    Oral lichen planus in children: an italian case series with literature review

    Objectives. Oral lichen planus is a chronic mucous inflammatory disease. The prevalence in adults ranges between 0.5% and 2%, whereas children are involved just in 0.03% of cases. The purpose of this paper is to review the current literature and to present six Italian cases of Oral Lichen Planus in children (OLPc). Methods. We reported the data about a retrospective study on 6 cases of oral lichen planus in children. A detailed history was retrieved from clinical notes of each patient. Patients younger than 18 years old were enrolled in the study. All of them had the OLP diagnosis confirmed by clinical findings, history, and histopathology. Results. The family history for lichen planus was negative in all patients, the mean age was 11 years (range 6-14); there was no cutaneous involvement, whereas 2 patients had concomitant autoimmune diseases. Topical corticosteroids was the only treatment used in 66.67% of patients with good response in a mean time of 4.75 weeks. Conclusions. The rarity of OLPc may be due to the lack of symptoms that prevents the patient or his family from noticing the presence of the condition, but also to misdiagnosis of the dentist or the paediatrician. Thus, though LP in children is uncommon and oral mucosal involvement extremely rare, clinicians should be aware of its existence and management, and this diagnosis should be taken into account in children presenting oral white lesions

    The size criteria in minimally invasive video-assisted thyroidectomy

    BACKGROUND: Thyroid size is a very important criteria of MIVAT exclusion because the working space provided by the technique is limited. The aim of this work has been to verify the suitability of MIVAT and its applicability in clinical practice, not only in patients with a thyroid volume up to 25 ml but also in patients with a thyroid volume included from 25 to 50 ml. METHODS: From January 2003 to February 2006, 33 patients have been selected for MIVAT. A completely gasless procedure was carried out through a central 20 to 35 mm skin incision performed "high" between the cricoid and jugular notch. RESULTS: The patients were separated in 2 groups. The first group (less than 25 ml) included 23 patients, the second group (from 25 to 50 ml) included 10 patients. The skin incision performed was from 20 to 25 mm (mean 23.61 mm ± 1.83) long in the first group and from 25 to 35 mm (mean 27.8 mm ± 2.20) long in the second one; this difference is significant (t test p < 0.001). CONCLUSION: Our study suggest that the MIVAT using for thyroids bigger than 25 ml and up to 50 ml in volume is feasible and safe. This way allows more patients, excluded before, to take the advantages of minimally invasive approach

    Numerical Calculation of Crack Parameters for Propagation Assessment in a Complex Component with Residual Stresses

    AbstractFatigue is an actual issue, especially in the structural assessment of highly critical components where failure may provoke the loss of the whole system they belong to. This is especially true in a damage tolerant scenario. In this case the use of a component is permitted even after damage has occurred, but extensive knowledge of the damage status and the effect of the damage on the reliability of the component is required. An experimental approach to damage tolerance is often very complex both for the damage creation and for the subsequent step of fatigue tests. A reliable and efficient methodology for fatigue simulation and crack propagation is therefore of interest both from a theoretical and an industrial point of view. The aim of the paper is to investigate crack parameters in a transmission shaft subjected to ballistic impact damage; cracks can nucleate from the point of damage and propagate due the application of service loads Starting from the outcome of a FEM simulation of a ballistic impact, different strategies for numerical crack propagation under service loads are presented and assessed. Specifically the damage under investigation is composed of two holes separated by a septum. Experiments reveal that the crack that nucleates from one of the two holes is strongly influenced by residual stresses. The complexity and the variety of the phenomena involved (complex damage, presence of residual stresses, shape of the component) make the numerical simulation of the phenomenon not trivial and give grounds for a thorough investigation of different modelling approaches

    The minimally invasive open video-assisted approach in surgical thyroid diseases

    BACKGROUND: The targets of minimally invasive surgery (MIVA) could be summarised by: achievement of the same results as those obtained with traditional surgery, less trauma, better post-operative course, early discharge from hospital and improved cosmetic results. The minimally invasive techniques in thyroid surgery can be described as either endoscopic "pure" approach (completely closed approach with or without CO(2 )insufflation), or "open approach" with central neck mini-incision or "open video-assisted approach". Traditionally, open thyroidectomy requires a 6 to 8 cm, or bigger, transverse wound on the lower neck. The minimally invasive approach wound is much shorter (1.5 cm for small nodules, up to 2–3 cm for the largest ones, in respect of the exclusion criteria) upon the suprasternal notch. Patients also experience much less pain after MIVA surgery than after conventional thyroidectomy. This is due to less dissection and destruction of tissues. Pathologies treated are mainly nodular goiter; the only kind of thyroid cancer which may be approached with endoscopic surgery is a small differentiated carcinoma without lymph node involvement. The patients were considered eligible for MIVA hemithyroidectomy and thyroidectomy on the basis of some criteria, such as gland volume and the kind of disease. In our experience we have chosen the minimally invasive open video-assisted approach of Miccoli et al. (2002). The aim of this work was to verify the suitability of the technique and the applicability in clinical practice. METHODS: A completely gasless procedure was carried out through a 15–30 mm central incision about 20 mm above the sternal notch. Dissection was mainly performed under endoscopic vision using conventional endoscopic instruments. The video aided group included 11 patients. All patients were women with a average age of 54. RESULTS: We performed thyroidectomy in 8 cases and hemithyroidectomy in 3 cases. The operative average time has been 170 minutes. CONCLUSION: Nowadays this minimally invasive surgery, in selected patients, clearly demonstrates excellent results regarding patient cure rate and comfort, with shorter hospital stay, reduced postoperative pain and most attractive cosmetic results

    Atomic size effects studied by transport in single silicide nanowires

    Ultrathin metallic silicide nanowires with extremely high aspect ratios can be easily grown, e.g., by deposition of rare earth elements on semiconducting surfaces. These wires play a pivotal role in fundamental research and open intriguing perspectives for CMOS applications. However, the electronic properties of these one-dimensional systems are extremely sensitive to atomic-sized defects, which easily alter the transport characteristics. In this study, we characterized comprehensively TbSi2 wires grown on Si(100) and correlated details of the atomic structure with their electrical resistivities. Scanning tunneling microscopy (STM) as well as all transport experiments were performed in situ using a four-tip STM system. The measurements are complemented by local spectroscopy and density functional theory revealing that the silicide wires are electronically decoupled from the Si template. On the basis of a quasiclassical transport model, the size effect found for the resistivity is quantitatively explained in terms of bulk and surface transport channels considering details of atomic-scale roughness. Regarding future applications the full wealth of these robust nanostructures will emerge only if wires with truly atomically sharp interfaces can be reliably grown. © 2016 American Physical Society.DFG/FOR/170

    Changes in knowledge, attitudes and behaviors of Italian university students regarding contraceptive methods and STDs (1998&#8211;2008): a cross-sectional study

    Aim: The aim of the study is to evaluate changes in opinions and behaviors of university students about sexual attitudes, use and effectiveness of contraceptive methods and sexually transmitted diseases (STDs). Subjects and methods: The study was carried out through three surveys in 1998, 2003 and 2008. A self-completion questionnaire, divided into three major sections, was administered. The groups were analyzed with the Chi-square test and the multiple logistic regression analysis; the significance level was set at p < 0.05. Results: Contraceptive methods were not used by 12.0, 12.5, and 16.2 % of the sample in 1998, in 2003 and 2008 respectively. The condom was the most common method used by responders, followed by the pill; both methods of birth control are also considered the most effective. In 2008, students demonstrated to have a good knowledge about STDs transmission through sexual intercourse and contraceptive use was higher among responders with multiple partners (p = 0.028), lower in smokers (p= 0.003) and female gender (p = 0.001). Conclusion: The results highlight the necessity to activate programs on sex education, on the use of contraceptives and STD prevention. Peer education should also be considered. © 2012 Springer-Verlag

    Systematic review of the literature on the clinical effectiveness of the cochlear implant procedure in paediatric patients

    The aim of this systematic review of the literature was to summarize the results of scientific publications on the clinical effectiveness of the cochlear implant (CI) procedure in children. The members of the Working Group first examined existing national and international literature and the principal international guidelines on the procedure. They considered as universally-accepted the usefulness/effectiveness of unilateral cochlear implantation in severely-profoundly deaf children. Accordingly, they focused attention on systematic reviews addressing clinical effectiveness and cost/efficacy of the CI procedure, with particular regard to the most controversial issues for which international consensus is lacking. The following aspects were evaluated: post-CI outcomes linked to precocity of CI; bilateral (simultaneous/ sequential) CI vs. unilateral CI and vs. bimodal stimulation; benefits derived from CI in deaf children with associated disabilities. With regard to the outcomes after implantation linked to precocity of intervention, there are few studies comparing post-CI outcomes in children implanted within the first year of life with those of children implanted in the second year. The selected studies suggest that children implanted within the first year of life present hearing and communicative outcomes that are better than those of children implanted after 12 months of age. Concerning children implanted after the first year of life, all studies confirm an advantage with respect to implant precocity, and many document an advantage in children who received cochlear implants under 18 months of age compared to those implanted at a later stage. With regard to bilateral CI, the studies demonstrate that compared to unilateral CI, bilateral CI offers advantages in terms of hearing in noise, sound localization and during hearing in a silent environment. There is, however, a wide range of variability. The studies also document the advantages after sequential bilateral CI. In these cases, a short interval between interventions, precocity of the first CI and precocity of the second CI are considered positive prognostic factors. In deaf children with associated disabilities, the studies analyzed evidence that the CI procedure is also suitable for children with disabilities associated with deafness, and that even these children may benefit from the procedure, even if these may be slower and inferior to those in children with isolated deafness, especially in terms of high communicative and perceptive skills
