    Falls and other geriatric syndromes in Blantyre, Malawi: A community survey of older adults

    Background: The prevalence of geriatric syndromes (falls, immobility,intellectual or memory impairment, and incontinence) is unknown inmany resource-poor countries. With an aging population such knowledgeis essential to develop national policies on the health and social needs ofolder people. The aim of this study was to provide a preliminary surveyto explore the prevalence of falls and other geriatric syndromes and theirassociation with known risk factors in people aged > 60 years in urbanBlantyre, Malawi.Methods: This was a cross-sectional, community survey of adults aged > 60 years. Subjects were recruited at home or in the waiting areas of chronic care clinics. They were interviewed to complete a questionnaire on ageassociated syndromes and comorbid problems. The Abbreviated MentalTest (AMT) and Timed Up and Go (TUG) tests were carried out.Results: Ninety-eight subjects were studied; 41% reported falling in the past 12 months, 33% of whom (13% of all subjects) were recurrent fallers.Twenty-five percent reported urine incontinence, 66% self-reported memory difficulties, and 11% had an AMT score < 7. A history of falling was significantly associated with urine incontinence (p=0.01), selfreportedmemory problems (p=0.004) and AMT score < 7 (p=0.02).Conclusions: Geriatric syndromes, including falls, appear to be prevalent in older people in Blantyre, Malawi. Falling is associated with cognitive impairment and urinary incontinence. There is an urgent need for more understanding of geriatric problems in this setting to develop national policies on health and social needs of older people. It is likely that many of the contributory factors to falls would be amenable to multifactorial interventions similar to those found to be effective in developed countries

    Documenting the absence of bovine brucellosis in dairy cattle herds in the southern region of Malawi and the associated knowledge, attitudes and practices of farmers

For this reason, a cross-sectional study was conducted, from 06 January 2020 to 27 February 2020, to estimate the seroprevalence of brucellosis in dairy cattle herds amongst smallholder farmers, government and private dairy farms in the southern region. A total of 529 serum samples were screened for anti-Brucella antibodies using the Rose Bengal test (RBT) and a competitive enzyme-linked immunosorbent assay (cELISA). A pre-tested electronic (Epicollect tool, Wellcome Sanger Institute, United Kingdom) questionnaire was administered to 378 smallholder farmers to assess their knowledge, attitudes and practices towards brucellosis. Descriptive statistics were used to analyse the data in Microsoft Excel¼ and Statistical Package for Social Sciences (SPSS¼) version 21. No animal tested positive for presence of anti-Brucella antibodies, indicating 0% prevalence (individual and herd levels). The majority (94.2%; 95% confidence interval [CI]: 91.8–96.5) of smallholder farmers had never heard about brucellosis. Furthermore, assisting during parturition without protective equipment (41.3%; 95% CI: 36.3–46.2) and using bulls for breeding (75%; 95% CI: 70.2–78.9) were amongst the common risk practices that were identified. We could not detect brucellosis in this study that indicates the disease could be very rare or even absent in the dairy cattle herds of the southern region of Malawi. However, further Brucella studies need to be conducted in cattle, small livestock, wildlife and humans to document the true status of brucellosis in the country. Brucellosis surveillance, monitoring, awareness and preventive measures are required to maintain this favourable situation.     Soil ingestion, nutrition and the seasonality of anthrax in herbivores of Etosha National Park

    Abstract. Anthrax, caused by the bacterium Bacillus anthracis, is a seasonally occurring infectious disease affecting primarily herbivorous wildlife and livestock. The seasonality of anthrax outbreaks varies among locations, making it difficult to develop a single consistent ecological description of this disease. Over 44 years of mortality surveillance, most anthrax cases in Etosha National Park, Namibia are observed in the wet season, although elephants have an anthrax mortality peak in the dry season. Focusing on three host species (plains zebra, Equus quagga; African elephant, Loxodonta africana; and springbok, Antidorcas marsupialis) occupying the endemic anthrax area of Etosha National Park, Namibia, we tested two commonly posited causes of anthrax seasonality in herbivores: increased pathogen exposure due to greater soil contact, and increased host susceptibility due to seasonal nutritional stress. These hypotheses were assessed using fecal sampling and measurement of the percentage of fecal silicates as an index of soil ingestion and fecal nitrogen, phosphorus and crude fiber as nutritional indices. Nutritional quality for all three species was higher in wet than dry seasons. Comparing among wet seasons, nutritional indices showed either a decline in nutrition with increasing rainfall or no significant pattern. All three species had greater soil ingestion in the wet season than the dry season. Higher soil contact during the anthrax peak suggests that anthrax seasonality may in part be due to heightened exposure to B. anthracis in wet seasons, for zebra and springbok. Elephant anthrax deaths do not correspond with the season of increased soil ingestion or grazing, suggesting that other behavioral mechanisms may overshadow foraging-based risk factors for this species. Nutritional stress is unlikely the primary causative factor in wet season anthrax systems, although nutritional stress sufficient to reduce resistance is difficult to assess non-invasively in wild herbivores. In contrast, increased soil ingestion may be an important predisposing factor for wet season anthrax outbreaks. Ultimately, the amount of soil ingested and its importance in the transmission of soil-borne pathogens will vary based on foraging behaviors, intake rates, grassland structure and on the likelihood that foraging areas intersect with pathogen aggregations in the environment

    City to city learning and knowledge exchange for climate resilience in southern Africa

    This paper argues that learning is essential for cities to become resilient to challenges. The Future Resilience for African CiTies And Lands (FRACTAL) project is profiled. Following FRACTAL’s city-to-city learning approach of sharing good practices, knowledge and experiences framed around transdisciplinary research, the study cities of Harare, Lusaka, Windhoek and Durban conducted city learning exchange visits between 2017 and 2018. FRACTAL contributes towards climate resilient development by providing relevant climate information for decision-making at the city regional scale in southern Africa.     Exploring the benefits and dis-benefits of climate migration as an adaptive strategy along the rural-peri-urban continuum in Namibia

    The scale of climate migration across the Global South is expected to increase during this century. By 2050, millions of Africans are likely to consider, or be pushed into, migration because of climate hazards contributing to agricultural disruption, water and food scarcity, desertification, flooding, drought, coastal erosion, and heat waves. However, the migration-climate nexus is complex, as is the question of whether migration can be considered a climate change adaptation strategy across both the rural and urban space. Combining data from household surveys, key informant interviews, and secondary sources related to regional disaster, demographic, resource, and economic trends between 1990 and 2020 from north central and central dryland Namibia, we investigate (i) human migration flows and the influence of climate hazards on these flows and (ii) the benefits and dis-benefits of migration in supporting climate change adaptation, from the perspective of migrants (personal factors and intervening obstacles), areas of origin, and areas of destination. Our analysis suggests an increase in climate-related push factors that could be driving rural out-migration from the north central region to peri-urban settlements in the central region of the country. While push factors play a role in rural-urban migration, there are also several pull factors (many of which have been long-term drivers of urban migration) such as perceived higher wages, diversity of livelihoods, water, health and energy provisioning, remittances, better education opportunities, and the exchange of non-marketed products. Migration to peri-urban settlements can reduce some risks (e.g. loss of crops and income due to climate extremes) but amplify others (e.g. heat stress and insecure land tenure). Adaptation at both ends of the rural–urban continuum is supported by deeply embedded linkages in a model of circular rural–urban-rural migration and interdependencies. Results empirically inform current and future policy debates around climate mobilities in Namibia, with wider implications across Africa

    The use of focus group discussion methodology: Insights from two decades of application in conservation

Available from Wiley via the DOI in this recordFocus group discussion is frequently used as a qualitative approach to gain an in-depth understanding of social issues. The method aims to obtain data from a purposely selected group of individuals rather than from a statistically representative sample of a broader population. Even though the application of this method in conservation research has been extensive, there are no critical assessment of the application of the technique. In addition, there are no readily available guidelines for conservation researchers. Here, we reviewed the applications of focus group discussion within biodiversity and conservation research between 1996 and April 2017. We begin with a brief explanation of the technique for first-time users. We then discuss in detail the empirical applications of this technique in conservation based on a structured literature review (using Scopus). The screening process resulted in 170 articles, the majority of which (67%, n = 114,) were published between 2011 and 2017. Rarely was the method used as a stand-alone technique. The number of participants per focus group (where reported) ranged from 3 to 21 participants with a median of 10 participants. There were seven (median) focus group meetings per study. Focus group discussion sessions lasted for 90 (median) minutes. Four main themes emerged from the review: understanding of people's perspectives regarding conservation (32%), followed by the assessment of conservation and livelihoods practices (21%), examination of challenges and impacts of resource management interventions (19%) and documenting the value of indigenous knowledge systems (16%). Most of the studies were in Africa (n = 76), followed by Asia (n = 44), and Europe (n = 30). We noted serious gaps in the reporting of the methodological details in the reviewed papers. More than half of the studies (n = 101) did not report the sample size and group size (n = 93), whereas 54 studies did not mention the number of focus group discussion sessions while reporting results. Rarely have the studies provided any information on the rationale for choosing the technique.     Africa’s drylands in a changing world: Challenges for wildlife conservation under climate and land-use changes in the Greater Etosha Landscape

    Proclaimed in 1907, Etosha National Park in northern Namibia is an iconic dryland system with a rich history of wildlife conservation and research. A recent research symposium on wildlife conservation in the Greater Etosha Landscape (GEL) highlighted increased concern of how intensification of global change will affect wildlife conservation based on participant responses to a questionnaire. The GEL includes Etosha and surrounding areas, the latter divided by a veteri nary fence into large, private farms to the south and communal areas of residential and farming land to the north. Here, we leverage our knowledge of this ecosystem to provide insight into the broader challenges facing wildlife conservation in this vulnerable dryland environment. We first look backward, summarizing the history of wildlife conservation and research trends in the GEL based on a literature review, providing a broad-scale understanding of the socioecological pro cesses that drive dryland system dynamics. We then look forward, focusing on eight key areas of challenge and opportunity for this ecosystem: climate change, water availability and quality, vegetation and fire management, adaptability of wildlife populations, disease risk, human wildlife conflict, wildlife crime, and human dimensions of wildlife conservation. Using this model system, we summarize key lessons and identify critical threats highlighting future research needs to support wildlife management. Research in the GEL has followed a trajectory seen elsewhere reflecting an increase in complexity and integration across biological scales over time. Yet, despite these trends, a gap exists between the scope of recent research efforts and the needs of wildlife conservation to adapt to climate and land-use changes. Given the complex nature of climate change, in addition to locally existing system stressors, a framework of forward-thinking adaptive management to address these challenges, supported by integrative and multidisciplinary research could be beneficial. One critical area for growth is to better integrate research and wildlife management across land-use types. Such efforts have the potential to support wildlife conservation efforts and human development goals, while building resilience against the impacts of climate change. While our conclusions reflect the specifics of the GEL ecosystem, they have direct relevance for other African dryland systems impacted by global change
