    Chitosan-based scaffold modified with D-(+) raffinose for cartilage repair: an in vivo study

    BackgroundOsteochondral defects significantly affect patients¿ quality of life and represent challenging tissue lesions, because of the poor regenerative capacity of cartilage. Tissue engineering has long sought to promote cartilage repair, by employing artificial scaffolds to enhance cell capacity to deposit new cartilage. An ideal biomaterial should closely mimic the natural environment of the tissue, to promote scaffold colonization, cell differentiation and the maintenance of a differentiated cellular phenotype. The present study evaluated chitosan scaffolds enriched with D-(+) raffinose in osteochondral defects in rabbits. Cartilage defects were created in distal femurs, both on the condyle and on the trochlea, and were left untreated or received a chitosan scaffold. The animals were sacrificed after 2 or 4 weeks, and samples were analysed microscopically.ResultsThe retrieved implants were surrounded by a fibrous capsule and contained a noticeable inflammatory infiltrate. No hyaline cartilage was formed in the defects. Although defect closure reached approximately 100% in the control group after 4 weeks, defects did not completely heal when filled with chitosan. In these samples, the lesion contained granulation tissue at 2 weeks, which was then replaced by fibrous connective tissue by week 4. Noteworthy, chitosan never appeared to be integrated in the surrounding cartilage.ConclusionsIn conclusion, the present study highlights the limits of D-(+) raffinose-enriched chitosan for cartilage regeneration and offers useful information for further development of this material for tissue repair

    Lattice Anharmonicity of Stereochemically Active Lone Pairs Controls Thermochromic Band Gap Reduction of PbVO\u3csub\u3e3\u3c/sub\u3eCl

Stereochemically active lone pairs of electrons play an important role in a diverse range of physical phenomena in many materials, ranging from semiconducting halide perovskites to thermochromic inorganic-organic hybrids. In this paper, we demonstrate the importance of the 6s2 lone pair of Pb on the reversible thermochromic transition in the mixed-anion inorganic compound, PbVO3Cl. This 6s2 stereochemically active lone pair results in subtle structural distortions upon heating while maintaining its overall orthorhombic structure. These distortions result in competing interactions with the Pb 6s2 lone pair and ultimately, a pronounced change between yellow and red at ∼200 °C. X-ray diffraction analyses of PbVO3Cl demonstrate two-dimensional features in contrast to the three-dimensional network in isostructural BaVO3Cl. X-ray and neutron pair distribution function experiments reveal that Pb-O interatomic distances decrease upon heating, while Pb-Cl distances are only affected by thermal motion. X-ray photoelectron spectroscopy measurements provide experimental evidence of the presence of the 6s2 lone pair at the valence band maximum, which are corroborated by first-principles calculations. The results demonstrate a broadly generalizable mechanism for using repulsions between lone-pair electrons of p-block cations to drive discontinuous changes of local symmetry and electronic structure

    User fees in private non-for-profit hospitals in Uganda: a survey and intervention for equity

    BACKGROUND: In developing countries, user fees may represent an important source of revenues for private-non-for-profit hospitals, but they may also affect access, use and equity. METHODS: This survey was conducted in ten hospitals of the Uganda Catholic Medical Bureau to assess differences in user fees policies and to propose changes that would better fit with the social concern explicitly pursued by the Bureau. Through a review of relevant hospital documents and reports, and through interviews with key informants, health workers and users, hospital and non-hospital cost was calculated, as well as overall expenditure and revenues. Lower fees were applied in some pilot hospitals after the survey. RESULTS: The percentage of revenues from user fees varied between 6% and 89% (average 40%). Some hospitals were more successful than others in getting external aid and government subsidies. These hospitals were applying lower fees and flat rates, and were offering free essential services to encourage access, as opposed to the fee-for-service policies implemented in less successful hospitals. The wide variation in user fees among hospitals was not justified by differences in case mix. None of the hospitals had a policy for exemption of the poor; the few users that actually got exempted were not really poor. To pay hospital and non-hospital expenses, about one third of users had to borrow money or sell goods and property. The fee system applied after the survey, based on flat and lower rates, brought about an increase in access and use of hospital services. CONCLUSION: Our results confirm that user fees represent an unfair mechanism of financing for health services because they exclude the poor and the sick. To mitigate this effect, flat rates and lower fees for the most vulnerable users were introduced to replace the fee-for-service system in some hospitals after the survey. The results are encouraging: hospital use, especially for pregnancy, childbirth and childhood illness, increased immediately, with no detrimental effect on overall revenues. A more equitable user fees system is possible

    Marine phycotoxin levels in shellfish-14 years of data gathered along the Italian coast

    Along the Italian coasts, toxins of algal origin in wild and cultivated shellfish have been reported since the 1970s. In this study, we used data gathered by the Veterinary Public Health Institutes (IZS) and the Italian Environmental Health Protection Agencies (ARPA) from 2006 to 2019 to investigate toxicity events along the Italian coasts and relate them to the distribution of potentially toxic species. Among the detected toxins (OA and analogs, YTXs, PTXs, STXs, DAs, AZAs), OA and YTX were those most frequently reported. Levels exceeding regulatory limits in the case of OA (≤2,448 μg equivalent kg-1) were associated with high abundances of Dinophysis spp., and in the case of YTXs (≤22 mg equivalent kg-1) with blooms of Gonyaulax spinifera, Lingulodinium polyedra, and Protoceratium reticulatum. Seasonal blooms of Pseudo-nitzschia spp. occur all along the Italian coast, but DA has only occasionally been detected in shellfish at concentrations always below the regulatory limit (≤18 mg kg-1). Alexandrium spp. were recorded in several areas, although STXs (≤13,782 μg equivalent kg-1) rarely and only in few sites exceeded the regulatory limit in shellfish. Azadinium spp. have been sporadically recorded, and AZAs have been sometimes detected but always in low concentrations (≤7 μg equivalent kg-1). Among the emerging toxins, PLTX-like toxins (≤971 μg kg-1 OVTX-a) have often been detected mainly in wild mussels and sea urchins from rocky shores due to the presence of Ostreopsis cf. ovata. Overall, Italian coastal waters harbour a high number of potentially toxic species, with a few HAB hotspots mainly related to DSP toxins. Nevertheless, rare cases of intoxications have occurred so far, reflecting the whole Mediterranean Sea conditions

    Changes in Community Mobility in Older Men and Women. A 13-Year Prospective Study

    Community mobility, defined as “moving [ones] self in the community and using public or private transportation”, has a unique ability to promote older peoples’ wellbeing by enabling independence and access to activity arenas for interaction with others. Early predictors of decreased community mobility among older men and women are useful in developing health promoting strategies. However, long-term prediction is rare, especially when it comes to including both public and private transportation. The present study describes factors associated with community mobility and decreased community mobility over time among older men and women. In total, 119 men and 147 women responded to a questionnaire in 1994 and 2007. Respondents were between 82 and 96 years old at follow-up. After 13 years, 40% of men and 43% of women had decreased community mobility, but 47% of men and 45% of women still experienced some independent community mobility. Cross-sectional independent community mobility among men was associated with higher ratings of subjective health, reporting no depression and more involvement in sport activities. Among women, cross-sectional independent community mobility was associated with better subjective health and doing more instrumental activities of daily living outside the home. Lower subjective health predicted decreased community mobility for both men and women, whereas self-reported health conditions did not. Consequently, general policies and individual interventions aiming to improve community mobility should consider older persons’ subjective health

Putative PRR dysfunction is associated with numerous immune disorders that affect the skin, such as systemic lupus erythematosus, cryopyrin-associated periodic syndrome, and primary inflammatory skin diseases including psoriasis and atopic dermatitis. As yet, the evidence is often confined to genetic association studies without additional proof of a causal relationship. However, insight into the role of PRRs in the pathophysiology of some disorders has already resulted in new therapeutic approaches based on immunomodulation of PRRs

    Primary biliary cirrhosis (PBC) is a classical autoimmune liver disease for which effective immunomodulatory therapy is lacking. Here we perform meta-analyses of discovery data sets from genome-wide association studies of European subjects (n=2,764 cases and 10,475 controls) followed by validation genotyping in an independent cohort (n=3,716 cases and 4,261 controls). We discover and validate six previously unknown risk loci for PBC (Pcombined<5 × 10(-8)) and used pathway analysis to identify JAK-STAT/IL12/IL27 signalling and cytokine-cytokine pathways, for which relevant therapies exist

Negli ultimi anni la produzione industriale di polimeri sintetici ha subito un importante crescita ed i materiali più tradizionali sono stati soppiantati da questi polimeri in svariati settori. I polimeri sintetici sono i biomateriali più usati grazie alla loro grande disponibilità in diverse forme: fibre, liquidi viscosi, pellicole, schiume, ecc., e alla loro struttura molto simile a quella dei polimeri naturali. I polimeri sono una classe di materiali che possono essere ingegnerizzati per interagire con i sistemi biologici con finalità terapeutiche o diagnostiche. Spesso quando utilizzati in campo biomedico si ha la formazione di legami tra i polimeri sintetici ed i tessuti naturali per facilitare il fissaggio delle protesi nella sede d’innesto; inoltre in molti casi l’utilizzo di questi materiali è preceduto da dei trattamenti superficiali in modo da prevenire qualsiasi effetto indesiderato. I poliuretani sono tra i biomateriali polimerici più utilizzati per applicazioni biomediche tra cui le camere di pompaggio dei cuori artificiali e protesi vascolari di piccolo calibro. Possiedono una buona biocompatibilità ed emocompatibilità fondamentali quando utilizzati a contatto con tessuti o organi all’interno del corpo umano, ed è possibile inoltre modularne le proprietà meccaniche in base allo scopo dell’applicazione. I poliuretani infatti, così come quasi tutti i biopolimeri, possono essere rigidi, elastici, adesivi o schiumosi in base ai reagenti di sintesi e alla tecnica di produzione. Tra i poliuretani più utilizzati in campo biomedico, e più in particolare in quello cardiovascolare, ci sono Biomer, Pellethone, Corathane, Cardiothane, Tecoflex e Chronoflex. Lo scopo di questo elaborato di tesi è quello di raccogliere e riassumere lo stato dell’arte riguardante l’utilizzo di Chronoflex nell’ambito biomedico, e di puntare lo sguardo su quelli che sembrano essere gli sviluppi futuri della ricerca in tale ambito
