    Fixation of an Anatomically Designed Cementless Stem in Total Hip Arthroplasty

    Purpose. The Anatomic Fiber Metal plus stem (Zimmer) is one of the anatomically designed cementless stems to achieve stable fixation by metaphyseal fit. We studied outcomes of cementless total hip arthroplasty using this stem and possible effects of metaphyseal fit on outcomes. Methods. The cementless total hip arthroplasty using this stem was performed for 155 hips. One hundred and thirty-seven hips of 122 patients were followed for 5 to 16 (mean, 9.7) years and entered into the study. The metaphyseal fit was defined as good or poor in an anteroposterior radiograph after surgery. We studied the fixation of the stem and bone reaction on an anteroposterior radiograph at the final followup. Results. Twelve hips had revision, six acetabular components and six acetabular liners. No stem was revised. The biological fixation of the stem was bone ingrown fixation for 136 hips and unstable for one. The metaphyseal fit was good for 83 hips and poor for 54 hips. There were no differences for stem fixation and bone reaction between the two groups. Conclusions. The fixation of the stem was stable at a mean followup of 9.7 years independently from metaphyseal fit

    Borrowable Fractional Ownership Types for Verification

    Automated verification of functional correctness of imperative programs with references (a.k.a. pointers) is challenging because of reference aliasing. Ownership types have recently been applied to address this issue, but the existing approaches were limited in that they are effective only for a class of programs whose reference usage follows a certain style. To relax the limitation, we combine the approaches of ConSORT (based on fractional ownership) and RustHorn (based on borrowable ownership), two recent approaches to automated program verification based on ownership types, and propose the notion of borrowable fractional ownership types. We formalize a new type system based on the borrowable fractional ownership types and show how we can use it to automatically reduce the program verification problem for imperative programs with references to that for functional programs without references. We also show the soundness of our type system and the translation, and conduct experiments to confirm the effectiveness of our approach.Comment: An extended version of the paper to appear in Proceedings of VMCAI 202

    Possible Excess in Charged Current Events with High-Q^2 at HERA from Stop and Sbottom Production

    We investigate a production process e^+p \to \st X \to \sb W^+ X at HERA, where we consider a decay mode \sb \to \bar{\nu}_e d of the sbottom in the framework of an R-parity breaking supersymmetric standard model. Both processes of the stop production e^+ d \to \st and the sbottom decay \sb \to \bar{\nu}_e d are originated from an R-parity breaking superpotential λ131L^1Q^3Dc^1\lambda'_{131} \hat{L}_1 \hat{Q}_3 \hat{D^c}_1. One of signatures of the process should be a large missing transverse momentum plus multijet events corresponding to hadronic decays of the WW. It is shown that the signal could appear as an event excess in the charged current (CC) processes e+pνXe^+p \to \nu X with the high Q2Q^2 at HERA. We compare expected event distributions with the CC data recently reported by the H1 and ZEUS groups at HERA.     The impact of a new exercise facility on physical activity at the community level: A non-randomized panel study in Japan

    Background Considering that building a sports facility is a major investment to promote population health, it is important to understand whether it is effective in increasing the level of physical activity (PA) in the community. This study examined the impact of building a new multipurpose exercise facility on community-level PA in Japan. Methods This non-randomised panel study compared two sites: an intervention site where a new exercise facility was built (opened after baseline data collection) and a control site where there was no such additional exercise facility. From each site, 3200 adult residents (aged 30–74 years) were randomly selected at baseline (2013) and at follow-up (2015). The number of participants retained for analysis was 845 at baseline and 924 at follow-up for the intervention site, and 821 at baseline and 1018 at follow-up for the control site. The outcomes were participants’ self-reported PA, perceived availability of PA facilities, awareness of others being active, and willingness to engage in PA. We examined the interaction terms between the sites and time of measurement in regression analyses to examine whether the magnitude of change from baseline to follow-up differed between the two sites. Results The changes in the proportion of participants meeting the PA guideline and those engaging in moderate-to-vigorous intensity PA were not significantly different between the intervention and control sites. The intervention site had a greater increase in the proportion of participants who were aware of PA facilities from baseline to follow-up than in the control site. The odds ratio for awareness of others being active approached significance, suggesting that there was a tendency at the intervention site towards a greater increase in the proportion of participants who noticed physically active people. Conclusions This study did not find community-level increases in PA after the construction of the exercise facility. However, a significant improvement in the awareness of PA facilities was observed in the intervention site. A sustained community-level effort to promote PA, possibly including social components, and a further tracking of residents’ PA are needed to take a full advantage of the new exercise facility and to assess its long-term impact.     Analyses of integrated EPID images for on-treatment quality assurance to account for interfractional variations in volumetric modulated arc therapy

    Purpose: To investigate the effects of interfractional variation, such as anatomical changes and setup errors, on dose delivery during treatment for prostate cancer (PC) and head and neck cancer (HNC) by courses of volumetric modulated arc therapy (VMAT) aided by on‐treatment electronic portal imaging device (EPID) images. Methods: Seven patients with PC and 20 patients with HNC who had received VMAT participated in this study. After obtaining photon fluence at the position of the EPID for each treatment arc from on‐treatment integrated EPID images, we calculated the differences between the fluence for the first fraction and each subsequent fraction for each arc. The passing rates were investigated based on a tolerance level of 3% of the maximum fluence during the treatment courses and the correlations between the passing rates and anatomical changes. Results: In PC, the median and lowest passing rates were 99.8% and 95.2%, respectively. No correlations between passing rates and interfractional variation were found. In HNC, the median passing rate of all fractions was 93.0%, and the lowest passing rate was 79.6% during the 35th fraction. Spearman’s correlation coefficients between the passing rates and changes in weight or neck volume were − 0.77 and − 0.74, respectively. Conclusions: Analyses of the on‐treatment EPID images facilitates estimates of the interfractional anatomical variation in HNC patients during VMAT and thus improves assessments of the need for re‐planning or adaptive strategies and the timing thereof

    BAFFアンタゴニストによる強皮症モデルマウスの治療

    金沢大学附属病院強皮症のモデルマウスであるtight skin(TSK/+)マウスおけるBAFFの発現異常を解析し,BAFFシグナルをターゲットとした分子標的療法がTSK/+マウスの治療において有用であるかを検証した.まずTSK/+マウスにおける血清BAFF濃度をELISAにて測定ししたところ,野生型マウスと比べTSK/+マウスでは生後4週,8週と有意に血清BAFF濃度の上昇を認めた.次にBAFFのアンタゴニストであるBAFF-R-Igを新生児マウスの時点から腹腔内投与を開始し,8週齢の時点で解析した.野生型マウスと比べTSK/+マウスでは皮膚疎性結合織が増生しているが,BAFF阻害により,有意に減弱した.またBAFF阻害によりB細胞の分化成熟が阻害され、さらにTSK/+マウスで認められる,高γグロブリン血症および自己抗体の産生が抑制された.TSK/+マウスの皮膚では繊維化促進作用のあるTh2サイトカインが有意であることが知られている.マウス皮膚よりmRNAを抽出し,各サイトカインをreal time RT-PCRにて測定したところ,TSK/+マウスで発現亢進しているTh2サイトカインがBAFF阻害により減弱し,反対にTh1サイトカインが亢進した.B細胞のサイトカイン産生能に対するBAFFの作用を解析するため,脾臓由来B細胞をBAFFとBCRシグナルを誘導するSACの存在下にて培養し上清のサイトカインをELISAにて測定した.適切なBAFF刺激にてB細胞はIL-6とIL-10を産生することが示された.特に,野生型マウスと比べTSK/+マウスでは線維化を促進するサイトカインであるIL-6の産生が有意に亢進していた.BAFFを介したシグナルがTSK/+マウスの病態に関与しているおり,BAFFをターゲットとした分子標的薬が強皮症の治療にも有用である可能性が示唆された.研究課題/領域番号:18799003, 研究期間(年度):2006 – 2007出典:「BAFFアンタゴニストによる強皮症モデルマウスの治療」研究成果報告書 課題番号18799003(KAKEN:科学研究費助成事業データベース(国立情報学研究所))(https://kaken.nii.ac.jp/ja/grant/KAKENHI-PROJECT-18799003/)を加工して作
