    Post-mortem examination of Hospital Inpatient COVID-19 Deaths in Lusaka, Zambia - A Descriptive Whole-body Autopsy Series

    BACKGROUND: Since information on the pathology of COVID-19 from sub-Saharan Africa (SSA) remains scarce, the objective of our study was to define the gross pathology and histological features of COVID-19. We report data from 29 whole-body autopsies of COVID-19 deaths occurring in hospitals in Lusaka, Zambia - the first large autopsy case series from Africa. METHODS: We performed a descriptive post-mortem examination study of inpatient COVID-19 related deaths at two hospitals in Lusaka, Zambia. Whole-body autopsies were conducted according to Standard Operating Procedures. Gross and histopathological examinations of all organs were performed. Patient demographics, history, co-morbidities, autopsy gross and microscopic findings, and cause(s) of death were recorded and analyzed using STATA version 14. Variables were grouped and presented as frequencies and percentages. FINDINGS: Autopsies were performed on 29 decedents (mean age = 44 ± 15.8years; age range = 19-82; 17/29 [58.8%] males). 22/29 [75.9%] cases were <55 years of age. A spectrum of pathological manifestations of COVID-19 were seen in all organs. The commonest causes of death were pulmonary thromboembolism (13/29, 45%), Diffuse Alveolar Damage (9/29, 31%), and COVID-19 pneumonia (7/29, 25%). 22/29 (76%) had co-morbidities. Common co-morbidities included HIV (8/29, 28%), Hypertension (6/29, 20%) Tuberculosis (3/29, 10%), Diabetes (3/29, 10%). CONCLUSIONS: A spectrum of gross anatomical and histopathological findings are seen in COVID-19 deaths in hospitalized decedents. These appear broadly similar to those reported from China, Europe and USA. Differences include a younger age group, and co-morbidities of HIV and TB co-infection which require further investigation

    Post-harvest use of banana in Uganda and Tanzania: Product characteristics and cultivar preferences of male and female farmers.

    Understanding user preferred traits is important for the successful development and adoption of improved banana cultivars. This article provides an overview of different banana products, cultivars used to make those products and the preferred characteristics as reported by men and women farmers in different regions of Uganda and Tanzania. Qualitative data from 23 focus group discussions (FGDs) conducted in six districts (Mbarara and Luwero in Uganda; and Bukoba, Meru, Moshi and Rungwe in Tanzania) were coded and analysed. Results show that farmers process banana to make a range of products that include staple foods such as steamed matooke, mbalaga and machalari; alcoholic beverages and snacks. Different cultivars are preferred for an assortment of products. There are minimal differences in the cultivar preferences for specific products and product utilization patterns by men and women farmers. Differences only occurred when men and women described reasons why products are important or preferred, and in the roles of men, women and children in the preparation and processing of products from different banana types. Both men and women mentioned socio-economic, cultural, consumption and health-related attributes of the different products. However, women (73% of women only FGDs) specifically highlighted health benefits attached to women, for example after giving birth, during pregnancy, during lactation and for children; compared to 44% for men. Men pointed out more the socio-cultural importance of the products and provided detailed descriptions on market-related traits and the corresponding value chain actors (88.9% of men only FGDs). A myriad of factors that include variety of products, cultivar attributes before processing, characteristics of the processed product and location differences are identified, highlighting the need for further assessments and physicochemical characterisation of the traits to gauge the feasibility of considering this valuable information in the product development process. Some of the products and attributes of cultivars cited by farmers are generalized descriptions lacking sufficient detail (e.g. ‘tastes nice’, ‘good flavour’, ‘long shelf life’). Farmers have tacit knowledge of these attributes. Hence, researchers need to probe further and use a range of methods to extract this knowledge. There is a need for social scientists to collect data on trait prioritisation and for food scientists to do biochemical analyses to ensure information collected is well defined, comprehensive, quantifiable and measurable to effectively guide breeding and other agricultural research and development

    Building demand-led and gender-responsive breeding programs

    Gender-responsive breeding is a new approach to making sure modern breeding takes advantage of opportunities to improve gender equality in agriculture. Conventional research on the acceptability of modern varieties has scarcely addressed gender differences during adoption studies. Gender-responsive breeding starts from a different premise that adoption and social impact will be enhanced if gender is addressed at early stages of variety design and priority setting in breeding. However, until recently, there was no concrete way to integrate gender considerations into the practice of breeding. This chapter draws lessons for the future from three RTB breeding programs innovating with gender-responsive breeding with a focus on piloting novel tools. The new G+ tools are designed to help gender researchers and breeders make joint, evidence-based decisions about the significance of gender differences for customer targeting and trait prioritization in variety development. Their piloting in the context of each program’s practice of gender-responsive breeding throws light on some valuable good practices that contributed to successful innovation

    Emerging concepts in biomarker discovery; The US-Japan workshop on immunological molecular markers in oncology

    Supported by the Office of International Affairs, National Cancer Institute (NCI), the "US-Japan Workshop on Immunological Biomarkers in Oncology" was held in March 2009. The workshop was related to a task force launched by the International Society for the Biological Therapy of Cancer (iSBTc) and the United States Food and Drug Administration (FDA) to identify strategies for biomarker discovery and validation in the field of biotherapy. The effort will culminate on October 28th 2009 in the "iSBTc-FDA-NCI Workshop on Prognostic and Predictive Immunologic Biomarkers in Cancer", which will be held in Washington DC in association with the Annual Meeting. The purposes of the US-Japan workshop were a) to discuss novel approaches to enhance the discovery of predictive and/or prognostic markers in cancer immunotherapy; b) to define the state of the science in biomarker discovery and validation. The participation of Japanese and US scientists provided the opportunity to identify shared or discordant themes across the distinct immune genetic background and the diverse prevalence of disease between the two Nations

    Why Gender Matters in Breeding: Lessons from Cooking Bananas in Uganda

    This study examined the gender-differentiated trait preferences of cooking banana (matooke) for farmers and consumers in Central Uganda to inform banana-breeding strategies. Women and men banana farmers might have differing production objectives, norms, and values which drive decisions on which varieties to adopt and grow. However, breeders rarely consider this in their variety development programs, leading to lost opportunities for equitable breeding. An exploratory sequential mixed-method approach was used to obtain a richer understanding of the trait preferences of women and men, which explains the acceptability of cooking bananas. Consumer preference tests for the candidate banana varieties and released hybrids were also conducted. The results showed that the universal attributes for variety selection were bunch size, taste, resistance to pests and diseases, drought tolerance, food texture/softness, maturity period, and finger size. Men appreciated agronomic and market-related traits, such as tolerance to drought and poor soils, bunch size and compactness, maturity period, and shelf life, while women valued processing and cooking traits such as flavour, food colour, ease of peeling, finger size, and agronomic traits such as plant height. These are plausible attributes for the gender-responsive breeding of bananas. The findings highlight the need to redesign the banana-breeding pipeline and process in Uganda to deliver varieties with attributes desired by women and men along this commodity value chain. A participatory demand-driven and gender-responsive process involving stepwise selection criteria that commences with quality traits followed by production traits while integrating gender-specific preferences should be employed to ensure the acceptability of cooking banana hybrids by women and men end users. This requires integrating different disciplines, including social scientists and gender experts, along the entire breeding process for more inclusive products and equitable outcomes

    Early Phase Pharmacokinetics but Not Pharmacodynamics Are Influenced by Propofol Infusion Rate

    Background: Conventional compartmental pharmacokinetic models wrongly assume instantaneous drug mixing in the central compartment, resulting in a flawed prediction of drug disposition for the first minutes, and the flaw affects pharmacodynamic modeling. This study examined the influence of the administration rate and other covariates on early phase kinetics and dynamics of propofol by using the enlarged structural pharmacokinetic model. Methods: Fifty patients were randomly assigned to one of five groups to receive 1.2 mg/kg propofol given with the rate of 10 to 160 mg . kg(-1) . h(-1). Arterial blood samples were taken frequently, especially during the first minute. The authors compared four basic pharmacokinetic models by using presystemic compartments and the time shift of dosing, LAG time. They also examined a sigmoidal maximum possible drug effect pharmacodynamic model. Patient characteristics and dose rate were obtained to test the model structure. Results: Our final pharmacokinetic model includes two conventional compartments enlarged with a LAG time and six presystemic compartments and includes following covariates: dose rate for transit rate constant, age for LAG time, and weight for central distribution volume. However, the equilibration rate constant between central and effect compartments was not influenced by infusion rate. Conclusions: This study found that a combined pharmacokinetic-dynamic model consisting of a two-compartmental model with a LAG time and presystemic compartments and a sigmoidal maximum possible drug effect model accurately described the early phase pharmacology of propofol during infusion rate between 10 and 160 mg . kg(-1) . h(-1). The infusion rate has an influence on kinetics, but not dynamics. Age was a covariate for LAG time
