    Cerebrovascular Accident Caused by Embolic Atrial Myxoma

    Background: Atrial myxomas, the most common type of cardiac tumors, can cause life-threatening complications. As most cardiac myxomas are surgically curable, early diagnosis is crucial. Cardiac tumors can present with cardiac and embolic manifestations, and should be considered in the differential diagnosis of patients presenting with such symptoms. In this case report, we describe a young, healthy patient who presented with stroke symptoms secondary to embolic atrial myxoma. Case Presentation: Case Presentation A 45-year-old male with no past medical history presented with an acute onset of dysarthria, described as garbled speech. He also experienced weakness and heaviness of his right hand, which impaired his ability to brush his teeth and write. He denied left-handed or lower extremity weakness, head trauma, facial droop, dysphagia, migraines, diplopia/visual changes, confusion, or memory loss. He denied tobacco, alcohol, or drug use. Review of systems was otherwise negative, although the patient reported that whenever he participated in strenuous physical activity over the past few years, he would become nauseated

    Integrating Web-based e-Learning in TVET to Enhance the Literacy and Socio-economic Condition for Sustainable Development of Bangladesh

    The purpose of the research is to use the technologies to shift the emphasis from teaching to learning. The ultimate goal of Web-based e-Learning is to bring the learning to the learners, not to bring the learners’ to learning. The teaching-learning phenomena of developed countries have changed from Brick to Click approaches (e-Learning). However, Web-based e-Learning is still new paradigm in developing countries like Bangladesh. Web-based e-Learning is absent in technical and vocational education and training (TVET) system. TVET in Bangladesh is depriving from the benefit of Web-based e-Leaning. Thus, the aims of the research are to illustrate the residence with mode of internet use and internet services with level of education; to explore the current literacy rate (%) by different economic classes in rural and urban areas; to identify the physical facilities of TVET classrooms; and to assess the infrastructural conditions of TVET institutions for providing the suggestions to Web-based e-Learning integration in Bangladesh. The researchers have visited 45 TVET institutions and observed 210 live classrooms in Bangladesh. The infrastructural conditions of 30 TVET institutions in urban areas were also observed with the structured questionnaires to get the results for applying the concept of Web-based e-Learning integration. The expected outcomes of the research are, once the e-Learning system can be introduced in TVET in developing countries like Bangladesh, there will be a breakthrough in the education system. More students will be enrolled, outreach will go further, better learning material will ensure quality output, ICT based technology will be adopted in the process of and finally bring an integrated benefit to the society.   Keywords: Web-based e-Learning, TVET, Literacy rate and Socio-economic development

    Students’ self-reflections of own participation in English language oral class

    The English language learners’ needs changes from time to time. In Malaysia, the focus of English teaching and learning has shifted from accuracy to fluency; the effective usage of the language. Learners are expected to use the English language in daily communication and to learn the workplace English language communication skill. Due to this demand, language lecturers at higher institutions are expected to assist English language learners to use the language fluently rather than to concentrate heavily on form. Nevertheless, instigating speaking activities in Malaysian classrooms has never been easy and successful. This research was conducted with an aim to identify the underlying factors that motivate or demotivate students’ participation in an English oral classroom. The data were qualitatively collected via students’ self-reflection reports on their own participation in the English language oral class. It was found that teacher’s personality traits, students’ communication skills, learning environment, students’ motivation, self-confidence and topical knowledge, and fascinating discussion topics play significant roles in students’ participation in the English oral class. Thus, these findings could guide language teachers to consider relevant and creative oral activities to encourage students’ active classroom participation

    History of Present Illness CHIEF COMPLAINT: Left breast pain/swelling 53-year-old female with PMHx of bilateral breast implants, IV drug use, and hypertension presents to the emergency department with swelling and pain to the left breast over the last 5 months after an assault with trauma to the area. She was seen at a previous hospital for these symptoms 3 months ago and reports fluid was drained from the left breast at that time. She reports subjective fevers and chills but denies other symptoms at this time including drainage or discharge from the breast/ nipple, chest pain, or shortness of breath.
