    Comparison of the Proseal, Supreme, and I-Gel SAD in Gynecological Laparoscopic Surgeries

    We compared proseal, supreme, and i-gel supraglottic airway devices in terms of oropharyngeal leak pressures and airway morbidities in gynecological laparoscopic surgeries. One hundred and five patients undergoing elective surgery were subjected to general anesthesia after which they were randomly distributed into three groups. Although the oropharyngeal leak pressure was lower in the i-gel group initially (mean ± standard deviation; 23.9 ± 2.4, 24.9 ± 2.9, and 20.9 ± 3.5, resp.), it was higher than the proseal group and supreme group at 30 min of surgery after the trendelenburg position (25.0 ± 2.3, 25.0 ± 1.9, and 28.3 ± 2.3, resp.) and at the 60 min of surgery (24.2 ± 2.1, 24.8 ± 2.2, and 29.5 ± 1.1, resp.). The time to apply the supraglottic airway devices was shorter in the i-gel group (12.2 (1.2), 12.9 (1.0), and 6.7 (1.2), resp., P=0.001). There was no difference between the groups in terms of their fiber optic imaging levels. pH was measured at the anterior and posterior surfaces of the pharyngeal region after the supraglottic airway devices were removed; the lowest pH values were 5 in all groups. We concluded that initial oropharyngeal leak pressures obtained by i-gel were lower than proseal and supreme, but increased oropharyngeal leak pressures over time, ease of placement, and lower airway morbidity are favorable for i-gel

    Nephrotic syndrome in a patient with Glycogen Storage Disease Type IXb.

    Introduction: Glycogen storage disorder (GSD) IXb is characterized by liver and muscle involvement. We present a GSD IXb patient with an incidental union of nephrotic syndrome. Case Report: A 4 year-old-patient was diagnosed with GSD IXb at 13 months of age with mildly elevated transaminases and hepatomegaly. During the follow-up period, there was no hypoglycemia. Development and growth were normal. In the last month, the onset of generalized edema was reported. Urinalysis showed a high protein level. He had low serum albumin, high serum triglycerides cholesterol. Complement levels were normal. The patient was diagnosed as minimal change disease with a renal biopsy. He was treated with oral prednisone. Discussion: Minimal Change Disease is the most common cause of idiopathic nephrotic syndrome cases in children and the first step for therapy is the usage of corticosteroids. This is the first report of nephrotic syndrome associated with GSD IXb disease

    PubMed ID: 19434424Betaine therapy was given for 2 years to a 2- year-old boy with 5,10-methylenetetrahydrofolate reductase deficiency. Used as a methyl donor to lower homocysteine levels through methylation of methionine, betaine has been reported to be effective in treating homocystinuria. Satisfactory biochemical and clinical responses were obtained with the following regimen: betaine started in the newborn period at increasing doses to reach 1 g given six times a day. It is suggested that frequent administration of a moderate dose may provide clinical and biochemical benefit. © Springer-Verlag 2009

    Research of cystic echinococcus serology in people living in rural areas and dealing with animals with the immunochromatographic method: A small-scale cross-sectional study

    Cystic echinococcus (CE) is a parasitic disease in humans and animals. Our study was performed with the aim of identifying the serology of hydatic cyst disease caused by the Echinococcus granulosus vector, causing a significant health problem in Eastern Anatolia and specifically in our province, among people living in rural areas and involved in animal husbandry. The population for this study comprised 75 volunteers living in rural regions of Erzurum practicing animal husbandry. Blood samples were stored at -40 °C until the time of the study. A survey form was completed to be able to identify the demographic data of participants and to reveal risk factors in terms of infectious vectors. After thawing blood samples at the time of the study, the immunochromatographic test procedure was implemented. The age distribution in the study group was 18 to 70 years, with a mean age of 46.8±15.97 years. The group comprised a total of 75 people, including 29 women (38.7%) and 46 men (61.3%). Of these 75 people with immunochromatographic screening performed, 72 were negative (96%) and 3 were weakly positive (0.5) (4%). It is notable that 4% CE seropositivity was found among individuals with no complaints. Linked to this seropositivity result, it is considered that creating a comprehensive CE screening program will be beneficial for individuals involved in animal husbandry.
