    Features of <i>ESCHERICHIA COLI</i> samples from patients with diarrheal syndrome in the Republic of Guinea

    Introduction. Diarrheal diseases are a global public health issue and cause 15% of deaths in children under 5 years old, of which about 80% occur in the regions of Africa and Southeast Asia. According to the Global Enteric Multicentre Study (GEMS) conducted in a number of African countries, one of the leading pathogens of high risk of death in infants and young children is diarrheagenic E. coli (DEC). In recent decades, antimicrobial resistance (AMR) has become globally ubiquitous. The Republic of Guinea urgently needs large-scale studies devoted to assessing DEC distribution and antibiotic resistance. The purpose of the study is to assess the pattern of E. coli infections and to test the susceptibility to antibiotics in strains of diarrheagenic E. coli sampled from individuals residing in the Republic of Guinea. Materials and methods. From 2019 to 2022, we studied 724 samples of faeces of patients with acute diarrhea, among them 72 (9.9%) children aged 1–5 years, 128 (17.7%) children aged 6–17 years, and 524 (72.4%) people aged 18 years and older; a method of polymerase chain reaction (PCR) was applied with the use of the AmpliSense¼ Escherichioses-FL reagent kit to identify the genetic determinants of DEC: EPEC, EHEC, ETEC, EIEC, and EAgEC (Central Research Institute of Epidemiology of Rospotrebnadzor, Russia). Susceptibility to 15 antimicrobial agents was found by the disc-diffusion method using Mueller–Hinton agar (Russia) and Oxoid discs (UK). Results were interpreted according EUCAST criteria, versions 2019–2022 (https://www.eucast.org/ast_of_bacteria/previous_versions_of_documents). Results. For the period from 2019 to 2022, the percentage of E. coli infections in the etiological pattern of acute intestinal infections amounted to 51.7%. In the age-related manner, DEC was significantly more common in young children aged 0–5 (96.9%, p 0.05) as compared to school age children aged 6–17 (53.9%) and adults (45.6%). In all years of observation, EAgEC strains prevailed, accounting for 38.4%. Other DEC pathotypes, EPEC, ETEC, EIEC and STEC, accounted for 27.2%, 17.5%, 11.8%, and 5.1%, respectively. DEC strains are susceptible to meropenem, amikacin, and nitrofurantoin. The activity of other antibiotics ranged from 11.3% for ampicillin, 28.3% for trimethoprim-sulfamethoxazole, and 34.0% for tetracycline to 73.6% for cephalosporins, 84.0% for aminoglycosides, and 98.1% for fluorinated quinolones. Conclusion. To reduce the burden of diarrheal diseases in the Republic of Guinea, it may be necessary to conduct targeted epidemiological and microbiological studies to identify DEC and monitor the development of antimicrobial resistance of E. coli infection pathogens in the population

    Chiroptera as a Potential Reservoir of Dangerous for Humans Viruses in the Territory of the Republic of Guinea. Part 2

    Among the viruses transmitted by Chiroptera, filoviruses, lissaviruses, henipaviruses, and coronaviruses pose the major threat to humans. The featured review is devoted to the analysis of the results of studies on the role of African Chiropterans in the circulation sustenance of the viruses. Chronology of investigations on the search of natural reservoir of Filoviruses and the evidence of Chiroptera participation in their circulation are described in the paper. Presented are summarized data on the involvement of chiropterans in the circulation of lissaviruses on the African continent. Given is the information concerning the role of African species of Chiroptera in the circulation of henipaviruses - dangerous for humans viruses of Paramyxoviridae family, associated with chiropterans of South-Eastern Asia. Considered is the possibility of SARS and MERS-like coronaviruses circulation in the populations of certain species of African Chiroptera. Engagement of the Chiroptera from Guinean Republic in maintenance of circulation of the mentioned above viruses in potential natural foci in the territory of the country and possibility of occurrence of dangerous human infection outbreaks is also assessed. The necessity for further investigation of the role of Chiroptera as carriers of the stated viruses to clarify the current epidemiological and epizootiological situation on the viral pathogens in the Republic of Guinea is substantiated

    Chiroptera is one of the largest in numbers orders of mammals which can take on the role of natural host and vector of various viruses, bacteria, and pathogenic fungi. Over the period of 2007–2013, 248 new viruses pertaining to 24 families were detected in chiropterans. Lately, a range of novel infections that provoked severe diseases in humans were associated with bats-carriers. Presently they are viewed as a potential reservoir of Ebola, Marburg, and Hendra viruses, severe acute respiratory syndrome agents, as well as many other lethal diseases. Chiropterans started drawing particular attention after establishment of the fact that they are involved in circulation of ebolavirus Zaire and possibly participate in the formation of natural foci of this infection. Currently chiropterans are considered to be natural reservoirs of ebolavirus Zaire and some other filoviruses (Filoviridae). Accept from filoviruses, other significant for public healthcare viruses were isolated from chiropterans on the African continent – lyssaviruses (Rhabdoviridae, Lyssavirus), henipaviruses (Paramixoviridae, Henipavirus), and coronaviruses (Coronaviridae, Alphacoronavirus, Betacoronavirus) that can also circulate among chiropterans in West Africa. The data available from literature sources on dissemination of chiropterans in the territory of the African continent, their species diversity, peculiarities of ecology and behavior are analyzed in this paper. The information on the infections for which chiropterans can serve as potential reservoirs is also provided. Specifics of interaction between the population residing on African continent and bats of different species are described too. The review is devoted to the assessment of results of studies on population, ecological, and epidemiological factors contributing to maintenance of circulation of a number of dangerous for humans viruses (filoviruses, lyssaviruses, henipaviruses, and coronaviruses) among African chiropterans. The need for further investigation of the mentioned factors affecting chiropteran populations in the Republic of Guinea is substantiated

    New data on the level of immune stratum against Q fever agent in population of the of Republic of Guinea

One of the factors showing the possibility of pathogen circulation in a certain territory is assessed by the presence of an immune stratum in the inhabitants of the region. In the 1980s, the study of the immune structure of the population of the Republic of Guinea in relation to coxiellosis has begun. The present study, carried out in 2015—2019, has been aimed to obtain new information about the immune stratum of the population of the Republic of Guinea against the causative agent of Q fever and to compare it with previous studies. Specific IgG antibodies in the blood of the Guinea residents were detected by using enzyme-linked immunosorbent assay (ELISA) with a set of reagents manufactured at the St. Petersburg Pasteur Institute (St. Petersburg, Russian Federation). The serum samples were tested in at 1:100 dilution. Antibodies against C. burnetii were detected in 124/2346 (5.3% [CI 4.5-6.3]) samples. This study confirms the previously obtained data on the circulation of the causative agent of coxiellosis in all landscape and geographical zones of the Republic of Guinea. The natural and climatic conditions of the region, the variety of ixodic tick species currently inhabiting this territory being a reservoir and vector of infection, as well as a large amount of livestock are the factors for active circulation of the Q fever pathogen and the emergence of related disease outbreaks. The data obtained necessitate continuing further studies on distribution of C. burnetii in the territory of the Republic of Guinea. Taking into consideration the epidemiological significance of Q fever, a pressing task is to study a proportion of this infectious disease in the overall structure of diseases registered in the territory of the Republic of Guinea. It is also necessary to conduct regular epizootological monitoring in order to clarify the types of carriers and vectors of C. burnetii in different landscape and geographical zones of the Republic of Guinea as well as to assess the immune stratum against the pathogen in large and small cattle being the main sources of infection for humans. The data obtained will allow us to determine presence of a natural focus of this infection as well as its borders and develop a set of preventive (anti-epidemic) measures

    Epidemiological Peculiarities of Ebola Virus Disease Epidemic, 2013-2015 in West Africa Countries

    Represented are the results of analysis of the on-going EVD epidemic, 2013-2015 in West Africa countries. Identified have been epidemiological peculiarities, the principal ones of which are: the scale of epidemic transmission; social factors of widespread occurrence; registration of EVD cases in the new territories of the African continent - West Africa (Guinea, Liberia, Sierra-Leone); genetic distinction between Ebola virus and the strains of the same virus, species Zaire ebolavirus, that caused previous outbreaks; prevalence of febrile syndrome over hemorrhagic; high risk of infection with EVD among the healthcare workers. Most probable carriers of Ebola virus may be fruit-bats of the three species - Hypsignathus monstrosus, Myonycteris torquata, and Epomops franqueti.     Detection of Specific Antibodies to Arboviruses in Blood Sera of Persons Residing in Kindia Province, the Republic of Guinea

    The aim of the work was to detect specific antibodies to West Nile, dengue, CCHF, and chikungunya viruses in blood sera of Guinean Kindia Province residents.Materials and methods. The obtained sera were analyzed in ELISA to discover IgG antibodies to abovementioned viruses.Results and conclusions. Detected were 267 (82 %) positive samples out of 326, containing immunoglobulins of G class to these arboviruses. The obtained data provide evidence for active circulation of dengue and West Nile fevers agents in this territory. Further studies of immune strata of the population, and possible carriers and vectors of arboviruses were demonstrated to be advisable for optimization of approaches to prophylactic (anti-epidemic) measures implementation

    Detection of Crimean-Congo Hemorrhagic Fever Virus Markers in Samples of Ixodes Ticks Collected in the Territory of the Republic of Guinea

    Objective of the study. This work was carried out to identify markers (antigen and RNA) of CrimeanCongo hemorrhagic fever (CCHF) virus in samples from ticks, collected in all landscape-geographical areas of Guinea: Lower, Middle, Upper and Forest, to obtain up-to-date data on the distribution of the pathogen in the country.Materials and methods. Total of 4276 specimens of 8 species of ticks collected in 2016–2019 in the territory of the Republic of Guinea were studied, which were compiled into 1406 samples. Ectoparasites were collected from livestock animals, dogs, and small mammals. Viral antigen was detected using enzyme immunoassay (ELISA). The presence of RNA of the CCHF virus was determined by reverse transcription polymerase chain reaction (RT-PCR).Results and discussion. As a result of the studies, the antigen of the CCHF virus was detected in 21 samples (1.5 %), and RNA – in 37 (2.6 %). All samples, in which the viral antigen was detected, contained RNA of the CCHF virus. Positive results were obtained in samples from all geographical areas of the country. The main vectors and reservoirs of the pathogen in Guinea are ticks of the species Rh. sanguineus, Rh. geigyi, Rh. annulatus and Am. variegatum. The data obtained confirm the previously available information on the possibility of the pathogen circulation in this region and determine the need for further study of the spread of the CCHF virus in the territory of the Republic of Guinea

    Studies of Ebola Virus persistence in the Body Fluids of a Patient at Advanced Stages of Convalescence

    At present EVD epidemic, which claimed the lives of more than 10000 people, is still underway in West Africa Countries. Promptness and quality of laboratory diagnostics, alongside with delivered anti-epidemic measures predetermine efficacy of Ebola response operation. Due to a lack of means for the specific prophylaxis and treatment of the disease, asymptomatic patients are discharged from hospitals, based on criteria recommended by WHO, which might be insufficient. Viral RNA is detected in different clinical samples taken from the patients even at the advanced stages of convalescence, which requires essential investigation of peculiarities of Ebola persistence in various biological fluids.
