Adipokines nouvellement impliquées dans l'obésité humaine : régulation et rôle dans le développement du syndrome métabolique
Accumulation of visceral rather than subcutaneous fat is an established risk factor for the
pathogenesis of the metabolic syndrome. Adipose tissue secretes a variety of bioactive peptides,
collectively referred to “adipokines” which may be involved in these obesity-related disorders.
However, comparison of secretome analysis between visceral adipose tissue of obese individuals
and that of lean ones has not been performed.
In this context, we carried out adipokine secretion profile of human visceral adipose
tissue and examined adipokines differentially secreted between lean and obese subjects. We
identified 6 adipokines as oversecreted by human visceral adipose tissue in obesity: 3
chemokines [Growth-Related Oncogen factor (GRO), Regulated upon Activation Normal T cells
Expressed and Secreted (RANTES), Macrophage Inflammatory Protein-1ß (MIP-1β)], 1 tissue
inhibitor of metalloproteinases (TIMP-1), 1 megakaryocytic growth factor (Thrombopoietin,
TPO) and 1 interleukin (IL-7). Both adipocytes and stromal-vascular cells contributed to their
altered secretion. Enhanced expression of these adipokines in adipocytes did correlate with
several features of the metabolic syndrome. In addition, circulating GRO-α, TPO and TIMP-1
levels were increased in obesity, as already demonstrated for RANTES. As these adipokines are
involved in cardiovascular disease and/or type 2 diabetes, our results may provide new insight
into the pathogenesis of the metabolic syndrome. Eventually, we investigated the molecular
mechanisms leading to this adipokine overproduction. We showed that TNF-α was, over all the
numerous factors secreted by stromal-vascular cells, a crucial determinant of adipokine
dysregulation in visceral adipocytes of obese subjects. This cytokine appears to be a potent and
proximal molecule in the hierarchy of cytokines’ and chemokines’ networks of adipose tissue.
TNF-α, which is oversecreted by stromal-vascular cells in obesity, acts on enlarged adipocytes,
which are hyper-responsive to this triggering signal. This adipocyte hyper-responsiveness results
in adipokine overproduction through TNFR1 and hyper-activation of NF-��B. This amplified
paracrine cross-talk between adipocytes and SVC in human obesity ultimately leads to
worsening of the inflammatory state and the metabolic syndrome. Targeting altered TNF-α
production, events of the TNF-α signalling cascade or the adipocyte hyper-responsiveness may
have therapeutic potential for the management of the metabolic syndrome.L’accumulation préférentielle de tissu adipeux viscéral plutôt que sous-cutané est un
facteur de risque majeur du développement du syndrome métabolique. Un grand nombre de
données suggère fortement l’implication de peptides régulateurs sécrétés par le tissu adipeux,
collectivement appelés “adipokines”, dans la pathogénie de ce syndrome. Toutefois, le
“secretome” du tissu adipeux viscéral de personnes obèses n’a jamais été comparé à celui de
sujets minces.
Dans ce cadre, nous avons déterminé le profil de sécrétion du tissu adipeux omental
humain et recherché quelles adipokines pouvaient être sécrétées de façon différentielle dans
l’obésité. Ainsi, nous avons identifié 6 adipokines hyper-sécrétées par le tissu adipeux omental
des personnes obèses : 3 chémokines (GRO, RANTES, MIP-1β), 1 inhibiteur de l’activité des
métalloprotéinases (TIMP-1), 1 facteur de croissance mégacaryocytaire (TPO) et 1 interleukine
(IL-7). Les adipocytes et les cellules stromales-vasculaires contribuent tous deux Ă ces
altérations. De plus, l’expression de ces adipokines dans les adipocytes est positivement corrélée
à plusieurs paramètres du syndrome métabolique. Nous avons également mesuré des
concentrations systémiques de GRO-α, TPO et TIMP-1 plus élevées chez les sujets obèses,
comme cela avait été démontré pour RANTES. Etant donné que ces adipokines sont impliquées
dans la survenue des maladies cardiovasculaires et/ou du diabète de type 2, elles pourraient
constituer un des chaînons manquant entre l’obésité centrale et le développement de ses comorbidités.
Enfin, nous avons mis en évidence certains mécanismes conduisant à la
surexpression des adipokines. Nous avons montré que le TNF-α, parmi les nombreux facteurs
sécrétés par les cellules stromales-vasculaires est un déterminant majeur dans la dérégulation des
adipokines associée à l’obésité. Le TNF-α jouerait donc un rôle puissant et proximal dans la
cascade menant à l’inflammation du tissu adipeux. Ainsi, le TNF-α, dont la sécrétion par les
cellules stromales-vasculaires de sujets obèses est accrue, agit sur les adipocytes hypertrophiques
qui y répondent de façon exacerbée. Cette hyper-réponse des adipocytes hypertrophiques est
médiée par le récepteur TNFR1 et par l’augmentation de l’activation du facteur NF-κB. Il en
résulte une surproduction d’adipokines qui aggrave l’état inflammatoire chronique et le
syndrome métabolique. Dans ce contexte, cibler la production du TNF-α ou la cascade de
signalisation qu’il induit semble rester une stratégie thérapeutique d’actualité dans le cadre du
syndrome métabolique. A la lumière de nos résultats, identifier les facteurs initiant l’hyperréponse

Adipokine dysregulation, adipose tissue inflammation and metabolic syndrome.
Obesity plays a causative role in the pathogenesis of the metabolic syndrome. Adipokines may link obesity to its co-morbidities. Most adipokines with pro-inflammatory properties are overproduced with increasing adiposity, while some adipokines with anti-inflammatory or insulin-sensitizing properties, like adiponectin are decreased. This dysregulation of adipokine production may promote obesity-linked metabolic disorders and cardiovascular disease. Besides considering adipokines, this review will also highlight the cellular key players and molecular mechanisms involved in adipose inflammation. Targeting the changes in the cellular composition of adipose tissue, the underlying molecular mechanisms, and the altered production of adipokines may have therapeutic potential in the management of the metabolic syndrome
Base moléculaire des perturbations circadiennes dans la pathogenèse de maladies métaboliques
Le système circadien permet à l’organisme d’anticiper et de répondre aux changements prédictibles de l’environnement. Les rythmes circadiens sont générés par une horloge moléculaire sensible au statut énergétique et présente dans toutes les cellules. Cette revue illustre l’impact des perturbations du système circadien sur la santé humaine, en s’appuyant sur les données épidémiologiques et cliniques, et reporte les découvertes réalisées grâce aux modèles expérimentaux génétiques. Cibler ce système a le potentiel d’améliorer les thérapies visant à traiter les maladies métaboliques et cardiovasculaires, en particulier chez les individus dont les rythmes sont perturbés (travail posté, décalage horaire chronique, troubles du sommeil, nutrition déséquilibrée et horaires de repas irréguliers…)
Epigenetic modification in chromatin machinery and its deregulation in pediatric brain tumors: Insight into epigenetic therapies
Malignancies are characterized by the reprogramming of epigenetic patterns. This reprogramming includes gains or losses in DNA methylation and disruption of normal patterns of covalent histone modifications, which are associated with changes in chromatin remodeling processes. This review will focus on the mechanisms underlying this reprogramming and, specifically, on the role of histone modification in chromatin machinery and the modifications in epigenetic processes occurring in brain cancer, with a specific focus on epigenetic therapies for pediatric brain tumors
Moleculaire basis van circadiaanse stoornissen bij de pathogenese van metabole aandoeningen
Het circadiaanse systeem stelt het lichaam in staat om te anticiperen en te reageren op voorspelbare veranderingen in de omgeving. De circadiaanse ritmes worden gestuurd door een moleculaire klok, die gevoelig is voor de energetische toestand en die in alle cellen zit. Dit overzicht illustreert de weerslag van afwijkingen van het circadiaanse systeem op de gezondheid van de mens (resultaten van epidemiologische en klinische studies) en beschrijft ontdekkingen die werden gedaan dankzij experimentele genetische modellen. Interventies gericht op dat systeem zouden de behandeling van metabole aandoeningen en hart- en vaataandoeningen kunnen verbeteren, vooral bij mensen bij wie de circadiaanse ritmes verstoord zijn (ploegenarbeid, chronische jetlag, slaapstoornissen, onevenwichtige voeding, eten op onregelmatige uren enz.)
Circadian disruption in the pathogenesis of metabolic syndrome
Metabolic syndrome is a multifactorial process induced by a combination of genetic and environmental factors and recent evidence has highlighted that circadian disruption and sleep loss contribute to disease pathogenesis. Emerging work in experimental genetic models has provided insight into the mechanistic basis for clock disruption in disease. Indeed, disruption of the clock system perturbs both neuroendocrine pathways within the hypothalamus important in feeding and energetics, in addition to peripheral tissues involved in glucose and lipid metabolism. This review illustrates the impact of molecular clock disruptions at the level of both brain and behavior and peripheral tissues, with a focus on how such dysregulation in turn impacts lipid and glucose homeostasis, inflammation and cardiovascular function. New insight into circadian biology may ultimately lead to improved therapeutics for metabolic syndrome and cardiovascular disease in humans
Circadian clock dysfunction in human omental fat links obesity to metabolic inflammation.
To unravel the pathogenesis of obesity and its complications, we investigate the interplay between circadian clocks and NF-ÎşB pathway in human adipose tissue. The circadian clock function is impaired in omental fat from obese patients. ChIP-seq analyses reveal that the core clock activator, BMAL1 binds to several thousand target genes. NF-ÎşB competes with BMAL1 for transcriptional control of some targets and overall, BMAL1 chromatin binding occurs in close proximity to NF-ÎşB consensus motifs. Obesity relocalizes BMAL1 occupancy genome-wide in human omental fat, thereby altering the transcription of numerous target genes involved in metabolic inflammation and adipose tissue remodeling. Eventually, clock dysfunction appears at early stages of obesity in mice and is corrected, together with impaired metabolism, by NF-ÎşB inhibition. Collectively, our results reveal a relationship between NF-ÎşB and the molecular clock in adipose tissue, which may contribute to obesity-related complications
Circadian rhythms and metabolic syndrome: from experimental genetics to human disease.
The incidence of the metabolic syndrome represents a spectrum of disorders that continue to increase across the industrialized world. Both genetic and environmental factors contribute to metabolic syndrome and recent evidence has emerged to suggest that alterations in circadian systems and sleep participate in the pathogenesis of the disease. In this review, we highlight studies at the intersection of clinical medicine and experimental genetics that pinpoint how perturbations of the internal clock system, and sleep, constitute risk factors for disorders including obesity, diabetes mellitus, cardiovascular disease, thrombosis and even inflammation. An exciting aspect of the field has been the integration of behavioral and physiological approaches, and the emerging insight into both neural and peripheral tissues in disease pathogenesis. Consideration of the cell and molecular links between disorders of circadian rhythms and sleep with metabolic syndrome has begun to open new opportunities for mechanism-based therapeutics
