    The challenges in the construction of psychopedagogical materials in health education

Universidad da Coruña, 2 de Setembro de 2015.A cidadania constitui um processo ativo, no qual o indivíduo e a sociedade interagem para o alcance de objetivos e de superação de dificuldades comuns. O contexto escolar, é assim considerado um dos meios mais privilegiados para que a aprendizagem se consolide, dado ser um local integrador, das várias áreas do saber, e de preocupações transversais à sociedade. Como tal, a aposta na educação para a cidadania e a sua inserção no currículo escolar torna-se importante desde os níveis de ensino mais precoces. A elaboração de projetos que permitam concretizar os objetivos que norteiam esta área de conhecimento deve ter em linha de conta não só as necessidades e fragilidades específicas da comunidade escolar, na qual se desenvolverá uma determinada ação, mas também possibilitar a extensão destas ações aos restantes contextos onde o indivíduo se encontra inserido. Uma das áreas de abordagem da educação para a cidadania diz respeito à Educação para a Saúde. Therefore, school settings are considered one of the privileged means of learning consolidation, as it consists in a context of integration of several areas of knowledge and expertise, as well as one of the major areas of concern to the society. As such, the investment in education for citizenship and the introduction of this discipline in regular school curricula from early school years is of central importance. The development of projects that allow the accomplishment of goals in this area of expertise must take into account not only the specific frailties and needs from the school community in which those actions will take place, but must also make possible to extend these actions to the remaining contexts in which the individuals participate.     A participação paterna na gravidez de adolescentes ainda é pouco discutida, embora a ausência do companheiro seja apontada como um dos problemas sociais dela decorrente. Esta investigação, de cunho qualitativo, teve por objetivo descrever como se dá a participação do pai do bebê durante a gestação, nacimento e criação do filho nos primeiros meses de vida, na perspectiva de adolescentes que passaram pela experiência de engravidar e ter um filho. Na sistematização dos dados, os mesmos foram agrupados em duas categorias: vivenciando a gravidez e valorizando a presença e participação do companheiro/pai.La participación del padre en el embarazo de adolescentes es poco discutida, aunque la ausencia del compañero sea apuntada como un de los problemas sociales della decorrente. Esta investigación cualitativa, tuve por objetivo describir como fue la participación del padre mientras la gravidez, nascimiento y criación del hijo en los primeros meses de vida, en la perspectiva de las adolescentes que pasaran por la experiencia de embarazar y tener un hijo.     Adolescentes e grávidas: onde buscam apoio?

    Adolescence is the age of crises, changes, adaptation to the new body and new attitudes towards life. If we add to this the meaning of pregnancy from personal, social and family points of view, we will understand how difficult it can be for the adolescents. The purpose of this study was to identify where the pregnant teenagers look for support. Results showed that they had family support, mainly from parents, and less frequently from the baby's father. It also showed the pregnancy acceptance, its retation to school abandonment, the girls' idealized vision of pregnancy and future expectations, the concern about biological aspects and the lack of concern about real problems.La Adolescencia es una época de crisis, cambio, adaptación al nuevo cuerpo y de nuevos comportamientos frente a la vida. Si sumamos a esto el significado del embarazo, con sus aspectos personales, sociales y familiares, comprenderemos como el embarazo puede ser un evento difícil para la adolescente. Este trabajo tuvo como objetivo identificar donde las adolescentes embarazadas buscan apoyo. Evidenció que las entrevistadas tuvieron el apoyo de la familia, principalmente de los padres y con menor frecuencia el apoyo del padre de su bebé, así como la aceptación del embarazo, su relación con el abandono escolar, la visón idealizada de esas jóvenes acerca del embarazo y futuras expectativas, la preocupación con aspectos biológicos y la despreocupación con problemas concretos.Adolescência é época de crise, mudança, readaptação ao novo corpo e de novas atitudes frente a vida. Se somarmos a isso o significado da gravidez, dos pontos de vista pessoal, social e familiar, compreenderemos como a gestação pode ser um evento difícil para a adolescente. O presente estudo teve como objetivo identificar onde as adolescentes grávidas buscam apoio. Evidenciou-se que as entrevistadas puderam contar com o apoio da família, principalmente dos pais e, com menos freqüência com o do pai do bebê, bem como a aceitação da gravidez, sua relação com o abandono escolar, a visão idealizada dessas garotas acerca da gestação e expectativas futuras, a preocupação com aspectos biológicos e a despreocupação com problemas concretos

    Multicentre Surveillance of Candida Species from Blood Cultures during the SARS-CoV-2 Pandemic in Southern Europe (CANCoVEU Project)

    Introduction: Surveillance of Candida species isolates from blood cultures (BCs) in Europe is considered fragmented, unable to allow the definition of targets of antifungal stewardship recommendations especially during the SARS-CoV-2 pandemic. Methods: We performed a multicentric retrospective study including all consecutive BC Candida isolates from six Southern European tertiary hospitals (1st January 2020 to 31st December 2021). Etiology, antifungal susceptibility patterns, and clinical setting were analyzed and compared. Results: C. albicans was the dominant species (45.1%), while C. auris was undetected. Candida species positive BC events increased significantly in COVID-19 ICUs in 2021 but decreased in other ICUs. Resistance to azole increased significantly and remained very high in C. albicans (fluconazole from 0.7% to 4.5%, p = 0.03) and C. parapsilosis complex (fluconazole up to 24.5% and voriconazole up to 8.9%), respectively. Resistance to caspofungin was remarkable in C. tropicalis (10%) and C. krusei (20%), while resistance to at least one echinocandin increased in 2021, especially in C. parapsilosis complex (from 0.8% to 5.1%, p = 0.05). Although no significant differences were observed over the study period, fluconazole and echinocandin resistance increased in COVID-19 ICUs by up to 14% and 5.8%, respectively, but remained undetected in non-intensive COVID-19 wards. Conclusions: Antifungal stewardship activities aimed at monitoring resistance to echinocandin in C. tropicalis and C. krusei, and against the spread of fluconazole resistant C. parapsilosis complex isolates are highly desirable. In COVID-19 patients, antifungal resistance was mostly present when the illness had a critical course.publishersversionpublishe

    Diagnosis of pulmonary tuberculosis in children and adolescents: comparison of two versions of the Brazilian Ministry of Health scoring system

    The aim of this study was to evaluate the concordance between two versions of the scoring system (2011 and 2019), recommended by the Brazilian Ministry of Health, for the diagnosis of pulmonary tuberculosis (PTB) in children and adolescents. A retrospective descriptive study was performed to assess the medical records of children and adolescents with PTB, in TB units from Brazilian cities located in Rio de Janeiro, Minas Gerais, and Parana States, from January 1st, 2004, to December 1st, 2018. Patients aged 0 to 18 years old with a diagnosis of PTB were included. The comparison between the two scoring systems showed a moderate concordance according to the κ coefficient value = 0.625. Fourteen patients showed a reduction in the TB score, going from 30 points in the 2011, to 25 points or less in the 2019 one. Seventy one percent of these 14 patients had radiological changes suggestive of PTB and 86% had tuberculin skin tests greater than 10 mm. The study concluded that a moderate agreement was observed between the 2011 and 2019 scoring systems, with an increase in the number of patients scoring 25 points or less in 2019, which can eventually hinder the diagnosis of PTB

    Identifying common and specific microRNAs expressed in peripheral blood mononuclear cell of type 1, type 2, and gestational diabetes mellitus patients

    Abstract\ud \ud \ud \ud Background\ud Regardless the regulatory function of microRNAs (miRNA), their differential expression pattern has been used to define miRNA signatures and to disclose disease biomarkers. To address the question of whether patients presenting the different types of diabetes mellitus could be distinguished on the basis of their miRNA and mRNA expression profiling, we obtained peripheral blood mononuclear cell (PBMC) RNAs from 7 type 1 (T1D), 7 type 2 (T2D), and 6 gestational diabetes (GDM) patients, which were hybridized to Agilent miRNA and mRNA microarrays. Data quantification and quality control were obtained using the Feature Extraction software, and data distribution was normalized using quantile function implemented in the Aroma light package. Differentially expressed miRNAs/mRNAs were identified using Rank products, comparing T1DxGDM, T2DxGDM and T1DxT2D. Hierarchical clustering was performed using the average linkage criterion with Pearson uncentered distance as metrics.\ud \ud \ud \ud Results\ud The use of the same microarrays platform permitted the identification of sets of shared or specific miRNAs/mRNA interaction for each type of diabetes. Nine miRNAs (hsa-miR-126, hsa-miR-1307, hsa-miR-142-3p, hsa-miR-142-5p, hsa-miR-144, hsa-miR-199a-5p, hsa-miR-27a, hsa-miR-29b, and hsa-miR-342-3p) were shared among T1D, T2D and GDM, and additional specific miRNAs were identified for T1D (20 miRNAs), T2D (14) and GDM (19) patients. ROC curves allowed the identification of specific and relevant (greater AUC values) miRNAs for each type of diabetes, including: i) hsa-miR-1274a, hsa-miR-1274b and hsa-let-7f for T1D; ii) hsa-miR-222, hsa-miR-30e and hsa-miR-140-3p for T2D, and iii) hsa-miR-181a and hsa-miR-1268 for GDM. Many of these miRNAs targeted mRNAs associated with diabetes pathogenesis.\ud \ud \ud \ud Conclusions\ud These results indicate that PBMC can be used as reporter cells to characterize the miRNA expression profiling disclosed by the different diabetes mellitus manifestations.     Enteropathic arthritis in Brazil: data from the brazilian registry of spondyloarthritis

    Inflammatory bowel diseases (Crohn's disease and ulcerative rectocolitis) have extraintestinal manifestations 25% of the patients, with the most common one being the enteropathic arthritis. METHODS: Prospective, observational, multicenter study with patients from 29 reference centers participating in the Brazilian Registry of Spondyloarthritis (RBE), which incorporates the RESPONDIA (Ibero-American Registry of Spondyloarthritis) group. Demographic and clinical data were collected from 1472 patients and standardized questionnaires for the assessment of axial mobility, quality of life, enthesitic involvement, disease activity and functional capacity were applied. Laboratory and radiographic examinations were performed. The aim of this study is to compare the clinical, epidemiological, genetic, imaging, treatment and prognosis characteristics of patients with enteropathic arthritis with other types of spondyloarthritis in a large Brazilian cohort. RESULTS: A total of 3.2% of patients were classified as having enteroarthritis, 2.5% had spondylitis and 0.7%, arthritis (peripheral predominance). The subgroup of individuals with enteroarthritis had a higher prevalence in women (P < 0.001), lower incidence of inflammatory axial pain (P < 0.001) and enthesitis (P = 0.004). HLA-B27 was less frequent in the group with enteroarthritis (P = 0.001), even when considering only those with the pure axial form. There was a lower prevalence of radiographic sacroiliitis (P = 0.009) and lower radiographic score (BASRI) (P = 0.006) when compared to patients with other types of spondyloarthritis. They also used more corticosteroids (P < 0.001) and sulfasalazine (P < 0.001) and less non-steroidal anti-inflammatory drugs (P < 0.001) and methotrexate (P = 0.001). CONCLUSION: There were differences between patients with enteroarthritis and other types of spondyloarthritis, especially higher prevalence of females, lower frequency of HLA-B27, associated with less severe axial involvement.As doenças inflamatórias intestinais (doença de Crohn e retocolite ulcerativa) apresentam manifestações extraintestinais em um quarto dos pacientes, sendo a mais comum a artrite enteropática. MÉTODOS: Estudo prospectivo, observacional e multicêntrico, realizado com pacientes de 29 centros de referência participantes do Registro Brasileiro de Espondiloartrites (RBE), que se incorpora ao grupo RESPONDIA (Registro Ibero-americano de Espondiloartrites). Dados demográficos e clínicos de 1472 pacientes foram colhidos, e aplicaram-se questionários padronizados de avaliação de mobilidade axial, de qualidade de vida, de envolvimento entesítico, de atividade de doença e de capacidade funcional. Exames laboratoriais e radiográficos foram realizados. Objetivamos, neste presente artigo, comparar as características clínicas, epidemiológicas, genéticas, imagenológicas, de tratamento e prognóstico de enteroartríticos com os outros espondiloartríticos nesta grande coorte brasileira. RESULTADOS: Foram classificados como enteroartrite 3,2% dos pacientes, sendo que 2,5% tinham espondilite e 0,7%, artrite (predomínio periférico). O subgrupo de indivíduos com enteroartrite apresentava maior prevalência de mulheres (P < 0,001), menor incidência de dor axial inflamatória (P < 0,001) e de entesite (P = 0,004). O HLA-B27 foi menos frequente no grupo de enteroartríticos (P = 0,001), mesmo se considerado apenas aqueles com a forma axial pura. Houve menor prevalência de sacroiliíte radiológica (P = 0,009) e também menor escore radiográfico (BASRI) (P = 0,006) quando comparado aos pacientes com as demais espondiloartrites. Também fizeram mais uso de corticosteroides (P < 0,001) e sulfassalasina (P < 0,001) e menor uso de anti-inflamatórios não hormonais (P < 0,001) e metotrexato (P = 0,001).
