Effect of in-hospital comprehensive geriatric assessment (CGA) in older people with hip fracture : the protocol of the Trondheim Hip Fracture Trial
Background: Hip fractures in older people are associated with high morbidity, mortality, disability and reduction in
quality of life. Traditionally people with hip fracture are cared for in orthopaedic departments without additional
geriatric assessment. However, studies of postoperative rehabilitation indicate improved efficiency of
multidisciplinary geriatric rehabilitation as compared to traditional care. This randomized controlled trial (RCT) aims
to investigate whether an additional comprehensive geriatric assessment of hip fracture patients in a special
orthogeriatric unit during the acute in-hospital phase may improve outcomes as compared to treatment as usual
in an orthopaedic unit.
Methods/design: The intervention of interest, a comprehensive geriatric assessment is compared with traditional
care in an orthopaedic ward. The study includes 401 home-dwelling older persons >70 years of age, previously
able to walk 10 meters and now treated for hip fracture at St. Olav Hospital, Trondheim, Norway. The participants
are enrolled and randomised during the stay in the Emergency Department. Primary outcome measure is mobility
measured by the Short Physical Performance Battery (SPPB) at 4 months after surgery. Secondary outcomes
measured at 1, 4 and 12 months postoperatively are place of residence, activities of daily living, balance and gait,
falls and fear of falling, quality of life and depressive symptoms, as well as use of health care resources and survival.
Discussion: We believe that the design of the study, the randomisation procedure and outcome measurements
will be of sufficient strength and quality to evaluate the impact of comprehensive geriatric assessment on mobility
and other relevant outcomes in hip fracture patients
Appropriate targeting of artemisinin-based combination therapy by community health workers using malaria rapid diagnostic tests: findings from randomized trials in two contrasting areas of high and low malaria transmission in south-western Uganda.
OBJECTIVE: To compare the impact of malaria rapid diagnostic tests (mRDTs), used by community health workers (CHWs), on the proportion of children <5 years of age receiving appropriately targeted treatment with artemisinin-based combination therapy (ACT), vs. presumptive treatment. METHODS: Cluster-randomized trials were conducted in two contrasting areas of moderate-to-high and low malaria transmission in rural Uganda. Each trial examined the effectiveness of mRDTs in the management of malaria and targeting of ACTs by CHWs comparing two diagnostic approaches: (i) presumptive clinical diagnosis of malaria [control arm] and (ii) confirmatory diagnosis with mRDTs followed by ACT treatment for positive patients [intervention arm], with village as the unit of randomisation. Treatment decisions by CHWs were validated by microscopy on a reference blood slide collected at the time of consultation, to compare the proportion of children <5 years receiving appropriately targeted ACT treatment, defined as patients with microscopically-confirmed presence of parasites in a peripheral blood smear receiving artemether-lumefantrine or rectal artesunate, and patients with no malaria parasites not given ACT. RESULTS: In the moderate-to-high transmission area, ACT treatment was appropriately targeted in 79.3% (520/656) of children seen by CHWs using mRDTs to diagnose malaria, vs. 30.8% (215/699) of children seen by CHWs using presumptive diagnosis (P < 0.001). In the low transmission area, 90.1% (363/403) children seen by CHWs using mRDTs received appropriately targeted ACT treatment vs. 7.8% (64/817) seen by CHWs using presumptive diagnosis (P < 0.001). Low mRDT sensitivity in children with low-density parasitaemia (<200 parasites/μl) was identified as a potential concern. CONCLUSION: When equipped with mRDTs, ACT treatments delivered by CHWs are more accurately targeted to children with malaria parasites. mRDT use could play an important role in reducing overdiagnosis of malaria and improving fever case management within iCCM, in both moderate-to-high and low transmission areas. Nonetheless, missed treatments due to the low sensitivity of current mRDTs in patients with low parasite density are a concern. For community-based treatment in areas of low transmission and/or non-immune populations, presumptive treatment of all fevers as malaria may be advisable, until more sensitive diagnostic assays, suitable for routine use by CHWs in remote settings, become available

This paper presents the effects of the concentration of solid nanoparticles in the liquid feedstock injection on the
high-velocity suspension flame spray (HVSFS) process. Four different concentrations of solid nanoparticles in suspension
droplets with various droplet diameters are used to study gas dynamics, vaporization rate, and secondary breakup. Two types of
injections, viz. surface and group, are used. The group-type injection increases the efficiency of droplet disintegration and the
evaporation process and reduces the gas cooling. The initiation of the fragmentation process is difficult for small droplets carrying
a high concentration of nanoparticles. Also, smaller droplets undergo rapid vaporization, leaving clogs of nanoparticles in the
middle of the barrel. For larger droplets, severe fragmentation occurs inside the combustion chamber. For a higher concentration
of nanoparticles, droplets exit the gun without complete evaporation. The results suggest that, in coating applications involving a
higher concentration of nanoparticles, smaller droplet sizes are preferred
Cost-effectiveness analysis of malaria rapid diagnostic tests for appropriate treatment of malaria at the community level in Uganda.
In Sub-Saharan Africa, malaria remains a major cause of morbidity and mortality among children under 5, due to lack of access to prompt and appropriate diagnosis and treatment. Many countries have scaled-up community health workers (CHWs) as a strategy towards improving access. The present study was a cost-effectiveness analysis of the introduction of malaria rapid diagnostic tests (mRDTs) performed by CHWs in two areas of moderate-to-high and low malaria transmission in rural Uganda. CHWs were trained to perform mRDTs and treat children with artemisinin-based combination therapy (ACT) in the intervention arm while CHWs offered treatment based on presumptive diagnosis in the control arm. Data on the proportion of children with fever 'appropriately treated for malaria with ACT' were captured from a randomised trial. Health sector costs included: training of CHWs, community sensitisation, supervision, allowances for CHWs and provision of mRDTs and ACTs. The opportunity costs of time utilised by CHWs were estimated based on self-reporting. Household costs of subsequent treatment-seeking at public health centres and private health providers were captured in a sample of households. mRDTs performed by CHWs was associated with large improvements in appropriate treatment of malaria in both transmission settings. This resulted in low incremental costs for the health sector at US13.3 were found in the low transmission area due to lower utilisation of CHW services and higher programme costs. Incremental costs from a societal perspective were marginally higher. The use of mRDTs by CHWs improved the targeting of ACTs to children with malaria and was likely to be considered a cost-effective intervention compared to a presumptive diagnosis in the moderate-to-high transmission area. In contrast to this, in the low transmission area with low attendance, RDT use by CHWs was not a low cost intervention

Long-term follow-up of disability pensioners having musculoskeletal disorders
<p>Abstract</p> <p>Background</p> <p>Previously we have conducted a randomised controlled trial (RCT) to evaluate the effect of a brief cognitive behavioural program with a vocational approach aiming to return disability pensioners with back pain to work, as compared to no intervention. One year after the intervention, 10 participants (22%) who received the program and 5 (11%) in the control group reported to have entered a return to work process. The aims of this study were to evaluate long-term effects of the intervention, and compare this effect to 2 reference populations not participating in the original trial.</p> <p>Methods</p> <p>Three groups of disability pensioners were investigated: 1) Disability pensioners having back pain (n = 89) previously participating in the RCT (randomized to either a brief cognitive behavioural intervention or to a control group), 2) 342 disability pensioners having back pain, but refusing to participate in the study and 3) 449 disability pensioners having other musculoskeletal disorders than back pain. Primary outcome was return to work, defined as a reduction in payment of disability pension.</p> <p>Results</p> <p>Only 2 of 89 (2.3%) participants from the RCT had reduced disability pension at 3-years follow-up, both from the control group. None of the participants that had been in a process of returning to work after 1 year had actually gained employment at 3-years follow-up. In the 2 groups not participating in the previous RCT, only 4 (1.2%) and 8 (1.6%) had returned to work after 3 years respectively.</p> <p>Conclusion</p> <p>The number of pensioners who returned to work was negligible in all groups regardless of having participated in a cognitive behavioural intervention or not.</p
Validity, Identification, and Distribution of the Roundscale Spearfish, Tetrapturus georgii (Teleostei: Istiophoridae): Morphological and Molecular Evidence
The roundscale spearfish, Tetrapturus georgii Lowe, 1840, is known only from four specimens from the Mediterranean and eastern North Atlantic. Additional specimens have not been identified since 1961, making the validity and distribution of this species unclear. Analysis of 16 billfish specimens from the western North Atlantic on the basis of scale morphology, anus position, and mitochondrial DNA confirms the validity of this species and extends its distribution. Mid-lateral scales are soft, notably rounded anteriorly, and bear 2–3 points distinct from those of the sympatric longbill spearfish (Tetrapturus pfluegeri Robins and de Sylva, 1963) and white marlin (Tetrapturus albidus Poey, 1860). Position of anus relative to first anal fin and a related morphometric ratio (distance from anus to first anal fin origin: height of first anal fin) are intermediate between T. pfluegeri and T. albidus. These characteristics match those described by Robins (1974) from the four eastern North Atlantic specimens of T. georgii. The mitochondrial ND4L, ND4, and cyt b gene sequences strongly support reciprocal monophyly of the western North Atlantic specimens relative to other Atlantic istiophorids. The difficulty in distinguishing between morphologically similar T. georgii and T. albidus in the field and the previously unrecognized presence of T. georgii in the western North Atlantic has implications for stock assessments of T. albidus, a species that is severely overfished
