Analysis of platelet-rich plasma extraction variations in platelet and blood components between 4 common commercial kits
Background: Platelet-rich plasma (PRP) has been extensively used as a treatment in tissue healing in tendinopathy, muscle injury, and osteoarthritis. However, there is variation in methods of extraction, and this produces different types of PRP.
Purpose: To determine the composition of PRP obtained from 4 commercial separation kits, which would allow assessment of current classification systems used in cross-study comparisons.
Study Design: Controlled laboratory study.
Methods: Three normal adults each donated 181 mL of whole blood, some of which served as a control and the remainder of which was processed through 4 PRP separation kits: GPS III (Biomet Biologics), Smart-Prep2 (Harvest Terumo), Magellan (Arteriocyte Medical Systems), and ACP (Device Technologies). The resultant PRP was tested for platelet count, red blood cell count, and white blood cell count, including differential in a commercial pathology laboratory. Glucose and pH measurements were obtained from a blood gas autoanalyzer machine.
Results: Three kits taking samples from the “buffy coat layer” were found to have greater concentrations of platelets (3-6 times baseline), while 1 kit taking samples from plasma was found to have platelet concentrations of only 1.5 times baseline. The same 3 kits produced an increased concentration of white blood cells (3-6 times baseline); these consisted of neutrophils, leukocytes, and monocytes. This represents high concentrations of platelets and white blood cells. A small drop in pH was thought to relate to the citrate used in the sample preparation. Interestingly, an unexpected increase in glucose concentrations, with 3 to 6 times greater than baseline levels, was found in all samples.
Conclusion:This study reveals the variation of blood components, including platelets, red blood cells, leukocytes, pH, and glucose in PRP extractions. The high concentrations of cells are important, as the white blood cell count in PRP samples has frequently been ignored, being considered insignificant. The lack of standardization of PRP preparation for clinical use has contributed at least in part to the varying clinical efficacy in PRP use.
Clinical Relevance: The variation of platelet and other blood component concentrations between commercial PRP kits may affect clinical treatment outcomes. There is a need for standardization of PRP for clinical use
The Improving Rural Cancer Outcomes Trial: a cluster-randomised controlled trial of a complex intervention to reduce time to diagnosis in rural cancer patients in Western Australia.
BACKGROUND: Rural Australians have poorer survival for most common cancers, due partially to later diagnosis. Internationally, several initiatives to improve cancer outcomes have focused on earlier presentation to healthcare and timely diagnosis. We aimed to measure the effect of community-based symptom awareness and general practice-based educational interventions on the time to diagnosis in rural patients presenting with breast, prostate, colorectal or lung cancer in Western Australia. METHODS: 2 Ă— 2 factorial cluster randomised controlled trial. Community Intervention: cancer symptom awareness campaign tailored for rural Australians. GP intervention: resource card with symptom risk assessment charts and local cancer referral pathways implemented through multiple academic detailing visits. Trial Area A received the community symptom awareness and Trial Area B acted as the community campaign control region. Within both Trial Areas general practices were randomised to the GP intervention or control. PRIMARY OUTCOME: total diagnostic interval (TDI). RESULTS: 1358 people with incident breast, prostate, colorectal or lung cancer were recruited. There were no significant differences in the median or ln mean TDI at either intervention level (community intervention vs control: median TDI 107.5 vs 92 days; ln mean difference 0.08 95% CI -0.06-0.23 P=0.27; GP intervention vs control: median TDI 97 vs 96.5 days; ln mean difference 0.004 95% CI -0.18-0.19 P=0.99). There were no significant differences in the TDI when analysed by factorial design, tumour group or sub-intervals of the TDI. CONCLUSIONS: This is the largest trial to test the effect of community campaign or GP interventions on timeliness of cancer diagnosis. We found no effect of either intervention. This may reflect limited dose of the interventions, or the limited duration of follow-up. Alternatively, these interventions do not have a measurable effect on time to cancer diagnosis
Mini-dose glucagon rescue for mild hypoglycaemia in children with type 1 diabetes: The Brisbane experience
Objectives: To evaluate the use of small doses of glucagon using an insulin syringe in mild or impending hypoglycaemia in children with type 1 diabetes. Methods: Data were collected from patients attending the Paediatric Diabetes Clinic at the Queensland Diabetes Centre at the Mater Hospital, Brisbane in 2002-2004 following the institution of a new protocol for home management of mild or impending hypoglycaemia associated with inability or refusal to take oral carbohydrate. The protocol recommended the use of subcutaneous injections of glucagon using insulin syringes at a dose of two ' units ' (20 mu g) in children 2 years of age or younger, and for older children one unit per year of age up to a maximum of 15 units (150 mu g), with an additional doubled dose given if the blood glucose had not increased in 20 min. Results: Over a 2-year period, 25 children were treated with mini-dose glucagon on a total of 38 occasions. Additional doses were required for recurring hypoglycaemia on 20 (53%) occasions. The child could be managed at home on 32 (84%) of these 38 occasions, with only 6 (16%) children needing hospital treatment. Conclusions: Our study confirmed that small doses of glucagon given subcutaneously with an insulin syringe is a simple, practical and effective home treatment of mild or impending hypoglycaemia due to gastroenteritis or food refusal in children with type 1 diabetes
Illusory touch temporarily improves sensation in areas of chronic numbness: A brief communication
Background: Creating the visual illusion of touch can improve tactile perception in healthy subjects.
Objective: We were interested in seeing if creating the illusion of touch in an insensate area could improve sensation in that area. Methods: Fourteen people with chronic numbness participated in a randomised cross-over experiment. The four conditions were: 1. Stimulation over the un-affected limb with mirror visual feedback (experimental condition). 2. Stimulation over the affected limb with mirror visual feedback 3. Stimulation over the un-affected limb without mirror visual feedback. 4. Stimulation over the affected limb without mirror visual feedback. Participants were assessed pre and post each condition using the Ten-Test and mechanical detection thresholds. Data were analysed using linear mixed models. Result: Only the experimental condition produced a change in the Ten-Test (mean difference -1.1 95% CI: -1.8 to -0.4; p=0.003), corresponding to a 24% improvement in sensation. No differences were observed for any condition in mechanical detection thresholds. Conclusion: Creating the illusion of touch may improve sensory function in areas of chronic numbness. This preliminary finding adds to the growing body of evidence supporting the use of techniques which directly target cortical function in people with peripheral nerve injury
Excisional treatment in women with cervical adenocarcinoma in situ (AIS): a prospective randomised controlled non-inferiority trial to compare AIS persistence/recurrence after loop electrosurgical excision procedure with cold knife cone biopsy: protocol for a pilot study
INTRODUCTION: Adenocarcinoma in situ (AIS) of the uterine cervix is the precursor to invasive endocervical adenocarcinoma. An excisional biopsy such as a cold knife cone biopsy (CKC) should be performed to exclude invasive adenocarcinoma. Loop electrosurgical excision procedure (LEEP) is an alternative modality to CKC but is controversial in AIS. There is a perception that there is a greater likelihood of incomplete excision of AIS with LEEP because the depth of excised tissue tends to be smaller and the tissue margins may show thermal artefact which can interfere with pathology assessment. In the USA, guidelines recommend that any treatment modality can be used to excise AIS, provided that the specimen remains intact with interpretable margins. However, there are no high-quality studies comparing LEEP with CKC and well-designed prospective studies are needed. If such a study were to show that LEEP was non-inferior to CKC for the outcomes of post-treatment persistence, recurrence and adenocarcinoma, LEEP could be recommended as an appropriate treatment option for AIS in selected patients. This would benefit women because, unlike CKC, LEEP does not require general anaesthesia and may be associated with reduced morbidity. METHODS AND ANALYSIS: The proposed exploratory study is a parallel group trial with an allocation ratio of 2:1 in favour of the intervention (LEEP: CKC). Participants are women aged ≥18 to ≤45 years diagnosed with AIS on cervical screening and/or colposcopically directed biopsy in Australia and New Zealand, who are to receive excisional treatment in a tertiary level centre. ETHICS AND DISSEMINATION: Ethical approval for the study has been granted by the St John of God Healthcare Human Research Ethics Committee (reference number #1137). Results from the study will be presented at conferences and published in a peer-reviewed scientific journal. Maternal age at birth and childhood type 1 diabetes: a pooled analysis of 30 observational studies.
OBJECTIVE—The aim if the study was to investigate whether
children born to older mothers have an increased risk of type 1
diabetes by performing a pooled analysis of previous studies
using individual patient data to adjust for recognized confounders.
RESEARCH DESIGN AND METHODS—Relevant studies published
before June 2009 were identified from MEDLINE, Web of
Science, and EMBASE. Authors of studies were contacted and
asked to provide individual patient data or conduct prespecified
analyses. Risk estimates of type 1 diabetes by maternal age were
calculated for each study, before and after adjustment for
potential confounders. Meta-analysis techniques were used to
derive combined odds ratios and to investigate heterogeneity
among studies.
RESULTS—Data were available for 5 cohort and 25 case-control
studies, including 14,724 cases of type 1 diabetes. Overall, there
was, on average, a 5% (95% CI 2–9) increase in childhood type 1
diabetes odds per 5-year increase in maternal age (P 0.006),
but there was heterogeneity among studies (heterogeneity I2
70%). In studies with a low risk of bias, there was a more marked
increase in diabetes odds of 10% per 5-year increase in maternal
age. Adjustments for potential confounders little altered these
estimates.
CONCLUSIONS—There was evidence of a weak but significant
linear increase in the risk of childhood type 1 diabetes across the
range of maternal ages, but the magnitude of association varied
between studies. A very small percentage of the increase in the
incidence of childhood type 1 diabetes in recent years could be
