    Resiliencia en adolescentes: el papel de la conectividad con la escuela y familia

    [Abstract] The sense of connectedness with family and school is an element associated with the psychosocial development in adolescents, being a factor that can influence the way they respond against adversity and adjust to change. This study aimed to establish the predictive value of intrafamilial relationships and connectedness with school, teachers, and between students on resilience in Mexican high school adolescents. The sample consisted of 645 students (50.7% female), with an age range of 12 to 16 years old (M = 13.35, SD = 0.99), from a public high school of México. Participants answered the Intrafamilial Relations Assessment Scale, the subscales of social connectedness with school and students of the MDS3 School Climate Survey, and the revised Resilience Questionnaire for Children and Adolescents. The results indicated moderate positive associations between the study variables and resilience. The higher coefficients were found between intrafamilial relationships and resilience. In the predictive analysis, intrafamilial relationship, connectedness with school and between students, conserved its significance for predicting resilience, with family relationships maintaining the highest coefficient. Results suggest that the family environment is the main factor that shapes how adolescents cope with adversity. Limitations and practical implications are discussed. We conclude it is important to generate strategies that promote a higher adolescent connectedness with key environments for the development of protective factors such as resilience.[Resumen] El sentido de conexión con la familia y escuela es un elemento asociado con el desarrollo psicosocial de los adolescentes, siendo un factor que puede influir en su manera de responder a la adversidad y ajustarse a los cambios. Este estudio tuvo como objetivo establecer el valor predictivo de las relaciones intrafamiliares y la conexión con la escuela, los maestros y los estudiantes sobre la resiliencia en los adolescentes de secundaria. La muestra estuvo conformada por 645 estudiantes (50.7% mujeres), con un rango de edad de 12 a 16 años (M = 13.35, DT = 0.99), de una escuela secundaria pública de México. Los participantes respondieron la Escala de Evaluación de Relaciones Intrafamiliares, las subescalas de conexión social con la escuela y los estudiantes de la Encuesta de Clima Escolar MDS3 y el Cuestionario de Resiliencia para Niños y Adolescentes revisado. Los resultados indicaron correlaciones positivas moderadas entre las variables de estudio y la resiliencia, siendo la más elevada la correlación entre relaciones intrafamiliares y resiliencia. En el análisis predictivo, la relación intrafamiliar, la conexión con la escuela y entre estudiantes, conservaron la significancia para predecir la resiliencia, manteniendo las relaciones familiares los coeficientes más altos. Los resultados sugieren que el entorno familiar es el factor principal que determina cómo los adolescentes afrontan la adversidad. Se discuten las limitaciones y las implicaciones prácticas. Concluimos que es importante generar estrategias que promuevan una mayor conexión del adolescente con los entornos que influyen en el desarrollo de factores protectores como la resiliencia

    Characteristics and Outcomes of Patients Discharged Directly Home From the Pediatric Intensive Care Unit

    Introduction: Patients admitted to the pediatric intensive care unit (PICU) typically transfer to an acute care floor prior to discharge (ACD). Various circumstances, including rapid clinical improvement, technology dependence, or capacity constraints, may lead to discharge directly to home from a PICU (DDH). This practice has been studied in adult intensive care units, but research is lacking for PICU patients. Methods: We aimed to describe characteristics and outcomes of patients requiring PICU admission who experienced DDH versus ACD. We conducted a retrospective cohort study of patients ≤18 years old admitted to our academic, tertiary care PICU between 1/1/15 and 12/31/20. Patients who died or were transferred to another facility were excluded. Baseline characteristics (including home ventilator dependence) and markers of illness severity, specifically the need for vasoactive infusion or new mechanical ventilation, were compared between groups. Admission diagnoses were categorized using the Pediatric Clinical Classification System (PECCS). Our primary outcome was hospital readmission within 30 days. Results: Of 4042 PICU admissions during the study period, 768 (19%) were DDH. Baseline demographic characteristics were similar, although DDH patients were more likely to have a tracheostomy (30% vs 5%, P < .01) and require a home ventilator at discharge (24% vs 1%, P < .01). DDH was associated with being less likely to have required a vasoactive infusion (7% vs 11%, P < .01), shorter median length of stay (LOS) (2.1 days vs 5.9 days, P < .01) and increased rate of readmission within 30 days of discharge (17% vs 14%, P < .05). However, repeat analysis after removing ventilator-dependent patients at discharge (n = 202) showed no difference in rates of readmission (14% vs 14%, P = .88). Conclusions: Direct discharge home from the PICU is a common practice. DDH and ACD groups had similar 30-day readmission rate when patient admissions with home ventilator dependence were excluded

    Propiedades Psicométricas de la Versión Corta del Inventario de Inteligencia Emocional (EQi-SF) en Población Mexicana

    Emotional intelligence is considered a protective factor against a variety of psychosocial problems. Due the relevance of this construct, a variety of instruments have been designed for its evaluation, among which the EQi-SF stands out; The objective of this work was to identify the psychometric properties of the EQi-SF in the Mexican population. The study was carried out in a sample of 829 high school graduates in the state of Jalisco, Mexico. The 28-item version of the EQi-SF was applied with a 5-point Likert structure that evaluates 4 factors: Intrapersonal Competence, Interpersonal Competence, Stress Management and Adaptability. Factorial analyzes were carried out, from which a four-dimensional factorial structure with adequate fit indices was corroborated, as well as acceptable to good reliability levels. The results are contrasted in the light of previous studies and implications and recommendations are discussed.La inteligencia emocional es considerada como un factor protector contra una variedad de problemáticas psicosociales. Debido a la relevancia de este constructo se han diseñado una variedad de instrumentos para su evaluación, entre los que destaca el EQi-SF; el objetivo de este trabajo fue identificar las propiedades psicométricas del EQi-SF en población mexicana. El estudio se llevó a cabo en una muestra de 829 bachilleres en el estado de Jalisco, México. Se aplicó la versión de 28 ítems del EQi-SF con una estructura Likert de 5 puntos que evalúa 4 factores: Competencia Intrapersonal, Competencia Interpersonal, Manejo del Estrés y Adaptabilidad. Se llevaron a cabo análisis factoriales, a partir de los cuales se corrobora una estructura factorial de cuatro dimensiones con índices de ajuste adecuados, así como niveles de confiabilidad de aceptables a buenos.     Confianza y redes sociales en productores de hortalizas en San Luis Potosí, México

    In this study, there were two objectives: a) analyzing the social networks of vegetable producers in the middle zone of San Luis Potosí, México, in order to determine their organization; and b) establishing the importance that trust has for business relations such as loaning money, asking for money or setting up a society. Surveys were applied to 35 producers, who referenced 170 names that were later concentrated into 39 producers. With the field information, symmetrical matrices were built to schematize the network structure, and three logistic regression models were formulated to establish the probability of intention for action based on trust. The results show the importance that proximity between actors has. It is concluded that different levels of trust explain the disposition to lending money, setting up a society and, to a lesser degree, asking for money on loan.En la presente investigación se tuvieron dos objetivos: a) analizar las redes sociales de los productores de hortalizas de la zona media de San Luis Potosí, México, a fin de determinar su organización; y b) establecer la importancia que tiene la confianza para las relaciones de negocios como prestar dinero, pedir dinero o hacer una sociedad. Se encuestó a 35 productores, que referenciaron 170 nombres que posteriormente se concentraron en 39 productores. Con la información de campo se construyeron matrices simétricas para esquematizar la estructura de la red y se formularon tres modelos de regresión logística para establecer la probabilidad de la intención de acción basada en la confianza. Los resultados muestran la importancia que tiene la proximidad entre actores. Se concluye que diferentes niveles de confianza explican la disposición a prestar dinero, hacer una sociedad y, en menor medida, a pedir dinero prestado

    Implementation of a multidisciplinary psychoeducational intervention for Parkinson's disease patients and carers in the community: study protocol

    Background: Parkinson’s disease progressively limits patients at different levels and as a result family members play a key role in their care. However, studies show lack of an integrative approach in Primary Care to respond to the difficulties and psychosocial changes experienced by them. The aim of this study is to evaluate the effects of a multidisciplinary psychoeducational intervention focusing on improving coping skills, the psychosocial adjustment to Parkinson’s disease and the quality of life in patients and family carers in a Primary Care setting. Methods: This quasi-experimental study with control group and mixed methods was designed to evaluate a multidisciplinary psychoeducational intervention. Based on the study power calculations, 100 people with Parkinson’s disease and 100 family carers will be recruited and assigned to two groups. The intervention group will receive the ReNACE psychoeducational intervention. The control group will be given a general educational programme. The study will be carried out in six community-based health centres. The results obtained from the two groups will be collected for evaluation at three time points: at baseline, immediately after the intervention and at 6 months post-intervention. The results will be measured with these instruments: the Quality of Life Scale PDQ39 for patients and the Scale of Quality of Life of Care-givers SQLC for family carers, and for all participants the Psychosocial Adjustment to Illness scale and the Brief COPE Inventory. Focus groups will be organised with some patients and family carers who will have received the ReNACE psychoeducational intervention and also with the healthcare professionals involved in its development. Discussion: An important gap exists in the knowledge and application of interventions with a psychosocial approach for people with PD and family carers as a whole. This study will promote this comprehensive approach in Primary Care, which will clearly contribute in the existing knowledge and could reduce the burden of PD for patients and family carers, and also in other long-term conditions

    Complications of Peripherally Inserted Central Venous Catheters: A Retrospective Cohort Study

    Background and Aim The use of venous catheters is a widespread practice, especially in oncological and oncohematological units. The objective of this study was to evaluate the complications associated with peripherally inserted central catheters (PICCs) in a cohort of patients. Materials and Methods In this retrospective cohort study, we included all patient carrying PICCs (n = 603) inserted at our institute between October 2010 and December 2013. The main variables collected were medical diagnosis, catheter care, location, duration of catheterization, reasons for catheter removal, complications, and nursing care. Complications were classified as infection, thrombosis, phlebitis, migration, edema, and/or ecchymosis. Results All patients were treated according to the same “nursing care” protocol. The incidence rate of complications was two cases per 1000 days of catheter duration. The most relevant complications were infection and thrombosis, both with an incidence of 0.17 cases per 1000 days of the total catheterization period. The total average duration of catheterization was 170 days [SD 6.06]. Additionally to “end of treatment” (48.42%) and “exitus”, (22.53%) the most frequent cause of removal was migration (displacement towards the exterior) of the catheter (5.80%). Conclusions PICCs are safe devices that allow the administration of long-term treatment and preserve the integrity of the venous system of the patient. Proper care of the catheter is very important to improve the quality life of patients with oncologic and hematologic conditions. Therefore, correct training of professionals and patients as well as following the latest scientific recommendations are particularly relevant

    A precision medicine test predicts clinical response after idarubicin and cytarabine induction therapy in AML patients

    Complete remission (CR) after induction therapy is the first treatment goal in acute myeloid leukemia (AML) patients and has prognostic impact. Our purpose is to determine the correlation between the observed CR/CRi rate after idarubicin (IDA) and cytarabine (CYT) 3 + 7 induction and the leukemic chemosensitivity measured by an ex vivo test of drug activity. Bone marrow samples from adult patients with newly diagnosed AML were included in this study. Whole bone marrow samples were incubated for 48 h in well plates containing IDA, CYT, or their combination. Pharmacological response parameters were estimated using population pharmacodynamic models. Patients attaining a CR/CRi with up to two induction cycles of 3 + 7 were classified as responders and the remaining as resistant. A total of 123 patients fulfilled the inclusion criteria and were evaluable for correlation analyses. The strongest clinical predictors were the area under the curve of the concentration response curves of CYT and IDA. The overall accuracy achieved using MaxSpSe criteria to define positivity was 81%, predicting better responder (93%) than non-responder patients (60%). The ex vivo test provides better yet similar information than cytogenetics, but can be provided before treatment representing a valuable in-time addition. After validation in an external cohort, this novel ex vivo test could be useful to select AML patients for 3 + 7 regimen vs. alternative schedules
