Transoral laser surgery for laryngeal carcinoma: has Steiner achieved a genuine paradigm shift in oncological surgery?
Transoral laser microsurgery applies to the piecemeal removal of malignant tumours of the upper aerodigestive tract using the CO2 laser under the operating microscope. This method of surgery is being increasingly popularised as a single modality treatment of choice in early laryngeal cancers (T1 and T2) and occasionally in the more advanced forms of the disease (T3 and T4), predomi- nantly within the supraglottis.
Thomas Kuhn, the American physicist turned philosopher and historian of science, coined the phrase ‘paradigm shift’ in his groundbreaking book The Structure of Scientific Revolutions. He argued that the arrival of the new and often incompatible idea forms the core of a new paradigm, the birth of an entirely new way of thinking. This article discusses whether Steiner and col- leagues truly brought about a paradigm shift in oncological surgery.
By rejecting the principle of en block resection and by replacing it with the belief that not only is it oncologically safe to cut through the substance of the tumour but in doing so one can actually achieve better results, Steiner was able to truly revolutionise the man- agement of laryngeal cancer. Even though within this article the repercussions of his insight are limited to the upper aerodigestive tract oncological surgery, his willingness to question other peoples’ dogma makes his contribution truly a genuine paradigm shift
Cholesterol and the risk of grade-specific prostate cancer incidence: evidence from two large prospective cohort studies with up to 37 years' follow up
<b>Background</b>
High cholesterol may be a modifiable risk factor for prostate cancer but results have been inconsistent and subject to potential "reverse causality" where undetected disease modifies cholesterol prior to diagnosis.<p></p>
<b>Methods</b>
We conducted a prospective cohort study of 12,926 men who were enrolled in the Midspan studies between 1970 and 1976 and followed up to 31st December 2007. We used Cox-Proportional Hazards Models to evaluate the association between baseline plasma cholesterol and Gleason grade-specific prostate cancer incidence. We excluded cancers detected within at least 5 years of cholesterol assay.<p></p>
<b>Results</b>
650 men developed prostate cancer in up to 37 years' follow-up. Baseline plasma cholesterol was positively associated with hazard of high grade (Gleason score[greater than or equal to]8) prostate cancer incidence (n=119). The association was greatest among men in the 4th highest quintile for cholesterol, 6.1 to <6.69 mmol/l, Hazard Ratio 2.28, 95% CI 1.27 to 4.10, compared with the baseline of <5.05 mmol/l. This association remained significant after adjustment for body mass index, smoking and socioeconomic status.<p></p>
<b>Conclusions</b>
Men with higher cholesterol are at greater risk of developing high-grade prostate cancer but not overall risk of prostate cancer. Interventions to minimise metabolic risk factors may have a role in reducing incidence of aggressive prostate cancer
Skyrmion fluctuations at a first-order phase transition boundary
Magnetic skyrmions are topologically protected spin textures with promising prospects for applications in data storage. They can form a lattice state due to competing magnetic interactions and are commonly found in a small region of the temperature - magnetic field phase diagram. Recent work has demonstrated that these magnetic quasi-particles fluctuate at the μeV energy scale. Here, we use a coherent x-ray correlation method at an x-ray free-electron laser to investigate these fluctuations in a magnetic phase coexistence region near a first-order transition boundary where fluctuations are not expected to play a major role. Surprisingly, we find that the relaxation of the intermediate scattering function at this transition differs significantly compared to that deep in the skyrmion lattice phase. The observation of a compressed exponential behavior suggests solid-like dynamics, often associated with jamming. We assign this behavior to disorder and the phase coexistence observed in a narrow field-window near the transition, which can cause fluctuations that lead to glassy behavior
Using joint species distribution modelling to predict distributions of seafloor taxa and identify vulnerable marine ecosystems in New Zealand waters
\ua9 The Author(s) 2024.Effective ecosystem-based management of bottom-contacting fisheries requires understanding of how disturbances from fishing affect seafloor fauna over a wide range of spatial and temporal scales. Spatial predictions of abundance for 67 taxa were developed, using an extensive dataset of faunal abundances collected using a towed camera system and spatially explicit predictor variables including bottom-trawl fishing effort, using a Joint Species Distribution Model (JSDM). The model fit metrics varied by taxon: the mean tenfold cross-validated AUC score was 0.70 \ub1 0.1 (standard deviation) for presence–absence and an R2 of 0.11 \ub1 0.1 (standard deviation) for abundance models. Spatial predictions of probability of occurrence and abundance (individuals per km2) varied by taxon, but there were key areas of overlap, with highest predicted taxon richness in areas of the continental shelf break and slope. The resulting joint predictions represent significant advances on previous predictions because they are of abundance, allow the exploration of co-occurrence patterns and provide credible estimates of taxon richness (including for rare species that are often not included in more commonly used single-species distribution modelling). Habitat-forming taxa considered to be Vulnerable Marine Ecosystem (VME) indicators (those taxa that are physically or functionally fragile to anthropogenic impacts) were identified in the dataset. Spatial estimates of likely VME distribution (as well as associated estimates of uncertainty) were predicted for the study area. Identifying areas most likely to represent a VME (rather than simply VME indicator taxa) provides much needed quantitative estimates of vulnerable habitats, and facilitates an evidence-based approach to managing potential impacts of bottom-trawling

Economic and other barriers to adopting recommendations to prevent childhood obesity: results of a focus group study with parents
Abstract Background Parents are integral to the implementation of obesity prevention and management recommendations for children. Exploration of barriers to and facilitators of parental decisions to adopt obesity prevention recommendations will inform future efforts to reduce childhood obesity. Methods We conducted 4 focus groups (2 English, 2 Spanish) among a total of 19 parents of overweight (BMI ≥ 85th percentile) children aged 5-17 years. The main discussion focused on 7 common obesity prevention recommendations: reducing television (TV) watching, removing TV from child's bedroom, increasing physically active games, participating in community or school-based athletics, walking to school, walking more in general, and eating less fast food. Parents were asked to discuss what factors would make each recommendation more difficult (barriers) or easier (facilitators) to follow. Participants were also asked about the relative importance of economic (time and dollar costs/savings) barriers and facilitators if these were not brought into the discussion unprompted. Results Parents identified many barriers but few facilitators to adopting obesity prevention recommendations for their children. Members of all groups identified economic barriers (time and dollar costs) among a variety of pertinent barriers, although the discussion of dollar costs often required prompting. Parents cited other barriers including child preference, difficulty with changing habits, lack of information, lack of transportation, difficulty with monitoring child behavior, need for assistance from family members, parity with other family members, and neighborhood walking safety. Facilitators identified included access to physical activity programs, availability of alternatives to fast food and TV which are acceptable to the child, enlisting outside support, dietary information, involving the child, setting limits, making behavior changes gradually, and parental change in shopping behaviors and own eating behaviors. Conclusions Parents identify numerous barriers to adopting obesity prevention recommendations, most notably child and family preferences and resistance to change, but also economic barriers. A Quantum Scattering Interferometer
The collision of two ultra-cold atoms results in a quantum-mechanical
superposition of two outcomes: each atom continues without scattering and each
atom scatters as a spherically outgoing wave with an s-wave phase shift. The
magnitude of the s-wave phase shift depends very sensitively on the interaction
between the atoms. Quantum scattering and the underlying phase shifts are
vitally important in many areas of contemporary atomic physics, including
Bose-Einstein condensates, degenerate Fermi gases, frequency shifts in atomic
clocks, and magnetically-tuned Feshbach resonances. Precise measurements of
quantum scattering phase shifts have not been possible until now because, in
scattering experiments, the number of scattered atoms depends on the s-wave
phase shifts as well as the atomic density, which cannot be measured precisely.
Here we demonstrate a fundamentally new type of scattering experiment that
interferometrically detects the quantum scattering phase shifts of individual
atoms. By performing an atomic clock measurement using only the scattered part
of each atom, we directly and precisely measure the difference of the s-wave
phase shifts for the two clock states in a density independent manner. Our
method will give the most direct and precise measurements of ultracold
atom-atom interactions and will place stringent limits on the time variations
