    A Stage-Specific Enhancer of Immunoglobulin J Chain Gene Is Induced by Interleukin-2 in a Presecretor B Cell Stage

    AbstractInterleukin-2 (IL-2)–induced transcription of the J chain gene was used as a model for analyzing cytokine regulation during B cell development. To determine whether IL-2 signals are targeted to a J chain gene enhancer as well as to its promoter, the sequences flanking the J chain gene were first examined for DNase I hypersensitivity. Of six sites identified, two strong ones, 7.5 kb upstream of the J chain gene, were found to be associated with an enhancer that is active only during the antigen-driven stages of B cell development. Further analyses of the enhancer in the IL-2–responsive presecretor BCL1 cells showed that the enhancer is activated at this stage by an IL-2 signal that functions by opening the enhancer chromatin and stimulating STAT5 to bind to a STAT5 element critical for the enhancer induction. Moreover, after this early induction stage, the enhancer was shown to be constitutively open and active in terminally differentiated plasma cells

    Optimization of propagation-based x-ray phase-contrast tomography for breast cancer imaging

    The aim of this study was to optimise the experimental protocol and data analysis for in-vivo breast cancer X-ray imaging. Results are presented of the experiment at the SYRMEP beamline of Elettra Synchrotron using the propagation-based phase-contrast mammographic tomography method, which incorporates not only absorption, but also X-ray phase information. In this study the images of breast tissue samples, of a size corresponding to a full human breast, with radiologically acceptable X-ray doses were obtained, and the degree of improvement of the image quality (from the diagnostic point of view) achievable using propagation-based phase-contrast image acquisition protocols with proper incorporation of X-ray phase retrieval into the reconstruction pipeline was investigated. Parameters such as the X-ray energy, sample-to-detector distance and data processing methods were tested, evaluated and optimized with respect to the estimated diagnostic value using a mastectomy sample with a malignant lesion. The results of quantitative evaluation of images were obtained by means of radiological assessment carried out by 13 experienced specialists. A comparative analysis was performed between the X-ray and the histological images of the specimen. The results of the analysis indicate that, within the investigated range of parameters, both the objective image quality characteristics and the subjective radiological scores of propagation-based phase-contrast images of breast tissues monotonically increase with the strength of phase contrast which in turn is directly proportional to the product of the radiation wavelength and the sample-to-detector distance. The outcomes of this study serve to define the practical imaging conditions and the CT reconstruction procedures appropriate for low-dose phase-contrast mammographic imaging of live patients at specially designed synchrotron beamlines

    Dimethyl fumarate in patients admitted to hospital with COVID-19 (RECOVERY): a randomised, controlled, open-label, platform trial

    Dimethyl fumarate (DMF) inhibits inflammasome-mediated inflammation and has been proposed as a treatment for patients hospitalised with COVID-19. This randomised, controlled, open-label platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]), is assessing multiple treatments in patients hospitalised for COVID-19 (NCT04381936, ISRCTN50189673). In this assessment of DMF performed at 27 UK hospitals, adults were randomly allocated (1:1) to either usual standard of care alone or usual standard of care plus DMF. The primary outcome was clinical status on day 5 measured on a seven-point ordinal scale. Secondary outcomes were time to sustained improvement in clinical status, time to discharge, day 5 peripheral blood oxygenation, day 5 C-reactive protein, and improvement in day 10 clinical status. Between 2 March 2021 and 18 November 2021, 713 patients were enroled in the DMF evaluation, of whom 356 were randomly allocated to receive usual care plus DMF, and 357 to usual care alone. 95% of patients received corticosteroids as part of routine care. There was no evidence of a beneficial effect of DMF on clinical status at day 5 (common odds ratio of unfavourable outcome 1.12; 95% CI 0.86-1.47; p = 0.40). There was no significant effect of DMF on any secondary outcome

    Development of HACCP analysis systems for beef slaughter

    End of Project ReportThe aim of this study was to establish the types and levels of bacterial contamination on beef carcasses slaughtered under commercial conditions. This information is necessary as baseline data for the implementation of a Hazard Analysis Critical Control Point (HACCP) plan in a meat factory. Samples were taken over a twelve month period from five carcass sites representing the fore and hind quarters of the carcass. These included the hock, bung, inside round, cranial back and brisket. The carcasses were sampled at different stages of dressing namely legging, hide removal, evisceration, carcass splitting, carcass washing and chilling (24 h later). Four meat cuts (inside round, outside round, chuck roll (cranial back) and brisket) were also sampled after boning. Counts were enumerated for the following groups of bacteria: total bacterial counts (25°C and 4°C); pseudomonad counts (25°C and 4°C); E nterobacteriaceae counts; E scherichia coli O157:H7 and L isteria spp.Teagasc acknowledges with gratitude grant aid under the Food Sub-Programme of the Operational Programme for Industrial Development.     Terrestrial Laser Scanning as an Effective Tool to Retrieve Tree Level Height, Crown Width, and Stem Diameter

    Accurate measures of forest structural parameters are essential to forest inventory and growth models, managing wildfires, and modeling of carbon cycle. Terrestrial laser scanning (TLS) fills the gap between tree scale manual measurements and large scale airborne LiDAR measurements by providing accurate below crown information through non-destructive methods. This study developed innovative methods to extract individual tree height, diameter at breast height (DBH), and crown width of trees in East Texas. Further, the influence of scan settings, such as leaf-on/leaf-off seasons, tree distance from the scanner, and processing choices, on the accuracy of deriving tree measurements were also investigated. DBH was retrieved by cylinder fitting at different height bins. Individual trees were extracted from the TLS point cloud to determine tree heights and crown widths. The R-squared value ranged from 0.91 to 0.97 when field measured DBH was validated against TLS derived DBH using different methods. An accuracy of 92% (RMSE = 1.51 m) was obtained for predicting tree heights. The R-squared value was 0.84 and RMSE was 1.08 m when TLS derived crown widths were validated using field measured crown widths. Examples of underestimations of field measured forest structural parameters due to tree shadowing have also been discussed in this study. The results from this study will benefit foresters and remote sensing studies from airborne and spaceborne platforms, for map upscaling or calibration purposes, for aboveground biomass estimation, and prudent decision making by the forest management

    A Case of Brucellosis Associated with Histiocytic Necrotizing Lymphadenitis: A Diagnostic Pitfall.

    Human cases of brucellosis are rare in the United States and difficult to diagnose. We report a case of a young female who underwent a diagnostic investigation of fever of unknown origin, which included a lymph node biopsy. The biopsy was consistent with Kikuchi’s Disease, or histiocytic necrotizing lymphadenitis, an entity where the major differential diagnosis is systemic lupus erythematosus. Interestingly, serologic studies supported the diagnosis of brucellosis. Brucellosis has rarely been associated with histiocytic necrotizing lymphadenitis. This association has never been reported in the United States, thus suggesting that brucellosis should be considered in the differential for histiocytic necrotizing lymphadenitis, along with lupus-like autoimmune disease. As the prognosis and treatment of histiocytic necrotizing lymphadenitis, brucellosis, and systemic lupus erythematosus are distinct, it is important to differentiate these entities.     The association between phase of illness and resource utilisation-a potential model for demonstrating clinical efficiency?

    Background: Healthcare efficiency involves demonstrating flexible inter-relationships between resource utilisation and patient need. In palliative care, five phases of patient illness have been identified: stable, unstable, deteriorating, terminal and bereaved. Evaluating the association between phase of illness and nursing activities could demonstrate clinical efficiency. Aim: The aim of this study was to evaluate the association between the phase of illness and the intensity of nursing care in a specialist palliative care unit. Methods: This was a prospective, observational cohort study of consecutive admissions (n=400) to a specialist palliative care unit. Patient phase of illness was documented on admission and daily thereafter. A nursing activity tool was developed, which scored daily nursing interventions (physical, psychological, family care and symptom control). This score was called the nursing total score (NTS) and reflected the intensity of nursing activities. Data were entered into SPSS and descriptive statistics weregenerated. Results: A total of 342 (85%) patients had full data recorded on admission. Stable, unstable, deteriorating and terminal phases were associated with progressively increasing median NTSs on days 1, 2, 3 and 4 (all P\u3c0.01). Phase stabilisation from the unstable to the stable phase during this timeframe resulted in reductions in physical care (p=0.038), symptom management (p=0.007) and near-significant reductions in family support (p=0.06). Conclusion: A significant association was demonstrated between phase of illness and intensity of nursing activities, which were sensitive to phase changes, from unstable to stable. This demonstrates technically efficient resource utilisation and identifies a potential efficiency model for future evaluations of inpatient palliative care
