Interhemispheric asymmetry profile and cognitive functions in adolescents with average and high intelligence
Background. The neuropsychological approach suggests that differences in cognitive processes are associated with the variability of combinations of signs of partial dominance of certain brain structures, manifested in the form of individual laterality profiles. However, the problem of identifying and studying the relationship between interhemispheric asymmetry and cognitive functions in adolescents with a high level of intelligence remains not fully resolved, which determines the relevance of this study.The aim of the research was to describe the relationship between the type of profile of interhemispheric asymmetry with the characteristics of cognitive functions in adolescents with average and high levels of intelligence.Materials and methods. The sample included 52 people aged 12–15 years: 26 adolescents with high level intelligence (main group), 26 adolescents with an average level of intelligence (control group). The following research methods were used: Wechsler test (WISC-IV), method for studying the profile of interhemispheric asymmetry (Khomskaya E.D., Efimova I.V.); method “Correction test” by B. Bourdon; method ”Story on a given topic”.Results. The majority of adolescents with a high level of intelligence revealed the severity of right-lateral features, which were traced in the manual, auditory and visual spheres of sensorimotor asymmetry. At the same time, the severity of right-lateral and ambilateral features is accompanied by more pronounced indicators of the development of cognitive functions. Adolescents with the type of profile of interhemispheric asymmetry “Left-handed” were characterized by an insufficient level of attention stability and a reduced ability for detailed speech utterance, regardless of the level of intelligence.Conclusions. The data obtained show that the peculiarity of individual profiles of interhemispheric asymmetry determines the uneven development of a number of mental functions in adolescence and the different readiness of brain mechanisms to support various cognitive processes. We assume that the results of the work will expand the understanding of the role of interhemispheric asymmetry of the brain in the organization of cognitive functions in adolescents with a high level of intelligence
Viral Load in COVID-19: Underestimated Clinical and Epidemiological Marker
Background. The viral load of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative agent of the new coronavirus infection, is becoming increasingly important in clinical and epidemiological contexts. Despite this, there are significant complexities in the implementation of viral load quantitative measurement into clinical practice due to the limited approaches to its assessment.The aim of this work was to develop an approach for SARS-CoV-2 viral load analysis by the value of sample threshold cycles (Ct) relative to the Ct of the internal control sample obtained in routine PCR diagnostics of the COVID-19, and to use this approach for quantitative monitoring of viral load in patients with first positive SARS-CoV-2 test from the Irkutsk region.Materials and methods. Using regression models based on the least squares method, an approach to determine the number of copies of SARS-CoV-2 RNA in 1 ml of nasopharyngeal secretion was developed. The viral load of SARS-CoV-2 was assessed in nasopharyngeal and pharyngeal samples obtained from 1370 patients from Irkutsk and Angarsk with primary diagnosed positive PCR result in the period from July 1 to November 10, 2020.Results. A tenfold increase in the average monthly viral load among patients in September-October 2020 was revealed. We assume that the change in the epidemiological pattern of the spread of the new coronavirus infection during this period is associated with an increase in the number of contacts in the population due to the school year beginning. Higher viral loads are observed in populations at risk for COVID-19 – among healthcare workers and adults/elderly patients. Conclusion. The development of a standardized quantification of SARS-CoV-2 viral load in the nasopharyngeal samples can be a predictive clinical marker and a reliable tool for improving COVID-19 surveillance using the proposed approach to assess average viral load in a local population
Capturing the essence of folding and functions of biomolecules using Coarse-Grained Models
The distances over which biological molecules and their complexes can
function range from a few nanometres, in the case of folded structures, to
millimetres, for example during chromosome organization. Describing phenomena
that cover such diverse length, and also time scales, requires models that
capture the underlying physics for the particular length scale of interest.
Theoretical ideas, in particular, concepts from polymer physics, have guided
the development of coarse-grained models to study folding of DNA, RNA, and
proteins. More recently, such models and their variants have been applied to
the functions of biological nanomachines. Simulations using coarse-grained

Многоуровневые инъекции ботулинического токсина типа А (Абоботулотоксина) при лечении спастических форм детского церебрального паралича: ретроспективное исследование опыта 8 российских центров
Background: The contemporary application of Botulinum toxin A (BTA) in cerebral palsy (CP) implies multilevel injections both in on-label and off-label muscles. However, there is no single international opinion on the effective and safe dosages, target muscles, and intervals between the injections.Objective: Our aim was to analyze the Russian multicenter independent experience of single and repeated multilevel injections of Abobotulinum toxin А in patients with spastic forms of CP.Methods: 8 independent referral CP-centers (10 hospitals) in different regions of Russia. Authors evaluated intervals between the injections, dosages of the BTA for the whole procedure, for the body mass, for the each muscle, and functional segment of the extremities.Results: 1872 protocols of effective BTA injections (1–14 repeated injections) for 724 patients with spastic CP were included. The age of the patients was between 8 months to 17 years 4 months at the beginning of the treatment (with a mean of 3 years 10 months). Multilevel BTA injections were indicated for the majority (n = 634, 87.6%) of the patients in all the centers. The medians of the dosages for the first BTA injection were between 30–31 U/kg (500 U), the repeated injections doses up to 45 U/kg (1000 U) (in most centers). The median intervals between the repeated injections were 180–200 days in 484 (66.9%) patients and 140–180 days in 157 (24.7%) patients. In 2 centers, children with GMFCS IV–V were injected more often than others.Conclusion: Multilevel BTA injections were indicated for the most patients. The initial dose of Abobotulinum toxin A was 30–31 U/kg. The repeated injections dose could increase up to 40 U/kg. The repeated injections were done in 140–200 days after the previous injection.Современная концепция ботулинотерапии при детском церебральном параличе (ДЦП) предлагает использование многоуровневых инъекций в расширенное число мышц. Однако по-прежнему отсутствует консенсус относительно выбора оптимальных доз, мышц и интервалов между инъекциями.Цель исследования: изучить российский опыт применения однократных и повторных многоуровневых инъекций абоботулотоксина при лечении спастичности у пациентов с ДЦП.Методы: в ретроспективном исследовании проанализирован опыт ботулинотерапии при ДЦПв 8 специализированных центрах России. Изучали протоколы клинически эффективных инъекций. Оценивали общие дозы препарата БТА, дозы на единицу массы тела пациентов, на всю инъекционную сессию и отдельные мышцы, а также интервалы между инъекциями.Результаты: изучено 1872 протокола клинически эффективных инъекций, всего от 1 до 14 повторных инъекций, сделанных 724 пациентам в возрасте от 8 мес до 17 лет 4 мес (медиана возраста на момент первой инъекции БТА — 3 года 10 мес) на момент начала ботулинотерапии. Большинство пациентов (n = 634; 87,6% инъекций) получили многоуровневую ботулинотерапию. Во всех центрах при первичных инъекциях БТА медиана доз находилась в пределах 30–31 Ед/кг массы тела (общая — 500 Ед). При повторных инъекциях в большинстве учреждений максимальные дозы превышали 45 Ед/кг (1000 Ед). Средние интервалы между повторными инъекциями колебались в пределах 140–180 сут для 157 (24,7%) и 180–200 сут для 484 (66,9%) пациентов. В 2 из 8 центров пациенты с наиболее выраженными двигательными нарушениями (GMFCS IV–V) требовали более частых повторных инъекций БТА.Заключение: в специализированных центрах большинству пациентов с ДЦП ботулинотерапию проводили по многоуровневой схеме. Общая доза абоботулотоксина при первичных инъекциях составляла 30–31 Ед/кг; при повторных инъекциях она могла быть увеличена до 40 Ед/кг и более. Вопрос о повторном проведении инъекции БТА рассматривался в интервале 140–200 сут после предшествующей инъекции
Менингококковая инфекция у детей в период 2012–2021 гг. Основные итоги ретроспективного многоцентрового исследования, проблемы сегодняшнего дня
The heavy burden of meningococcal infection is associated not only with life-threatening complications in the acute period and high mortality in invasive forms of the disease, but also with severe consequences in survivors, who are not recorded in our country.The aim of study: to analyze clinical manifestations, complications of the acute period and outcomes of invasive forms of meningococcal disease in children in various regions of the Russian Federation.Materials and methods: an analysis of data from 1327 inpatient medical records of children with an invasive meningococcal infection from 14 regional centers of the Russian Federation for 2012-2021 was carried out (28.3% of cases of the disease in children in the represented federal districts).Results: it was found that young children predominated among the patients – the median was 27.4 (10.7-70.4) months. Complications of the acute period, often combined, were observed in 47.6% of cases. The development of septic shock was noted in 30.4%, Waterhouse-Friderichsen syndrome in 6.6%, carditis in 2.9%, cerebral edema in 15.7%, arthritis in 1.4% of cases; the formation of hydrocephalus, subdural effusion, sensorineural hearing loss in 1.8%, 0.6%, 1% of children, respectively. The presence of soft tissue necrosis requiring surgical intervention was noted in 3.5% of cases. Mortality rate was 10.1%. At the time of discharge from the hospital, 30% of children had complications associated with meningococcal infection: organ dysfunction/ failure in 13.2% of patients (severe in 1.3%), cerebral insufficiency in 19.6%; severe psycho-neurological deficits, sensorineural hearing loss, problems associated with the need for orthopedic/surgical interventions accounted for 0.7%, 0.6% and 0.8%, respectively.Conclusion. Considering the epidemiological features of meningococcal infection – the risk of a sharp increase in morbidity in short periods of time, the life-threatening nature of the disease itself, it is necessary to remain alert to these risks and take all possible measures to prevent the disease using all available means, the most effective of which is vaccine prevention.Тяжелое бремя менингококковой инфекции связано не только с жизнеугрожающими осложнениями острого периода и высокой летальностью при генерализованных формах заболевания, но и с тяжелыми последствиями у выживших, учет которых в нашей стране не ведется.Цель: проведение анализа клинических проявлений, осложнений острого периода и исходов генерализованных форм менингококковой инфекции у детей в различных регионах Российской Федерации.Материалы и методы: проведен анализ данных 1327 медицинских карт (форма 003/у) детей с генерализованной формой менингококковой инфекции из 14 региональных центров Российской Федерации за 2012– 2021 гг. (28,3% случаев заболевания у детей в представляемых федеральных округах).Результаты: установлено, что среди больных преобладали дети раннего возраста – медиана составила 27,4 (10,7–70,4) месяцев. Осложнения, часто сочетанные, в остром периоде заболевания наблюдались в 47,6% случаев: септический шок в 30,4%, синдром Уотерхауза – Фридериксена в 6,6%, кардит в 2,9%, отек головного мозга в 15,7%, артриты в 1,4%, гидроцефалия в 1,8%, сенсоневральная тугоухость в 1%, субдуральный выпот в 0,6% случаев. Наличие некрозов мягких тканей, требовавших хирургического вмешательства, отмечено в 3,5% случаев. Летальность составила 10,1%. На момент выписки из стационара у 30% детей выявлялись осложнения, в том числе выраженная органная дисфункция в 1,3%, грубый психоневрологический дефицит, сенсоневральная тугоухость; осложнения, требующие проведения ортопедических/хирургических вмешательств, составили 0,7%, 0,6% и 0,8% соответственно.Анализ полученных данных позволил вскрыть существующие проблемы, касающиеся клинической и этиологической диагностики заболевания, возможностей выявления осложнений острого периода и учета последствий генерализованных форм менингококковой инфекции.Заключение. Учитывая эпидемиологические особенности менингококковой инфекции (риск резкого подъема заболеваемости в короткие временные промежутки, жизнеугрожающий характер самого заболевания), необходимо сохранять настороженность в отношении данных рисков и предпринимать все возможные меры для профилактики заболевания с использованием всех доступных средств, наиболее эффективным из которых является вакцинопрофилактика
AUTOMATIC GENERATION OF OPTIMIZED BUSINESS PROCESS MODELS FROM CONSTRAINT-BASED SPECIFICATIONS
Business process (BP) models are usually defined manually by business analysts through imperative languages considering activity properties, constraints imposed on the relations between the activities as well as different performance objectives. Furthermore, allocating resources is an additional challenge since scheduling may significantly impact BP performance. Therefore, the manual specification of BP models can be very complex and time-consuming, potentially leading to non-optimized models or even errors. To overcome these problems, this work proposes the automatic generation of imperative optimized BP models from declarative specifications. The static part of these declarative specifications (i.e. control-flow and resource constraints) is expected to be useful on a long-term basis. This static part is complemented with information that is less stable and which is potentially unknown until starting the BP execution, i.e. estimates related to (1) number of process instances which are being executed within a particular timeframe, (2) activity durations, and (3) resource availabilities. Unlike conventional proposals, an imperative BP model optimizing a set of instances is created and deployed on a short-term basis. To provide for run-time flexibility the proposed approach additionally allows decisions to be deferred to run-time by using complex late-planning activities, and the imperative BP model to be dynamically adapted during run-time using replanning. To validate the proposed approach, different performance measures for a set of test models of varying complexity are analyzed. The results indicate that, despite the NP-hard complexity of the problems, a satisfactory number of suitable solutions can be produced.Ministerio de Ciencia e Innovación TIN2009-1371
Effects of flanking loops on membrane insertion of transmembrane helices: a role for peptide conformational equilibrium
The ability of a transmembrane helix (TMH) to insert into a lipid bilayer has been mainly understood based on the total hydrophobicity of the peptide sequence. Recently, Hedin et al. investigated the influence of flanking loops on membrane insertion of a set of marginally hydrophobic TMHs using translocon-based membrane integration assays. While the flanking loops were found to facilitate the insertion in most cases, counter examples also emerged where the flanking loops hinder membrane insertion and contradict the hydrophobicity and charge distribution analyses. Here, coarse-grained free energy calculations and atomistic simulations were performed to investigate the energetics and conformational details of the membrane insertion of two representative marginally hydrophobic TMHs with (NhaL and EmrL) and without (NhaA and EmrD) the flanking loops. The simulations fail to directly recapitulate the contrasting effects of the flanking loops for these two TMHs, due to systematic overprediction of the stabilities of the transmembrane states that has also been consistently observed in previous studies. Nonetheless, detailed force decomposition and peptide conformation analyses suggest a novel mechanism on how the peptide conformational equilibrium in the aqueous phase may modulate the effects of flanking loops on membrane insertion. Specifically, the flanking loops in peptide EmrL interact strongly with the TMH segment and form stable compact conformations in the aqueous phase, which can hinder membrane absorption and insertion as these processes require extended conformations with minimal interactions between the flanking loops and TMH segment. This work also emphasizes the general importance of considering the peptide conformational equilibrium for understanding the mechanism and energetics of membrane insertion, an aspect that has not yet been sufficiently addressed in the literature
