This document has been developed by the Lazio regional chapters of two scientific associations, the Italian National Association of Hospital Cardiologists (ANMCO) and the Italian Society of Emergency Medicine (SIMEU), whose members are actively involved in the everyday management of Acute Coronary Syndromes (ACS). The document is aimed at providing a specific, practical, evidence-based guideline for the effective management of antithrombotic treatment (antiplatelet and anticoagulant) in the complex and ever changing scenario of ACS. The document employs a synthetic approach which considers two main issues: the actual operative context of treatment delivery and the general management strategy
Emergency Department as an epidemiological observatory of Human Mobility: the experience of the Moroccan population
We conducted a retrospective study of the accesses to the Emergency Department registered from January 2000 to December 2014 in 5 major hospitals in the Metropolitan Area of Rome. We extrapolated data relating to patients of Moroccan origin from about 5 million total accesses, so we compared with Italians data which, in the same period, came to ED.
The Moroccan population is distinguished by a larger number of diagnoses belonging to the ICD-9 code of Infectious Diseases and, more precisely, to Respiratory Infectious Diseases. There are also no differences in the assignment of such diagnoses to Moroccans with Italian citizenship, and this led to think that this could play an important role in the use of the ED and moreover that enrollment to the National Health Service may reduce its inappropriate use.
Regarding to Degenerative Disorders, the result of our analysis is quite emblematic, showing that the accesses to the ED is due to Cardiovascular Diseases: 6.33% of Italians' accesses against 1.81% of Moroccans and 2.36% of Moroccans with Italian citizenship. The main explanation for this difference is, obviously, due to the age of the population: about 60% of Moroccans who accessed to ED was less than 40 years old.
It is interesting how, in the field of ​​Cardiovascular Diseases, Moroccans have a lower percentage of diagnosis compared to Italians for acute diseases and a greater percentage of diagnoses for chronic diseases, suggesting once again that accesses to ED for migrants often is due to the inability to use the general services of the National Health Service.
In conclusion, from the point of view of the Emergency Department, Migration Medicine still has Infectious Diseases as the main reason for access. Degenerative Disorders remain a prerogative of the Italians, but we could certainly assume that the Moroccan population would develop at some point with the aging
A pilot study on the application of the current European guidelines for the management of acute coronary syndrome without elevation of ST segment (NSTEMI) in the Emergency Department setting in the Italian region Lazio.
Background: In 2011 the European Society of Cardiology published the new guidelines for the treatment and management of acute coronary syndrome without elevation of the ST segment (NSTEMI). For the treatment of the syndrome, the use of P2Y12 inhibitors in addition to aspirin was strongly recommended (evidence IA). We studied the application of this recommendation in the setting of the emergency department in the vast and uneven area of the Italian region Lazio, three years after the release of these drugs in Italy. Methods: 121 consecutive patients (65% older than 65 years) affected by NSTEMI were recruited between May and July 2013. During the transition in the emergency department data was collected on patient's symptoms, syndrome severity and type & timing of treatments chosen. Adherence to the guidelines was evaluated considering the number of "good treated" patients: these being the patients that received at least 80% of the main five recommendations on percutaneous coronary intervention (PCI) timing, antiplatelet and anti-coagulant therapy suggested by the European Cardiology Task Force (ESC guidelines, 2011) for the very acute phase of NSTEMI. Results: Patients were treated with: 1) 35% of cases with double antiplatelet therapy and anticoagulation (DAPT+AC), 2) 22% of cases with single antiplatelet and anticoagulation (SAPT+AC), 3) 6% of cases with a single antiplatelet therapy (SAPT), 4) 6% of cases with a double antiplatelet therapy (DAPT) and 5) 24% of cases did not receive any therapy. Data on PCI was available for 95 patients and, of these, only 82% of the patients underwent the procedure. The percentage of "good treated" patients were among of 20-40%, depending on PCI timing – as guidelines suggested – was considered as mandatory (20,5%) or as the extreme time limit (40%). Significant differences were found between patients treated in a central hospital with a hemodynamic laboratory active 24/24hr (HUB) and patients treated in the other hospital (SPOKE). HUBs showed a higher percent of "good treated" patients, a higher percentage of early invasive treated and a better adherence to recommended pharmacological therapy. Conclusions: A significant number of patients did not receive adequate treatment during the emergency department stay. The absence of hemodynamic services increases the risk of inadequate treatment
Total Skin Electron Therapy Stanford Technique Evolution With Monte Carlo Simulation Toward Personalized Treatments For Cutaneous Lymphoma
Current Total Skin Electron Therapy (TSET) Stanford technique for cutaneous
lymphoma, established in the 70's, involves a unique irradiation setup, i.e.
patient's position and beam arrangement, for all patients with ensuing great
variability in dose distribution and difficult dose optimization. A
Geant4-based simulation has been developed to explore the possibility of
personalizing the dose to each patient's anatomy. To achieve this optimization
of the treatment method, this project enrolls different aspects of the clinical
and computational techniques: starting with the knowledge of the experimental
parameters involving TSET practice, passing through an innovative approach to
model the patient's anatomy, a precise description of the electron beam and a
validated configuration of the physics models handling the interactions of the
electrons and of secondary particles. The Geant4-based simulation models the
patient as a tessellated solid derived from the optical scan of her/his body,
realistically reproduces the irradiation environment in detail and calculates
the energy deposition corresponding to each facet of the patient's scanned
surface. The resulting three-dimensional dose distribution constitutes the
basis for the personalization of the medical treatement as appropriate to each

Cyclic Vomiting Syndrome in Children
Cyclic Vomiting Syndrome (CVS) is an underdiagnosed episodic syndrome characterized
by frequent hospitalizations, multiple comorbidities, and poor quality of life. It is often
misdiagnosed due to the unappreciated pattern of recurrence and lack of confirmatory
testing. CVS mainly occurs in pre-school or early school-age, but infants and elderly onset
have been also described. The etiopathogenesis is largely unknown, but it is likely to be
multifactorial. Recent evidence suggests that aberrant brain-gut pathways, mitochondrial
enzymopathies, gastrointestinal motility disorders, calcium channel abnormalities, and
hyperactivity of the hypothalamic-pituitary-adrenal axis in response to a triggering
environmental stimulus are involved. CVS is characterized by acute, stereotyped and
recurrent episodes of intense nausea and incoercible vomiting with predictable periodicity
and return to baseline health between episodes. A distinction with other differential
diagnoses is a challenge for clinicians. Although extensive and invasive investigations
should be avoided, baseline testing toward identifying organic causes is recommended
in all children with CVS. The management of CVS requires an individually tailored therapy
Management of acute phase is mainly based on supportive and symptomatic care. Early
intervention with abortive agents during the brief prodromal phase can be used to attempt
to terminate the attack. During the interictal period, non-pharmacologic measures as
lifestyle changes and the use of reassurance and anticipatory guidance seem to be
effective as a preventive treatment. The indication for prophylactic pharmacotherapy
depends on attack intensity and severity, the impairment of the QoL and if attack
treatments are ineffective or cause side effects. When children remain refractory to
acute or prophylactic treatment, or the episode differs from previous ones, the clinician
should consider the possibility of an underlying disease and further mono- or combination
therapy and psychotherapy can be guided by accompanying comorbidities and specific
sub-phenotype. This review was developed by a joint task force of the Italian Society

Grey Matter Atrophy and its Relationship with White Matter Lesions in Patients with Myelin Oligodendrocyte Glycoprotein Antibody-associated Disease, Aquaporin-4 Antibody-Positive Neuromyelitis Optica Spectrum Disorder, and Multiple Sclerosis
OBJECTIVE:
To evaluate: (1) the distribution of gray matter (GM) atrophy in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4+NMOSD), and relapsing–remitting multiple sclerosis (RRMS); and (2) the relationship between GM volumes and white matter lesions in various brain regions within each disease.
METHODS:
A retrospective, multicenter analysis of magnetic resonance imaging data included patients with MOGAD/AQP4+NMOSD/RRMS in non-acute disease stage. Voxel-wise analyses and general linear models were used to evaluate the relevance of regional GM atrophy. For significant results (p < 0.05), volumes of atrophic areas are reported.
RESULTS:
We studied 135 MOGAD patients, 135 AQP4+NMOSD, 175 RRMS, and 144 healthy controls (HC). Compared with HC, MOGAD showed lower GM volumes in the temporal lobes, deep GM, insula, and cingulate cortex (75.79 cm3); AQP4+NMOSD in the occipital cortex (32.83 cm3); and RRMS diffusely in the GM (260.61 cm3). MOGAD showed more pronounced temporal cortex atrophy than RRMS (6.71 cm3), whereas AQP4+NMOSD displayed greater occipital cortex atrophy than RRMS (19.82 cm3). RRMS demonstrated more pronounced deep GM atrophy in comparison with MOGAD (27.90 cm3) and AQP4+NMOSD (47.04 cm3). In MOGAD, higher periventricular and cortical/juxtacortical lesions were linked to reduced temporal cortex, deep GM, and insula volumes. In RRMS, the diffuse GM atrophy was associated with lesions in all locations. AQP4+NMOSD showed no lesion/GM volume correlation.
INTERPRETATION:
GM atrophy is more widespread in RRMS compared with the other two conditions. MOGAD primarily affects the temporal cortex, whereas AQP4+NMOSD mainly involves the occipital cortex. In MOGAD and RRMS, lesion-related tract degeneration is associated with atrophy, but this link is absent in AQP4+NMOSD. ANN NEUROL 2024;96:276–28

Improving quality through clinical risk management: A triage sentinel event analysis
"Triage" is a useful tool used in emergency departments (EDs) to prioritize the care of patients. Through a methodical process of different sequential steps, the triage nurse assigns a color code which goes from red-critical patient with immediate access to medical examination-to a white code that represents no urgency. Clinical studies have shown that patients can be victims of errors during the process of care, especially in complex systems such as EDs. To reduce errors it is essential to map the risks in order to identify the causes (both individual and organizational); the introduction of corrective changes cannot be postponed. The incorrect assessment at triage represents one of the major errors in EDs. By monitoring this activity, through the analysis of sentinel events we can reduce adverse consequences. Missed recognition of a red code indicates a sentinel event. We used a "root cause analysis" to explain an episode of missed recognition of red code at triage. A nurse without specific training in triage and inexperienced in critical care was identified as the "root cause" of the sentinel event. To make improvements we planned a triage training course (for newly employed nurses and a refresher course for existing staff) and created a team of dedicated triage nurses. © 2011 SIMI
