Case Study: Cancrum oris (noma) in a malnourished HIV-positive child from rural Kwazulu-Natal
Cancrum oris (noma – derived from the Greek nomein, ‘to devour\') is an infectious disease with a fulminating course that destroys the oro-facial tissues and other neighbouring structures.1 Although cancrum oris can occur at any age, it is most commonly in malnourished children between the ages of 1 and 5 years whose general health has been further weakened by some infectious disease, usually measles but also tuberculosis, gastro-enteritis, typhoid, whooping cough, or malignant disease such as leukaemia. The possible relevance to HIV has not been fully investigated. This report details a case presenting to East Griqualand and Usher Memorial Hospital, Kokstad, KwaZulu-Natal.

Baseline chest radiographic features of HIV-infected children eligible for antiretroviral therapy
Background. South Africa’s HIV mortality is primarily due to pulmonary disease. No evidence exists regarding a correlation between specific chest radiographic patterns and CD4 levels of immunity in HIV-infected children.Objectives. We aimed to determine the prevalence of specific radiographic features in HIV-infected children initiating antiretroviral therapy (ART) to develop a guideline of expected baseline radiographic appearances, and the radiographic features that predominate at specific levels of immune suppression (defined by CD4 percentage ranges), which would narrow the radiological differential diagnosis.Method. Retrospective review of the baseline chest radiographs of 92 consecutive paediatric outpatients initiating ART.Results. Normal radiographs were reported in 54% of patients. Those with radiographic abnormalities had parenchymal disease (34%), mediastinal disease (22%) and pleural disease (1%). Parenchymal disease was predominantly air space (28%), and mediastinal disease was predominantly cardiomegaly (21%); lymphadenopathy was rare (1%). Radiological appearances of TB were seen in 9% of patients. A statistically significant association was shown between immune suppression and air space disease (p=0.049) with a relative risk of 0.46 (95% CI 0.24 - 0.88) for air space disease in immune-suppressed children. This association was independent of age.Conclusion. Baseline chest radiographs in paediatric outpatients presenting for initiation of ART are predominantly normal, but also demonstrate a significant number of pathological radiological features – primarily air space disease and cardiomegaly. The only statistically significant association between radiographic features and immune suppression was air space disease, which correlated with a higher level of immunity.S Afr Med J 2011;101:829-834
Issue in Child Health: Can a new paediatric sub-specialty improve child health in South Africa?
Compared with other middle-income countries, child health in South Africa is in a poor state, and should be addressed by focusing on the healthcare needs of all children across a system or region. Paediatricians have had little effect on this situation, partly because their training is not aligned with South African needs. The proposed re-engineering of primary healthcare will belimited by the skewed distribution of staff and the lack of suitable skills. A ‘community’ placement during specialist training, and the creation of a sub-specialty in Community Paediatrics and Child Health, could address the skills shortage and possibly attract health personnel to under-served areas through creating an appropriate career path. This proposal would also support the Department of Health’s encouraging plans to re-engineer primary healthcare
Child mortality in South Africa: Fewer deaths, but better data are needed
South Africa is committed to reducing under-5 mortality rates in line with the Sustainable Development Goal (SDG) targets. Policymakers and healthcare service managers require accurate and complete data on the number and causes of child deaths to plan and monitor healthcare service delivery and health outcomes. This study aimed to review nationally representative data on under-5 mortality and the cause of deaths among children under 5 years of age. We also reviewed systems that are currently used for generating these data. Child mortality has declined substantially in the past decade. Under-5 mortality in 2015 is estimated at 37 - 40 deaths per 1 000 live births, with an estimated infant mortality rate of 27 - 33 deaths per 1 000 live births. Approximately one-third of under-5 deaths occur during the newborn period, while diarrhoea, pneumonia and HIV infection remain the most important causes of death outside of the newborn period. The proportion of deaths owing to non-natural causes, congenital disorders and non-communicable diseases has increased. However, many discrepancies in data collected through different systems are noted, especially at the sub-national level. There is a need to improve the completeness and accuracy of existing data systems and to strengthen reconciliation and triangulation of data
Need for timely paediatric HIV treatment within primary health care in rural South Africa
<p>Background: In areas where adult HIV prevalence has reached hyperendemic levels, many infants remain at risk of acquiring HIV infection. Timely access to care and treatment for HIV-infected infants and young children remains an important challenge. We explore the extent to which public sector roll-out has met the estimated need for paediatric treatment in a rural South African setting.</p>
<p>Methods: Local facility and population-based data were used to compare the number of HIV infected children accessing HAART before 2008, with estimates of those in need of treatment from a deterministic modeling approach. The impact of programmatic improvements on estimated numbers of children in need of treatment was assessed in sensitivity analyses.</p>
<p>Findings: In the primary health care programme of HIV treatment 346 children <16 years of age initiated HAART by 2008; 245(70.8%) were aged 10 years or younger, and only 2(<1%) under one year of age. Deterministic modeling predicted 2,561 HIV infected children aged 10 or younger to be alive within the area, of whom at least 521(20.3%) would have required immediate treatment. Were extended PMTCT uptake to reach 100% coverage, the annual number of infected infants could be reduced by 49.2%.</p>
<p>Conclusion: Despite progress in delivering decentralized HIV services to a rural sub-district in South Africa, substantial unmet need for treatment remains. In a local setting, very few children were initiated on treatment under 1 year of age and steps have now been taken to successfully improve early diagnosis and referral of infected infants.</p>
Dissecting the Active Site of the Collagenolytic Cathepsin L3 Protease of the Invasive Stage of Fasciola hepatica
Background: A family of secreted cathepsin L proteases with differential activities is essential for host colonization and survival in the parasitic flatworm Fasciola hepatica. While the blood feeding adult secretes predominantly FheCL1, an enzyme with a strong preference for Leu at the S2 pocket of the active site, the infective stage produces FheCL3, a unique enzyme with collagenolytic activity that favours Pro at P2. Methodology/Principal Findings: Using a novel unbiased multiplex substrate profiling and mass spectrometry methodology (MSP-MS), we compared the preferences of FheCL1 and FheCL3 along the complete active site cleft and confirm that while the S2 imposes the greatest influence on substrate selectivity, preferences can be indicated on other active site subsites. Notably, we discovered that the activity of FheCL1 and FheCL3 enzymes is very different, sharing only 50% of the cleavage sites, supporting the idea of functional specialization. We generated variants of FheCL1 and FheCL3 with S2 and S3 residues by mutagenesis and evaluated their substrate specificity using positional scanning synthetic combinatorial libraries (PS-SCL). Besides the rare P2 Pro preference, FheCL3 showed a distinctive specificity at the S3 pocket, accommodating preferentially the small Gly residue. Both P2 Pro and P3 Gly preferences were strongly reduced when Trp67 of FheCL3 was replaced by Leu, rendering the enzyme incapable of digesting collagen. In contrast, the inverse Leu67Trp substitution in FheCL1 only slightly reduced its Leu preference and improved Pro acceptance in P2, but greatly increased accommodation of Gly at S3. Conclusions/Significance: These data reveal the significance of S2 and S3 interactions in substrate binding emphasizing the role for residue 67 in modulating both sites, providing a plausible explanation for the FheCL3 collagenolytic activity essential to host invasion. The unique specificity of FheCL3 could be exploited in the design of specific inhibitors selectively directed to specific infective stage parasite proteinases. © 2013 Corvo et al
