    Mini-invasive foam sclerotherapy-assisted ligation versus surgical flush ligation for incompetent sapheno-popliteal junction treatment

    Objectives: Comparison of a flush sapheno-popliteal junction ligation versus a mini-invasive foam sclerotherapy-assisted ligation. Methods: Forty-eight chronic venous disease patients underwent sapheno-popliteal junction flush ligation (group A). Forty-one patients underwent small saphenous vein ligation by means of mini-invasive incision with foam sclerotherapy of the popliteal stump (group B). Results: At 4.1 ± 3.3 years mean follow-up, sapheno-popliteal junction recurrence was detected in four patients of group A (4/48; 8.3%) and in two cases of group B (3/41; 7.3%) (P=ns). Mean procedural time was 36 ± 11 minutes versus 21 ± 6 minutes (p<0.0001). A mild post-operative paresthesia lasting more than 24 h was reported in 6.3% (3/48) of group A versus 2.4% (1/41) (p<0.009) of group B. At one-year check-up, Aberdeen Varicose Vein Questionnaire significantly improved in both groups with no significant difference between group A and B. Conclusions: Foam-assisted mini-invasive sapheno-popliteal ligation represents a time and clinical-effective option, associated with a decrease in post-operative paresthesia risk

    Il territorio nazionale è disseminato di monumenti ai caduti della Prima Guerra Mondiale, che incarnano sia valori antropologici e sociali sia valori storico-artistici e architettonici da riscoprire e tutelare. Il contributo, a partire dall’analisi di due casi studio esemplari, propone una metodologia integrata finalizzata alla catalogazione di questo patrimonio culturale italiano capace di integrare alle informazioni storiche i relativi modelli conoscitivi digitali (2D e 3D) e un abaco di linee guida propedeutiche agli interventi di restauro, conservazione e valorizzazione. La possibilità di mettere a punto protocolli di documentazione e catalogazione capaci di innescare processi di gestione, ricerca, diagnostica, conservazione, restauro e valorizzazione del patrimonio culturale, apre nuovi scenari in cui i modelli conoscitivi digitali diventano il supporto per la raccolta di diverse informazioni utilizzabili a livello multidisciplinare. La disponibilità dei database di integrare dati diversi, acquisiti attraverso tecniche di rilievo tradizionali e metodologie avanzate, consente di mettere a punto un sistema informativo multimediale in cui le diverse informazioni possono essere utilizzate come base per progetti di comunicazione dedicati. La metodologia proposta è applicata nello specifico ai monumenti ai caduti della Prima Guerra Mondiale, particolarmente significativi a scala nazionale in quanto espressione fisica della memoria e di grande valore storico-artistico, architettonico e sociale, ed è finalizzata alla catalogazione (e quindi alla documentazione e alla conoscenza) e alla definizione di linee guida su base tipologica propedeutiche alla conservazione, al restauro e alla valorizzazione.The Italian territory is full of Monuments to the Fallen Soldiers of the First World War, which embody both anthropological and social significances and historical-artistic and architectural values to be rediscovered and protected. The paper, starting from the analysis of two exemplary case studies, proposes an integrated methodology aimed at cataloguing this Italian cultural heritage and able to integrate historical information with digital models (2D and 3D) and a set of guidelines preliminary to restoration, conservation and enhancement. The opportunity of developing documentation and cataloguing protocols able to trigger processes of management, research, diagnostics, conservation, restoration and enhancement of cultural heritage opens up new scenarios in which digital models become the support for the collection of different information to be managed at a multidisciplinary level. The capability of databases to integrate different data, acquired through traditional survey techniques and advanced methodologies, allows the development of a multimedia information system in which different information can be used as a basis for specific communication projects. The proposed methodology is specifically applied to the Monuments to the Fallen of the First World War, particularly significant on a national scale as a physical expression of memory and of great historical-artistic, architectural and social value. The research is aimed at cataloguing (and therefore at documentation and knowledge) and to the definition of guidelines set for this specific typology of monument and preliminary to restoration, conservation and enhancement

    The experience of patients with diabetes with the use of telemedicine and teleassistance services during the COVID-19 pandemic in Italy: Factors associated with perceived quality and willingness to continue

    Aim: The purpose of this study is to investigate the individual and contextual determinants of the perceived quality (PQ) of the telemedicine and teleassistance (TMTA) services and the willingness to continue (WC) with them among patients with diabetes using TMTA services during the COVID-19 pandemic in one large region of Italy. Methods: A structured survey was administered to patients with type 1 and 2 diabetes who used TMTA during the first wave of the COVID-19 pandemic. The questionnaire contained questions on TMTA service experience and participants' socio-demographic characteristics. Multiple regression models investigated the independent factors associated with PQ and WC. Results: The final analysis included 569 patients with diabetes (54.7% female), with an average age of 58.1 years. TMTA services' PQ and WC were high. A higher education and being unemployed were factors associated with an increased WC. Older age was negatively related to PQ. Perceived support from TMTA service was positively associated with PQ and WC. Perceived increase in disease self-management was positively associated with PQ and WC. Conclusions: Our study identified several determinants of PQ and WC. These sociodemographic and TMTA-related factors should be considered in the implementation of care pathways integrating in-person visits with TMTA. (c) 2021 Elsevier B.V. All rights reserved

    Eyelid Dermatitis and Contact Sensitization to Nickel: Results from an Italian Multi-Centric Observational Study

    Background: Due to the sensitizing constituents of eye cosmetics, allergic contact dermatitis is considered a frequent cause of eyelid dermatitis. An association between eyelid dermatitis and nickel contained in make-ups remains controversial.Objective: The study aimed to assess the association between nickel allergy, the use of pigmented makeup products and self-reported eyelid dermatitis.Method: This multi-centric, cross-sectional study enrolled 165 women sensitized to nickel (patients) and 103 women without intolerance to metals (controls). We recorded: demographics, atopy, use of pigmented eye cosmetics (mascara, eyeshadow, eyeliner, eyebrow pencil), and previous eyelid dermatitis. Among the patients, any co-sensitization to cosmetics or metals was recorded.Results: 87.3% of the patients and 91.3% of the controls reported their use of eye make-up; 44.9% and 52.4%, respectively, reported previous episodes of eyelid dermatitis, without significant differences. The occurrence of eyelid dermatitis was significantly associated with the use of eye make-up products, both in general and considering each product separately. Age, atopy, or co-sensitization to other metals or cosmetics did not affect the occurrence of eyelid dermatitis.Conclusion: Nickel allergy should not be considered the main risk factor for eyelid dermatitis. The use of pigmented eye make-up may be a triggering factor for eyelid dermatitis, probably due to an irritant action

    Lipid Plaque Burden in NSTE-ACS patients with or without COPD: insights from the SCAP trial

    Background: COPD patients have higher recurrence of adverse events and worse prognosis after acute coronary syndrome (ACS). The underlying pathophysiological mechanism is not fully elucidated. Methods: In screening for COPD in ACS (SCAP) trial (NCT02324660), ACS patients with smoking habit underwent a predischarge screening procedure to detect undiagnosed chronic obstructive pulmonary disease (UCOPD) confirmed with spirometry at 60 days. Patients were then categorized as UCOPD or no-COPD. In 65 NSTE-ACS patients, we performed near infrared spectroscopy (NIRS) in the culprit and at least one non-culprit vessel (151 vessels overall), and we calculated the SYNTAX I score. Primary endpoint was max lipid core burden index (LCBI) 4 mm. Secondary endpoints were SYNTAX score I and vessel LCBI. Results: Max LCBI 4 mm and vessel LCBI were significantly higher in the UCOPD compared to the no-COPD group (UCOPD 388±122, no-COPD 264±131, p &lt;0.001; UCOPD 118±50, no-COPD 82±42, p&lt;0.001, respectively). UCOPD patients showed higher max LCBI 4 mm and LCBI vessel both in culprit and non-culprit vessels. SYNTAX score I was comparable between the two groups (UCOPD: 13.5 [5.5-24], no-COPD: 12.5 [5-24.5], p=0.7). Conclusions: NSTE-ACS patients with UCOPD showed a higher LCBI compared to those without COPD, while SYNTAX score I was comparable between the two groups

    The frailty in elderly patients receiving cardiac interventional procedures (FRASER) program: rational and design of a multicenter prospective study

    Background: Frailty has become a high-priority issue in cardiovascular medicine because of the aging of cardiovascular patients. Simple and reproducible tools to assess frailty in elderly patients are clearly on demand. Their application may help physicians in the selection of invasive and medical treatments and in the timing and modality of the follow-up. The frailty in elderly patients receiving cardiac interventional procedures (FRASER) program is designed with the aim to validate the use of the short physical performance battery (SPPB) as prognostic tools in patients admitted to hospital for acute coronary syndrome (ACS). Methods: The FRASER program is a multicenter prospective study involving 4 Italian cardiology units. The FRASER program enrolls only patients aged ≥70 years. The core of the FRASER program includes patients admitted to hospital for ACS. The aims are (1) to describe SPPB distribution before hospital discharge and (2) to investigate the prognostic role of SPPB score. The primary outcome is a composite of 1-year all-cause mortality and hospital readmission for any cause. Ancillary analyses will be focused on different study populations (patients hospitalized for arrhythmias or acute heart failure or symptomatic severe aortic stenosis) and on different tools to assess frailty (multidimensional prognostic index, clinical frailty score, grip strength). Discussion: The FRASER program will fill critical gaps in the knowledge regarding the link between frailty, cardiovascular disease, interventional procedures and outcome and will help physicians in the generation of a more personalized risk assessment and in the identification of potential targets for interventions
