A Case of Hepatocellular Carcinoma Metastasizing to the Palatine Tonsil
This report presented the very rare case of a patient with a metastasis in the left palatine tonsil from hepatocellular carcinoma (HCC). A 69-year-old man suffered from an about 20-mm mass in the left palatine tonsil. The biopsy revealed that the mass in the palatine tonsil was a metastatic lesion from HCC. The patient underwent a left palatine tonsillectomy, and metastasis from HCC has not recurred after the operation. Long-term monitoring of the short period SU UMa-type dwarf nova, V844 Herculis
We report on time-resolved CCD photometry of four outbursts of a short-period
SU UMa-type dwarf nova, V844 Herculis. We successfully determined the mean
superhump periods to be 0.05584(64) days, and 0.055883(3) for the 2002 May
superoutburst, and the 2006 April-May superoutburst, respectively. During the
2002 October observations, we confirmed that the outburst is a normal outburst,
which is the first recorded normal outburst in V844 Her. We also examined
superhump period changes during 2002 May and 2006 April-May superoutbursts,
both of which showed increasing superhump period over the course of the plateau
stage. In order to examine the long-term behavior of V844 Her, we analyzed
archival data over the past ten years since the discovery of this binary.
Although photometry is not satisfactory in some superoutbursts, we found that
V844 Her showed no precursors and rebrightenings. Based on the long-term light
curve, we further confirmed V844 Her has shown almost no normal outbursts
despite the fact that the supercycle of the system is estimated to be about 300
days. In order to explain the long-term light curves of V844 Her, evaporation
in the accretion disk may play a role in the avoidance of several normal
outbursts, which does not contradict with the relatively large X-ray luminosity

Chemogenetic attenuation of cortical seizures in nonhuman primates

Because pharmacological and surgical treatments occasionally interfere with normal brain function, a more focused and on-demand approach is desirable. Here we examined the efficacy of a chemogenetic tool—designer receptors exclusively activated by designer drugs (DREADDs)—for treating focal seizure in a nonhuman primate model. Acute infusion of the GABAA receptor antagonist bicuculline into the forelimb region of unilateral primary motor cortex caused paroxysmal discharges with twitching and stiffening of the contralateral arm, followed by recurrent cortical discharges with hemi- and whole-body clonic seizures in two male macaque monkeys. Expression of an inhibitory DREADD (hM4Di) throughout the seizure focus, and subsequent on-demand administration of a DREADD-selective agonist, rapidly suppressed the wide-spread seizures. These results demonstrate the efficacy of DREADDs for attenuating cortical seizure in a nonhuman primate model
Current status of a helicopter transportation system on remote islands for patients undergoing mechanical thrombectomy
Background: Mechanical thrombectomy (MT) is standard treatment for acute ischemic stroke (AIS) with large-vessel occlusion within 6 h of symptom onset to treatment initiation (OTP). Recent trials have extended the therapeutic time window for MT to within 24 h. However, MT treatment remains low in remote areas. Nagasaki Prefecture, Japan has many inhabited islands with no neurointerventionalists. Our hospital on the mainland is a regional hub for eight island hospitals. We evaluated clinical outcomes of MT for patients with AIS on these islands versus on the mainland. Methods: During 2014–2019, we reviewed consecutive patients with AIS who received MT at our hospital. Patients comprised the Islands group and Mainland group. Patient characteristics and clinical outcomes were compared between groups. Results: We included 91 patients (Islands group: 15 patients, Mainland group: 76 patients). Seven patients (46.7%) in the Islands group versus 43 (56.6%) in the Mainland group achieved favorable outcomes. Successful recanalization was obtained in 11 patients (73.3%) on the islands and 67 (88.2%) on the mainland. The median OTP time in the Islands was 365 min. In both the Islands and Mainland groups, the OTP time and successful recanalization were associated with functional outcome. The modified Rankin Scale (mRS) score at 90 days ≤2 was obtained in two patients and mRS = 3 in four patients among eight patients with OTP time >6 h. Conclusions: Few patients with AIS on remote islands have received MT. Although patients who underwent MT on the islands had longer OTP, the clinical outcomes were acceptable. OTP time on remote islands must be shortened, as this is related to functional outcome. In some cases with successful recanalization, a favorable outcome can still be obtained even after 6 h. Even if OTP exceeds 6 h, it is desirable to appropriately select patients and actively perform MT
Far-East Asian Toxoplasma isolates share ancestry with North and South/Central American recombinant lineages

Clonal lineages predominate in Europe, North America, Africa, and China, whereas highly recombinant parasites are endemic in South/Central America. Far East Asian T. gondii isolates are not included in current global population genetic structure analyses at WGS resolution. Here we report a genome-wide population study that compared eight Japanese and two Chinese isolates against representative worldwide T. gondii genomes using POPSICLE, a novel population structure analyzing software. Also included were 7 genomes resurrected from non-viable isolates by target enrichment sequencing. Visualization of the genome structure by POPSICLE shows a mixture of Chinese haplogroup (HG) 13 haploblocks introgressed within the genomes of Japanese HG2 and North American HG12. Furthermore, two ancestral lineages were identified in the Japanese strains; one lineage shares a common ancestor with HG11 found in both Japanese strains and North American HG12. The other ancestral lineage, found in T. gondii isolates from a small island in Japan, is admixed with genetically diversified South/Central American strains. Taken together, this study suggests multiple ancestral links between Far East Asian and American T. gondii strains and provides insight into the transmission history of this cosmopolitan organism
