Trauma outcome of road traffic accident at UAE
Road traffic accident (RTA) is of growing public health importance worldwide contributing significantly to the global disease burden. Road traffic accidents are a major cause of both internal and external wounds, many of which cannot be treated with simple dressings. There is paucity of published data on road traffic accident in environment of UAE. This study was carried out to describe the injury characteristics and outcome of road traffic accident victims in UAE setting and establishment of treatment protocols. Materials ad methods: A total of 100 road traffic crash victims were studied from November 2000 to November 2006, and recorded in Um Aquin hospital UAE depended on the R.T.A Form data on age, sex, nationality, the state of patient in the street, the region of trauma, the nature of injuries, treatment, and outcome were obtained from the registry. The results: Road traffic accident constitute a major public health problem in AUE setting and contribute significantly to unacceptably high morbidity and mortality, about 84% male age between 21-31 year old about 29%, occurred in head and neck 43% for one regen 61%, about 81.82 treated surgically with mortality rate 12%. Conclusion: Injury is the leading cause of death among young adults using of motor vehicles is growing in UAE a particular concern in emerging nations where increasing urbanization, overcrowding and scant regard for the 'rules of the road' are the standard. Pure polyurethane and castor oil based polyurethane: Synthesis and characterization
In this study, the physico-chemical properties of high performance polyurethane synthesized from poly propylene glycol (PPG) in comparison with a combination of PPG and castor oil, is studied using the in-situ polymerization technique. The variations in properties of both types of polyurethanes are evaluated by Fourier transform infrared spectroscopy (FTIR), scanning electron microscopy (SEM), X-ray diffraction (XRD) and the thermogravimetric analysis (TGA) technique. Tensile strength properties were investigated by film tensile testing equipment. The results indicated the presence of large -CH stretching in the castor oil mixed polyurethane with a larger oxidative thermal stability over pure PPG polyurethanes. The tensile properties were found to be almost comparable in pure and mixed polymers, signifying the usage of mixed polymers in the future to overcome the environmental and economic crisis in polyurethane synthesis
Significance of selenium in ameliorating the effects of irrigation deficit via improving photosynthesis efficiency, cell integrity, osmo-protectants, and oil profile of anise crop
Anise is one of the plants with therapeutic potential, which is classified among the most important medicinal plants with interesting biological effects. Its components could be perceived so as “natural” and “safe” alternatives to antibiotics as well as they are applied in different industries such as food and cosmetic purposes. Selenium (Se) is an essential micronutrient, however, its importance to improve oil yield and quality of anise has not been adequately investigated, specifically under drought. Therefore, two successive seasons were conducted to investigate the effect of selenium foliar application upon anise plants under drought stress. Selenium was applied at three different concentrations (0.0 1.0 and 2.0 mM denoted Se0, Se1 and Se2, respectively) along with two levels of crop evapotranspiration (ET): Full irrigation, 100% of ET (FI) and 60% of ET (DI). The promotive effect of combinations of DI × Se1 or Se2 (for Fv/Fm, RWC%, and MSI% in the first season) and DI × Se2 (for Fv/Fm, and MSI% in the second season) were as similar as FI × Se2. Compared to the counterpart control treatment (DI × Se0), the highest increases in total free amino acids (31.5 and 31.6%), total soluble sugars (84.2 and 86.4%) and free proline content (84.2 and 86.4%) were recorded with application of DI × Se2 practice in both seasons, respectively. Under DI, Se2 recorded the maximum values of root length, shoot fresh weight and shoot dry weight in the second season. Under drought, the increases in seed yield due to application of Se1 and Se2 amounted to 1.72 and 1.62 folds in the 1st season and 1.50 and 1.43 folds in 2nd one, respectively. The most effective practice for improving IWUE was Se1× DI in both seasons, followed by Se2 × DI. Based on the chromatographical detection, the maximum values of Anethole were recorded with FI × Se2 while L-Linalool has greatly increased with DI × Se2. In conclusion, the growers in arid and semi-arid zones are advised to use selenium (2.0 mM) in anise fertilization to mitigate the adverse impacts of drought, and keeping crop yield and quality
Transcriptomic and Protein Expression Analysis Reveals Clinicopathological Significance of Bloom Syndrome Helicase (BLM) in Breast Cancer
BLM has key roles in homologous recombination repair, telomere maintenance and DNA replication. Germ-line mutation in the BLM gene causes Bloom’s syndrome, a rare disorder characterised by premature aging and predisposition to multiple cancers including breast cancer. The clinicopathological significance of BLM in sporadic breast cancers is unknown. We investigated BLM mRNA expression in the Molecular Taxonomy of Breast Cancer International Consortium cohort (n=1950) and validated in an external dataset of 2413 tumours. BLM protein level was evaluated in the Nottingham Tenovus series comprising 1650 breast tumours. High BLM mRNA expression was highly significantly associated with high histological grade, larger tumour size, ER negative, PgR negative and triple negative phenotypes (ps<0.0001). High BLM mRNA expression was also linked to aggressive molecular phenotypes including PAM50.Her2 (p<0.0001), PAM50.Bas al (p<0.0001) and PAM50.LumB (p<0.0001) and Genufu subtype (ER+/Her2-/High proliferation) (p<0.0001). PAM50.LumA tumours and Genufu subtype (ER+/Her2-/low proliferation) were more likely to express low levels of BLM mRNA (ps<0.0001). Integrative molecular clusters (intClust) intClust.1 (p<0.0001), intClust.5 (p<0.0001), intClust.9 (p<0.0 001) and intClust.10 (p<0.0001) were also more likely in tumours with high BLM mRNA expression. High BLM mRNA expression was associated with poor breast cancer specific survival (BCSS) (ps<0.000001). At the protein level, altered sub-cellular localisation with high cytoplasmic BLM and low nuclear BLM was linked to aggressive phenotypes. In multivariate analysis, BLM mRNA and BLM protein levels independently influenced BCSS ( p=0.03). This is the first and the largest study to provide evidence that BLM is a promising biomarker in breast cancer
Elaboration and characterisation of novel low-cost adsorbents from grass-derived sulphonated lignin
AbstractThis study investigated the use of water-soluble sulphonated lignin (SL) extracted from grass, which has not been used before as a precursor of activated carbon (AC). Chemical activation of SL with three dehydrating salts (ZnCl2, KCl, Fe2(SO4)3·xH2O) at various salt concentrations (10%, 20%, 30%w/w), charring temperatures (600,700°C) and charring times (1,2h) has been carried out. The surface characteristics and removal efficiencies of cadmium, copper and zinc ions from aqueous solutions were affected by the activation conditions. The sulphonated lignin-based activated carbons (SLACs) with the highest specific surface area, total pore and micropore volume were produced at the lowest dehydrating salt concentration (10%w/w) and at 700°C and 2-h charring. These optimal sulphonated lignin-based ACs were named SLAC-ZC (optimal grass-derived SLAC activated by zinc chloride); SLAC-PC (optimal grass-derived SLAC activated by potassium chloride) and SLAC-FS (optimal grass-derived SLAC activated by ferric sulphate). The central composite design and surface response methodology of different SLACs characteristics showed that the optimal responses were achieved at the same operating conditions. These SLACs also achieved the highest removal efficiencies of Cd2+, Cu2+ and Zn2+ ions from aqueous solutions. The chemical activation had significantly increased the total porosity, microporosity and surface area of water-soluble SL. The activation mechanism depended on the used dehydrating salt where the porosity developed by the dehydration effect of ZnCl2, and by a series of hydrolysis and redox reactions for the other two salts. The results of this research demonstrated that water-soluble SL has a great potential as a novel precursor for the production of activated carbons
Population and fertility by age and sex for 195 countries and territories, 1950–2017: a systematic analysis for the Global Burden of Disease Study 2017
Background: Population estimates underpin demographic and epidemiological research and are used to track progress on numerous international indicators of health and development. To date, internationally available estimates of population and fertility, although useful, have not been produced with transparent and replicable methods and do not use standardised estimates of mortality. We present single-calendar year and single-year of age estimates of fertility and population by sex with standardised and replicable methods. Methods: We estimated population in 195 locations by single year of age and single calendar year from 1950 to 2017 with standardised and replicable methods. We based the estimates on the demographic balancing equation, with inputs of fertility, mortality, population, and migration data. Fertility data came from 7817 location-years of vital registration data, 429 surveys reporting complete birth histories, and 977 surveys and censuses reporting summary birth histories. We estimated age-specific fertility rates (ASFRs; the annual number of livebirths to women of a specified age group per 1000 women in that age group) by use of spatiotemporal Gaussian process regression and used the ASFRs to estimate total fertility rates (TFRs; the average number of children a woman would bear if she survived through the end of the reproductive age span [age 10–54 years] and experienced at each age a particular set of ASFRs observed in the year of interest). Because of sparse data, fertility at ages 10–14 years and 50–54 years was estimated from data on fertility in women aged 15–19 years and 45–49 years, through use of linear regression. Age-specific mortality data came from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017 estimates. Data on population came from 1257 censuses and 761 population registry location-years and were adjusted for underenumeration and age misreporting with standard demographic methods. Migration was estimated with the GBD Bayesian demographic balancing model, after incorporating information about refugee migration into the model prior. Final population estimates used the cohort-component method of population projection, with inputs of fertility, mortality, and migration data. Population uncertainty was estimated by use of out-of-sample predictive validity testing. With these data, we estimated the trends in population by age and sex and in fertility by age between 1950 and 2017 in 195 countries and territories. Findings: From 1950 to 2017, TFRs decreased by 49\ub74% (95% uncertainty interval [UI] 46\ub74–52\ub70). The TFR decreased from 4\ub77 livebirths (4\ub75–4\ub79) to 2\ub74 livebirths (2\ub72–2\ub75), and the ASFR of mothers aged 10–19 years decreased from 37 livebirths (34–40) to 22 livebirths (19–24) per 1000 women. Despite reductions in the TFR, the global population has been increasing by an average of 83\ub78 million people per year since 1985. The global population increased by 197\ub72% (193\ub73–200\ub78) since 1950, from 2\ub76 billion (2\ub75–2\ub76) to 7\ub76 billion (7\ub74–7\ub79) people in 2017; much of this increase was in the proportion of the global population in south Asia and sub-Saharan Africa. The global annual rate of population growth increased between 1950 and 1964, when it peaked at 2\ub70%; this rate then remained nearly constant until 1970 and then decreased to 1\ub71% in 2017. Population growth rates in the southeast Asia, east Asia, and Oceania GBD super-region decreased from 2\ub75% in 1963 to 0\ub77% in 2017, whereas in sub-Saharan Africa, population growth rates were almost at the highest reported levels ever in 2017, when they were at 2\ub77%. The global average age increased from 26\ub76 years in 1950 to 32\ub71 years in 2017, and the proportion of the population that is of working age (age 15–64 years) increased from 59\ub79% to 65\ub73%. At the national level, the TFR decreased in all countries and territories between 1950 and 2017; in 2017, TFRs ranged from a low of 1\ub70 livebirths (95% UI 0\ub79–1\ub72) in Cyprus to a high of 7\ub71 livebirths (6\ub78–7\ub74) in Niger. The TFR under age 25 years (TFU25; number of livebirths expected by age 25 years for a hypothetical woman who survived the age group and was exposed to current ASFRs) in 2017 ranged from 0\ub708 livebirths (0\ub707–0\ub709) in South Korea to 2\ub74 livebirths (2\ub72–2\ub76) in Niger, and the TFR over age 30 years (TFO30; number of livebirths expected for a hypothetical woman ageing from 30 to 54 years who survived the age group and was exposed to current ASFRs) ranged from a low of 0\ub73 livebirths (0\ub73–0\ub74) in Puerto Rico to a high of 3\ub71 livebirths (3\ub70–3\ub72) in Niger. TFO30 was higher than TFU25 in 145 countries and territories in 2017. 33 countries had a negative population growth rate from 2010 to 2017, most of which were located in central, eastern, and western Europe, whereas population growth rates of more than 2\ub70% were seen in 33 of 46 countries in sub-Saharan Africa. In 2017, less than 65% of the national population was of working age in 12 of 34 high-income countries, and less than 50% of the national population was of working age in Mali, Chad, and Niger. Interpretation: Population trends create demographic dividends and headwinds (ie, economic benefits and detriments) that affect national economies and determine national planning needs. Although TFRs are decreasing, the global population continues to grow as mortality declines, with diverse patterns at the national level and across age groups. To our knowledge, this is the first study to provide transparent and replicable estimates of population and fertility, which can be used to inform decision making and to monitor progress. The global retinoblastoma outcome study : a prospective, cluster-based analysis of 4064 patients from 149 countries
DATA SHARING : The study data will become available online once all analyses are complete.BACKGROUND : Retinoblastoma is the most common intraocular cancer worldwide. There is some evidence to suggest that major differences exist in treatment outcomes for children with retinoblastoma from different regions, but these differences have not been assessed on a global scale. We aimed to report 3-year outcomes for children with retinoblastoma globally and to investigate factors associated with survival. METHODS : We did a prospective cluster-based analysis of treatment-naive patients with retinoblastoma who were diagnosed between Jan 1, 2017, and Dec 31, 2017, then treated and followed up for 3 years. Patients were recruited from 260 specialised treatment centres worldwide. Data were obtained from participating centres on primary and additional treatments, duration of follow-up, metastasis, eye globe salvage, and survival outcome. We analysed time to death and time to enucleation with Cox regression models. FINDINGS : The cohort included 4064 children from 149 countries. The median age at diagnosis was 23·2 months (IQR 11·0–36·5). Extraocular tumour spread (cT4 of the cTNMH classification) at diagnosis was reported in five (0·8%) of 636 children from high-income countries, 55 (5·4%) of 1027 children from upper-middle-income countries, 342 (19·7%) of 1738 children from lower-middle-income countries, and 196 (42·9%) of 457 children from low-income countries. Enucleation surgery was available for all children and intravenous chemotherapy was available for 4014 (98·8%) of 4064 children. The 3-year survival rate was 99·5% (95% CI 98·8–100·0) for children from high-income countries, 91·2% (89·5–93·0) for children from upper-middle-income countries, 80·3% (78·3–82·3) for children from lower-middle-income countries, and 57·3% (52·1-63·0) for children from low-income countries. On analysis, independent factors for worse survival were residence in low-income countries compared to high-income countries (hazard ratio 16·67; 95% CI 4·76–50·00), cT4 advanced tumour compared to cT1 (8·98; 4·44–18·18), and older age at diagnosis in children up to 3 years (1·38 per year; 1·23–1·56). For children aged 3–7 years, the mortality risk decreased slightly (p=0·0104 for the change in slope). INTERPRETATION : This study, estimated to include approximately half of all new retinoblastoma cases worldwide in 2017, shows profound inequity in survival of children depending on the national income level of their country of residence. In high-income countries, death from retinoblastoma is rare, whereas in low-income countries estimated 3-year survival is just over 50%. Global, regional, and national burden of diabetes from 1990 to 2021, with projections of prevalence to 2050: a systematic analysis for the Global Burden of Disease Study 2021

on September 28, 2023BACKGROUND : Diabetes is one of the leading causes of death and disability worldwide, and affects people regardless of country, age group, or sex. Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050. METHODS : Estimates of diabetes prevalence and burden were computed in 204 countries and territories, across 25 age groups, for males and females separately and combined; these estimates comprised lost years of healthy life, measured in disability-adjusted life-years (DALYs; defined as the sum of years of life lost [YLLs] and years lived with disability [YLDs]). We used the Cause of Death Ensemble model (CODEm) approach to estimate deaths due to diabetes, incorporating 25 666 location-years of data from vital registration and verbal autopsy reports in separate total (including both type 1 and type 2 diabetes) and type-specific models. Other forms of diabetes, including gestational and monogenic diabetes, were not explicitly modelled. Total and type 1 diabetes prevalence was estimated by use of a Bayesian meta-regression modelling tool, DisMod-MR 2.1, to analyse 1527 location-years of data from the scientific literature, survey microdata, and insurance claims; type 2 diabetes estimates were computed by subtracting type 1 diabetes from total estimates. Mortality and prevalence estimates, along with standard life expectancy and disability weights, were used to calculate YLLs, YLDs, and DALYs. When appropriate, we extrapolated estimates to a hypothetical population with a standardised age structure to allow comparison in populations with different age structures. We used the comparative risk assessment framework to estimate the risk-attributable type 2 diabetes burden for 16 risk factors falling under risk categories including environmental and occupational factors, tobacco use, high alcohol use, high body-mass index (BMI), dietary factors, and low physical activity. Using a regression framework, we forecast type 1 and type 2 diabetes prevalence through 2050 with Socio-demographic Index (SDI) and high BMI as predictors, respectively. FINDINGS : In 2021, there were 529 million (95% uncertainty interval [UI] 500–564) people living with diabetes worldwide, and the global age-standardised total diabetes prevalence was 6·1% (5·8–6·5). At the super-region level, the highest age-standardised rates were observed in north Africa and the Middle East (9·3% [8·7–9·9]) and, at the regional level, in Oceania (12·3% [11·5–13·0]). Nationally, Qatar had the world’s highest age-specific prevalence of diabetes, at 76·1% (73·1–79·5) in individuals aged 75–79 years. Total diabetes prevalence—especially among older adults—primarily reflects type 2 diabetes, which in 2021 accounted for 96·0% (95·1–96·8) of diabetes cases and 95·4% (94·9–95·9) of diabetes DALYs worldwide. In 2021, 52·2% (25·5–71·8) of global type 2 diabetes DALYs were attributable to high BMI. The contribution of high BMI to type 2 diabetes DALYs rose by 24·3% (18·5–30·4) worldwide between 1990 and 2021. By 2050, more than 1·31 billion (1·22–1·39) people are projected to have diabetes, with expected age-standardised total diabetes prevalence rates greater than 10% in two super-regions: 16·8% (16·1–17·6) in north Africa and the Middle East and 11·3% (10·8–11·9) in Latin America and Caribbean. By 2050, 89 (43·6%) of 204 countries and territories will have an age-standardised rate greater than 10%. INTERPRETATION : Diabetes remains a substantial public health issue. Type 2 diabetes, which makes up the bulk of diabetes cases, is largely preventable and, in some cases, potentially reversible if identified and managed early in the disease course. However, all evidence indicates that diabetes prevalence is increasing worldwide, primarily due to a rise in obesity caused by multiple factors. Preventing and controlling type 2 diabetes remains an ongoing challenge. Global, regional, and national incidence of six major immune-mediated inflammatory diseases : findings from the global burden of disease study 2019
DATA SHARING STATEMENT : Data used for the analyses are publicly available from the Institute of Health Metrics and Evaluation (http://www.healthdata.org/; http:// ghdx.healthdata.org/gbd-results-tool).BACKGROUND : The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. METHODS : We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. FINDINGS : In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. INTERPRETATION : The incidence patterns of IMIDs varied considerably across the world. Innovative prevention and integrative management strategy are urgently needed to mitigate the increasing ASR of rheumatoid arthritis and upsurging new cases of other five IMIDs, respectively.The Global Burden of Disease Study is funded by the Bill and Melinda Gates Foundation. Antimicrobial activities of coriander in chicken meat products: A review
Chicken meat products contribute significantly as a fairly priced substitute for red meat, which is critically undersupplied in Egypt. This type of meat is high in animal protein, vital amino acids, and trace elements. Furthermore, as a result of rapid improvements in food processing and technology, a variety of chicken meat products, including as chicken burgers, chicken fillets, chicken sandwiches, chicken nuggets, and chicken panne, were manufactured and released into the chicken meat markets. Such key products are defined by their distinct flavor and aroma, which captivates buyers, particularly children. Microorganisms can contaminate chicken meat products at any stage of production, including raw material preparation, manufacture, distribution, and storage. As a result, chicken products are regarded to be a possible source of bacteria that cause food poisoning, such as Staphylococcus aureus (S. aureus) and Salmonella spp. Essential oils derived from the coriander plant (Coriandrum sativum L.), either from the seeds or the leaves, are among the most extensively utilized. Coriander and coriander essential oils are antibacterial, anti-oxidant, anti-diabetic, anxiolytic, anti-epileptic, depressive, anti-mutagenic, anti-inflammatory, antidyslipidemic, antihypertensive, neuroprotective, and diuretic.
