The number and types of hypotensive drugs prescribed for patients with poorly controlled arterial hypertension admitted to specialized units
Wstęp Nadciśnienie tętnicze nieleczone lub niedostatecznie
leczone jest istotnym czynnikiem ryzyka
chorób sercowo-naczyniowych i powikłań
narządowych. Zgodnie z zaleceniami ESH/ESC
wszyscy chorzy z nadciśnieniem 2. lub 3. stopnia
wymagają długotrwałego leczenia hipotensyjnego.
Celem pracy była retrospektywna ocena liczby
i rodzaju leków hipotensyjnych zalecanych przy
wypisie chorym przyjmowanym do ośrodka specjalistycznego
z powodu niekontrolowanego nadciśnienia
tętniczego.
Materiał i metody Na podstawie historii chorób 350
pacjentów (210 kobiet - K, średnia wieku 62 ± 16 lat
i 140 mężczyzn - M, średnia wieku 54 ± 15 lat)
dokonano retrospektywnej analizy liczby i rodzaju leków
zalecanych przy wypisie ze szpitala chorym hospitalizowanym
w specjalistycznej klinice hipertensjologicznej
w latach 2004-2007 z powodu wysokich
wartości ciśnienia tętniczego (≥ 180/100 mm Hg). Wyniki Na podstawie przeprowadzonej analizy
stwierdzono, że w celu normalizacji ciśnienia tętniczego
83,4% chorym zalecano inhibitory konwertazy
angiotensyny (ACE), następnie zaś w kolejności stosowano:
b-adrenolityki, leki z grupy antagonistów
wapnia, leki moczopędne, pochodne klonidyny,
a-adrenolityki. U 17,1% włączono antagonistę aldosteronu,
natomiast antagonistę receptora angiotensynowego
AT1 zalecono 13,7% chorych. W latach
2004-2007 istotnie statystycznie zwiększyła się częstość
stosowania sartanów oraz obserwowano wyraźną
tendencję do częstszego stosowania inhibitorów
ACE, a rzadszego b-adrenolityków. Odsetek
pacjentów, którzy przy wypisie otrzymali jeden lek
wynosił 4%, dwa leki - 16,3%, trzy leki - 26,9%,
a cztery i więcej - 52%. Pomimo zastosowanej politerapii
wysokość ciśnienia tętniczego poniżej 140/90 mm Hg w chwili wypisu uzyskano zaledwie
u 64% pacjentów.
Wnioski Najczęściej stosowanymi lekami hipotensyjnymi
są inhibitory ACE. Chorzy z 2. lub 3. stopniem
nadciśnienia tętniczego w celu jego normalizacji
wymagają zwykle skojarzonego leczenia czterema
lub nawet więcej lekami hipotensyjnymi.Background Both untreated or inadequately controlled
arterial hypertension is a serious risk factor of cardiovascular
diseases and target organ complications. According
to the ESH/ESC guidelines, all patients with AH
(arterial hypertension) stage 2. or 3. require chronic antihypertensive
therapy. The aim of the study was to retrospectively
evaluate the number and classes of antihypertensive
drugs administered to the patients discharged
from hospital who were hospitalized due to uncontrolled
hypertension.
Material and methods The retrospective evaluation of the
number and classes of prescribed antihypertensive drugs
on discharge from hospital was based on medical records
of 350 patients (210 women - F, age 62 ± 16 years;
140 men - M, age 54 ± 15 years), admitted to the hypertension
reference center in the years 2004-2007 due to high
blood pressure (≥ 180/100 mm Hg).
Results 83.4% of patients received angiotensin converting
enzyme inhibitors; and than in order of decreasing frequency: adrenergic b-antagonists, Ca-channel blockers,
diuretics, clonidine, adrenergic a-antagonists. Aldosterone
antagonists were prescribed to 17.1% and angiotensin type-
1 receptor antagonist was prescribed to 13.7% of patients.
In the analyzed period the administration of angiotensin
type-1 receptor antagonists significant increased from year
to year and we observed tendency to more frequent use of
angiotensin converting enzyme inhibitors, in parallel to
a decreasing frequency of b-blockers use. The percentage

An association between polymorphism of the heme oxygenase-1 and -2 genes and age-related macular degeneration
Iron may be implicated in the generation of oxidative stress by the catalyzing the Haber–Weiss or Fenton reaction. On the other hand, oxidative stress has been implicated in the pathogenesis of age-related macular degeneration (AMD) and heme oxygenase-1 (HO-1), encoded by the HMOX1 gene and heme oxygenase-2 (HO-2), encoded by the HMOX2 gene are important markers of iron-related oxidative stress and its consequences. Therefore, variability of the HMOX1 and HMOX2 genes might be implicated in the pathogenesis of AMD through the modulation of the cellular reaction to oxidative stress. In the present work, we investigated the association between AMD and a G → C transversion at the 19 position in the HMOX1 gene (the 19G>C-HMOX1 polymorphism, rs2071747) and a A → G transition at the −42 + 1444 position in the HMOX2 gene (the −42 + 1444A>G-HMOX2 polymorphism, rs2270363) and its modulation by some environmental factors. 279 patients with AMD and 105 controls were recruited in this study and the polymorphisms were typed by restriction fragment length polymorphism and allele-specific polymerase chain reaction (PCR). We observed an association between the occurrence of dry AMD and the G/A genotype of the −42 + 1444A>G-HMOX2 polymorphism (odds ratio (OR) 2.72), whereas the G/G genotype reduced the risk of dry AMD (OR 0.41). The G/C genotype and the C allele of the 19 G>C-HMOX1 polymorphism and the G/G genotype and the G allele of the −42 + 1444A>G-HMOX2 polymorphism were associated with progression of AMD from dry to wet form (OR 4.83, 5.20, 2.55, 1.69, respectively). On the other hand, the G/G genotype and the G allele of the 19 G>C-HMOX1 polymorphism and the A/G genotype and the A allele of the −42 + 1444A>G-HMOX2 polymorphism protected against AMD progression (OR 0.19, 0.19, 0.34, 0.59, respectively). Therefore, the 19G>C-HMOX1 and the −42 + 1444A>G-HMOX2 polymorphisms may be associated with the occurrence and progression of AMD
