    Therapeutic Effect of Fenugreek Seed on the Patients Suffering from Diabetes Mellitus type II

    Nowadays diabetes is the most dreadful disease for humankind in the whole world. Indians are more centrally obese at a given level of BMI compared to white Caucasians and that Indians are more insulin resistant even at lower level of BMI so they are more prone to have diabetes.. Consumption of fenu greek seed decreases the blood glucose level as its amount is inversely associated with insulin resistance. A detailed clinical examination done on selected patients. The criteria for the selection are as follows: 1.All patients are sedentary middle aged workers. 2. Patients with complications like were not included 3.Patients selected were explained the outline and aim of the study. They were advised to have fenugreek seed  in the form of powder. 4. We made 3 groups. Group A fed 5 gm, group B fed 10 gm and group C fed 15 gm of fenugreek seed . 5.Patients are recalled after 2 weeks for the investigation to know the proper effect of fenugreek seed. 6.The investigations include Anthropometric data, GTT, blood glucose, and serum lipid profile. The test applied for clinical estimation of blood glucose is GOD-POD (glucose oxidase peroxiadse) method.     Continuous increase of global reliance on sessional staff in higher education has not been accompanied by the development of strategies to enhance quality learning and teaching or understanding the experiences of these staff. This has resulted in a general discontent among this category of academics. The growing importance of building respectful working environments that integrate inclusive care and performance quality leads to the emergence of standards frameworks. Explorations of sessional staff lived experiences are vital to inform policy and practice. This study examined sessional staff predominantly in a marking role in a local context of an Aotearoa/New Zealand University. We explored our markers’ agreement with selected principles of an Australian national framework. Based on an anonymous Qualtrics survey, the quantitative data showed our markers agreed with most of the selected Australian framework principles at the individual and department levels. An appreciative inquiry of the qualitative data identified markers’ desires for increased professional development, infrastructure and resources, and personal motivation. Considering our markers’ experiences, we propose several actions in a workshop format to provide insights for developing better structures and policies for retaining, training, and engaging with sessional staff

    Ovarian Hemangioma: a rare case in a young girl

    The ovary has a rich vascular supply. But the vascular tumors of the ovary are extremely rare. Ovarian masses are mostly discovered accidentally during surgery or imaging. These tumors may rarely be associated with systemic manifestations. Here we report a case of 17 year old young unmarried obese girl who presented with acute pain in abdomen and was treated laproscopically with conservation of ovaries. Histopathology report was suggestive of ovarian haemangioma. The diagnostic challenge and therapeutic rarity of such a tumour in a young unmarried girl has proven to be an exceptional case and an excellent investigative opportunity

    Study of management of postpartum hemorrhage and its complications

    Background: Postpartum hemorrhage (PPH) is globally one of the most common causes of maternal death, especially in developing country like India. Pregnancy and childbirth involve significant health risks, even to women with no preexisting health problem. The objective of this study was to analyze the role of various interventions in the management of PPH and its complications.Methods: This prospective observational study was conducted in the department of obstetrics and gynecology of Department of Obstetrics and Gynecology, Dr. D Y Patil Medical college, Pimpri, Pune, Maharashtra, India. A total number of 80 cases of postpartum hemorrhage that fulfilled the selection criteria were included. Data collected and analyzed in PPH patients with medical and surgical management.Results: In present study, most of cases were multigravida (60%) and more than 50 percent of patients required blood and blood products. In present study, most of the postpartum bleeding or postpartum hemorrhage (PPH) cases managed by medical methods. Uterotonic drugs (42.5%) This was possible due to early identification and timely intervention.Conclusions: Active management of third stage of labour is recommended in all cases. Seventy percent cases were  managed by medical methods while rest of the cases required surgical management. Among the medical management uterotonic drugs and bimanual uterine compression was used while among the surgical methods repair of cervical and vaginal laceration was mostly required

    Green-Synthesized Graphene for Supercapacitors—Modern Perspectives

    Graphene is a unique nanocarbon nanostructure, which has been frequently used to form nanocomposites. Green-synthesized graphene has been focused due to environmentally friendly requirements in recent technological sectors. A very important application of green-synthesized graphene-based nanocomposite has been observed in energy storage devices. This state-of-the-art review highlights design, features, and advanced functions of polymer/green-synthesized graphene nanocomposites and their utility in supercapacitor components. Green graphene-derived nanocomposites brought about numerous revolutions in high-performance supercapacitors. The structural diversity of conjugated polymer and green graphene-based nanocomposites has facilitated the charge transportation/storage capacity, specific capacitance, capacitance retention, cyclability, and durability of supercapacitor electrodes. Moreover, the green method, graphene functionality, dispersion, and matrix–nanofiller interactions have affected supercapacitance properties and performance. Future research on innovative polymer and green graphene-derived nanocomposites may overcome design/performance-related challenging factors for technical usages

    Life-Saving Combined Heart–Kidney Transplantation in a Previous Sequential Heart and Kidney Transplant Recipient

    Purpose: Solid organ re-transplantation in the context of allograft failure is a challenging clinical and ethical problem. Ideally, solid organ re-transplantation after initial allograft failure should be performed in all recipients, but this is often not clinically or logistically feasible. Methods: This report details what we believe is the first combined heart–kidney transplant in a recipient of a previous sequential heart and kidney transplant. Results: Eight years after a combined heart and kidney transplant after initially receiving a sequential heart and kidney transplant, a 31-year-old man is doing extremely well, with no rejection episodes or significant complications after transplantation. Summary: This case confirms that combined heart and kidney transplantation is a viable option for tackling the complex issue of graft failure in recipients of previous cardiac and renal grafts

    Anti-vascular endothelial growth factor treatment normalizes tuberculosis granuloma vasculature and improves small molecule delivery

    Tuberculosis (TB) causes almost 2 million deaths annually, and an increasing number of patients are resistant to existing therapies. Patients who have TB require lengthy chemotherapy, possibly because of poor penetration of antibiotics into granulomas where the bacilli reside. Granulomas are morphologically similar to solid cancerous tumors in that they contain hypoxic microenvironments and can be highly fibrotic. Here, we show that TB-infected rabbits have impaired small molecule distribution into these disease sites due to a functionally abnormal vasculature, with a low-molecular-weight tracer accumulating only in peripheral regions of granulomatous lesions. Granuloma-associated vessels are morphologically and spatially heterogeneous, with poor vessel pericyte coverage in both human and experimental rabbit TB granulomas. Moreover, we found enhanced VEGF expression in both species. In tumors, antiangiogenic, specifically anti-VEGF, treatments can “normalize” their vasculature, reducing hypoxia and creating a window of opportunity for concurrent chemotherapy; thus, we investigated vessel normalization in rabbit TB granulomas. Treatment of TB-infected rabbits with the anti-VEGF antibody bevacizumab significantly decreased the total number of vessels while normalizing those vessels that remained. As a result, hypoxic fractions of these granulomas were reduced and small molecule tracer delivery was increased. These findings demonstrate that bevacizumab treatment promotes vascular normalization, improves small molecule delivery, and decreases hypoxia in TB granulomas, thereby providing a potential avenue to improve delivery and efficacy of current treatment regimens

    A Comparison of Four Treatments for Generalized Convulsive Status Epilepticus

    ABSTRACT Background and Methods Although generalized convulsive status epilepticus is a life-threatening emergency, the best initial drug treatment is uncertain. We conducted a five-year randomized, doubleblind, multicenter trial of four intravenous regimens: diazepam (0.15 mg per kilogram of body weight) followed by phenytoin (18 mg per kilogram), lorazepam (0.1 mg per kilogram), phenobarbital (15 mg per kilogram), and phenytoin (18 mg per kilogram). Patients were classified as having either overt generalized status epilepticus (defined as easily visible generalized convulsions) or subtle status epilepticus (indicated by coma and ictal discharges on the electroencephalogram, with or without subtle convulsive movements such as rhythmic muscle twitches or tonic eye deviation). Treatment was considered successful when all motor and electroencephalographic seizure activity ceased within 20 minutes after the beginning of the drug infusion and there was no return of seizure activity during the next 40 minutes. Analyses were performed with data on only the 518 patients with verified generalized convulsive status epilepticus as well as with data on all 570 patients who were enrolled. Results Three hundred eighty-four patients had a verified diagnosis of overt generalized convulsive status epilepticus. In this group, lorazepam was successful in 64.9 percent of those assigned to receive it, phenobarbital in 58.2 percent, diazepam and phenytoin in 55.8 percent, and phenytoin in 43.6 percent (P=0.02 for the overall comparison among the four groups). Lorazepam was significantly superior to phenytoin in a pairwise comparison (P=0.002). Among the 134 patients with a verified diagnosis of subtle generalized convulsive status epilepticus, no significant differences among the treatments were detected (range of success rates, 7.7 to 24.2 percent). In an intention-to-treat analysis, the differences among treatment groups were not significant, either among the patients with overt status epilepticus (P=0.12) or among those with subtle status epilepticus (P=0.91). There were no differences among the treatments with respect to recurrence during the 12- hour study period, the incidence of adverse reactions, or the outcome at 30 days. Conclusions As initial intravenous treatment for overt generalized convulsive status epilepticus, lorazepam is more effective than phenytoin. Although lorazepam is no more efficacious than phenobarbital or diazepam and phenytoin, it is easier to use. (N Engl J Med 1998;339:792-8.

    Early onset obesity and adrenal insufficiency associated with a homozygous POMC mutation

    Isolated hypocortisolism due to ACTH deficiency is a rare condition that can be caused by homozygous or compound heterozygous mutations in the gene encoding proopiomelanocortin (POMC). Loss of function mutations of POMC gene typically results in adrenal insufficiency, obesity and red hair. We describe an 18 month old Hispanic female with congenital adrenal insufficiency, a novel POMC mutation and atypical clinical features. The patient presented at the age of 9 months with hypoglycemia and the endocrine evaluation resulted in a diagnosis of ACTH deficiency. She developed extreme weight gain prompting sequence analysis of POMC, which revealed a homozygous c.231C > A change which is predicted to result in a premature termination codon. The case we report had obesity, hypocortisolism but lacked red hair which is typical for subjects with POMC mutations. Mutations of POMC should be considered in individuals with severe early onset obesity and adrenal insufficiency even when they lack the typical pigmentary phenotype
