Estimating the cost-effectiveness of brief interventions for heavy drinking in primary health care across Europe
Background
Screening and Brief Interventions for alcohol are an effective public health measure to tackle
alcohol-related harm, however relatively few countries across the European Union (EU) have
implemented them widely. This may be due to a lack of understanding of the specific financial
implications of such policies within each country.
Methods
A novel ‘meta-modelling’ approach was developed based on previous SBI cost-effectiveness models
for four EU countries. Data was collected on the key factors which drive cost-effectiveness for all 28
EU countries (mean per capita alcohol consumption, proportion of the population to be screened
over a 10-year SBI programme; per capita alcohol-attributable mortality; per capita alcoholattributable
morbidity; mean cost of an alcohol-related hospitalisation and mean SBI-delivery staff
cost). Regression analysis was used to fit two meta-models estimating net programme costs and
quality-adjusted life-years (QALYs) gained, to calculate cost-effectiveness estimates specific to each
EU country.
Results
Costs are dependent upon the proportion of the population covered by the screening programme,
the country-specific per capita mortality and morbidity rate and the country-specific costs of GP care
and hospitalisation. QALYs depend on the proportion of the population screened and per capita
alcohol consumption. Despite large inter-country variability in factor values, SBI programmes are
likely to be cost-effective in 24 out of 28 EU countries and cost-saving in about 50% of countries.
Conclusion
Implementing national programmes of SBI in primary health care would be a cost-effective means of
reducing alcohol-attributable morbidity and deaths in almost all countries of the EU
Ending the abuse: the human rights implications of obstetric violence and the promise of rights-based policy to realise respectful maternity care
In September 2019, Dubravka Šimonovic, the United Nations (UN) Special Rapporteur on violence against women (VAW), submitted her report to the UN General Assembly on “mistreatment and VAW during reproductive health services with a focus on childbirth and obstetric violence”. This report solidified obstetric violence as a form of VAW – a human rights violation to be addressed by the UN, rather than solely a matter of quality of care for maternal health professionals. The positioning opens new channels for interdisciplinary advocacy to be translated into multisectoral policy. However, the risk is that rights-based approaches remain siloed within the human rights community, rather than serving as a foundation for broadbased policy reforms
Injury Alcohol-Attributable Fractions: Methodological Issues and Developments
Background: Alcohol-attributable fractions (AAFs) are routinely used to estimate the burden of injury resulting from alcohol. Recent methodological advances allow AAFs to be estimated using national survey data. However, this requires assuming that the drinking patterns are equivalent to those used by epidemiological studies estimating the relative risk of injury. This study explores the implications of this assumption and presents an improved method of estimating injury AAFs. Methods: Diary survey is used to describe individuals’ drinking occasions and estimate AAFs. Statistical methods and numerical integration are used to combine the evidence on the risk of injury when intoxicated with the diary data. Alternative assumptions are chosen to explore the implications of using national survey data. Results:Overall, an estimated 27% of road traffic accident (RTA) and 23% of non-RTA injuries in Britain are attributable to alcohol. AAF estimates for RTAs range from 54% to 2% and for non-RTAs from 36% to 8% in men aged 16-24 and women aged 55-64 respectively. Two potentially more realistic assumptions relating to the use of national survey data resulted in substantially lower AAF estimates for RTAs. Conclusion: Current methods of estimating injury AAFs using national survey data are flawed for some harms, particularly RTAs, where the data is not consistent with the epidemiological literature. Our findings indicate that the burden of injuries from RTAs in England has been previously overestimated. Further research into the prevalence of risky behaviours when intoxicated is required to refine these methods and produce more robust burden of injury estimates
Neutron structure function and inclusive DIS from H-3 and He-3 at large Bjorken-x
A detailed study of inclusive deep inelastic scattering (DIS) from mirror A =
3 nuclei at large values of the Bjorken variable x is presented. The main
purpose is to estimate the theoretical uncertainties on the extraction of the
neutron DIS structure function from such nuclear measurements. On one hand,
within models in which no modification of the bound nucleon structure functions
is taken into account, we have investigated the possible uncertainties arising
from: i) charge symmetry breaking terms in the nucleon-nucleon interaction, ii)
finite Q**2 effects neglected in the Bjorken limit, iii) the role of different
prescriptions for the nucleon Spectral Function normalization providing baryon
number conservation, and iv) the differences between the virtual nucleon and
light cone formalisms. Although these effects have been not yet considered in
existing analyses, our conclusion is that all these effects cancel at the level
of ~ 1% for x < 0.75 in overall agreement with previous findings. On the other
hand we have considered several models in which the modification of the bound
nucleon structure functions is accounted for to describe the EMC effect in DIS
scattering from nuclei. It turns out that within these models the cancellation
of nuclear effects is expected to occur only at a level of ~ 3%, leading to an
accuracy of ~ 12 % in the extraction of the neutron to proton structure
function ratio at x ~ 0.7 -0.8$. Another consequence of considering a broad
range of models of the EMC effect is that the previously suggested iteration
procedure does not improve the accuracy of the extraction of the neutron to
proton structure function ratio.Comment: revised version to appear in Phys. Rev. C; main modifications in

Crop loading studies on ‘Caricia’ and ‘Eva’ apples grown in a mild winter area
The crop load level of an apple (Malus × domestica Borkh.) tree impacts fruit yield and quality parameters, tree vigor and biennial bearing. The optimal crop load is that which allows for consistent annual cropping and fruit quality acceptable to the market. We evaluated the effect of crop load on yield and fruit quality of two low-chill apples cv. ‘Caricia’ and ‘Eva’, growing in a mild winter area. During 2010 and 2011 crop load was manually adjusted from 2 or 3 to 17 fruits cm−2 of trunk cross-sectional area (TCSA). Fruit yield was positively related to crop load in both cultivars but mean fruit weight diminished as the crop load increased. For both cultivars, the production of non-commercial and small-sized fruit increased, whereas production of middle-sized fruit diminished as the fruit load increased. Shoot length was not affected by crop load in ‘Eva’ whereas it was reduced in ‘Caricia’. Red skin color (RSC %) had a quadratic response to crop load in ‘Caricia’. On the other hand, the RSC % of ‘Eva’ fruit was adjusted to a negative logarithmic model as an effect of crop load increment. No biennial bearing was observed in either cultivar. This research study suggests that the maximum limit of crop load for both cultivars is 7 fruits cm−2 of TCSA, and the lower limit of crop load was 3 fruits cm−2 of TCSA for ‘Eva’ and 5 fruits cm−2 of TCSA for ‘Caricia’
Model-independent search for CP violation in D0→K−K+π−π+ and D0→π−π+π+π− decays
A search for CP violation in the phase-space structures of D0 and View the MathML source decays to the final states K−K+π−π+ and π−π+π+π− is presented. The search is carried out with a data set corresponding to an integrated luminosity of 1.0 fb−1 collected in 2011 by the LHCb experiment in pp collisions at a centre-of-mass energy of 7 TeV. For the K−K+π−π+ final state, the four-body phase space is divided into 32 bins, each bin with approximately 1800 decays. The p-value under the hypothesis of no CP violation is 9.1%, and in no bin is a CP asymmetry greater than 6.5% observed. The phase space of the π−π+π+π− final state is partitioned into 128 bins, each bin with approximately 2500 decays. The p-value under the hypothesis of no CP violation is 41%, and in no bin is a CP asymmetry greater than 5.5% observed. All results are consistent with the hypothesis of no CP violation at the current sensitivity
Search for the lepton-flavor-violating decays Bs0→e±μ∓ and B0→e±μ∓
A search for the lepton-flavor-violating decays Bs0→e±μ∓ and B0→e±μ∓ is performed with a data sample, corresponding to an integrated luminosity of 1.0 fb-1 of pp collisions at √s=7 TeV, collected by the LHCb experiment. The observed number of Bs0→e±μ∓ and B0→e±μ∓ candidates is consistent with background expectations. Upper limits on the branching fractions of both decays are determined to be B(Bs0→e±μ∓)101 TeV/c2 and MLQ(B0→e±μ∓)>126 TeV/c2 at 95% C.L., and are a factor of 2 higher than the previous bounds
Measurements of long-range near-side angular correlations in TeV proton-lead collisions in the forward region
Two-particle angular correlations are studied in proton-lead collisions at a
nucleon-nucleon centre-of-mass energy of TeV, collected
with the LHCb detector at the LHC. The analysis is based on data recorded in
two beam configurations, in which either the direction of the proton or that of
the lead ion is analysed. The correlations are measured in the laboratory
system as a function of relative pseudorapidity, , and relative
azimuthal angle, , for events in different classes of event
activity and for different bins of particle transverse momentum. In
high-activity events a long-range correlation on the near side, , is observed in the pseudorapidity range . This
measurement of long-range correlations on the near side in proton-lead
collisions extends previous observations into the forward region up to
. The correlation increases with growing event activity and is found
to be more pronounced in the direction of the lead beam. However, the
correlation in the direction of the lead and proton beams are found to be
compatible when comparing events with similar absolute activity in the
