Activity of isoflavones and berberine on vasomotor symptoms and lipid profile in menopausal women

The aim of this study was to evaluate the efficacy of a food supplement combination based on isoflavones and berberine (ISB) in the treatment of menopausal symptoms and dyslipidaemia. Isoflavones are extracted from soy and absorbed in the body after being activated by lactobacillus. Berberine, extracted from the plant Berberis aristata, lowers plasma cholesterol and triglycerides (TG) by increasing low-density lipoprotein (LDL) receptors and reducing hepatic synthesis of TG. One hundred twenty women with a mean age of 54.8 ± 0.6 years were enrolled and randomized to treatment with ISB (estromineral lipid [EL] = 60 cases) or calcium and vitamin D(3) (CaD = 60 cases). Menopausal symptoms, plasma cholesterol, and TG were evaluated at baseline, and after 4 and 12 weeks. EL treatment significantly lowered plasma total cholesterol (-13.5% ± 0.7 vs -0.2% ± 0.5), LDL cholesterol (-12.4% ± 1.5 vs + 0.8 % ± 0.7) and TG (-18.9% ± 2.5 vs -1.3% ± 1.2) and improved menopausal symptoms compared with CaD treatment. SURGICAL TREATMENT OF STRESS URINARY INCONTINENCE: A 3-YEAR FOLLOW-UP

OBJECTIVES: To evaluate the objective and subjective midterm outcomes of transobturator tape (TOT) in the treatment of female stress urinary incontinence (SUI). MATERIALS AND METHODS: A total of 114 consecutive patients affected by stress urinary incontinence (mean age 59±10 years), who underwent the TOT procedure between July 2004 and May 2008 (46 patients received the Safyre, 30 patients the PelviLace, 20 patients the Monarc, 10 patients the DynaMesh and 8 the Align sling), were assessed in June 2008. An evaluation of the patients, based on history, physical examination, stress test, urodynamic tests, and the compilation of two specific quality of life questionnaires, was performed before and after surgery. RESULTS: The therapeutic midterm failure rate for the TOT procedure was 4.4% (5 of 114 patients). The midterm cure rate was 95.6% (109 of 114 patients), 7 (6.1%) of those patients only improved, still with minimal residual urinary leakage. Clinical signs for bowel, urethral, or bladder injuries were undetectable. Intraoperative bleeding, postoperative field infections, or postoperative pelvic floor relaxations were not noted. Three patients (2.6%) presented a little small area of mesh erosion within 6 months from surgery. We also noticed a few obstructive symptoms and irritative symptoms: 4 of 114 (3.,5%) patients showed urge de novo urge symptoms. A high grade degree of satisfaction was assessed reported by QoL questionnaires ( p<0.001). CONCLUSIONS : The midterm results of this study show that the TOT procedure is a simple, safe, and effective technique in thefor treatingment of female stress urinary incontinence

DOUBLE TRANSOBTURATOR APPROACH TO TREATING CYSTOCELE

The aim of this study is to show the safety and efficacy of the double transobturator approach, a new technique for anterior vaginal wall prolapse, using several different kinds of meshes (synthetic and biological).This is a retrospective study of 74 women treated between 2005 and 2007. The patients underwent a 3, 6 and 12 month follow-up. The anatomical cure rate, defined as grade of prolapse <2, was 79.7%. Fifteen patients had a recurrence, 12 were treated with biological VS meshes and 3 were treated with synthetic meshes. Vaginal erosion was reported in 5 (6.7%) of these patients, two of whom were treated with topic estrogenic therapy, and three of whom with partial excision. These results suggest that this technique is safe and efficacious and that the best meshes to use are synthetic meshes, because they have a lower recurrence rate than biological meshes

Benefits of automated acclimatization during the pre-milking phase of lactating girolando cows Benefícios da climatização automatizada na pré-ordenha de vacas girolando em lactação

The aim of this paper was to evaluate the automated acclimatization effects during pre-milking of cows on thermal conditioning, physiology, milk production and cost-benefit of the automated adiabatic evaporative cooling system (AECS). The treatments 20; 30; 40 min and control consisted of exposure time of pre-milking cows to the automated AECS. Sixteen cows were used with an average daily milk yield of 19 kg, distributed in a 4 x 4 Latin square design. The Tukey's test (P<0.05) was used to compare the means. The environmental variables, dry bulb temperature (DBT, ºC) and relative humidity (RH, %), were recorded every minute, which allowed the determination of the system efficiency through the Temperature and Humidity Index (THI). The respiratory rate (RR), rectal temperature (RT) and temperature of the coat (TC) were measured before and after the acclimatization. The 40 min treatment kept the environmental variables and the comfort indexes within recommended limits. The physiological variables (RR, RT and TC) were lower in the 40 min treatment and reflected positively on milk production, which increased 3.66% compared to the control treatment. The system was profitable, having a 43 days return on investment and a monthly revenue increase of R$1,992.67.<br>Objetivou-se com esta pesquisa avaliar os efeitos da climatização na pré-ordenha sobre o acondicionamento térmico, fisiologia, produção de leite e relação custo/benefício do sistema de resfriamento adiabático evaporativo (SRAE) automatizado. Os tratamentos adotados foram os tempos de exposição dos animais ao SRAE no curral de espera, 20; 30; 40 min e o tratamento-controle. Utilizaram-se 16 vacas com produção de 19 kg de leite dia-1, adotando-se delineamento em quadrado latino 4 x 4 e comparação entre as médias pelo teste de Tukey (P<0,05). Foram registradas as variáveis ambientais temperatura de bulbo seco (Tbs) e umidade relativa (UR) a cada minuto, o que permitiu determinar a eficiência do sistema por meio do índice de temperatura e umidade (ITU). A frequência respiratória (FR), temperatura retal (TR) e temperatura de pelame (TP) foram medidas antes e depois da climatização. O tratamento 40 min permitiu manter as variáveis ambientais e os índices de conforto entre os limites recomendados. As variáveis fisiológicas (FR, TR e TP) mostraram valores inferiores no tratamento 40 min, com aumento de 3,66$ 1.992,67 com tempo de retorno do capital de 43 dias

Recurrence rate after loop electrosurgical excision procedure (LEEP) and laser Conization: A 5-year follow-up study

Objective: Conization aims to remove pre-neoplastic lesions of the uterine cervix. Several techniques for conization have been compared, but evidence regarding the most effective therapeutic option is scant. Here, we aimed to compare the recurrence rate following laser conization and loop electrosurgical excision procedure (LEEP) in patients with high-grade cervical dysplasia (HSIL/CIN2+). Methods: This is a retrospective multi-institutional study. Medical records of consecutive patients with HSIL/CIN2+ undergoing conization between 2010 and 2014 were retrieved. A propensity-score matching (PSM) was applied in order to reduce allocation bias. The risk of developing recurrence was estimated using Kaplan-Meir and Cox hazard models. Results: Overall, 2966 patients had conization over the study period, including 567 (20%) and 2399 (80%) patients having laser conization and LEEP, respectively. Looking at predictors of recurrence, diagnosis of CIN3 (HR:3.80 (95%CI:2.01,7.21); p &lt; 0.001) and HPV persistence (HR:1.81 (95%CI:1.11,2.96); p &lt; 0.001) correlated with an increased risk of recurrence. After applying a PSM we selected 500 patients undergoing laser conization and 1000 undergoing LEEP. Patients undergoing LEEP were at higher risk of having positive surgical margins in comparison to patients undergoing laser conization (11.2% vs. 4.2%). The risk of having persistence of HPV was similar between the two groups (15.0% vs. 11.6%;p = 0.256). Five-year recurrence rate was 8.1% and 4% after LEEP and laser conization, respectively (p = 0.023). HPV persistence was the only factor associated with [5-]year recurrence after both laser conization (p = 0.003) and LEEP (p = 0.001). Conclusions: HPV persistence is the only factor associated with an increased risk of recurrence after either laser conization or LEEP. Owing to the lack of data regarding obstetrical outcomes, we are not able to assess the best therapeutic option for women with cervical dysplasia

Background: Primary prevention through vaccination is a prophylactic approach aiming to reduce the risk of developing human papillomavirus (HPV)-related lesions. No mature and long-term data supported the adoption of vaccination in women undergoing conization. Methods: This is a retrospective multi-institutional study. Charts of consecutive patients undergoing conization between 2010 and 2014 were collected. All patients included had at least 5 years of follow-up. We compared outcomes of patients undergoing conization plus vaccination and conization alone. A propensity-score matching algorithm was applied in order to reduce allocation biases. The risk of developing recurrence was estimated using Kaplan-Meir and Cox hazard models. Results: Overall, charts of 1914 women were analyzed. The study group included 116 (6.1%) and 1798 (93.9%) women undergoing conization plus vaccination and conization alone, respectively. Five-year recurrence rate was 1.7% (n = 2) and 5.7% (n = 102) after conization plus vaccination and conization alone, respectively (p = 0.068). After the application of a propensity-score matching, we selected 100 patients undergoing conization plus vaccination and 200 patients undergoing conization alone. The crude number of recurrences was 2 (2%) and 11 (5.5%) for patients undergoing conization plus vaccination and conization alone, respectively (p = 0.231). Vaccination had no impact on persistent lesions (no negative examination between conization and new cervical dysplasia; p = 0.603), but reduced the risk of recurrent disease (patients who had at least one negative examination between conization and the diagnosis of recurrent cervical dysplasia; p = 0.031). Conclusions: Patients having vaccination experience a slightly lower risk of recurrence than women who had not, although not statistically significantly different. Further evidence is needed to assess the cost effectiveness of adopting vaccination in this setting

Prognostic impact of diabetes and prediabetes on survival outcomes in patients with chronic heart failure: A post-hoc analysis of the GISSI-HF (Gruppo Italiano per lo Studio della Sopravvivenza nella Insufficienza Cardiaca-Heart Failure) trial

826BACKGROUND: The independent prognostic impact of diabetes mellitus (DM) and prediabetes mellitus (pre-DM) on survival outcomes in patients with chronic heart failure has been investigated in observational registries and randomized, clinical trials, but the results have been often inconclusive or conflicting. We examined the independent prognostic impact of DM and pre-DM on survival outcomes in the GISSI-HF (Gruppo Italiano per lo Studio della Sopravvivenza nella Insufficienza Cardiaca-Heart Failure) trial. METHODS AND RESULTS: We assessed the risk of all-cause death and the composite of all-cause death or cardiovascular hospitalization over a median follow-up period of 3.9 years among the 6935 chronic heart failure participants of the GISSI-HF trial, who were stratified by presence of DM (n=2852), pre-DM (n=2013), and non-DM (n=2070) at baseline. Compared with non-DM patients, those with DM had remarkably higher incidence rates of all-cause death (34.5% versus 24.6%) and the composite end point (63.6% versus 54.7%). Conversely, both event rates were similar between non-DM patients and those with pre-DM. Cox regression analysis showed that DM, but not pre-DM, was associated with an increased risk of all-cause death (adjusted hazard ratio, 1.43; 95% CI, 1.28-1.60) and of the composite end point (adjusted hazard ratio, 1.23; 95% CI, 1.13-1.32), independently of established risk factors. In the DM subgroup, higher hemoglobin A1c was also independently associated with increased risk of both study outcomes (all-cause death: adjusted hazard ratio, 1.21; 95% CI, 1.02-1.43; and composite end point: adjusted hazard ratio, 1.14; 95% CI, 1.01-1.29, respectively).
