Negative diffraction pattern dynamics in nonlinear cavities with left-handed materials
We study a ring cavity filled with a slab of a right-handed material and a
slab of a left-handed material. Both layers are assumed to be nonlinear Kerr
media. First, we derive a model for the propagation of light in a left-handed
material. By constructing a mean-field model, we show that the sign of
diffraction can be made either positive or negative in this resonator,
depending on the thicknesses of the layers. Subsequently, we demonstrate that
the dynamical behavior of the modulation instability is strongly affected by
the sign of the diffraction coefficient. Finally, we study the dissipative
structures in this resonator and reveal the predominance of a two-dimensional
up-switching process over the formation of spatially periodic structures,
leading to the truncation of the homogeneous hysteresis cycle.Comment: 8 pages, 5 figure
Surface polaritons on left-handed cylinders: A complex angular momentum analysis
We consider the scattering of electromagnetic waves by a left-handed cylinder
-- i.e., by a cylinder fabricated from a left-handed material -- in the
framework of complex angular momentum techniques. We discuss both the TE and TM
theories. We emphasize more particularly the resonant aspects of the problem
linked to the existence of surface polaritons. We prove that the long-lived
resonant modes can be classified into distinct families, each family being
generated by one surface polariton propagating close to the cylinder surface
and we physically describe all the surface polaritons by providing, for each
one, its dispersion relation and its damping. This can be realized by noting
that each surface polariton corresponds to a particular Regge pole of the
matrix of the cylinder. Moreover, for both polarizations, we find that there
exists a particular surface polariton which corresponds, in the large-radius
limit, to the surface polariton which is supported by the plane interface.
There exists also an infinite family of surface polaritons of
whispering-gallery type which have no analogs in the plane interface case and

Carrier capture processes in strain-induced InxGa1-xAs/GaAs quantum dot structures
We investigate carrier capture processes in strain-induced quantum dot structures. The quantum dots consist of a near-surface InGaAs/GaAs quantum well in which a lateral confining potential is generated by the strain from InP stressor islands grown on the sample surface. Using photoluminescence spectroscopy, we show that the rate of carrier capture into the quantum dots increases dramatically when the energetic depth of the confinement potential is reduced by enlarging the quantum well/surface separation D. While carriers in the quantum well region between the quantum dots are found to experience D-dependent nonradiative surface recombination, this process seems to be negligible for carriers in the quantum dots, presumably due to the protecting InP islands.Peer reviewe
Abnormal phenomena in a one-dimensional periodic structure containing left-handed materials
The explicit dispersion equation for a one-dimensional periodic structure
with alternative layers of left-handed material (LHM) and right-handed material
(RHM) is given and analyzed. Some abnormal phenomena such as spurious modes
with complex frequencies, discrete modes and photon tunnelling modes are
observed in the band structure. The existence of spurious modes with complex
frequencies is a common problem in the calculation of the band structure for
such a photonic crystal. Physical explanation and significance are given for
the discrete modes (with real values of wave number) and photon tunnelling
propagation modes (with imaginary wave numbers in a limited region).Comment: 10 pages, 4 figure
Green's function for metamaterial superlens: Evanescent wave in the image
We develop a new method to calculate the evanescent wave, the subdivided
evanescent waves (SEWs), and the radiative wave, which can be obtained by
separating the global field of the image of metamaterial superlens. The method
is based on Green's function, and it can be applied in other linear systems.
This study could help us to investigate the effect of evanescent wave on
metamaterial superlens directly, and give us a new way to design new devices.Comment: 15 pages, 3 figure
Self-induced and induced transparencies of two-dimensional and three- dimensional superlattices
The phenomenon of transparency in two-dimensional and three-dimensional
superlattices is analyzed on the basis of the Boltzmann equation with a
collision term encompassing three distinct scattering mechanisms (elastic,
inelastic and electron-electron) in terms of three corresponding distinct
relaxation times. On this basis, we show that electron heating in the plane
perpendicular to the current direction drastically changes the conditions for
the occurrence of self-induced transparency in the superlattice. In particular,
it leads to an additional modulation of the current amplitudes excited by an
applied biharmonic electric field with harmonic components polarized in
orthogonal directions. Furthermore, we show that self-induced transparency and
dynamic localization are different phenomena with different physical origins,
displaced in time from each other, and, in general, they arise at different

The effect of continuous ultrasound on chronic low back pain: protocol of a randomized controlled trial
<p>Abstract</p> <p>Background</p> <p>Chronic non-specific low-back pain (LBP) is one of the most common and expensive musculoskeletal disorders in industrialized countries. Similar to other countries in the world, LBP is a common health and socioeconomic problem in Iran. One of the most widely used modalities in the field of physiotherapy for treating LBP is therapeutic ultrasound. Despite its common use, there is still inconclusive evidence to support its effectiveness in this group of patients. This randomised trial will evaluate the effectiveness of continuous ultrasound in addition to exercise therapy in patients with chronic LBP.</p> <p>Methods and design</p> <p>A total of 46 patients, between the ages 18 and 65 years old who have had LBP for more than three months will be recruited from university hospitals. Participants will be randomized to receive continuous ultrasound plus exercise therapy or placebo ultrasound plus exercise therapy. These groups will be treated for 10 sessions during a period of 4 weeks. Primary outcome measures will be functional disability and pain intensity. Lumbar flexion and extension range of motion, as well as changes in electromyography muscle fatigue indices, will be measured as secondary outcomes. All outcome measures will be measured at baseline, after completion of the treatment sessions, and after one month.</p> <p>Discussion</p> <p>The results of this trial will help to provide some evidence regarding the use of continuous ultrasound in chronic LBP patients. Improvement of lower extremity electrodiagnostic findings following a trial of spinal manipulation and motion-based therapy
BACKGROUND: Lumbar disc herniation is a problem frequently encountered in manual medicine. While manual therapy has shown reasonable success in symptomatic management of these cases, little information is known how manual therapy may affect the structure and function of the lumbar disc itself. In cases where lumbar disc herniation is accompanied by radicular symptoms, electrodiagnostic testing has been used to provide objective clinical information on nerve function. This report examines the treatment rendered for a patient with lower extremity neurological deficit, as diagnosed on electrodiagnostic testing. Patient was treated using spinal manipulation and exercises performed on a Pettibon Wobble Chair™, using electrodiagnostic testing as the primary outcome assessment. CASE PRESENTATION: An elderly male patient presented to a private spine clinic with right-sided foot drop. He had been prescribed an ankle-foot orthosis for this condition. All sensory, motor, and reflex findings in the right leg and foot were absent. This was validated on prior electromyography and nerve conduction velocity testing, performed by a board certified neurologist. Patient was treated using spinal manipulation twice-weekly and wobble chair exercises three times daily for 90 days total. Following this treatment, the patient was referred for follow-up electrodiagnostic studies. Significant improvements were made in these studies as well as self-rated daily function. CONCLUSION: Motion-based therapies, as part of a comprehensive rehabilitation program, may contribute to the restoration of daily function and the reversal of neurological insult as detected by electrodiagnostic testing. Electrodiagnostic testing may be a useful clinical tool to evaluate the progress of chiropractic patients with lumbar disc herniation and radicular pain syndromes
Advice or exercise for chronic whiplash disorders? Design of a randomized controlled trial
BACKGROUND: Whiplash-associated disorder (or "whiplash") is a common condition incurring considerable expense in social and economic terms. A lack of research on effective therapy for patients with chronic whiplash associated disorders prompted the design of the current study. The primary aim of this randomised controlled trial is to determine the effects of a physical activity program for people with chronic (symptoms of > 3 months duration) whiplash. A secondary aim is to determine if pain severity, level of disability and fear of movement/(re)injury predict response to a physical activity program. METHODS / DESIGN: This paper presents the rationale and design of a randomised controlled trial examining the effects of advice and individualized sub-maximal exercise programs in the treatment of whiplash associated disorders. Predictors of outcome after 6 and 12 months following anthroposophic therapy for adult outpatients with chronic disease: a secondary analysis from a prospective observational study
<p>Abstract</p> <p>Background</p> <p>Anthroposophic medicine is a physician-provided complementary therapy system involving counselling, artistic and physical therapies, and special medications. The purpose of this analysis was to identify predictors of symptom improvement in patients receiving anthroposophic treatment for chronic diseases.</p> <p>Methods</p> <p>913 adult outpatients from Germany participated in a prospective cohort study. Patients were starting anthroposophic treatment for mental (30.4% of patients, n = 278/913), musculoskeletal (20.2%), neurological (7.6%), genitourinary (7.4%) or respiratory disorders (7.2%) or other chronic indications. Stepwise multiple linear regression analysis was performed with the improvement of Symptom Score (patients' assessment, 0: not present, 10: worst possible) after 6 and 12 months as dependent variables. 61 independent variables pertaining to socio-demographics, life style, disease status, co-morbidity, health status (SF-36), depression, and therapy factors were analysed.</p> <p>Results</p> <p>Compared to baseline, Symptom Score improved by average 2.53 points (95% confidence interval 2.39-2.68, p < 0.001) after six months and by 2.49 points (2.32-2.65, p < 0.001) after 12 months. The strongest predictor for improvement after six months was baseline Symptom Score, which alone accounted for 25% of the variance (total model 32%). Improvement after six months was also positively predicted by better physical function, better general health, shorter disease duration, higher education level, a diagnosis of respiratory disorders, and by a higher therapy goal documented by the physician at baseline; and negatively predicted by the number of physiotherapy sessions in the pre-study year and by a diagnosis of genitourinary disorders. Seven of these nine variables (not the two diagnoses) also predicted improvement after 12 months. When repeating the 0-6 month analysis on two random subsamples of the original sample, three variables (baseline Symptom Score, physical function, general health) remained significant predictors in both analyses, and three further variables (education level, respiratory disorders, therapy goal) were significant in one analysis.</p> <p>Conclusion</p> <p>In adult outpatients receiving anthroposophic treatment for chronic diseases, symptom improvement after 6 and 12 months was predicted by baseline symptoms, health status, disease duration, education, and therapy goal. Other variables were not associated with the outcome. This secondary predictor analysis of data from a pre-post study does not allow for causal conclusions; the results are hypothesis generating and need verification in subsequent studies.</p
