains of Microcystis. In these strains, a halogenase gene was identified between 2 genes coding for NRPS modules in respective gene clusters, whereas it was consistently absent when the strains produced only nonchlorinated corresponding congeners. Nucleotide sequences were obtained for 12 complete halogenase genes and 14 intermodule regions of gene clusters lacking a halogenase gene or containing only fragments of it. When a halogenase gene was found absent, a specific, identical excision pattern was observed for both synthetase gene clusters in most strains. A phylogenetic analysis including other bacterial halogenases showed that the NRPS-related halogenases of Microcystis form a monophyletic group divided into 2 subgroups, corresponding to either the cyanopeptolin or the aeruginosin peptide synthetases. The distribution of these peptide synthetase gene clusters, among the tested Microcystis strains, was found in relative agreement with their phylogeny reconstructed from 16S–23S rDNA intergenic spacer sequences, whereas the distribution of the associated halogenase genes appears to be sporadic. The presented data suggest that in cyanobacteria these prevalent halogenase genes originated from an ancient horizontal gene transfer followed by duplication in the cyanobacterial lineage. We propose an evolutionary scenario implying repeated gene losses to explain the distribution of halogenase genes in 2 NRPS gene clusters that subsequently defines the seemingly erratic production of halogenated and nonhalogenated aeruginosins and cyanopeptolins among Microcystis strains
Enhanced transfection of cell lines from Atlantic salmon through nucoleofection and antibiotic selection
Background
Cell lines from Atlantic salmon kidney have made it possible to culture and study infectious salmon anemia virus (ISAV), an aquatic orthomyxovirus affecting farmed Atlantic salmon. However, transfection of these cells using calcium phosphate precipitation or lipid-based reagents shows very low transfection efficiency. The Amaxa Nucleofector technology™ is an electroporation technique that has been shown to be efficient for gene transfer into primary cells and hard to transfect cell lines.
Findings
Here we demonstrate, enhanced transfection of the head kidney cell line, TO, from Atlantic salmon using nucleofection and subsequent flow cytometry. Depending on the plasmid promoter, TO cells could be transfected transiently with an efficiency ranging from 11.6% to 90.8% with good viability, using Amaxa's cell line nucleofector solution T and program T-20. A kill curve was performed to investigate the most potent antibiotic for selection of transformed cells, and we found that blasticidin and puromycin were the most efficient for selection of TO cells.
Conclusions
The results show that nucleofection is an efficient way of gene transfer into Atlantic salmon cells and that stably transfected cells can be selected with blasticidin or puromycin
