Acidente com material perfurocortante entre profissionais de enfermagem de um hospital universitĂĄrio
Os riscos ocupacionais a que os profissionais da equipe de enfermagem estĂŁo sujeitos no desempenho de suasfunçÔes sĂŁo considerĂĄveis. Assim estabeleceu-se como objetivo deste estudo analisar os acidentes perfurocortantesno perĂodo de 2002 a 2006, envolvendo a equipe de enfermagem de um hospital universitĂĄrio, para compreender ocontexto em que ocorrem. A utilização destas informaçÔes pode ser ferramenta de prevenção. Estudo descritivo,retrospectivo quantitativo e qualitativo. Na anĂĄlise quantitativa foi utilizada estatĂstica descritiva e na qualitativao discurso do sujeito coletivo. Por meio das fichas de notificação do NĂșcleo de VigilĂąncia EpidemiolĂłgica doHospital, foi possĂvel identificar acidentes do gĂȘnero no perĂodo pesquisado. Entrevistas foram direcionadas Ă svĂtimas de acidentes que tiveram como paciente-fonte portador de SĂndrome da ImunodeficiĂȘncia Adquirida/VĂrus da ImunodeficiĂȘncia Humana, hepatite B e C. Os achados demonstraram que ainda persiste um grau significativodo desconhecimento ou banalização dos acidentes entre profissionais da saĂșde
A Prognostic Model for Estimating the Time to Virologic Failure in HIV-1 Infected Patients Undergoing a New Combination Antiretroviral Therapy Regimen
<p>Abstract</p> <p>Background</p> <p>HIV-1 genotypic susceptibility scores (GSSs) were proven to be significant prognostic factors of fixed time-point virologic outcomes after combination antiretroviral therapy (cART) switch/initiation. However, their relative-hazard for the time to virologic failure has not been thoroughly investigated, and an expert system that is able to predict how long a new cART regimen will remain effective has never been designed.</p> <p>Methods</p> <p>We analyzed patients of the Italian ARCA cohort starting a new cART from 1999 onwards either after virologic failure or as treatment-naĂŻve. The time to virologic failure was the endpoint, from the 90<sup>th </sup>day after treatment start, defined as the first HIV-1 RNA > 400 copies/ml, censoring at last available HIV-1 RNA before treatment discontinuation. We assessed the relative hazard/importance of GSSs according to distinct interpretation systems (Rega, ANRS and HIVdb) and other covariates by means of Cox regression and random survival forests (RSF). Prediction models were validated via the bootstrap and c-index measure.</p> <p>Results</p> <p>The dataset included 2337 regimens from 2182 patients, of which 733 were previously treatment-naĂŻve. We observed 1067 virologic failures over 2820 persons-years. Multivariable analysis revealed that low GSSs of cART were independently associated with the hazard of a virologic failure, along with several other covariates. Evaluation of predictive performance yielded a modest ability of the Cox regression to predict the virologic endpoint (c-indexâ0.70), while RSF showed a better performance (c-indexâ0.73, p < 0.0001 vs. Cox regression). Variable importance according to RSF was concordant with the Cox hazards.</p> <p>Conclusions</p> <p>GSSs of cART and several other covariates were investigated using linear and non-linear survival analysis. RSF models are a promising approach for the development of a reliable system that predicts time to virologic failure better than Cox regression. Such models might represent a significant improvement over the current methods for monitoring and optimization of cART.</p
Workloads, strain processes and sickness absenteeism in nursing
OBJECTIVE: to analyze the workloads, strain processes and sickness absenteeism among nursing workers from a teaching hospital in the Brazilian Central-West. METHOD: a descriptive and cross-sectional study was developed with a quantitative approach, based on the theoretical framework of the social determination of the health-disease process. Data were collected between January and December 2009, based on records of complaints related to occupational exposure among nursing professionals, filed in the software Monitoring System of Nursing Workers' Health. For the sake of statistical analysis, relative and absolute frequencies of the variables and the risk coefficient were considered. RESULTS: 144 notifications of occupational exposure were registered across the analysis period, which represented 25% of the total nursing population at the hospital. The physiological and psychic workloads were the most representative, corresponding to 37% and 36%, respectively. These notifications culminated in 1567 days of absenteeism for disease treatment. CONCLUSIONS: the findings evidence the impact of occupational illnesses on the absenteeism of nursing workers, and can be used to demonstrate the importance of institutional investments in occupational health surveillance
School Effects on the Wellbeing of Children and Adolescents
Well-being is a multidimensional construct, with psychological, physical and social components. As theoretical basis to help understand this concept and how it relates to school, we propose the Self-Determination Theory, which contends that self-determined motivation and personality integration, growth and well-being are dependent on a healthy balance of three innate psychological needs of autonomy, relatedness and competence. Thus, current indicators involve school effects on childrenâs well-being, in many diverse modalities which have been explored. Some are described in this chapter, mainly: the importance of peer relationships; the benefits of friendship; the effects of schools in conjunction with some forms of family influence; the school climate in terms of safety and physical ecology; the relevance of the teacher input; the school goal structure and the implementation of cooperative learning. All these parameters have an influence in promoting optimal functioning among children and increasing their well-being by meeting the above mentioned needs. The empirical support for the importance of schools indicates significant small effects, which often translate into important real-life effects as it is admitted at present. The conclusion is that schools do make a difference in childrenâs peer relationships and well-being
