    Diet, physical exercise and cognitive behavioral training as a combined workplace based intervention to reduce body weight and increase physical capacity in health care workers - a randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Health care workers comprise a high-risk workgroup with respect to deterioration and early retirement. There is high prevalence of obesity and many of the workers are overweight. Together, these factors play a significant role in the health-related problems within this sector. The present study evaluates the effects of the first 3-months of a cluster randomized controlled lifestyle intervention among health care workers. The intervention addresses body weight, general health variables, physical capacity and musculoskeletal pain.</p> <p>Methods</p> <p>98 female, overweight health care workers were cluster-randomized to an intervention group or a reference group. The intervention consisted of an individually dietary plan with an energy deficit of 1200 kcal/day (15 min/hour), strengthening exercises (15 min/hour) and cognitive behavioral training (30 min/hour) during working hours 1 hour/week. Leisure time aerobic fitness was planned for 2 hour/week. The reference group was offered monthly oral presentations. Body weight, BMI, body fat percentage (bioimpedance), waist circumference, blood pressure, musculoskeletal pain, maximal oxygen uptake (maximal bicycle test), and isometric maximal muscle strength of 3 body regions were measured before and after the intervention period.</p> <p>Results</p> <p>In an intention-to-treat analysis from pre to post tests, the intervention group significantly reduced body weight with 3.6 kg (p < 0.001), BMI from 30.5 to 29.2 (p < 0.001), body fat percentage from 40.9 to 39.3 (p < 0.001), waist circumference from 99.7 to 95.5 cm (p < 0.001) and blood pressure from 134/85 to 127/80 mmHg (p < 0.001), with significant difference between the intervention and control group (p < 0.001) on all measures. No effect of intervention was found in musculoskeletal pain, maximal oxygen uptake and muscle strength, but on aerobic fitness.</p> <p>Conclusion</p> <p>The significantly reduced body weight, body fat, waist circumference and blood pressure as well as increased aerobic fitness in the intervention group show the great potential of workplace health promotion among this high-risk workgroup.     Sick leave among home-care personnel: a longitudinal study of risk factors

    BACKGROUND: Sick leave due to neck, shoulder and back disorders (NSBD) is higher among health-care workers, especially nursing aides/assistant nurses, compared with employees in other occupations. More information is needed about predictors of sick leave among health care workers. The aim of the study was to assess whether self-reported factors related to health, work and leisure time could predict: 1) future certified sick leave due to any cause, in nursing aides/assistant nurses (Study group I) and 2) future self-reported sick leave due to NSBD in nursing aides/assistant nurses (Study group II). METHODS: Study group I, comprised 443 female nursing aides/assistant nurses, not on sick leave at baseline when a questionnaire was completed. Data on certified sick leave were collected after 18 months. Study group II comprised 274 of the women, who at baseline reported no sick leave during the preceding year due to NSBD and who participated at the 18 month follow-up. Data on sick leave due to NSBD were collected from the questionnaire at 18 months. The associations between future sick leave and factors related to health, work and leisure time were tested by logistic regression analyses. RESULTS: Health-related factors such as previous low back disorders (OR: 1.89; 95% CI 1.20–2.97) and previous sick leave (OR 6.40; 95%CI 3.97–10.31), were associated with a higher risk of future sick leave due to any cause. Factors related to health, work and leisure time, i.e. previous low back disorders (OR: 4.45; 95% CI 1.27–15.77) previous sick leave, not due to NSBD (OR 3.30; 95%CI 1.33–8.17), high strain work (OR 2.34; 95%CI 1.05–5.23) and high perceived physical exertion in domestic work (OR 2.56; 95%CI 1.12–5.86) were associated with a higher risk of future sick leave due to NSBD. In the final analyses, previous low back disorders and previous sick leave remained significant in both study groups. CONCLUSION: The results suggest a focus on previous low back disorders and previous sick leave for the design of early prevention programmes aiming at reducing future sick leave due to any cause, as well as due to NSBD, among nursing aides/assistant nurses. A multifactorial approach may be of importance in the early prevention of sick leave due to NSBD

    The Effects of a Change in Work Organization Upon the Work Environment and Musculoskeletal Symptoms Among Letter Carriers

    An organizational change among 82 postal workers was studied with the aim of evaluating the effects on the work environment, work ability, and musculoskeletal complaints. The study was undertaken in 2 suburbs of Stockholm, Sweden. Psychological work demands were estimated to be reduced at the 1-year follow-up but physical work demands had changed very little. In an observation study in a subgroup of older workers, the risk of overexertion at work and musculoskeletal complaints was reduced. In spite of that, most of the older participants (>35 years) had unchanged or increased musculoskeletal symptoms. This shows the need for early preventive measures

    Validation of a self-administered questionnaire for assessing exposure to back pain mechanical risk factors

    Objective: To validate a self-administered questionnaire assessing exposure to mechanical risk factors, developed for a cohort study aiming at assessing the influence of physical and psycho-social factors on the incidence of low back pain (LBP). Methods: The study first involved a criterion validity test. A sample of the cohort workers (n=152) was observed at the workplace during four 30 min periods randomly distributed along the shift. At the end of the work shift, the questionnaire was filled in both by the worker and the observer. Agreements were tested between self-reports and observations, and between self-reports and observer opinion. Secondly, a comparison of exposure-effect relationships based on self-reports to those based on observations was carried out on the whole study cohort (n=716). Both sets of Relative Risks of being an incident case (LBP lasting at least 7 consecutive days in the follow-up year) were tested for heterogeneity. Results: Self-reports agreement levels were better with observer opinion than with observational data and were higher for answers at a dichotomous level. Vehicle driving, manual handling without estimation of weight and frequencies, or trunk bending without rotation showed a fair to good agreement with the external criteria. Limits in the validation procedure did not allow validating the sitting and standing durations. As regards the health outcome comparison, questionnaire and observations led to homogeneous Relative Risks for the variables tested. Conclusions: Results show that self-reports provide a limited accuracy to assess actual frequencies and durations of work activities. Using a questionnaire, classifying the workers into exposure categories is rather relative, but questionnaire and observations seem similar in their relationships to outcome

    Effect of individual finger skin temperature on vibrotactile perception threshold

    Objectives: In healthy people, the vibrotactile perception threshold (VPT) at fingertips depends on a given measurement method and on individual characteristics such as age, gender and finger skin temperature. The aim of the study was to compare the VPT values in 2 groups of healthy subjects with different finger skin temperature. Materials and Methods: The study group comprised 56 males and 76 females, who formed pairs matched with respect to age, gender and body mass index (BMI) but differing in terms of finger skin temperature at pre-launch testing. The finger skin temperature of less than 29°C indicated the subjects with "cold hands" and that of more than 29°C, the subjects with "warm hands". The measuring system made use of P8 pallesthesiometer (EMSON-MAT, Poland) and the measurement procedure was in compliance with the ISO 13091-1:2001 standard. VPT measurements were performed for the index, middle and ring fingers of both hands at the frequencies of 4 Hz, 25 Hz, 31.5 Hz, 63 Hz, 125 Hz and 250 Hz. Results: The findings of the study revealed that the mean VPTs among the subjects with "cold hands" were significantly higher than the corresponding values among the subjects with "warm hands". Conclusions: The type of individual peripheral thermoregulation should be considered when assessing the VPT and determining its reference values
