    Results of therapy of chronic urticaria in patients with IgE-dependent and IgE-independent disease profile

    The main mechanism for the occurrence of urticaria is the degranulation of mast cells. It has been proven that, regardless of the activation pathway, clinical manifestations will not differ. According to the literature, up to half of cases of chronic spontaneous urticaria are autoimmune in nature, can be combined with autoimmune thyroid disease, SLE, etc., and have a more severe course.In therapy, antihistamines are traditionally used. However, some patients do not respond to the treatment, even with a multiple increase in doses. In the treatment of urticaria resistant to traditional antihistamines, the use of Omalizumab is recommended. The purpose of the study: to determine the profile of patients with chronic urticaria, as well as to evaluate the effectiveness of treatment with Omalizumab in patients with IgE- dependent and IgE-independent chronic urticaria.Eight-one patients with chronic urticaria (60 adults, 21 children) were examined. Patients before the start of therapy had a long history of CU: from 1 to 20 years. Patients before the start of therapy were treated with antihistamines, but no control was obtained. An increase in the level of serum IgE was detected in 51.7% of cases in adults and 42% in children. Concomitant sensitization was determined in 48.3% of adults and 76.2% of children. In children, food, epidermal and pollen sensitization was the most common. Pollen and epidermal sensitization were more common in adults. The level of eosinophilia in the group with IgE-dependent was more pronounced than in other group (p = 0.0097). After 6 months, the group with IgE-dependent showed an improvement in the symptom score (UCT) from 3.1 CI (1.5-4.6) to 12.2 CI (10.8-13.7), (p = 0.0001). In other group, symptoms improved from 0.63 CI (0.36-1.6) to 8.1 CI (5-11.2) after 6 months (no control). After 6 months of genetically engineered biological therapy (GIBT), complete control over the symptoms of CU in group 1 was obtained in 66.7% of patients, partial — in 33.7%. In the second group, in 33.3% of cases, positive treatment results could not be achieved. Thus, genetically engineered biological therapy with Omalizumab increases the control over the course of CU. Treatment outcomes are higher in patients with an IgE-dependent disease profile

    The results of complex hydrochemical, hydrobiological and ichthyological investigations by IBSS, NAS of Ukraine, realized in 6 regions of the coastal zone of the Crimean peninsula in the Black Sea and the Sea of Azov are given. The main negative factors causing changes in structural and functional characteristics of hydrobiocenoses in the regions studied are analyzed and “hot ecological spots” are isolated. Variants of different methods of management of the coastal ecosystems, including construction of artificial reefs and usage of biological filters for water cleaning, protection and recreation of biological diversity are taken into consideration

    CLINICAL AND IMMUNOLOGICAL EFFECTIVENESS OF TREATMENT OF RAGWEED POLLINOSIS IN THE SAMARA REGION

    Allergic rhinitis and bronchial asthma are widespread respiratory allergic diseases. In some territories of the Russian Federation, the dominant cause of pollinosis is ragweed. The aim of the study was to evaluate the clinical and immunological efficacy of ASIT with the allergen Ambrosia artemisiifolia in patients sensitized to Ambrosia trifida in the Samara region. Patients with proven sensitization to Ambrosia trifida was held immunotherapy with Ambrosia artemisiifolia allergoid preseason. After treatment, patients had a decrease in the severity of symptoms of allergic rhinitis according to VAS (p = 0.00001), a decrease in the need for medications (p = 0.0003), as well as the need for corticosteroids against the background of therapy from 34.6% to 0% (p = 0.00001). In 8% of cases, the result of treatment was good, in 69% satisfactory, in 23% unsatisfactory. In the control group, there were no changes in the severity of symptoms (p = 0.858). Also, in the control group, the need for medications remained unchanged and 14.3% of patients continued to use corticosteroids.After ASIT, there was a decrease in the level of IL-4 (p = 0.002), and a decrease in the ratio of IL-4/ IL-10 (p = 0.0063); at the same time, changes in the level of other cytokines (IL-10; IFNγ) were statistically insignificant (p > 0.05). Before treatment, the levels of IL-4/ IL-10 in both groups were comparable, and after treatment, the differences became statistically significant (p = 0.031). We did not get a statistically significant change in the level of IgG4 Amb a 1 or IgG4 Amb trifida. There was no correlation between the level of individual cytokines and the results of treatment. As a result of the conducted ASIT, positive clinical and immunological results were obtained. In most patients, the disease has acquired a controlled course. At the same time, the lack of excellent and low number of good results of ASIT is probably due to the intraspecific allergenic properties of ragweed

    Building nonparametric nn-body force fields using Gaussian process regression

    Constructing a classical potential suited to simulate a given atomic system is a remarkably difficult task. This chapter presents a framework under which this problem can be tackled, based on the Bayesian construction of nonparametric force fields of a given order using Gaussian process (GP) priors. The formalism of GP regression is first reviewed, particularly in relation to its application in learning local atomic energies and forces. For accurate regression it is fundamental to incorporate prior knowledge into the GP kernel function. To this end, this chapter details how properties of smoothness, invariance and interaction order of a force field can be encoded into corresponding kernel properties. A range of kernels is then proposed, possessing all the required properties and an adjustable parameter nn governing the interaction order modelled. The order nn best suited to describe a given system can be found automatically within the Bayesian framework by maximisation of the marginal likelihood. The procedure is first tested on a toy model of known interaction and later applied to two real materials described at the DFT level of accuracy. The models automatically selected for the two materials were found to be in agreement with physical intuition. More in general, it was found that lower order (simpler) models should be chosen when the data are not sufficient to resolve more complex interactions. Low nn GPs can be further sped up by orders of magnitude by constructing the corresponding tabulated force field, here named "MFF".Comment: 31 pages, 11 figures, book chapte

    Associated Complication of Gastrointestinal Stromal Tumours of the Small Intestine

    Aim. To consider the possibility of the diagnosis of a rare small bowel tumour complicated by small bowel obstruction and small bowel bleeding, as well as the surgical treatment of this pathology using a minimally invasive surgical approach.Results and discussion. The authors present a clinical case of associated complication of a gastrointestinal stromal tumour (GIST) of the small intestine in a 48-year-old patient. The patient was admitted to a surgical clinic with small bowel obstruction, episodes of intestinal bleeding and anaemia. The diagnosis was determined using CT enterography. In line with the current trends for minimally invasive surgery, minimally invasive laparotomy was performed drawing on the CT mapping of the anterior abdominal wall. The last stage involved in the morphological verification of GIST, which employs a standard procedure of IHC testing, revealed a malignant GIST, spindle cell variant. The verified histotype of a small intestinal tumour provides the opportunity to choose the necessary variant of adjuvant chemotherapy, as well as to improve general and relapse-free survival.Conclusion. Minimally invasive operations can be performed in the complicated course of GIST due to the biological properties of this pathology (absence of lymphogenic metastasis and infiltrative growth) without decreasing five-year survival rate. The use of CT helps make a topical diagnosis and plan minimally invasive surgical treatment

    Unexpected Room Temperature Ferromagnetism of a Ball-Milled Graphene Oxide—Melamine Mixture

    Nitrogen-doped carbon nanomaterial (NDCNM) was synthesized by grinding a mixture of graphene oxide and melamine in a planetary mill with both balls and milling chamber of zirconium dioxide. In the electron spin resonance spectrum of NDCNM, a broad signal with g = 2.08 was observed in addition to a narrow signal at g = 2.0034. In the study using a vibrating-sample magnetometer, the synthesized material is presumably a ferromagnet with a coercive force of 100 Oe. The specific magnetization at 10,000 Oe is approximately 0.020 and 0.055 emu/g at room temperature and liquid nitrogen temperature, respectively

    Reduced Graphene Oxide Aerogel inside Melamine Sponge as an Electrocatalyst for the Oxygen Reduction Reaction

    A graphene oxide aerogel (GOA) was formed inside a melamine sponge (MS) framework. After reduction with hydrazine at 60 °C, the electrical conductive nitrogen-enriched rGOA-MS composite material with a specific density of 20.1 mg/cm3 was used to fabricate an electrode, which proved to be a promising electrocatalyst for the oxygen reduction reaction. The rGOA-MS composite material was characterized by elemental analysis, scanning electron microscopy, X-ray photoelectron spectroscopy, and Raman spectroscopy. It was found that nitrogen in the material is presented by different types with the maximum concentration of pyrrole-like nitrogen. By using Raman scattering it was established that the rGOA component of the material is graphene-like carbon with an average size of the sp2-domains of 5.7 nm. This explains a quite high conductivity of the composite obtained
