    Abstract: The essence of human existence depends on the environment and fellow humans. The activity of human life takes place in space and to show its existence human beings always create space, so that there is a process of human settlement in a certain place. The culture of settling in the people of East Nusa Tenggara creates certain patterned settlements. The phenomenon of living culture-based occurs in the Sikka community. Space philosophy emerged in the process of building houses and villages in the Sikka community. This paper presents the meaning of space in the culture of the settlements of the Sikka community. Research carried out with a qualitative paradigm, presented descriptively and associated with relevant literature. As a result, the culture settled and built the residence of the tribes within the village (Wisung-wagang), kampung, magical concepts (Lero and kala), and Amerta were closely related to the concepts and meaning of space according to the culture of the Sikka community.Keywords:space, culture, house, Sikka communityAbstrak: Hakekat keberadaan manusia bergantung pada lingkungan dan sesama manusia. Budaya bermukim pada masyarakat Nusa Tenggara Timur menciptakan permukiman berpola tertentu. Fenomena bermukim berbasis budaya terjadi di kalangan masyarakat Sikka. Filosofi ruang muncul pada proses pembangunan rumah maupun kampung di kalangan masyarakat Sikka. Tulisan ini memaparkan makna ruang dalam budaya bermukim masyarakat Sikka. Penelitian dilakukan dengan paradigma kualitatif, disajikan secara deskriptif dan dikaitkan dengan pustaka yang relevan.     Uncertain pasts and risk-sensitive futures in sub-Saharan urban transformation

    This chapter explores the status and the scope for transition of risk- sensitive and transformative urban development in diverse cities of sub-Saharan Africa. Sub-Saharan Africa is important because of its large proportions of urban populations with high vulnerability and growing exposure to risks. High rates of urban growth pose increasing risks as we go into the future, yet there is also opportunity to reduce risk through integrating risk management into development. However, this opportunity space is often constrained by limited capacities to plan and manage the rapid urbanisation process, particularly in informal settlements. Limited capacities to prevent processes of risk accumulation pose threats to poverty reduction and sustainable development. In this context, there is an increasingly urgent need for squarely recognising and addressing the underlying vulnerabilities of urban populations and their root causes. Transitioning towards such sustainable urban pathways will require the strengthening of capacities and accountability of city authorities and broader governance systems, both formal and informal

    Complex Urban Systems for Sustainability and Health: A structured approach to support the development and implementation of city policies for population and planetary health

    Context: The multi-disciplinary and multi-partner CUSSH project (Complex Urban Systems for Sustainability and Health) seeks to support cities to take transformative action towards population and planetary health goals. Rationale: As all cities are complex systems with unique contexts and priorities, our approach is to engage with partner cities in a participatory process to build a shared understanding of relevant systems that will inform the development and implementation of new city policies. Description: Six partner cities were selected to represent larger and smaller cities from across the global spectrum of income: London (UK) and Rennes (France) in Europe, Nairobi and Kisumu in Kenya, and Beijing and Ningbo in China. In each setting we are engaging stakeholders in a broadly similar structured approach that integrates evidence gathering and modelling, participatory engagement framework, and ongoing tracking and evaluation. In addition, we are developing a working theory of change in each setting to explain how and why the chosen policies may work. Achievements: Our city engagement to date has focused on indoor air pollution (Nairobi), green infrastructure (London) and GHG emissions (Rennes), where findings highlighted not only multiple pathways by which policy interventions could affect health, but also potential counter-intuitive influences and tensions, and synergistic opportunities for solving both sustainability and health problems

    Improving the health and welfare of people who live in slums

    Summary In the first paper in this Series we assessed theoretical and empirical evidence and concluded that the health of people living in slums is a function not only of poverty but of intimately shared physical and social environments. In this paper we extend the theory of so-called neighbourhood effects. Slums offer high returns on investment because beneficial effects are shared across many people in densely populated neighbourhoods. Neighbourhood effects also help explain how and why the benefits of interventions vary between slum and non-slum spaces and between slums. We build on this spatial concept of slums to argue that, in all low-income and-middle-income countries, census tracts should henceforth be designated slum or non-slum both to inform local policy and as the basis for research surveys that build on censuses. We argue that slum health should be promoted as a topic of enquiry alongside poverty and health

    A system dynamics-based scenario analysis of residential solid waste management in Kisumu, Kenya

    The problem of solid waste management presents an issue of increasing importance in many low-income settings, including the progressively urbanised context of Kenya. Kisumu County is one such setting with an estimated 500 t of waste generated per day and with less than half of it regularly collected. The open burning and natural decay of solid waste is an important source of greenhouse gas (GHG) emissions and atmospheric pollutants with adverse health consequences. In this paper, we use system dynamics modelling to investigate the expected impact on GHG and PM_{2.5} emissions of (i) a waste-to-biogas initiative and (ii) a regulatory ban on the open burning of waste in landfill. We use life tables to estimate the impact on mortality of the reduction in PM_{2.5} exposure. Our results indicate that combining these two interventions can generate over 1.1 million tonnes of cumulative savings in GHG emissions by 2035, of which the largest contribution (42%) results from the biogas produced replacing unclean fuels in household cooking. Combining the two interventions is expected to reduce PM_{2.5} emissions from the waste and residential sectors by over 30% compared to our baseline scenario by 2035, resulting in at least around 1150 cumulative life years saved over 2021–2035. The contribution and novelty of this study lies in the quantification of a potential waste-to-biogas scenario and its environmental and health impact in Kisumu for the first time

    A spectrum of methods for a spectrum of risk: Generating evidence to understand and reduce urban risk in sub-Saharan Africa

    Many African towns and cities face a range of hazards, which can best be described as representing a “spectrum of risk” of events that can cause death, illness or injury, and impoverishment. Yet despite the growing numbers of people living in African urban centres, the extent and relative severity of these different risks is poorly understood. This paper provides a rationale for using a spectrum of methods to address this spectrum of risk, and demonstrates the utility of mixed‐methods approaches in planning for resilience. It describes activities undertaken in a wide‐ranging multi‐country programme of research, which use multiple approaches to gather empirical data on risk, in order to build a stronger evidence base and provide a more solid base for planning and investment. It concludes that methods need to be chosen in regard to social, political economic, biophysical and hydrogeological context, while also recognising the different levels of complexity and institutional capacity in different urban centres. The paper concludes that as well as the importance of taking individual contexts into account, there are underlying methodological principles – based on multidisciplinary expertise and multi‐faceted and collaborative research endeavours – that can inform a range of related approaches to understanding urban risk in sub‐Saharan Africa and break the cycle of risk accumulation

    Towards risk-sensitive and transformative urban development in sub Saharan Africa

    Risk-sensitive urban development is required to reduce accumulated risk and to better consider risk when planning new developments. To deliver a sustainable city for all requires a more frank and comprehensive focus on procedure: On who makes decisions, under which frameworks, based upon what kind of data or knowledge, and with what degree and direction of accountability? Acting on these procedural questions is the promise of transformative urban development. This paper explores the status of risk sensitive and transformative urban development and the scope for transition towards these components of sustainability in urban sub-Saharan Africa through the lens of diverse city cases: Karonga (Malawi), Ibadan (Nigeria), Niamey (Niger) and Nairobi (Kenya). The paper draws from a 3-year research and capacity building programme called Urban Africa: Risk Knowledge that aims to address gaps in data, understandings and capacity to break cycles of risk accumulation. A common analytical framework is presented to help identify blockages and opportunities for transition towards a risk-sensitive and transformative urban development. This framework is then illustrated through each city in turn and a concluding discussion reflects on city observations to draw out recommendations for city level and wider action and research partnerships

    Disparities in access to diagnosis and care in Blantyre, Malawi, identified through enhanced tuberculosis surveillance and spatial analysis

    BACKGROUND: A sizeable fraction of tuberculosis (TB) cases go undiagnosed. By analysing data from enhanced demographic, microbiological and geospatial surveillance of TB registrations, we aimed to identify modifiable predictors of inequitable access to diagnosis and care. METHODS: Governmental community health workers (CHW) enumerated all households in 315 catchment areas during October-December 2015. From January 2015, government TB Officers routinely implemented enhanced TB surveillance at all public and private TB treatment registration centres within Blantyre (18 clinics in total). This included collection from registering TB patients of demographic and clinical characteristics, a single sputum sample for TB microscopy and culture, and geolocation of place of residence using an electronic satellite map application. We estimated catchment area annual TB case notification rates (CNRs), stratified by microbiological status. To identify population and area-level factors predictive of CHW catchment area TB case notification rates, we constructed Bayesian spatially autocorrelated regression models with Poisson response distributions. Worldpop data were used to estimate poverty. RESULTS: In total, the 315 CHW catchment areas comprised 753,489 people (range 162 to 13,066 people/catchment area). Between 2015 and 2017, 6077 TB cases (61% male; 99% HIV tested; 67% HIV positive; 55% culture confirmed) were geolocated, with 3723 (61%) resident within a CHW catchment area. In adjusted models, greater distance to the nearest TB registration clinic was negatively correlated with TB CNRs, which halved for every 3.2-fold (95% CI 2.24-5.21) increase in distance. Poverty, which increased with distance from clinics, was negatively correlated with TB CNRs; a 23% increase (95% CI 17-34%) in the mean percentage of the population living on less than US$2 per day corresponded to a halving of the TB case notification rates. CONCLUSIONS: Using enhanced surveillance of TB cases in Blantyre, we show an ecological relationship consistent with an 'inverse care law' whereby poorer neighbourhoods and those furthest from TB clinics have lower relative CNRs. If confirmed as low case detection, then pro-poor strategies to facilitate equitable access to TB diagnosis and treatment are required

    Developing a programme theory for a transdisciplinary research collaboration: Complex Urban Systems for Sustainability and Health [version 1; peer review: 2 approved]

    Background: Environmental improvement is a priority for urban sustainability and health and achieving it requires transformative change in cities. An approach to achieving such change is to bring together researchers, decision-makers, and public groups in the creation of research and use of scientific evidence. / Methods: This article describes the development of a programme theory for Complex Urban Systems for Sustainability and Health (CUSSH), a four-year Wellcome-funded research collaboration which aims to improve capacity to guide transformational health and environmental changes in cities. / Results: Drawing on ideas about complex systems, programme evaluation, and transdisciplinary learning, we describe how the programme is understood to “work” in terms of its anticipated processes and resulting changes. The programme theory describes a chain of outputs that ultimately leads to improvement in city sustainability and health (described in an ‘action model’), and the kinds of changes that we expect CUSSH should lead to in people, processes, policies, practices, and research (described in a ‘change model’). / Conclusions: Our paper adds to a growing body of research on the process of developing a comprehensive understanding of a transdisciplinary, multiagency, multi-context programme. The programme theory was developed collaboratively over two years. It involved a participatory process to ensure that a broad range of perspectives were included, to contribute to shared understanding across a multidisciplinary team. Examining our approach allowed an appreciation of the benefits and challenges of developing a programme theory for a complex, transdisciplinary research collaboration. Benefits included the development of teamworking and shared understanding and the use of programme theory in guiding evaluation. Challenges included changing membership within a large group, reaching agreement on what the theory would be ‘about’, and the inherent unpredictability of complex initiatives

    Trapped in the prison of the mind: notions of climate-induced (im)mobility decision-making and wellbeing from an urban informal settlement in Bangladesh

    The concept of Trapped Populations has until date mainly referred to people ‘trapped’ in environmentally high-risk rural areas due to economic constraints. This article attempts to widen our understanding of the concept by investigating climate-induced socio-psychological immobility and its link to Internally Displaced People’s (IDPs) wellbeing in a slum of Dhaka. People migrated here due to environmental changes back on Bhola Island and named the settlement Bhola Slum after their home. In this way, many found themselves ‘immobile’ after having been mobile—unable to move back home, and unable to move to other parts of Dhaka, Bangladesh, or beyond. The analysis incorporates the emotional and psychosocial aspects of the diverse immobility states. Mind and emotion are vital to better understand people’s (im)mobility decision-making and wellbeing status. The study applies an innovative and interdisciplinary methodological approach combining Q-methodology and discourse analysis (DA). This mixed-method illustrates a replicable approach to capture the complex state of climate-induced (im)mobility and its interlinkages to people’s wellbeing. People reported facing non-economic losses due to the move, such as identity, honour, sense of belonging and mental health. These psychosocial processes helped explain why some people ended up ‘trapped’ or immobile. The psychosocial constraints paralysed them mentally, as well as geographically. More empirical evidence on how climate change influences people’s wellbeing and mental health will be important to provide us with insights in how to best support vulnerable people having faced climatic impacts, and build more sustainable climate policy frameworks
