    Modification of cellulose ether with organic carbonate for enhanced thermal and rheological properties: Characterization and analysis

    Reduction in viscosity at higher temperatures is the main limitation of utilizing cellulose ethers in high thermal reservoir conditions for petroleum industry applications. In this study, cellulose ether (hydroxyethyl methyl cellulose (HEMC)) is modified using organic carbonates, i.e., propylene carbonate (PC) and diethyl carbonate (DEC), to overcome the limitation of reduced viscosity at high temperatures. The polymer composites were characterized through various analytical techniques, including Fourier-transform infrared (FTIR), H-NMR, X-ray diffraction (XRD), scanning electron microscope (SEM), thermogravimetric analysis (TGA), differential scanning calorimetry (DSC), -potential measurement, molecular weight determination, and rheology measurements. The experimental results of structural and morphological characterization confirm the modification and formation of a new organic carbonate-based cellulose ether. The thermal analysis revealed that the modified composites have greater stability, as the modified samples demonstrated higher vaporization and decomposition temperatures. -potential measurement indicates higher stability of DEC- and PC-modified composites. The relative viscometry measurement revealed that the modification increased the molecular weight of PC- and DEC-containing polymers, up to 93,000 and 99,000 g/moL, respectively. Moreover, the modified composites exhibited higher levels of stability, shear strength and thermal resistance as confirmed by viscosity measurement through rheology determination. The observed increase in viscosity is likely due to the enhanced inter- and intramolecular interaction and higher molecular weight of modified composites. The organic carbonate performed as a transesterification agent that improves the overall properties of cellulose ether (HEMC) at elevated temperatures as concluded from this study. The modification approach in this study will open the doors to new applications and will be beneficial for substantial development in the petroleum industry

    WHAT DIABETICS KNOW ABOUT DIABETES?

    Patient’s lack of understanding of disease hinders in proper control of disease thus increasing the morbidity and mortality. Review of the studies done in this region, dose not provide a detail account of patients understanding about the disease. Majority of patients when interviewed in clinics do not know the right answer to cause, level of desired sugar, possible complication. Material and methods: Hospital-based cross-sectional KAP study was conducted on diabetic patients visiting the diabetic clinic in two tertiary care Hospitals and a diabetic poly clinic in different areas of Karachi during the month of September 2012. Results: The mean age was 56 years, with 122 male and 118 female, 40.4 % patients were uneducated, 22.9% metric, 12.9% intermediate and 22.9% were graduate. Considering the knowledge, as to what is diabetes? only 46% answered correctly. What could cause the disease? 35% thought there was no reason for sugar. Regarding most frequent symptom, 27% thought there were no specific symptoms, 24.2% weight loss and 23.8% excessive urine and 21.7% thought not healing of wound was first indicator and 2.1 % thought that increase appetite is the cause. The desired levels of random sugar only 24.2% thought it be around 180 mg /dl while 32.5% had no idea about the value. Desired fasting sugar was correctly known by 34.6% as less than 100 mg/dl and 22.9% as less than 120 mg/dl. , Duration of treatment was believed to be life long treatment by 85%.Oral hypoglycemic were consumed by 68%, 24% were on insulin. And 6% were using both. 75% patients did not do exercise regularly. Regarding the harms created by diabetes, 51.3 % knew that heart or kidneys may get damaged. Regular doctor visits were done by 80% , and 85 % were satisfied with the level of care. The average sugar check cost was less than 100 rupees for 62.1 % . Conclusion: Lack of proper concepts regarding the disease, desired level of sugars, possible complications could be a big hinder in achieving good diabetic control. Thus by addressing the specific deficiencies in the knowledge and practices of patients, a better outcome may be achievable

    Impact of service provision platforms on maternal and newborn health in conflict areas and their acceptability in Pakistan: a systematic review

    Various models and strategies have been implemented over the years in different parts of the world to improve maternal and newborn health (MNH) in conflict affected areas. These strategies are based on specific needs and acceptability of local communities. This paper has undertaken a systematic review of global and local (Pakistan) information from conflict areas on platforms of health service provision in the last 10 years and information on acceptability from local stakeholders on effective models of service delivery; and drafted key recommendations for improving coverage of health services in conflict affected areas. The literature search revealed ten studies that described MNH service delivery platforms. The results from the systematic review showed that with utilisation of community outreach services, the greatest impacts were observed in skilled birth attendance and antenatal consultation rates. Facility level services, on the other hand, showed that labour room services for an internally displaced population (IDP) improved antenatal care coverage, contraceptive prevalence rate and maternal mortality. Consultative meetings and discussions conducted in Quetta and Peshawar (capitals of conflict affected provinces) with relevant stakeholders revealed that no systematic models of MNH service delivery, especially tailored for conflict areas, are available. During conflict, even previously available services and infrastructure suffered due to various barriers specific to times of conflict and unrest. A number of barriers that hinder MNH services were discussed. Suggestions for improving MNH services in conflict areas were also laid down by participants. The review identified some important steps that can be undertaken to mitigate the effects of conflict on MNH services, which include: improve provision and access to infrastructure and equipment; development and training of healthcare providers; and advocacy at different levels for free access to healthcare services and for the introduction of the programme model in existing healthcare system. The obligation is enormous, however, for a sustainable programme, it is important to work closely with both the IDP and host community, and collaborating with the government and non-government organisations

    Impact of local terminology on the design of a community-based diagnostic and management algorithm for postpartum sepsis: Findings of formative research

    Background: Postpartum sepsis is one of the leading causes of maternal mortality and morbidity in developing countries. This formative research elicits local terms used for postpartum illnesses and symptoms of postpartum sepsis with the aim of improving postpartum diagnosis and management in Pakistan.Methods: We conducted 34 in-depth interviews with recently delivered women (RDW), traditional birth attendants (TBAs), healthcare providers and family members of RDW from rural Sindh to explore local Sindhi terms used to describe postpartum sepsis and related symptoms. During interviews, all participants were asked to orally free list common symptoms of postpartum illnesses; those who were aware of the concept were asked to free list possible symptoms of postpartum sepsis. The responses were recorded by the interviewer. Free listing data were analyzed for frequency and salience.Results: Most participants, including TBAs, were not familiar with the concept of postpartum sepsis as a distinct disease or of a local term denoting the concept. Almost all could identify and report symptoms related to postpartum sepsis in the local language. Only physicians were able to recognize the term postpartum sepsis and related symptoms. Multiple local terms were used for a particular symptom; still others were used to denote gradations of severity. \u27Bukhar\u27 (fever) was the most commonly named symptom although it was often considered a normal part of puerperium. Many postpartum illnesses were related to the highly non-specific local term \u27kamzori\u27 (weakness).Conclusions: Better awareness about local terminology used in rural areas related to postpartum sepsis could improve communication, care-seeking patterns, diagnosis and management

    A Novel Low-Cost Compact High-Performance Flower-Shaped Radiator Design for Modern Smartphone Applications

    This manuscript examines the design principle and real-world validation of a new miniaturized high-performance flower-shaped radiator (FSR). The antenna prototype consists of an ultracompact square metallic patch of 0.116λ0 × 0.116λ0 (λ0 is the free space wavelength at 3.67 GHz), a rectangular microstrip feed network, and a partial metal ground plane. A novel, effective, and efficient approach based on open circuit loaded stubs is employed to achieve the antenna’s optimal performance features. Rectangular, triangular, and circular disc stubs were added to the simple structure of the square radiator, and hence, the FSR configuration was formed. The proposed antenna was imprinted on a low-cost F4B laminate with low profile thickness of 0.018λ0, relative permittivity εr = 2.55, and dielectric loss tangent δ = 0.0018. The designed radiator has an overall small size of 0.256λ0 × 0.354λ0. The parameter study of multiple variables and their influence on the performance results has been extensively studied. Moreover, the impact of different substrate materials, impedance bandwidths, resonance tuning, and impedance matching has also been analyzed. The proposed antenna model has been designed, simulated, and fabricated. The designed antenna exhibits a wide bandwidth of 5.33 GHz ranging from 3.67 to 9.0 GHz at 10 dB return loss, which resulted in an 83.6% fractional impedance bandwidth; a maximum gain of 7.3 dBi at 8.625 GHz; optimal radiation efficiency of 89% at 4.5 GHz; strong intensity current flow across the radiator; and stable monopole-like far-field radiation patterns. Finally, a comparison between the scientific results and newly published research has been provided. The antenna’s high-performance simulated and measured results are in a good agreement; hence, they make the proposed antenna an excellent choice for modern smartphones’ connectivity with the sub-6 GHz frequency spectrum of modern fifth-generation (5G) mobile communication application
