    Oxidative Stress: A Double Edged Sword

    Oxidative stress (OS) in simple words is defined as a state of imbalance between antioxidant and pro-oxidants defenses. The purpose of this article is to find out the connection of oxidative stress and free radical species with different aspects of human health. Owing to its harmful effects on proteins and nucleic acids, oxidative stress causes chronic diseases such as cardiovascular and neurodegenerative diseases, diabetes and cancer. It highlights the impacts of antioxidants and pro-oxidants particularly on fertility and infertility. It also focuses on the adverse effects caused by the long-term exposure to pro-oxidant factors leading to structural defects of mitochondrial DNA. As modern life style consists of more reliance on the processed foods and lack of physical activity, a nutrition deficiency, which is common in the present lifestyle, is also one of the reasons for oxidative stress to cause inflammation. However, this review also focuses on how diet affects and triggers inflammation. Redox mechanism with potential threats to health of mankind is discussed, how mere stress can provoke biological stress responses leading to development of disease or metabolic errors. Methods for reduction of oxidative stress are discussed in this review. It also highlights mitochondria as an aging factor as many ROS, particularly mitochondria ROS contribute directly to aging in human body.     Forensic Analysis of Tor Browser on Windows 10 and Android 10 Operating Systems

    Smartphones and Internet have become prevalent in our society with various applications in businesses, education, healthcare, gaming, and research. One of the major issues with the Internet today is its lack of security since an eavesdropper can potentially intercept the communication. This has contributed towards an increased number of cyber-crime incidents, resulting in an increase in users’ consciousness about the security and privacy of their communication . One example is the shift towards using private browsers such as Tor. Tor is a well-recognized and widely used privacy browser based on The Onion Router network that provisions anonymity over the insecure Internet. This functionality of Tor has been a major hurdle in cybercrime investigations due to the complex nature of its anonymity. This paper investigates artifacts from the Tor privacy browser on the latest Windows 10 and Android 10 devices to determine potential areas where evidence can be found. We examine the registry, storage, and memory of Windows 10 devices and the memory, storage, logs, and Zram of Android 10 devices for three possible scenarios i.e. before, during, and after use of the Tor browser. Our results do not support the claims made by the Tor Project regarding user privacy and anonymity. We find that it is possible to retrieve significant details about a user’s browsing activities while the Tor browser is in use as well as after it is closed (on both operating systems). This paper also provides an investigative methodology for the acquisition and analysis of Tor browser artifacts from different areas of the targeted operating systems. Therefore, it can serve as a base to expand research in the forensic analysis of other privacy browsers and improve the efficiency of cybercrime investigations efficiency

    Prevalence of obesity in patients suffering from migraine

    Background: Migraine is one of the most common primary headaches, accounting for significant morbidity in patients suffering from it. An association between obesity and migraine has been documented in the past, despite some studies pointing to the contrary. Author’s purpose is to calculate the prevalence of obesity in migraine patients in order to contribute to the existing concepts. A positive correlation could lead to the employment of weight loss interventions in the management of obese patients with migraine.Methods: In this cross-sectional study, 400 patients, recently diagnosed with migraine, attending the Neurology Outpatient Department at the Dr. Ruth KM Pfau Civil Hospital Karachi were enrolled after taking informed consent.  Migraine was diagnosed using International Classification of Headache Disorders III (ICHD III) criteria. Height (meters) and weight (kilograms) were measured and body mass index calculated. This data was kept confidential. The results were tabulated and analyzed using SPSS version 19.  Continuous quantitative data were analyzed using chi square test. A p Value of less than or equal to 0.05 was considered significant.Results: The mean age of enrolled participants was 30.69±6.96 years, 204(51%) were >30 years of age, 159(39.8%) were male and 241(60.3%) were female, mean height was 1.55±0.1 meters, mean weight was 56.26±12.98 kg, and mean duration of migraine was 5.04±2.02 weeks. The prevalence of obesity in patients with migraine was 108 (27%).Conclusions: It was concluded that the prevalence of obesity in patients with migraine was 27%

    Massive Fluid Requirements and an Unusual BUN/Creatinine Ratio for Pre-Renal Failure in Patients with Cholera

    Background: Cholera is an important infectious cause of secretory diarrhea. The primary symptom of infection is the sudden onset of watery diarrhea with subsequent volume depletion causing renal insufficiency. The objective of this research is to study the level of dehydration at presentation and subsequent fluid management in Patients with cholera. Methods: This study was conducted on 191 Patients of Cholera admitted at a tertiary care hospital in Karachi, Pakistan during the period of 5 years. Medical charts were evaluated retrospectively for initial hydration status, baseline lab investigations on admission and discharge and fluid therapy given to all the Patients while their stay in the hospital and the data was analyzed on SPSS 15.0. Results: Out of the 191 Patients, 83(43%) were males and 108 (57%) were females with mean age of 42.3 years (SD+/-18.34). The average duration of symptoms was 3.75 days (SD+/-2.04). Of 191 Patients, 175 (92.1%) presented with dehydration, 80 (42.3%) were given Ringer\u27s Lactate (R/L) + Normal Saline (N/S), 45 (24%) Patients were given R/L + N/S + Oral Rehydration Therapy (ORS), 27 (14.3%) of the Patients were kept on R/L only and remaining were given various combinations of R/L, N/S, ORS and Dextrose Saline (D/S). On admission mean Blood Urea Nitrogen (BUN) was 24.54 (SD+/-16.6), mean creatinine was 2.47 (SD+/-2.35) and mean BUN/Creatinine ratio was 11.63 (SD+/-5.7). Conclusion: Aggressive fluid rehydration remains the cornerstone of management of cholera. Instead of presenting with a classical BUN/Creatinine ratio of \u3e20:1, Patients with pre-renal failure in cholera may present with a BUN/Creatinine ratio of \u3c15:1

    Precision Agriculture using Internet of thing with Artificial intelligence: A Systematic Literature Review

    Machine learning with its high precision algorithms, Precision agriculture (PA) is a new emerging concept nowadays. Many researchers have worked on the quality and quantity of PA by using sensors, networking, machine learning (ML) techniques, and big data. However, there has been no attempt to work on trends of artificial intelligence (AI) techniques, dataset and crop type on precision agriculture using internet of things (IoT). This research aims to systematically analyze the domains of AI techniques and datasets that have been used in IoT based prediction in the area of PA. A systematic literature review is performed on AI based techniques and datasets for crop management, weather, irrigation, plant, soil and pest prediction. We took the papers on precision agriculture published in the last six years (2013-2019). We considered 42 primary studies related to the research objectives. After critical analysis of the studies, we found that crop management; soil and temperature areas of PA have been commonly used with the help of IoT devices and AI techniques. Moreover, different artificial intelligence techniques like ANN, CNN, SVM, Decision Tree, RF, etc. have been utilized in different fields of Precision agriculture. Image processing with supervised and unsupervised learning practice for prediction and monitoring the PA are also used. In addition, most of the studies are forfaiting sensory dataset to measure different properties of soil, weather, irrigation and crop.     Role of community engagement in maternal health in rural Pakistan: Findings from the CLIP randomized trial

    Background: Community-based strategies to promote maternal health can help raise awareness of pregnancy danger signs and preparations for emergencies. The objective of this study was to assess change in birth preparedness and complication readiness (BPCR) and pregnant women\u27s knowledge about pre-eclampsia as part of community engagement (CE) activities in rural Pakistan during the Community Level Interventions for Pre-eclampsia (CLIP) Trial.Methods: The CLIP Trial was a cluster randomized controlled trial that aimed to reduce maternal and perinatal morbidity and mortality using CE strategies alongside mobile health-supported care by community health care providers. CE activities engaged pregnant women at their homes and male stakeholders through village meetings in Hyderabad and Matiari in Sindh, Pakistan. These sessions covered pregnancy complications, particularly pre-eclampsia/eclampsia, BPCR and details of the CLIP intervention package. BPCR was assessed using questions related to transport arrangement, permission for care, emergency funds, and choice of facility birth attendant for delivery during quarterly household surveys. Outcomes were assessed via multilevel logistic regression with adjustment for relevant confounders with effects summarized as odds ratios and 95% confidence intervals.Results: There were 15 137 home-based CE sessions with pregnant women and families (n = 46 614) and 695 village meetings with male stakeholders (n = 7784) over two years. The composite outcomes for BPCR and pre-eclampsia knowledge did not differ significantly between trial arms. However, CE activities were associated with improved pre-eclampsia knowledge in some areas. Specifically, pregnant women in the intervention clusters were twice as likely to know that seizures could be a complication of pregnancy (odds ratio (OR) = 2.17, 95% confidence interval (CI) = 1.11, 4.23) and 2.5 times more likely to know that high blood pressure is potentially life-threatening during pregnancy (OR = 2.52, 95% CI = 1.31, 4.83) vs control clusters.Conclusions: The findings suggested that a CE strategy for male and female community stakeholders increased some measures of knowledge regarding complications of pre-eclampsia in low-resource settings. However, the effect of this intervention on long-term health outcomes needs further study.Trial registration: Clinical Trials.gov - INCT01911494

    Building Resilience Against ViolencE (BRAVE): protocol of a parenting intervention for mothers and fathers with post-traumatic stress disorder in Pakistan

    Abstract Background Prevalence of post-traumatic stress disorder (PTSD) is high in Pakistan both due to natural disasters and ongoing conflicts. Offspring of trauma survivors are at increased risk for mental and physical illnesses. Parental PTSD has been linked to troubled parent–child relationships, behaviour problems, trauma symptoms, and depression in children. This study aims to explore the acceptability, feasibility and indications of the effectiveness of group learning through play plus trauma-focused cognitive behaviour therapy (LTP Plus TF-CBT) for parents experiencing PTSD. Methods/Design This is a two-arm pilot cluster randomised controlled trial (RCT). We aim to recruit 300 parents with a diagnosis of PTSD. The screening will be done using the Impact of Event Scale-Revised. Diagnosis of PTSD will be confirmed using the Clinician-Administered PTSD Scale-5 (CAPS-5). Union Councils from Peshawar and Karachi will be randomised into either group LTP Plus TF CBT arm or treatment as usual (TAU). The intervention includes 12 sessions of LTP Plus TF-CBT delivered weekly in the first 2 months and then fortnightly in a group setting by trained psychologists. The groups will be co-facilitated by the community health workers (CHWs). Parents will be assessed at baseline and 4th month (end of the intervention), using the Patient Health Questionnaire (PHQ-9), Generalised Anxiety Disorder (GAD-7) Scale, Client Service Receipt Inventory (CSRI), and Ages and Stages Questionnaire (ASQ-3) Discussion This trial would help build an understanding of the acceptability, feasibility and indications of the effectiveness of a low-cost parenting intervention
