Background: Significant progress has been made in reducing the incidence of childhood lead poisoning in the United States in the past three decades. However, the prevalence of elevated blood lead in children (≥ 10 μg/dL) remains high in some communities, particularly those with high proportions of pre-1978 housing in poor condition. Increasingly, municipalities are using local policy tools to reduce lead poisoning in high-risk areas, but little is known about the effectiveness of such policies
Analysis of the Ribonuclease a superfamily of antimicrobial peptides in patients undergoing chronic peritoneal dialysis
Infectious peritonitis is a common complication in patients undergoing chronic peritoneal dialysis (PD), limiting the duration of PD as a modality for renal replacement therapy and increasing patient morbidity and mortality. Antimicrobial peptides (AMPs) serve critical roles in mucosal defense, but their expression and activity during peritonitis are poorly understood. We hypothesized that AMPs belonging to the Ribonuclease (RNase) A Superfamily are present in peritoneal fluid and increase during peritonitis in patients undergoing chronic PD. In the absence of peritonitis, we detected RNase 3, RNase 6, and RNase 7 in cell-free supernatants and viable cells obtained from peritoneal fluid of chronic PD patients. The cellular sources of these RNases were eosinophils (RNase 3), macrophages (RNase 6), and mesothelial cells (RNase 7). During peritonitis, RNase 3 increased 55-fold and RNase 7 levels increased 3-fold on average, whereas RNase 6 levels were unchanged. The areas under the receiver-operating characteristic curves for RNase 3 and RNase 7 were 0.99 (95% confidence interval (CI): 0.96–1.0) and 0.79 (95% CI: 0.64–0.93), respectively, indicating their potential as biomarkers of peritonitis. Discrete omental reservoirs of these RNases were evident in patients with end stage kidney disease prior to PD initiation, and omental RNase 3 reactive cells increased in patients undergoing PD with a history of peritonitis. We propose that constitutive and inducible pools of antimicrobial RNases form a network to shield the peritoneal cavity from microbial invasion in patients undergoing chronic PD
Bush the transnationalist: a reappraisal of the unilateralist impulse in US foreign policy, 2001-2009
This article challenges the common characterisation of George W. Bush’s foreign policy as “unilateral.” It argues that the Bush administration developed a new post-9/11 understanding of terrorism as a transnational, networked phenomenon shaped by the forces of globalisation. This led to a new strategic emphasis on bi- and multilateral security co-operation and counterterrorism operations, especially outside of Afghanistan and Iraq, driven by the perceived need to counter a transnational security challenge present in multiple locations. This (flawed) attempt to engage with transnational security challenges supplemented the existing internationalist pillar of the Bush administration’s foreign policy. Highlighting the transnational realm of international relations and the ways in which the Bush administration was able to co-opt other states to tackle perceived transnational challenges also shows the high importance the administration attached to concerted action even as it frequented eschewed institutional multilateralism
Beliefs and attitudes about endemic dental fluorosis among adolescents in rural Brazil Creencias y actitudes con relación a la fluorosis dentaria de jóvenes en región rural de Brasil Crenças e atitudes em relação à fluorose dentária de jovens em região rural do Brasil
OBJECTIVE: To understand beliefs and attitudes about fluorosis among young people living in a rural area. METHODOLOGICAL PROCEDURES: Qualitative study consisting of semi-structured interviews with 23 adolescents with dental fluorosis, 14 teachers and three health authorities in the city of São Francisco, Southeastern Brazil, in 2002. Content analysis and social representation theory were applied. ANALYSIS OF RESULTS: The organoleptic characteristics of carbonates that affect groundwater (salty flavor, whitish coloration, and turbidity) associated with negative aspects of household use of this water are considered a cause of mottled enamel. Even after contact with researchers who investigated this phenomenon and helped find a solution for this condition, the local population is still unwilling to accept fluoride as the cause of the problem and does not fully agree to use water from other sources because they are afraid of the quality of water. CONCLUSIONS: Misperceptions of the causes of dental fluorosis and water treatment costs compromise the implementation of uncontaminated surface water supplies. Health education strategies are required in parallel with solutions for securing water supply in drought-ravaged areas.<br>OBJETIVO: Comprender creencias y actitudes con relación a la fluorosis dentaria de jóvenes residentes en región rural de Brasil. MÉTODOS: Estudio cualitativo en que fueron realizadas entrevistas semi-estructuradas con 23 adolescentes con fluorosis dentaria, 14 profesores y tres gestores públicos en el municipio de Sao Francisco, en Sureste de Brasil, en 2002. Fueron empleadas la técnica de análisis de contenido y la teoría de las representaciones sociales. RESULTADOS: Por las características organolépticas que imprime al agua (sabor salado, coloración blanquecina y turbidez) y su asociación con aspectos negativos relacionados al uso doméstico, el carbonato es considerado la causa de la fluorosis por adolescentes y profesores. Aún posterior al contacto con científicos que investigaron el fenómeno y que participaron de su solución, la población continúa resistiendo en aceptar el flúor como causa del problema y no concuerda plenamente con el uso de agua de otras fuentes por dudar de su calidad. CONCLUSIONES: Percepciones equivocadas con relación a las causas de la fluorosis dentaria y la dificultad en costear el tratamiento del agua comprometen la implantación del suplemento de agua de superficie no contaminada. Estrategias de educación en salud deben ser implementadas paralelamente a soluciones para captación de agua de fuentes alternativas no contaminadas en localidades afectadas por la seca.<br>OBJETIVO: Compreender crenças e atitudes em relação à fluorose dentária de jovens residentes em região rural do Brasil. PROCEDIMENTOS METODOLÓGICOS: Estudo qualitativo em que foram realizadas entrevistas semi-estruturadas com 23 adolescentes com fluorose dentária, 14 professores e três gestores públicos no município de São Francisco, MG, em 2002. Foram empregadas a técnica de análise de conteúdo e a teoria das representações sociais. ANÁLISE DOS RESULTADOS: Pelas características organolépticas que imprime à água (sabor salgado, coloração esbranquiçada e turbidez) e sua associação com aspectos negativos relacionados ao uso doméstico, o carbonato é considerado a causa da fluorose por adolescentes e professores. Mesmo após o contato com pesquisadores que investigaram o fenômeno e que participaram da sua solução, a população continua a resistir em aceitar o flúor como causa do problema e não concorda plenamente com o uso de água de outras fontes por duvidar de sua qualidade. CONCLUSÕES: Percepções equivocadas em relação às causas da fluorose dentária e a dificuldade em custear o tratamento da água comprometem a implantação de suprimento de água de superfície não contaminada. Estratégias de educação em saúde devem ser implementadas paralelamente a soluções para captação de água de fontes alternativas não contaminadas em localidades afetadas pela seca
Protective effect of gallic acid against sodium fluoride-induced oxidative stress in rat’s kidney
In the present study, the nephroprotective effect of gallic acid isolated from Peltiphyllum peltatum was examined in sodium fluoride (NaF) treated rats. Nephrotoxicity was induced by 1-week intoxication of NaF at 600 ppm through drinking water. The levels of thiobarbituric acid reactive substances, reduced glutathione as well as activities of superoxide dismutase and catalase in renal tissues homogenates were determined. The serum biochemical markers of renal injuries including creatinine, serum urea, blood urea nitrogen, uric acid levels as well as the levels of phosphate and calcium were also assessed. Intoxication with NaF caused a significant increase in the levels of thiobarbituric acid reactive substances (46 % versus to control) and reduced the glutathione concentration (47 %) and the activities of superoxide dismutase (46 %) and catalase (41 %) in renal tissues homogenates. NaF intoxication also induced significant alterations in the kidney biochemical markers increasing the levels of urea, uric acid, blood urea nitrogen, creatinine, and phosphate and decreasing the levels of calcium. Daily administration of gallic acid (20 mg/kg) for 1 week before NaF intoxication brought the antioxidant–oxidant balance similar to the NaF-untreated group. Silymarin, used a standard antioxidant agent, also showed a nephroprotective activity. We concluded that NaF caused nephrotoxicity and oxidative stress in renal tissues and daily administration of gallic acid for 1 week prior to intoxication inhibited toxicity and oxidative stress
