    Introduction : Le but de notre travail est préciser le profil épidémio-clinique de cette pathologie, l’apport de l’imagerie dans le diagnostic, et discuter les modalités de l’exérèse chirurgicale et l’apport de la voie endoscopique.Patients et méthode : il s’agit d’une étude rétrospective de 18 patients porteurs d’ostéomes des sinus paranasaux opérés entre 1993 à 2010. Ont été exclus les patients non opérés et les malades perdus de vue.Résultats : il s’agit de 18 patients porteurs d’un ostéome symptomatique des sinus de la face, d’âge moyen 40 ans, sex ratio 0,38. La localisation la plus fréquente était le sinus frontal dans 55.5% des cas. Le traitement chirurgical était de mise pour les patients symptomatiques. La voie endonasale était indiquée dans 6 cas.la récidive a intéressé deux patients (11%) ayant un ostéome frontal et fronto-ethmoïdal. Pour le reste des malades l’évolution était favorable avec un recul moyen de 36 mois.Conclusion : L’ostéome des sinus paranasaux reste longtemps asymptomatique mais peut se compliquer par extension orbitaire ou cérébrale. L’abord endonasal a permis d’améliorer la prise en charge.Mots clés : Ostéome, sinus de la face, tomodensitométrie, chirurgie, voie endonasaleObjective : Lymph node infection is the most frequent localization of extrapulmonary tuberculosis. The treatment does not make general agreement. The aim of this study is to specify, from a review of the literature, the different ways of antimicrobial treatment and the indications of surgery.Patients and methods : il is about a retrospective study including 18 patients with paranasal sinus osteomas operated between 1993 and 2010. Were excluded unoperated and lost patients.Results : in this group of 18 patients with symptomatic sinus osteoma of the face, the mean age was 40 years, sex ratio was 0.38. the frontal sinus was the most affected, 55.5% of cases. Surgical treatment was set for symptomatic patients. The endonasal route was indicated in 6 cases. Recurrence was observed in two patients (11%) having a frontal osteoma and fronto-ethmoid. Outcomes were favorable in the remaining patients, mean follow-up was 36 months.Conclusion: Paranasal sinuses osteoma is a long asymptomatic tumor, but it may be complicated with orbital extending or stroke.     Introduction : L’otospongiose est une ostéodystrophie de la capsule otique se traduisant par une surdité de transmission ou plus rarement une surdité mixte d’apparition progressive. Le but de notre travail est d’analyser les caractéristiques épidémiologiques, cliniques, paracliniques et comparer les résultats en fonction de l’âge, du stade de l’ostéodystrophie, de la technique chirurgicale et de la prothèse.Patients et méthodes : notre étude est rétrospective portant sur 124 patients (149 oreilles) colligés sur une période de 9 ans (2000-2008).Résultats : notre population a comporté 86 femmes et 38 hommes. La moyenne d’âge à l’intervention était de 39,82 ans. La surdité était bilatérale dans 61,3% des cas et unilatérale dans 38,3%. Les seuils moyens préopératoires de la conduction osseuse et aérienne étaient respectivement de 17,33 dB et 52,52 dB, le rinne moyen était de 35,19 dB. La tomodensitométrie a été réalisée chez 63 patients. On a réalisé 21 platinectomie totale, 24 platinectomie partielle et 101 platinotomie calibrée. Selon la classification de Portmann, on a trouvé une répartition sensiblement égale entre les stades II, III et IV. On a utilisé un piston téflon 6/10 mm dans 119 cas et 4/10 mm dans 27 cas. Le rinne moyen postopératoire était de 15,95 dB. a 1 an, le rinne était de 14,81 dB avec un résultat satisfaisant de 94,2%.Conclusion : Il ressort de notre travail que certains facteurs semblent prédictifs de bons résultats et qui sont : le sexe féminin, l’âge <50 ans, le stade chirurgical< stade IV, la platinotomie, et le piston 0,4 mm.Mots clés : otospongiose, conduction osseuse, conduction aérienne, platinotomie, platinectomie.Objective : Otosclerosis is a common disorder of the otic capsule characterized by the presence of a progressive conductive or rarely mixed hearing loss. The aims of this study were to analyses the epidemiologic, clinical, paraclinical characteristic and compare our results according to age, stage, surgical technique and prosthesis. Patients et méthodes : Our study is retrospective about 124 patients (149 ears) in the period of 9 years (2000-2008).Results: Our population included 86 female and 38 male patients. mean age at intervention time was 39,82 years. The hearing loss was bilateral in 61,3% of cases and unilateral in 38,3%. The bone and air conduction threshold was 17.33 dB and 52,52 dB, the air-bone gap was 35,19 dB. The CT scan was realized in 63 patients. We have realized 21stapedectomy, 24 partial stapedectomy and 101 stapedotomy. In accordance with Portmann classification, the distribution between II, III and IV stages was almost equal. We used 6/10 mm Teflon piston prosthesis in 119 cases and 4/10 mm in 27 cases. The postoperative air-bone gap was 15,95 dB. after 1 year, the air-bone gap was 14,81 dB with a satisfying results in 94,2%.Conclusion: it seems that some factors are predictive for good results like a female sex, age<50 years, stage< stage IV, platinotomy and piston 4/10 mm.Keyswords : otosclerosis, bone conduction, air conduction, stapedectomy, stapedotom

    Analyzing Recent Coronary Heart Disease Mortality Trends in Tunisia between 1997 and 2009.

    BACKGROUND: In Tunisia, Cardiovascular Diseases are the leading causes of death (30%), 70% of those are coronary heart disease (CHD) deaths and population studies have demonstrated that major risk factor levels are increasing. OBJECTIVE: To explain recent CHD trends in Tunisia between 1997 and 2009. METHODS: DATA SOURCES: Published and unpublished data were identified by extensive searches, complemented with specifically designed surveys. ANALYSIS: Data were integrated and analyzed using the previously validated IMPACT CHD policy model. Data items included: (i)number of CHD patients in specific groups (including acute coronary syndromes, congestive heart failure and chronic angina)(ii) uptake of specific medical and surgical treatments, and(iii) population trends in major cardiovascular risk factors (smoking, total cholesterol, systolic blood pressure (SBP), body mass index (BMI), diabetes and physical inactivity). RESULTS: CHD mortality rates increased by 11.8% for men and 23.8% for women, resulting in 680 additional CHD deaths in 2009 compared with the 1997 baseline, after adjusting for population change. Almost all (98%) of this rise was explained by risk factor increases, though men and women differed. A large rise in total cholesterol level in men (0.73 mmol/L) generated 440 additional deaths. In women, a fall (-0.43 mmol/L), apparently avoided about 95 deaths. For SBP a rise in men (4 mmHg) generated 270 additional deaths. In women, a 2 mmHg fall avoided 65 deaths. BMI and diabetes increased substantially resulting respectively in 105 and 75 additional deaths. Increased treatment uptake prevented about 450 deaths in 2009. The most important contributions came from secondary prevention following Acute Myocardial Infarction (AMI) (95 fewer deaths), initial AMI treatments (90), antihypertensive medications (80) and unstable angina (75). CONCLUSIONS: Recent trends in CHD mortality mainly reflected increases in major modifiable risk factors, notably SBP and cholesterol, BMI and diabetes. Current prevention strategies are mainly focused on treatments but should become more comprehensive

    CZT-Based Harmonic Analysis in Smart Grid Using Low-Cost Electronic Measurement Boards

    This paper validates the use of a harmonic analysis algorithm on a microcontroller to perform measurements of non-stationary signals in the context of smart grids. The increasing presence of electronic devices such as inverters of distributed generators (DG), power converters of charging stations for electric vehicles, etc. can drain non-stationary currents during their operation. A classical fast Fourier transform (FFT) algorithm may not have sufficient spectral resolution for the evaluation of harmonics and inter-harmonics. Thus, in this paper, the implementation of a chirp-Z transform (CZT) algorithm is suggested, which has a spectral resolution independent from the observation window. The CZT is implemented on a low-cost commercial microcontroller, and the absolute error is evaluated with respect to the same algorithm implemented in the LabVIEW environment. The results of the tests show that the CZT implementation on a low-cost microcontroller allows for accurate measurement results, demonstrating the feasibility of reliable harmonic analysis measurements even in non-stationary conditions on smart grids

    Premature mortality attributable to smoking among Tunisian men in 2009

    INTRODUCTION: Tobacco smoking is a significant public health threat in the world, a risk factor for many diseases, and has been increasing in prevalence in many developing countries. In this study, we aimed to estimate the burden of premature deaths attributable to smoking among Tunisian men aged 35-69 years in 2009. METHODS: The number of deaths attributable to smoking was estimated using the population attributable risk fraction method. Smoking prevalence was obtained from a nationally representative survey. Causes of death were obtained from the registry of the National Public Health Institute. Relative risks were taken from the American Cancer Society Prevention Study (CPS-II). RESULTS: Total estimated premature deaths attributable to smoking among men in Tunisia were 2601 (95% CI: 2268-2877), accounting for 25% (95% CI: 23.3-26.6) of total male adult mortality. Cancer, cardiovascular and respiratory diseases were the major causes of premature deaths attributable to smoking with 1272 (95% CI: 1188-1329), 966 (95% CI: 779-1133) and 364 (300-415) deaths, respectively. CONCLUSIONS: Tobacco smoking is highly relevant and is related to substantial premature mortality in Tunisia, around double that estimated for the region as a whole. This also has not decreased over the past 20 years. Urgent actions are needed to reduce this pandemic

    Forecasting Tunisian type 2 diabetes prevalence to 2027: validation of a simple model.

    BACKGROUND: Most projections of type 2 diabetes (T2D) prevalence are simply based on demographic change (i.e. ageing). We developed a model to predict future trends in T2D prevalence in Tunisia, explicitly taking into account trends in major risk factors (obesity and smoking). This could improve assessment of policy options for prevention and health service planning. METHODS: The IMPACT T2D model uses a Markov approach to integrate population, obesity and smoking trends to estimate future T2D prevalence. We developed a model for the Tunisian population from 1997 to 2027, and validated the model outputs by comparing with a subsequent T2D prevalence survey conducted in 2005. RESULTS: The model estimated that the prevalence of T2D among Tunisians aged over 25 years was 12.0% in 1997 (95% confidence intervals 9.6%-14.4%), increasing to 15.1% (12.5%-17.4%) in 2005. Between 1997 and 2005, observed prevalence in men increased from 13.5% to 16.1% and in women from 12.9% to 14.1%. The model forecast for a dramatic rise in prevalence by 2027 (26.6% overall, 28.6% in men and 24.7% in women). However, if obesity prevalence declined by 20% in the 10 years from 2013, and if smoking decreased by 20% over 10 years from 2009, a 3.3% reduction in T2D prevalence could be achieved in 2027 (2.5% in men and 4.1% in women). CONCLUSIONS: This innovative model provides a reasonably close estimate of T2D prevalence for Tunisia over the 1997-2027 period. Diabetes burden is now a significant public health challenge. Our model predicts that this burden will increase significantly in the next two decades. Tackling obesity, smoking and other T2D risk factors thus needs urgent action. Tunisian decision makers have therefore defined two strategies: obesity reduction and tobacco control. Responses will be evaluated in future population surveys

    Moving Beyond the Stigma: Understanding and Overcoming the Resistance to the Acceptance and Adoption of Artificial Intelligence Chatbots

    Artificial intelligence chatbots may fundamentally transform academic research, automate mundane tasks, and enhance productivity. However, the integration of artificial intelligence chatbots (AIc) is impeded by a complex stigma deeply rooted in individuals’ misconceptions and apprehension, including concerns about academic integrity, job displacement, data privacy, and algorithmic bias. The aim of this study was to scrutinize the origins and impacts of the stigma associated with artificial intelligence chatbots within the realm of academic research and to propose strategies to mitigate such stigmas. This study draws parallels between the reception of artificial intelligence chatbots and previous transformative technologies, presenting case studies illustrating the spectrum of responses to the integration of artificial intelligence chatbots into academic research. This study identifies the need for a shift in mindset from perceiving artificial intelligence chatbots as threats to recognizing them as facilitators of efficiency and innovation. It also underscores the importance of understanding these models as tools that aid researchers but do not replace the need for human expertise and judgment. We further highlighted the role of education, transparency, regulation, and ethical guidelines in overcoming the stigma associated with artificial intelligence chatbots. Given how adaptable people are, the surrounding stigma will likely fade with time. We support a cooperative strategy with continuing education and discussion to maximize the benefits of artificial intelligence chatbots while minimizing their drawbacks, hopefully paving the way for their ethical and successful application in scholarly research

    A cost effectiveness analysis of salt reduction policies to reduce coronary heart disease in four Eastern Mediterranean countries.

    BACKGROUND: Coronary Heart Disease (CHD) is rising in middle income countries. Population based strategies to reduce specific CHD risk factors have an important role to play in reducing overall CHD mortality. Reducing dietary salt consumption is a potentially cost-effective way to reduce CHD events. This paper presents an economic evaluation of population based salt reduction policies in Tunisia, Syria, Palestine and Turkey. METHODS AND FINDINGS: Three policies to reduce dietary salt intake were evaluated: a health promotion campaign, labelling of food packaging and mandatory reformulation of salt content in processed food. These were evaluated separately and in combination. Estimates of the effectiveness of salt reduction on blood pressure were based on a literature review. The reduction in mortality was estimated using the IMPACT CHD model specific to that country. Cumulative population health effects were quantified as life years gained (LYG) over a 10 year time frame. The costs of each policy were estimated using evidence from comparable policies and expert opinion including public sector costs and costs to the food industry. Health care costs associated with CHDs were estimated using standardized unit costs. The total cost of implementing each policy was compared against the current baseline (no policy). All costs were calculated using 2010 PPP exchange rates. In all four countries most policies were cost saving compared with the baseline. The combination of all three policies (reducing salt consumption by 30%) resulted in estimated cost savings of 235,000,000and6455LYGinTunisia;235,000,000 and 6455 LYG in Tunisia; 39,000,000 and 31674 LYG in Syria; 6,000,000and2682LYGinPalestineand6,000,000 and 2682 LYG in Palestine and 1,3000,000,000 and 378439 LYG in Turkey. CONCLUSION: Decreasing dietary salt intake will reduce coronary heart disease deaths in the four countries. A comprehensive strategy of health education and food industry actions to label and reduce salt content would save both money and lives
